Cíl studie: Sledování sérových hladin markerů karcinomu prostaty PSA, %freePSA, [-2]proPSA a výpočet PHI v časné dia - gnostice karcinomu prostaty. Typ studie: Pilotní studie Materiál a metody: V imunoanalytické laboratoři Fakultní nemocnice Plzeň (FN) byla vyšetřena séra 76 pacientů Urologické kliniky FN s podezřením na karcinom prostaty a indikovaných k biopsii prostaty pod ultrasonografickou kontrolou (TRUS). U všech pacientů byla stanovena hladina celkového PSA, a pokud byla mezi 0 – 30 μg/l, bylo doplněno stanovení hladin freePSA, [-2]proPSA, vypočítán poměr %freePSA a Prostate Health Index (PHI). Stanovení sledovaných biomarkerů se prováděla chemiluminiscenční metodou na přístroji DxI 800 (Beckman Coulter, USA). Pro veškeré statistické výpočty byl použit statistický software SAS verze 9.2. Výsledky: Nalezli jsme statisticky signifikantně zvýšené hladiny [-2]proPSA a PHI u pacientů s histologicky potvrzeným karcinomem prostaty oproti pacientům s benigní hyperplazií prostaty ([-2]proPSA medián 14 vs. 27 ng/l, PHI medián 35 vs. 77). Naopak u celkového PSA a %freePSA jsme statisticky významné rozdíly v hladinách nenalezli (mediány tPSA 7,1 vs. 7,7 μg/l a %freePSA 16 vs. 11,4 %). Závěr: Pro zpřesnění diferenciální diagnostiky benigní hyperplazie se zdá být stanovení [-2]proPSA a vypočtení indexu PHI velkým přínosem., Objective: Monitoring changes in the levels of biomarkers PSA, %fPSA, [-2]proPSA and calculation of PHI in the diagnostic algorithm of early prostate cancer. Design: Pilot study Material and Methods: The Immunoanalytical Laboratory of University Hospital in Pilsen examined sera of 76 patients from the Urology department of the University Hospital with suspected prostate cancer who have undergone TRUS biopsy. We assessed the levels of PSA and, if the interval of PSA was between 0-30 μg/l, we also assessed the levels of freePSA, [-2]proPSA and we calculated %fPSA and Prostate Health Index (PHI). The monitored biomarkers were measured using the chemiluminescent DxI 800 instrument (Beckman Coulter, USA). All statistical analyses were calculated using the SAS version 9.2 software. Results: We found statistically significant increased levels of [-2]proPSA and PHI in patients diagnosed with prostate cancer by prostate biopsy vs. patients with benign prostate hypertrophy ([-2]proPSA median 14 vs. 27 ng/l, PHI median 35 vs. 77). On the contrary, we did not find any significant difference in tPSA and %freePSA (median tPSA 7.1 vs. 7.7 μg/l and %freePSA 16 vs.11.4%). Úvod: Zhoršení výživy u seniorů, bývá popisováno ve spojitosti s institucionální péčí. Jako vhodný nástroj pro posouzení stavu výživy se jeví Škála pro hodnocení stavu výživy – MNA. Cíl výzkumu: Zjistit stav výživy u seniorů ve vybraných institucích prostřednictvím Škály pro hodnocení stavu výživy – MNA a následně analyzovat hodnoty Body mass index (BMI) u sledované populace. Metodika: Výzkumný soubor tvořilo 200 osob ve věku nad 60 let v institucionální péči. Statistické testy byly hodnoceny na hladině významnosti 5 %. Výsledky: Z 200 seniorů v institucionální péči dle Škály pro hodnocení stavu výživy (MNA) bylo ve stavu dobré výživy 32 %, rizikem podvýživy bylo ohroženo 38 % seniorů a podvýživou trpělo 30 %. Byl zjištěn statisticky významný rozdíl ve stavu výživy v závislosti na druhu instituce (p=0,001). Byl zjištěn statisticky významný rozdíl mezi sledovanými institucemi v hodnotách BMI (p=0,0084). Hodnoty BMI vyšší než 23 byly u 65 % seniorů, hodnoty BMI nižší než 19 byly u 16 % seniorů. Závěr: Z výsledku studie vyplývá, že pouze jedna třetina seniorů byla ve stavu dobré výživy, což vede k nutnosti včasné detekce stavu výživy prostřednictvím MNA., Introduction: Nutritional disorders in the elderly are described in relation to institutional care. Mini Nutritional Assessment (MNA) was used for assessing nutritional status. Objective: To assess the