Cíl: Průzkumné šetření zjišťovalo schopnosti spolupráce nemocných při léčbě Warfarinem a dodrţování léčebného reţimu v průběhu dlouhodobé antikoagulační terapie Warfarinem. Konkrétně byla mapována a vyhodnocena úroveň edukace sestrou, informovanost a adherence k léčbě u pacientů léčených Warfarinem. Metodika: Průzkumný soubor tvořilo 108 pacientů evidovaných v ambulantním léčení interní a kardiologické ambulance při interním oddělení Nemocnice ve Frýdku-Místku. Průzkumné šetření probíhalo v prosinci roku 2010 a lednu roku 2011. Pro sběr dat byla vyuţita metoda dotazníkového šetření. Výsledky: Výsledky průzkumného šetření ukazují, ţe naprostá většina pacientů léčených Warfarinem se aktivně podílí na léčebném procesu. Nemocným se od zdravotníků dostává poučení o léčbě, sestry se na edukaci podílejí v 75 %. Většina dotázaných dochází na pravidelné kontroly a zná zpaměti uţívanou dávku léku. Čtvrtina nemocných však není schopna uvést krvácení jako hlavní riziko léčby. Převáţná většina respondentů si je vědoma skutečnosti, ţe účinek léčby můţe být ovlivněn sloţením přijímané potravy, méně neţ polovina respondentů zná interakci Warfarinu s jinými léky či doplňky stravy. Nemocní mají na zřeteli, ţe je nutno informovat kaţdého lékaře o této léčbě, jen polovina respondentů však informuje i lékárníka. Závěr: Adherence k léčbě ze strany pacienta je základem úspěšné a bezpečné terapie Warfarinem. Cílem všech zdravotníků by měla být snaha o kvalitní a efektivní komunikaci s nemocnými, snaha o dosaţení co nejvyšší úrovně spolupráce, ochoty a schopnosti pacientů řídit se pokyny zdravotníků. Tím se výrazně zvýší úspěšnost léčby a eliminuje se riziko moţných komplikací léčby., Aim: Exploratory investigation ascertained capabilities of cooperation of patients when treated with warfarin and adherence to treatment regimen during long term anticoagulant therapy by warfarin. Specifically was mapped and assessed the level of education by nurse, foreknowledge and adherence to treatment in patients treated with warfarin. Methods: The exploratory set consisted of 108 patients, who were registered in ambulant treatment of internal and cardiac ambulance at internal department of Hospital in Frydek-Mistek. Exploratory investigation took place in December 2010 and January 2011. For data collection was used method of questionnaire survey. Results: The results of questionnaire survey show, that absolute majority of patients treated with warfarin is actively participating in the treatment process. The patients receive from the health care providers education about the treatment, nurses are participating in education in 75%. Majority of respondents have regular check ups and knows by heart the dose of the drug. A quarter of patients is not able to state bleeding as the main risk of treatment. The vast majority of respondents are aware of the fact, that the treatment effect may be influenced by the composition of the diet, less than half of the respondents know the warfarin interactions with other medication or supplements. Patients have in mind that every doctor should be informed about this treatment, only half of the respondents, however, informs as well the pharmacist. Conclusion: Adherence to treatment by the patient is the foundation of a successful and safe warfarin therapy. The goal of all health professionals should be to aim for quality and effective communication with patients, effort to achieve the highest level of cooperation, willingness and ability of patients to follow medical instructions. Autoři popisují případ 78leté ženy s masivní hemoptýzou, zdrojem které byla hypertrofická bronchiální tepna anomálně odstupující z a. thoracica interna. CT angiografie provedená na multidetektorovém CT přístroji ektopickou bronchiální tepnu zobrazila. Následná selektivní embolizace dané tepny akrylátovým lepidlem vedla k zástavě hemoptýzy. Nemocná neměla recidivu krvácení během 24měsíčního sledování. Anomální odstup bronchiální tepny není vzácný a proto je nutné na něj myslet zejména v případech, kdy není patrná žádná tepna zásobující bronchy v normální anatomické lokalizaci. CT angiografie provedená na multidetektorovém CT přístroji má vysokou úspěšnost v detekci odstupu bronchiálních tepen a slouží k navádění při jejich katetrizaci a embolizaci., The authors describe a case of a 78-year-old woman with massive hemoptysis, which was from to hypertrophic bronchial artery anomalously originating from the internal thoracic artery. CT angiography performed on multidetector CT shows anomalous origin of the right bronchial artery. The bronchial bleeding was treated with embolization. The patient had no recurrent bleeding during 24 month follow-up. Ectopic origin of bronchial arteries is