The aim of our study was to determine the effect of selected cytostatics on a human ovarian cancer cell line A2780 as a model system for ovarian cancer treatment. This cell line is considered cisplatin-sensitive. Panel of tested cytostatics included cisplatin, paclitaxel, carboplatin, gemcitabine, topotecan and etoposide. These cytostatics have a different mechanism of action. To evaluate cytotoxic potential of the tested compounds, the methods measuring various toxicological endpoints were employed including morphological studies, MTT assay, dynamic monitoring of cell proliferation with xCELLigence, cell cycle analysis, caspase 3 activity and expression of proteins involved in cell cycle regulation and cell death. Metastatický karcinom prsu je nevyléčitelné onemocnění, ale použití nových léčebných možností může zmírnit obtíže pacientky, prodloužit dobu do progrese a prodloužit celkové přežití. Léčba protinádorovou chemoterapií je často doprovázena řadou závažných nežádoucích účinků a v průběhu léčení časem dojde k rezistenci na chemoterapii. Nové naděje se tedy vkládají do kombinace chemoterapie s cílenou léčbou. Vaskulární endoteliální růstový faktor (VEGF) je jedním z nejdůležitějších regulátorů angiogeneze. Humanizovaná monoklonální protilátka proti VEGF – bevacizumab prokázal účinnost u řady nádorových onemocnění. Významný úspech liečby s inhibítormi tyrozínkinázy (TKI) u chorých s Philadelphia chromozóm (Ph) pozitívnou chronickou myelocytovou leukémiou (CML) dramaticky zmenil prognózu tohto ochorenia. Aj napriek tomu, že prvolíniová liečba CML použitím imatinibu znamená významný benefit pre väčšinu chorých, ukazuje sa, že 25–35 % pacientov potrebuje v priebehu 5 rokov zmenu liečby z dôvodu rezistencie alebo zlyhania liečby. Najčastejším mechanizmom zodpovedným za rezistenciu proti imatinibu u pacientov s CML je rozvoj mutácií v BCR-ABL kinázovej doméne. Tieto mutácie vyvolávajú rôznu mieru rezistencie proti imatinibu a zatiaľ čo niektoré z nich môžu reagovať na zvýšené dávky imatinibu, iné sú rezistentné proti nilotinibu a ďalšie vykazujú zvýšenú mieru rezistencie proti dasatinibu. Práca je prehľad liečebných možností v primárnej liečbe chronickej fázy CML a pri jej zlyhaní. Ukazuje sa, že pokroky v molekulových metódach umožňujú lepšie porozumieť podstatu choroby, zvážiť benefit a riziko terapie, individualizovať terapeutický prístup a prispôsobiť liečbu CML včas tak, aby sa minimalizovalo riziko progresie do akcelerovaných fáz., The phenomenal success of therapy with tyrosine kinase inhibitors (TKI) in Philadelphia chromosome (Ph) – positive chronic myeloid leukemia (CML) has dramatically changed the prognosis of this disease. Despite the fact that first-line therapy of CML using imatinib represents revolutionized the treatment of this disease, it became clear that during 5 years 25–35 % of the patients require change in the therapy due to the development of imatinib resistance or failure. The most frequent mechanism responsible for imatinib resistance is development of mutation in BCR-ABL kinase domain. Mutations cause different level of imatinib resistance and while some of them can be overcome by increased dose of imatinib, others seem to be resistant to nilotinib and others are more resistant to dasatinib. This review is focused on primary treatment and treatment after imatinib failure. Úvod: Radioimunoterapie patří mezi rychle se rozvíjející klinické metody. Často je nejvhodnější možností terapie u pacientů s relabujícím lymfomem, případně u starších pacientů. Vývoj nových typů imunokonjugátů patří proto v posledních letech mezi klíčová témata ve vývoji radiofarmak. Metody: Imunoglobulin G M75 byl značen metodou elektrofilní jodace radionuklidem 125I, který je optimální pro vývoj metody značení a také pro stanovení vlastností značeného preparátu (vazebná aktivita, imunospecifita). Výhody 125I jsou zejména jeho dlouhý poločas (59,408 dne) a nízká radiační zátěž. Zachování vazby aktivního místa na antigen bylo testováno metodou ELISA, koncentrace byla stanovena modifikovanou metodou podle Bradfordové. Fyziologická distribuce byla testována na myších CD1-Foxn1nu. Výsledky: Stanovená kumulace značené protilátky v nádorové tkáni byla > 7 % podané radioaktivity, vazebná aktivita protilátky byla první den po značení vyšší než 90 % ve srovnání s vazebnou aktivitou neznačené protilátky. Závěr: Připravený radioimunokonjugát IgG M75 s 125I si zachovává dostatečnou schopnost vazby antigenu, a má proto potenciál v terapii tumorů exprimujících karbonátdehydratasu. Klasifikácia SZO (2008) je jediná celosvetovo akceptovaná a v praxi používaná