nutritional status of the elderly in selected institutions through Mini Nutritional Assessment (MNA) and to analyze the Body Mass Index (BMI) of the studied population. Methods: The research sample consisted of 200 people aged over 60 years in institutional care. Statistical tests were evaluated at a significance level of 5%. Results: Of the 200 seniors in institutional care according to MNA, 32% were in good nutrition condition, 38% of respondents were at risk of malnutrition and 30% of seniors suffered from malnutrition. A statistically significant difference in nutritional status was revealed depending on the type of institution (p=0.001) and a statistically significant difference between the observed values of institutions in terms of BMI (p=0.0084). Values of BMI in excess of 23 were identified in 65% of the elderly, values of BMI below 19 were found in 16% of elderly. Conclusion: The results show that only one third of the elderly were in good nutritional status. Účel studie: Retrospektivně jsme zpracovali výsledky léčby mozkových nádorů stereotaktickou radioterapií a radiochirurgií. Použité metody: Pacienti podstoupili plánovací CT a MR vyšetření. Poté bylo provedeno plánování cílového objemu a ozařovací techniky v BrainLab systému a pacienti byli ozářeni na X-noži Varian s vícelamelovým kolimátorem BrainLab. Radiochirurgie byla aplikována jednorázově s fixací ve stereotaktickém rámu. Pacienti, kteří podstoupili frakcionovanou stereotaktickou radioterapii, byli fixováni pomocí speciální stereotaktické masky. Statistické zpracování bylo provedeno ve spolupráci s Ústavem matematiky a statistiky PřF MU Brno. Výsledky: Celkem bylo ozářeno 101 pacientů s primárními mozkovými tumory. Nejčastěji se jednalo o meningeomy a high-grade gliomy. Medián sledování byl 22,4 měsíců. Pacienti, kteří podstoupili radiochirurgii, měli medián dávky 18 Gy. Pacienti, ozařovaní frakcionovaně, měli medián dávky 25 Gy. Toxicita léčby byla nízká. Stabilizace onemocnění byla zaznamenána u 68 % pacientů, parciální remisi dosáhlo 10 % a jeden pacient měl kompletní remisi. Závěr: Výsledky naší léčby jsou srovnatelné s publikovanými studiemi. Stereotaktické ozařovací metody mají své důležité místo v léčbě primárních tumorů mozku., We analysed retrospectively our treatment of primary brain tumors using stereotactic radiotherapy and radiosurgery. Methods: Patients underwent CT and MR examinations. Target volume and technique were planned in BrainLab system. Patients were irradiated on X knife. Radiosurgery was apllied with fixation using stereotactic frame. Stereotactic mask was used in cases of stereotactic radiotherapy. Statistical processing was performed at Department of mathematics and statistics, Faculty of Science, Masaryk university Brno. Results: 101 paients were included in our study. In the most of cases there were diagnosis of meningeoma and high-grade glioma. Median follow up was 22,4 months. Median of dose of radiosurgery was 18 Gy. Patients, whose underwent stereotactic radiotherapy, were median of doses 25 Gy. Treatment toxicity was low. Stable disease was recorded in 68 % of cases, partial remission achieved in 10 % patients and one patient had the complete remission. Conclusion: Results of our study are comparable with results other publicated studies. Obezita dětí je v oblasti veřejného zdraví vážným celosvětovým problémem. Výživa, fyzická (in)aktivita a sedavý životní styl, ke kterému patří sledování televize, práce či hraní her na počítači, jsou nezávislými rizikovými faktory nárůstu BMI (nárůstu hmotnosti). Cílem předkládané práce bylo posoudit asociace mezi stravovacími návyky, fyzickou aktivitou, životním stylem, nadváhou a obezitou u dětí ve věku 8–14 let v Královéhradeckém, Libereckém a Pardubickém kraji. Cílovou skupinou (n = 2042) byli žáci ze základních škol z 2., 4., 6. a 8. ročníku. Všechny sledované parametry (vynechávání