not rare, therefore it is necessary to consider it especially in cases when no bronchial artery arises between the upper margin of the fifth thoracic vertebra and the lower margin of the sixth thoracic vertebra. Metastatický melanom je agresivní onemocnění. Dosud využívané léčebné možnosti jako aplikace dakarbazinu, temozolomidu, high dose interleukinu-2 byly relativně omezené. Vemurafenib je malá molekula, která má schopnost inhibovat mutovanou BRAF proteinkinázu. Cíl: Posouzení efektivity biopsie sentinelové uzliny a na ní navazující terapie karcinomu prsu po implementaci metodiky do klinické praxe. Soubor a metodika: V průběhu let 2006-2010 bylo sledováno 94 pacientek s karcinomem prsu po biopsii sentinelové uzliny, průměrného věku 60,6 roku, 100 % ve stádiu do T2 v TNM klasifikaci (85 % T1, 10 % T2, 5 % TIS). Biopsie sentinelové uzliny byla provedena s periareolární aplikací radiofarmaka s většími koloidními částicemi. Pozitivní sentinelové lymfatické uzliny byly nalezeny u 27 % pacientek, z nichž se ve 23 % jednalo o mikrometastatické postižení nebo izolované tumorózní buňky. V případě pozitivity sentinelové uzliny byla indikována exenterace axily. U 79 % z celého souboru pacientek byla provedena radioterapie prsu a výběrově i axily. Následně byly pacientky léčeny podle standardního protokolu a sledovány v onkologické ambulanci Nemocnice Pelhřimov. Průměrná doba sledování byla v období vyhodnocování souboru 3,9 roku. Byly sledovány běžně používané parametry v hodnocení péče o onkologické pacienty. Výsledky: Lokoregionální rekurence byla 1 %, axilární rekurence 0 %, vzdálená rekurence 0 %, přežití bez nemoci dosaženo v 99 % a celkové přežití v 99 % případů. Cieľ: Cieľom štúdie bolo preskúmať, ako sestry vnímajú svoju úroveň dôstojného prístupu k pacientom v domovoch sociálnych služieb (DSS) a identifikovať, či existujú významné vzťahy medzi úrovňou dôstojného prístupu k pacientom v DSS a demografickými znakmi (vek sestier, dĺžka praxe, vzdelanie a pod.). Metodika: Výskumný súbor tvorilo 106 sestier pracujúcich v domovoch sociálnych služieb v zriaďovateľskej pôsobnosti Trnavského samosprávneho kraja. Výskum prebiehal v období december 2011 až február 2012. Na zistenie úrovne dôstojného prístupu sme využili autorizovaný dotazník: Posudzovanie etických problémov – dôstojný prístup k pacientovi. Tvorí ho 5 subškál (informovanosť pacientov; autonómia; dodržiavanie dôvernosti a ochrany informácií; princíp spravodlivosti; intimita). Štatistická analýza bola realizovaná softvérom SPSS, verzia 15.0. Výsledky: Svoju úroveň dôstojného prístupu k pacientom v domovoch sociálnych služieb hodnotili sestry ako primeranú. Medzi vnímanou úrovňou dôstojného prístupu sestier a demografickými údajmi boli nízke korelácie. Signifikantne vyššiu úroveň dôstojného prístupu sme zistili u sestier s vyšším vzdelaním (Bc.). Najnižšiu úroveň dôstojného prístupu k pacientom v domovoch sociálnych služieb sme zaznamenali v subškále zameranej na autonómiu pacienta. Záver: Na základe poznania týchto výsledkov môžeme podporovať zmeny, ktoré budú prispievať k zvýšenej kvalite poskytovanej ošetrovateľskej starostlivosti v domovoch sociálnych služieb., Aim: The aim of the study was to examine nurses’ perception of their own level of dignified approach to patients in social care homes and to identify potential statistically significant relationships between the level of dignified approach to patients in social care homes and demographic data (nurses’ age, duration of clinical experience, education etc.). Methods: The sample consisted of 106 nurses working in social care homes run by the Trnava Region. An authorized questionnaire called Assessment of Ethical Issues – Dignified Approach to a Patient was used to determine the level of dignified approach. The questionnaire consisted of five subscales (patient awareness; autonomy; respecting confidentiality; principle of justice; intimacy). Statistical analysis was carried out using the SPSS version 15.0 software. Results: The level of dignified approach to patients in social care homes was self-assessed by nurses as adequate. There was a low correlation between the perceived level of dignified approach to patients and demographic data. Nurses with higher education (bachelor’s degree) demonstrated significantly higher levels of dignified approach. The lowest level of dignified approach was found in the subscale focused on patient autonomy. ABSTRAKT Cieľ: Zámerom štúdie bolo identifikovať štatisticky významné rozdiely v úrovni emočnej inteligencii (EI) u študentov ošetrovateľstva vzhľadom k demografi ckým údajom (vek, dĺžka štúdia) a zároveň zistiť či úroveň EI študentov ošetrovateľstva ovplyvňuje ich záujem o problematiku geriatrického