klasifikácia nádorových ochorení kostnej drene (KD), ktorá definuje aj postavenie a význam biopsie KD v diagnostickom algoritme. Myeloproliferatívne neoplázie (MPN) tvoria pestrú skupinu ochorení s variabilne dynamickým rozvojom z počiatočných štádií, cez štádiá s plno rozvinutou klinicko-laboratórnou manifestáciou až do pokročilých štádií – fibrotickej, resp. blastovej transformácie, sprevádzaných zmenami morfológie a laboratórneho obrazu. V počiatočných štádiách je určenie typu MPN len na základe klinicko-laboratórnych údajov pre podobnosť ich klinickej manifestácie nespoľahlivé. Dôkaz pre MPN typických genetických mutácií potvrdzuje klonalitu ochorenia, ale jeho typizačný význam je limitovaný. Naopak, bioptické vyšetrenie KD v kontexte dostatočných klinických informácií, výsledkov laboratórnych vyšetrení a s podporou genetických vyšetrení umožňuje okrem potvrdenia MPN aj bližšiu typizáciu, ktorá je prognosticky relevantná a ovplyvňuje aj voľbu terapie. Zároveň umožňuje odlíšenie reaktívnych zmien hemopoézy, zhodnotenie prognosticky významných morfologických znakov (stupeň myelofibrózy, množstvo blastov) a sledovanie progresie ochorenia do fibrotickej, resp. blastovej transformácie. V týchto pokročilých štádiách a po terapii nie je však typizácia MPN len na základe morfológie KD možná. Výpovedná hodnota biopsie závisí od kvality a kvantity vzorky drene, hodnotenie morfológie KD závisí aj od vedomostí a skúseností vyšetrujúceho patológa., WHO classification (2008) is the only world-wide accepted and in routine praxis used classification of bone marrow (BM) neoplasms defining also the importance of BM biopsy within the diagnostic algorithm. Myeloproliferative neoplasias (MPN) represent a heterogenous group of disorders with variable dynamic transformational potential from initial into advanced stages such as blast crisis or myelofibrotic transformation. Because of similar manifestations, the typing of MPN in their initial stages based on clinical and laboratory parameters is not optimal. The finding of MPN-typical genetic mutations proves the disease clonality, but not a diagnosis of individual MPN types. In contrast, the BM biopsy verifies both the diagnosis and MPN type. It helps also to discriminate MPN from reactive hematopoietic changes, to evaluate prognostically relevant BM features (e.g. grade of myelofibrosis, blasts number) and the follow-up to terminal stages. However, the MPN typing in terminal and posttherapeutical stages based on morphology alone is not sufficient. The diagnostic value of BM biopsy depends on specimen´s quality, the objectivity of BM morphology evaluation depends on pathologist´s training and experiences. Autoři popisují jednotlivé bioptické metody, které jsou dnes součástí úplné mamární diagnostiky, jejich indikace, techniku provedení, kontraindikace a komplikace, spolehlivost pro stanovení histologické diagnózy a význam pro následnou léčbu u maligních nádorů prsu. Jednotlivé bioptické techniky jsou podány v historickém přehledu. Nejdéle užívaná aspirace tenkou jehlou je dnes ve většině screeningových zemí užívána pro biopsii solidních lézí velmi omezeně a její hlavní význam je v aspiraci tekutinových útvarů v prsu. Core cut biopsie (CCB) je standardní metodou pro získání histologické diagnózy u solidních lézí prsní žlázy a její zavedení v devadesátých letech 20. století znamenalo zásadní změnu pro následnou chirurgickou a onkologickou léčbu karcinomu prsu. Jsou referovány nejnovější bioptické metody, které využívají k odběru většího množství tkáně z prsu vakua. Jedná se o vakuovou biopsii, která stanoví velmi přesnou histologickou diagnózu u lézí, kde core biopsie může podhodnotit histologickou diagnózu. Je to zejména v případě histologicky hraničních lézí, jako jsou in situ karcinomy a atypické hyperplazie. Intact BLES biopsie (radiofrekvenční ablace) je vysoce přesná bioptická metoda, která může odebrat ložisko v jednom vzorku tkáně. Jako jediná bioptická metoda je schopná zachovat pro patologa hodnotitelný okraj ložiska, a může tak při další technické inovaci uspět i jako terapeutická metoda u minimálních maligních lézí prsu., The authors describe the different biopsy methods, which are now a part of the complete mamodiagnostics, their indications, technique, contraindications and complications, its reliability for histological diagnosis and the importance of subsequent treatment in malignant breast tumors. The core cut biopsy is the standard method for obtaining histological diagnosis in solid lesions and its introduction in the 90s of the last century marked a major change for the post-surgical and oncological