jídla, celkový energetický příjem, preference potravin a nápojů...) byly hodnoceny formou četnosti (%) v celé skupině, dle pohlaví a dle tělesné hmotnosti. Ve všech sledovaných parametrech měly děti s nadváhou/obezitou vždy horší výsledky než děti s normální hmotností, i když ne vždy signifikantní. Největší rozdíly byly zjištěny při dodržování pitného režimu, kde děti s nadváhou/obezitou pijí především slazené nápoje., Childhood obesity is a serious public health concern worldwide. Dietary behaviour, physical activity and a sedentary lifestyle which includes television viewing, working or playing games on the computer are independent risk factors for increased BMI (increasing weight). The aim of this study was to explore dietary habits, physical activity and lifestyle associated with overweight and obesity among children 8–14 years of age, in Hradec Králové, Liberec and Pardubice district. The target population (N = 2042) comprised elementary school children in the 2nd, 4th, 6th and 8th grade. All monitored parameters (skipping meals, total energy intake, food and beverage preferences...) were evaluated by frequency (%) in the group, by gender and according to body weight. In all monitored parameters overweight/obese children have worse results, although not always significantly, than children with normal weight. Ačkoliv konvenční techniky zobrazení magnetickou rezonancí mají dominantní postavení při vyšetření centrálního nervového systému, přesto tyto techniky nedokážou dát informaci o funkčních vlastnostech mozkové tkáně. Kromě konvenčních MR technik však existují i MR metody umožňující některé z těchto vlastností zobrazit. Mezi ně můžeme zařadit funkční magnetickou rezonanci, difuzně vážené zobrazování, resp. zobrazení difuzního tenzoru a morfometrickou analýzu. Introduction: We studied influence of mud-bath on bone status in male Wistar rats with subchronic arthritis. Methods: Arthritis was induced by 2 subplantar injections of Freund’s adjuvans with heat-killed Streptoccocus pyogenes into paw. Groups: intact (int) on chippings; (con) arthritis on chippings; (san38) arthritis on hot sand; (mu38) arthritis on hot mud; (mu21) arthritis on mild mud. Bone mineral density (BMD, g/cm2) was measured by dual energy X-ray absorptiometry and femurs were tested biomechanically. Bone markers osteocalcin (OC), PINP and CTX were analysed in bone. Results: BMD of right femur decreased vs. left in san38 (p = 0.030) and mu38 (p = 0.047). Fracture load of right/left femur (N) decreased in experimental groups, significantly in san38 (p = 0.05). Fracture threshold of neck decreased in right vs. left in experimental groups, but significantly in san38 (p = 0.05). OC decreased in mu38 vs. con (1.84 ± 0.14/2.62 ± 0.23). PINP decreased in int vs. san38 (p = 0.005) and mu21 (p < 0.001). CTX decreased in int vs. mu38 (p = 0.006) and mu21 (p = 0.005). Conclusion: The hot bath appears indifferent in relation to osteoporosis, while cold mud-bath shows good effect on bone metabolism. Regulatory T cells (Treg) are a specialized subpopulation of T cells that act to suppress inadequate immune response. Psoriasis is recognized as a T -cell driven immune-mediated systemic inflammatory disease with skin manifestation. Effective therapeutical approach to treat psoriasis is Goeckerman therapy (GT ). The aim of this study was to compare the number of Treg in the peripheral blood of 27 psoriatic patients and 19 controls and to evaluate the influence of GT on Treg population in peripheral blood of patients with psoriasis. There was no significant difference in the relative number of Treg cells in the peripheral blood of healthy blood donors and patients with psoriasis before initiation of GT (P = 0.2668). In contrary, the relative number of Treg cells in peripheral blood of patients with psoriasis after GT was significantly higher than those found in