ošetrovateľstva. Metódy: Výskumu sa zúčastnilo 86 univerzitných študentov denného bakalárskeho štúdia odboru ošetrovateľstvo. Na meranie úrovne EI boli využité dotazníky reprezentujúce rôzne chápania EI. Prostredníctvom SIT-EMO (Situational Test of Emotional Understanding) bola zisťovaná EI ako schopnosť. EI ako črta bola zisťovaná dotazníkom SEIS(Schutte Emotional Intelligence Scale). Dotazníkom vlastnej konštrukcie ESE-GP (Emotional self-effi cacygeriatric patient) bola zisťovaná úroveň EI študentov v procese starostlivosti o geriatrických pacientov. Výsledky: Nezistili sme štatisticky významné rozdiely v úrovni EI študentov z hľadiska demografi ckých údajov (vek a ročník štúdia). Medzi úrovňou EI (SEIS)a perspektívnym záujmom študentov venovať sa geriatrickému ošetrovateľstvu bola zistená štatisticky významná súvislosť (p < 0,05). Závery: Zistené výsledky môžu uplatniť najmä inštitúcie poskytujúce ošetrovateľské vzdelávanie, ktorým odporúčame zamerať pozornosť na rozvoj EI v procese vzdelávania študentov ošetrovateľstva., Aim: The aim of the study was to identify statistically significant differences in levels of emotional intelligence (EI) of nursing students in relation with demographic data (age and year of study) and also determine whether the EI level of nursing students affects their interest in the issue of geriatric nursing. Methods: The sample consisted of 86 full-time undergraduate university students of nursing. To measure the level of EI, questionnaires representing diff erent understanding of EI were used. EI as an ability was determined by the SIT-EMO (Situational Test of Emotional Understanding). EI as a trait was evaluated by the questionnaire SEIS (Schutte Emotional Intelligence Scale). The questionnaire of own construction ESE-GP (Emotional self-effi cacygeriatric patient) assessed the level of EI of students in the process of caring for geriatric patients. Results: There were no statistically signifi cant diff erences in the students’ level of EI in terms of demographic data (age and year of study). A statistically signifi cant association (p < 0.05) was found between the students’ level of EI (SEIS) and their promising interest to pursue geriatric nursing. Motivation Until recently it was considered that 65 years is cutoff for defining patients as elderly, but newer reports indicate that this age limit shift to 70 years of age. Elderly patients with advanced non small cell lung cancer, associated comorbidities and poor performance status represent a specific population and a challenge for use of chemotherapy. Primary aim was to evaluate the impact of mono therapy with oral etoposide on overall survival in elderly patients (≥ 70 years of age) with advanced non small cell lung cancer and poor performance status (PS) ≥ 2 (clinical stage IIIb and IV ), and as well to evaluate tolerability of this therapy. Secondary aim was to evaluate response rate. Methods Retrospectively, medical records of 79 female and male patients with advanced non small cell lung cancer and poor performance status treated with oral etoposide (2x25 mg 20 days/10 days pause) in period from 2007 till 2010 were checked for relevant data. Data regarding demographics, performance status, overall survival, response rates and drug toxicity were collected. For statistical analysis we used Pearson chi-square test, T-test, Kaplan-Meier product limited method and Cox regression. Results Median overall survival (OS) was 31 weeks, in patients with PS 2 overall survival was 34 weeks, and in group with PS 3 was only 24 weeks. Partial response was registered in 20.2% of patients, stable disease in 41.85 % and disease progression in 38% of patients. Treatment was well tolerated, febrile neutropenia and toxic deaths were not registered. Toxic effects didn't have statistically significant influence on OS. Účel studie: Hlavním cílem studie bylo odhadnout dosažení patologické kompletní odpovědi nemocných trpících lokálně pokročilým adenokarcinomem rekta po předoperační (neoadjuvantní) chemoradioterapii (CHRT) s využitím individuálních farmakokinetických parametrů 5-fluorouracilu (5-FU). Sekundárním cílem bylo hodnocení bezpečnosti a snášenlivosti léčby. Tato otevřená prospektivní studie podporovaná grantem IGA NS 9693–04/2008 zahrnula 34 dospělých s lokálně pokročilým karcinomem konečníku ozařovaných do celkové dávky 50,4 Gy ve 28 frakcích po 1,8 Gy 10–15 MV paprsky v režimu 5 + 2 (5 dní radioterapie, 2 dny volno) s kontinuálně podávaným 5-FU v i. v. infuzi 200–1000 mg/m2 po dobu 4–5 týdnů kontinuálně 7denní infuzí. Chirurgická resekce následovala 4–6 týdnů po ukončení CHRT a po klinickém restagingu. Hodnocena byla klinická a patologická odpověď na chemoradioterapii pomocí zobrazovacího (MR) a histopatologického vyšetření, vyjádřená v % jako reziduální choroba. Výsledky a závěr: Výsledek dokládá vztah mezi velikostí kumulativní dávky 5-FU a kumulativní AUC 5-FU (r = 0,61, p < 0,001). Podobný vztah byl prokázán mezi kumulativní AUC 5-FU a metabolickým poměrem (poměr dosažených plazmatických koncentrací neaktivního metabolitu dihydrofluorouracilu (5-FUH2) ku koncentracím 5-FU, r = -0,80, p < 0,0001). Kumulativní AUC korelovala se stupněm odpovědi na léčbu (r = – 0,53, p < 0,005) a určovala také stupeň toxicity léčby. Pokud bychom chtěli dosáhnout kompletní patologickou odpověď, pak by denní dávka 5-FU měla být u středně rychlého metabolizéra >350 mg/m2 a kumulativní AUC1–39 dní > 50 mg/L*h. U žádného z nemocných nebyla nalezena mutace v genu pro dihydropyrimidindehydrogenázu (DPD) a multidrug resistance-1 protein (MDR-1), přesto byla nalezena vysoká interindividuální variabilita v dosažených plazmatických koncentracích 5-FU, a to i s ohledem na cirkadiální rytmus kinetiky léčiva., Background and Purpose: The main goal of the present study was to estimate the early patients´response following neoadjuvant chemoradiotherapy (CHRT) based on 5-fluorouracil (5-FU) with curative aim in relation to plasma concentrations and pharmacokinetic parameters of 5-FU. Secondary objectives included evaluation of the safety and tolerability of the regimen. Patients and Methods: This open prospective study enrolles 34 adult patients with locally advanced rectal cancer, who received 5-FU 200 -1000 mg/m2 administered as a continuous i. v. infusion over 4–5 week and radiotherapy delivered with 10–15 MV photon beams at 1.8 Gy/fraction up o 50.4 Gy in 28 daily fractions for 5 days a week. Surgical resection with curative aim followed 4–6 weeks after the completion of CHRT and clinical restaging. Pathologic response evaluation and the rate of tumor regression was evaluated using tumor downstaging by MR, histopathological staging, and expressed as residual disease (%). Results and Conclusion: The outcome evidenced the correlation between the cumulative 5-FU dose and cumulative AUC of 5-FU (r = 0.61; p < 0.001). The similar relationship was demonstrated between the cumulative AUC and metabolic ratio (the plasma concentration od inactive metabolite dihydrofluotouracile 5-FUH2 to 5-FU; r = -0.80; p < 0.0001). The cumulative AUC was correlated with tumor regression rate (r = -0.53; p < 0.005) and determined toxicity grade. To reach pCR, the daily dose of 5-FU in patient with average metabolic ratio of 5-FUH2/5-FU should be >350 mg/m2 and the cumulative AUC1–39days > 50 mg/L*h. INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease. Cíl studie: Na základě soudobých studií podat přehled o hlavních biologických funkcích lipoproteinů o vysoké hustotě (HDL), zejména ve vztahu k reverznímu transportu cholesterolu a k jejich protizánětlivému působení. Popsat pravděpodobné mechanismy vzniku dysfunkčních prozánětlivých HDL a uvést některé parametry asociované s tímto procesem. Podat přehled o laboratorních metodách stanovení kvantitativních, strukturních a funkčních vlastností HDL, včetně moderních metod založených na hmotové spektrometrii. Typ studie: přehledový článek Závěr: V případě systémového zánětu a/nebo oxidačního stresu dochází ke vzniku dysfunkčních HDL akumulujících oxidanty. Probíhá strukturní modifikace apolipoproteinu AI, což vede mimo jiné k inhibici reverzního transportu cholesterolu a tvorbě prozánětlivých HDL. Na ztrátě příznivých vlastností HDL se podílí modifikace proteinové i lipidové složky HDL. Izolované měření HDL cholesterolu nemusí u některých syndromů bezvýhradně korelovat s mírou kardiovaskulárního rizika a v některých situacích je vhodné stanovit markery mapující strukturu a funkci HDL., Objective: On the basis of recent studies to make an overview concerning the crucial biological functions of high density lipoproteins (HDL), with emphasis on the role in reverse cholesterol transport and their antiinflammatory traits. The aim was to describe probable mechanisms of dysfunctional proinflammatory HDL formation with introduction some of associated parameters. To present laboratory methods for determination of quantitative, structural and functional qualities of HDL, including advanced mass spectrometry techniques. Study design: review Conclusion: In the case of systemic inflammation and/or oxidative stress, the formation of dysfunctional HDL accumulating oxidants takes place, including apo AI structural modifications. This process can lead to the inhibition of reverse cholesterol transport and proinflammatory HDL generation, among others. Loss of beneficial qualities results from modification of both lipid and protein components of HDL.
