Nízká pravdivost a nedostatečná srovnatelnost výsledků měření patří mezi chronické problémy laboratorní medicíny a může být jedním z faktorů rizika zdravotní péče o pacienty. Situace by měla být teoreticky řešena požadavkem na návaznost kalibrátorů, obsaženým ve Směrnici 98/79 IVD. Tento požadavek je však podle mého názoru plněn v řadě případů formálně a výrobcům uniká často smysl návaznosti jako nástroje zvyšování srovnatelnosti a pravdivosti měření. Za této situace je nutno počítat se signifi kantními hodnotami bias jako se součástí denní praxe činnosti klinických laboratoří a považovat je za významný zdroj dílčí nejistoty výsledků měření. Ukazujeme na několika příkladech, že hodnoty nejistot se započtenými hodnotami bias se dramaticky neliší od klasicky řadu let používaných hodnot celkových chyb měření., Low value of trueness and insufficient level of comparability and interchangeability belongs to main problems of laboratory medicine and can be one of many risks of patients care. Theoretically is situation resolved by requirements on the traceability of calibrators, introduced in Directive IVD 98/79 EC. Practically it seems that reason of traceability as tool for elevation of level in comparability is not for many producers clear. Often only formal documentation is taken in account seriously. In these situation is necessary to calculate that signifi cant bias is part of daily work in clinical laboratories and its exclusion from uncertainty estamation is illusion. Nejistota měření je důležitým konceptem laboratorní medicíny. Tento koncept je popsán v dokumentu ISO „Guide to the Expression of Uncertainty in Measurement”, všeobecně známém pod zkratkou GUM. Výpočet nejistoty má dva důležité aspekty – jeden statistický, založený na zákonu o šíření standardních odchylek, a druhý analytický, zabývající se použitelností výsledku v důsledku různých vlivů působících v procesech měření. Tento článek je stručným úvodem do problematiky stanovení nejistot podle GUM a poskytuje laboratořím orientaci k aplikaci nejistoty měření v praxi. Kromě některých základních postupů vycházejících z GUM jsou popsány i přístupy vycházející z dostupných dat uváděných v pracovních návodech výrobců. Sdělení se také zabývá důležitým dilematem, jak postupovat při měření nejistoty s hodnotami bias. Rovněž demonstruje použití hodnot vnitřní kontroly kvality k výpočtu nejistot., Measurement uncertainty is an important evolving concept in laboratory medicine. This concept is described in the ISO document “Guide to the Expression of Uncertainty in Measurement”, often referred to as “GUM”. GUM has two important aspects; a statistical one, which deals with the propagation of standard deviations; an analytical one, which counts on the expertise of the analytical chemist for addressing measurement uncertainty that extends beyond the obvious (sample related effects, stability of analytes, etc.). This paper gives a short introduction to the GUM concept and addresses the main challenges for its application in laboratory medicine (for example, the treatment of bias). Besides some basic GUM calculations, it describes a generic laboratory approach for calculating measurement uncertainty from available data (typically from manufacturers’ data sheets). The latter example shows the “spirit of GUM” which is equally important to the “statistics of GUM”. Imunochemické analytické metody jsou nenahraditelnou součástí toxikologie díky svému technickému provedení a rychlosti získání výsledku. Těchto výhod je využíváno převážně v klinické toxikologii, kdy je nutno rychle diagnostikovat, zda se jedná o intoxikaci a jaká noxa tuto intoxikaci způsobila. Široké používání imunoanalytických metod má i své nevýhody a úskalí. Nejobtížnější a zároveň nejproblematičtější je interpretace naměřených dat. Pomocí imunotechnik lze zachytit jen relativně úzké spektrum analytů. Pro hledání neznámé noxy v biologických materiálech je proto nutné použít i jiné analytické screeningové metody. Nálezy získané pomocí imunotechnik je vždy potřeba potvrdit jinou nezávislou metodou. Testy pro záchyt toxikologicky významných nox (drogy i léčiva) jsou zaměřeny na celou skupinu strukturně podobných látek a nelze jednoznačně říci, která noxa otravu způsobila., Immunochemical analytical methods represent an irreplaceable part of the toxicology for their technical implementation and rapid outcome. These advantages are used predominantly in clinical toxicology for quick diagnose whether intoxication occurs and what has been the cause. A broad application of immunoassays has certain disadvantages and pitfalls too. The interpretation of measured data is the most diffi cult step and brings a lot of problems. The spectrum of drugs detected using immunoassays is relatively narrow. Therefore, the use of other analytical screening procedures is required for searching of unknown compounds in biological materials. Always, the results obtained by immunoassays need to be confi rmed using another, independent method like chromatography coupled to mass spectrometry. The immunoassays used in toxicology or drug-abuse screening are designed to be broadly cross-reactive with a whole drug group.