healthy blood donors (P = 0.0019). Moreover, the relative number of Treg is significantly increased in psoriatic patients after Goeckerman therapy compared to the pre-treatment level (P = 0.0042). Cíl práce: Lipoperoxidace je dávána do příčinné souvislosti s rozvojem nejdůležitějších rizikových faktorů kardiovaskulárních onemocnění. Lipoperoxidační procesy jsou důsledkem reakcí lipidů s volnými radikály bez ohledu na jejich původ. Vztah mezi triacylglyceroly (TAG) a volnými radikály (VR) není zcela jasný. Může se dokonce stát, že hypertriacylglycerolémie je doprovázena nízkými koncentracemi VR a naopak. Jedno z možných vysvětlení spočívá ve vztahu lipoperoxidace a složení mastných kyselin v molekulách TAG. Cílem této studie bylo analyzovat vztah složení mastných kyselin v TAG a produkci VR u pacientů trpících diabetes mellitus 2. typu. Typ studie: observační s kontrolní skupinou Materiál a metody: Studie zahrnovala 18 normotriacylglycerolemických (NTG) jedinců (M/F 12/6) a 24 hypertriacylglycerolemických (HTG) jedinců (M/F 16/8). Pacienti v obou skupinách byli diabetici 2. typu. Analýzu mastných kyselin v triacylglycerolech plazmy jsme provedli kapilární plynovou chromatografií a koncentrace volných radikálů byly změřeny přímou spektrofotometrií. Statistické analýzy (parametrické i neparametrické) byly provedeny pomocí statistického software STATISTICA pro Windows. Výsledky: Ve skupině HTG jsme pozorovali nižší zastoupení kyseliny myristové (14:0) (1.62 [1.23-2.07] vs. 1.90 [1.72-2.79], p = 0.022, [medián (1.-3. kvartil]) a vyšší poměr alfa-linolenové kyseliny (18:3n-3) (1.51 [1.21-1.86] vs 1.16 [1.02-1.42], p = 0.021). Poměr kyselin arachidonové/eikosapentaenové byl ve skupině HTG nižší (5.54 [3.62-7.98] vs 7.25 [5.98-15.12], p = 0.027). Když jsme HTG skupinu dále rozdělili podle koncentrací VR na tercily, prokázali jsme nižší hodnoty poměru kyseliny arachidonová/eikosapentaenová ve druhém tercilu (4.05 [2.66-4.67] vs 7.64 [5.83-11.00], 1. tercil, a 6.21 [3.20-8.31], 3. tercil p = 0.019). V spojených HTG+NTG skupinách jsme zaznamenali negativní korelaci mezi MA a TAG (r = -0.3153, p = 0.045). V triaclylglycerolech jsme prokázali pozitivní korelace VR a kyseliny stearové (p < 0.001) spolu s negativní korelací některých n-6 vícenenasycených mastných kyselin (18:3n-6, p < 0.05, 22:5n-6, p < 0.01). Závěr: Zastoupení mastných kyselin v molekulách TAG je spojeno s tvorbou VR. Nicméně do tohoto vztahu vstupují i jiné faktory., Objective: The lipoperoxidation plays a causal role in the development of most important risk factors of cardiovascular disease. The lipoperoxidation results from the reaction of lipid molecules with free radicals, regardless of their origin. The relationship between triacylglycerols (TAG) and the free radicals (FR) amount is not clear. Thus, it can turn up that the hypertriacylglyceridaemia is accompanied with the low values of FR, or vice versa. One possible explanation of this phenomenon is that the degree of lipoperoxidation of TAG is dependent on the composition of fatty acids (FA) in their molecule. Therefore, the aim of this study was to analyze FA composition in TAG and its relationship to FR production in type 2 diabetes mellitus. Design: observational study with internal control group Material and Methods: 18 normotriacylglycerolemic (NTG) individuals (12M/6F) and 24 hypertriacylglycerolemic (HTG) patients (16M/8F). Patients in both groups were persons, suffering from type 2 diabetes mellitus. Fatty acid analysis of plasma triacylglycerols was performed by gas chromatography and free radicals were determined by direct spectrophotometric method. Statistical analyses (both parametric and non-parametric) were performed with the statistical software STATISTICA for Windows. Results: We found lower content of myristic acid (14:0) (1.62 [1.23-2.07] vs. 1.90 [1.72-2.79], p = 0.022, [median (1st-3rd