treatment of breast cancer. The latest vacuum-assisted biopsy methods used to receive a larger amount of tissue from the breast. It is a vacuum-assisted biopsy, which provides very accurate histological diagnosis for lesions where the core biopsy may underestimate the histological diagnosis. It is especially in the case of histologically borderline lesions such as carcinoma in situ and atypical hyper-plasia. Nádorová onemocnění urogenitálního traktu mají skelet jako predilekční místo metastazování. Bez léčby zaměřené na kostní postižení má většina nemocných s metastatickým karcinomem prostaty v průběhu onemocnění zkušenost s SREs – skeletal-related events – zahrnujících patologické fraktury, kompresi míchy, potřebu paliativní radioterapie, chirurgické výkony na kosti, cementoplastiku a přítomnost hyperkalcemie, které zhoršují kvalitu života a zvyšují mortalitu. V léčbě kostních metastáz karcinomu prostaty, močového měchýře a ledviny byl hodnocen efekt různých bisfosfonátů. Podání kyseliny zoledronové signifikantně redukuje riziko vzniku SREs nejen u karcinomu prostaty, ale také u karcinomu ledviny a močového měchýře., The skeleton is a predilection site for metastasis of genitourinary tumours. If there is no treatment for bone disease, the majority of patients with metastatic prostate cancer experience skeletal-related events (SREs) during the course of the disease, including pathological fractures, spinal cord compression, the need for palliative radiotherapy, surgical procedures on the bones, cementoplasty, and the presence of hypercalcaemia, that all reduce the quality of life and increase the mortality rate. Pozornost věnovaná účinku bisfosfonátů v léčbě kostního postižení je zaměřena především na kostní postižení u karcinomu prsu a prostaty, výjimečně i u jiných solidních nádorů. Nové informace o bisfosfonátech přináší každoročně sympozium ZENITH Meeting. Tématem jednání během posledních let byly především důkazy o protinádorovém účinku bisfosfonátů a jejich možném použití v adjuvantním podání. A bylo překvapením, že letošní téma ZENITH Meetingu bylo zaměřeno na kostní postižení u maligního myelomu, tedy oblast, která je diskutovaná spíše hematology. V březnu 2012 se sešlo v Mnichově 150 odborníků z celého světa, věnujících se problematice bisfosfonátů, aby pohovořili o současném postavení bisfosfonátů v léčbě kostního postižení maligního myelomu. Většina sdělení prezentovala nové údaje, ve snaze přehlédnout současný stav v dané indikaci. V následujícím je uveden výběr z přednesených sdělení., The attention devoted to the effect of bisphosphonates in treating bone disease is particularly focused on bone disease in breast and prostate cancer and only exceptionally in other solid tumours. Every year, new information on bisphosphonates is presented at the ZENITH Meeting. In recent years, the meetings’ topics have centred around evidence of antitumour activity of bisphosphonates and their potential use in adjuvant therapy. Surprisingly, this year’s topic of the ZENITH Meeting was focused on bone disease in malignant myeloma, i.e. an area more likely debated by haematologists. In March 2012, 150 bisphosphonates experts from all over the world met in Munich, Germany, to discuss the current role of bisphosphonates in treating bone disease in malignant myeloma. Bisphosphonates (BP) are potent inhibitors of bone resorption used mainly in the treatment of metastatic bone disease and osteoporosis. By inhibiting bone resorption, they prevent complications as pathological fracture, pain, tumor-induced hypercalcemia. Even though patient’s benefit of BP therapy is huge, various side effects may develop. Bisphosphonate-related osteonecrosis of the jaws (BRONJ) is among the most serious ones. Oncologic patients receiving high doses of BP intravenously are at high risk of BRONJ development. BPs impair bone turnover leading to compromised bone healing which may result in the exposure of necrotic bone in the oral cavity frequently following tooth extraction or trauma of the oral mucosa. Frank bone exposure may be complicated by secondary infection leading to osteomyelitis development with various symptoms and radiological findings. In the management of BRONJ, conservative therapy aiming to reduce the symptoms plays the main role. In patients with extensive bone involvement resective surgery may lead to complete recovery, provided that the procedure is correctly indicated. Since the treatment of BRONJ is difficult, prevention is the main goal. Therefore in high risk patients dental preventive measures should be taken prior to bisphosphonate administration.