quartile)]) and higher content of alpha-linolenic acid (18:3n-3) (1.51 [1.21-1.86] vs. 1.16 [1.02-1.42], p = 0.021) in HTG group. The ratio of arachidonic/eicosapentaenoic acid was in HTG group lower (5.54 [3.62-7.98] vs. 7.25 [5.98-15.12], p = 0.027). When the HTG group was stratified into the tertiles by the FR concentrations, we observed lower values in the second tertile for ratios based on arachidonic acid (20:4n-6) and eicosapentaenoic acid (20:5n-3) (4.05 [2.66-4.67] vs. 7.64 [5.83-11.00], 1st tertile, and 6.21 [3.20-8.31], 3rd tertile, p = 0.019). In pooled groups, we proved negative correlation between MA and TAG (r = -0.3153, p = 0.045) and the link between FR and FA composition of TAG expressed as positive relationship of FR with content of stearic acid (p < 0.001) and negative correlation with some n-6 polyunsaturated fatty acids (18:3n-6, p < 0.05, 22:5n-6, p < 0.01). Conclusion: The composition of FA in TAG molecules has undoubtedly some relationship to the FR generation. Kontinuální tvorba výpotků jakékoliv etiologie zásadním způsobem ovlivňuje kvalitu života nemocných. Akumulace tekutiny v pleurální dutině postupně komprimuje plicní parenchym. Prvním příznakem bývá progrese námahové dušnosti, později se přidává i klidová dušnost. S nárůstem výpotku dochází ke kompresi plicního parenchymu s rozvojem plicní atelektázy. Nemocný je ohrožen vznikem zánětového onemocnění plic v nevzdušném plicním parenchymu. U diagnózy maligního onemocnění je riziko zánětových komplikací výrazně vyšší vzhledem k předchozí chemoterapii či radioterapii, která zásadním způsobem snižuje imunitu. Opakovaná tvorba výpotku se výrazně podílí na rozvoji terminálního stavu nádorové kachexie. Efektivní metodou léčby je chemická pleurodéza, která se užívá u vybraného spektra nemocných. Principem je navození aseptické pleuritidy. Nejefektivnější cestou aplikace léčiva je videotorakoskopie. V souboru bylo 47 nemocných, 29 (61,7 %) mužů a 18 (38,3 %) žen, s cytologicky prokázaným recidivujícím maligním pleurálním výpotkem. Věkové rozmezí 42–80 let, Ø věk 65 let. Cílem práce bylo objektivizovat průběh celkových a lokálních zánětových změn po talkáži. Efekt léčby nebyl ovlivněn základním typem nádorového onemocnění. U obou skupin nebyly významně odlišné pooperační komplikace, což dokládá bezpečnost zvolené metody a podporuje správně zvolený algoritmus léčby., Continuous pleural effusion production of any aetiology can significantly affect the quality of patients life. Chronic effusion accumulation in the pleural cavity can lead to lung parenchyma compression. The first symptom of this entity is a progressive exertional dyspnea, which can later turn in dyspnea at rest. With the increase of the pleural effusion volume, pulmonary atelectasis can develop due to chronic parenchycha compression. The patient is at risk of inflamatory complications from the territory of non-ventilated parenchyma. Patients with a diagnosis of malignant disease have this risk considerably higher due to their previous chemotherapy or radiation, which substantially affect the immunity system. Repeated pleural effusion formation can significantly take part in the development of the terminal status of cancer cachexia. An effective method of palliative treatment is a chemical pleurodesis, which is used in selected patients. The principle is inducing aseptic inflammation. The most effective way of application is via videothoracoscopy. In a group of 47 patients, there were 29 (61.7 %) men and 18 (38.3 %) women with cytologically diagnosed recurrent malignant pleural. The age ranged between 42 and 80 with average age of 65 years. The aim was to assess the course of local and systemic inflammatory changes after talc application. The effect of treatment was not influenced by the type of malignancy . In both groups there was no significant difference in postoperative complications.
