To study defects and chromosomal abnormalities of the fetus, we retrospectively analyzed results of comprehensive dynamic survey of 26,404 pregnant women aged 1850 years old at 6 to 40 weeks of pregnancy. Of them, 25,956 (98.3%) women had physiological course of pregnancy, 448 (1.7%) women had abnormal pregnancy. For the diagnosis of fetal defects, we carried out ultrasound, biochemical, invasive and cytogenetic studies. The results of study showed that the majority of fetal defects and pathological course of pregnancy was noted in women at the age of 2125 years old, since at this age period women have the highest number of pregnancies. At the older age, we noted a gradual decrease in the number of pregnant women, as well as the number of abnormalities of the fetus. Based on the analysis of the results obtained, we have developed an algorithm for early fetal ultrasound examination. Introduction Cutaneous malignant melanoma prevalence, incidence and mortality rates are increasing in white populations worldwide more rapidly than any other cancer site (American Cancer Society, 2006). Despite the potential importance of regular skin selfexamination and promotion of selfprotection practices, little is known about the prevalence of these practices in medical students in Albanian population. Methods This is a descriptive, quantitative crosssectional study. In this study were included a sample of 150 individuals chosen among the students of Faculty of Medicine based on their family history for skin cancer. This study was started on October the 3rd and finished on November the 12th. Subjects had to fill in a structured, selfadministered questionnaire. All participants lived within Republic of Albania but at the time of the study were students in University of Tirana, Faculty of Medicine. Results In this study that we conducted we included 150 individuals supposed to have a risk for skin cancer based on their family history. 200 individuals were approached for participation. Of these, 150 individuals returned questionnaire data, yielding a response rate of 75% among eligible, successfully contacted participants. The mean age of the sample was 20.05 years (Std. Dev. = 0.925), with males (22%) and females (78%) represented unequally, because the gender configuration of the faculty itself has gender disparities with more than 80% females and only 20% male students. The majority of the sample was born in city or town (85.3%), 14.7% was born in a village. In terms of income level, 79.3% of participants had a medium income family background, 15.3% high income and 5.3% had a low income level family background. Conclusion It is important for those individuals with family history for skin cancer (which inherently indicates risk for skin cancer) to develop selfexamination and SSE behaviours and practices in order to have a protection and at least an early detection (if onset) of the different forms of Melanoma. Cieľ: Cieľom prieskumu bolo zistiť prevalenciu a rizikové faktory hypertenzie bieleho plášťa (WCH) u detí. Metodika: Formou retrospektívnej štúdie sme analyzovali zdravotné záznamy 140 detí (36 dievčat, 104 chlapcov) s vysokým krvným tlakom nameraným v kardiologickej ambulancii, u ktorých bolo vykonané 24-hodinové ambulantné monitorovanie krvného tlaku (ABPM) a určený typ hypertenzie. Hypertenzia bola diagnostikovaná, keď bol systolický a/alebo diastolický krvný tlak meraný auskultačnou technikou ≥ 95. percentil pre pohlavie, vek a výšku. WCH bola definovaná ako priemerná hodnota krvného tlaku v ambulancii nachádzajúca sa v percentilovom pásme hypertenzie a hodnota tlaku krvi počas 24-hodinového monitorovania ambulantného tlaku nachádzajúca sa v pásme normálnych hodnôt krvného tlaku. Výsledky: V skupine 140 detí bola potvrdená prítomnosť WCH u 51 detí (36,4 %) – 13 dievčat a 38 chlapcov. Z celkového súboru sa WCH vyskytovala u 36,1 % zaradených dievčat a 36,5 % zaradených chlapcov. Systolický TK sa v kardiologickej ambulancii nachádzal nad 95. percentilom, diastolický TK pod 90. percentilom. Väčšina pacientov s WCH bola vo veku 15 – 19 rokov (80,4 %) a v pásme od 3. do 90. percentilu BMI (72,5 %). U pacientov prevládala negatívna rodinná anamnéza hypertenzie (54,9 %) a zriedkavé návštevy kardiologickej ambulancie (2-4x). Krvný tlak v ambulancii merala deťom výlučne sestra. Záver: Zistenie prevalencie WCH a jej rizikových faktorov je u detí významné, pretože prevažuje názor, že v detskom veku je WCH predispozíciou k trvalej hypertenzii. Na základe zistení rizikových faktorov, môžeme vykonávať prevenciu WCH., Aim: The aim of this study was to find out the prevalence and risk factors of white coat hypertension (WCH) in children. Methods: We realized the retrospective study and analyzed records of 140 children (36 girls, 104 boys) with high blood pressure measured in cardiology outpatients' clinic, to whom a 24-hour ambulatory blood pressure monitoring (ABPM) was made and the type of hypertension was specified. Hypertension was diagnosed when systolic and/or diastolic blood pressure measurements with auscultatory technique were ≥ 95th percentile for sex, age and height. White coat hypertension (WCH) has been defined as mean blood pressure (BP) readings in outpatients' clinic being located in the hypertensive range while 24-hour ambulatory blood pressure monitoring is in the normal range. Results: In the group of 140 children the presence of WCH was confirmed in 51 children (36,4 %) - 13 girls and 38 boys. WCH occurred in 36,1 % of girls and 36,5 % of boys in the total sample of respondents. Systolic BP in cardiology outpatients' clinic reached the value above the 95th percentile, diastolic BP below the 90th percentile. The most patients with WCH were 15 – 19 years old (80,4 %) and in 3rd - 90th percentile of BMI (72,5 %). Patients prevailed negative family history of hypertension (54,9 %) and fewer visit (2-4 times) of cardiology outpatients' clinic. Blood pressure of children was measured exclusively by a nurse in this outpatients' clinic. Conclusion: Determination of the prevalence of WCH and its risk factors in children is important because of prevailing opinion that WCH in children is predisposition in permanent hypertension. Cieľ: Práca má design prierezovej, deskriptívnej štúdie, ktorej cieľom bolo zhodnotiť, aký je vzťah medzi psychickou záťažou, výskytom depresívnej symptomatológie a subjektívnou pohodou u sestier. Metodika: Vzorku respondentov tvorilo 78 sestier pracujúcich v NsP v Levoči a Spišskej Novej Vsi. Na meranie psychickej záťaže bol použitý Meisterov dotazník neuropsychickej záťaže, na meranie výskytu depresie skríningový nástroj Patient Health Questionnaire 9 (PHQ 9) a na meranie subjektívnej pohody Personal Well-Being Index (PWI) pre zdravú dospelú populáciu. Výsledky: Bol potvrdený významný vzťah medzi psychickou záťažou, výskytom depresívnej symptomatológie a subjektívnou pohodou u sestier. Vyššia frekvencia výskytu depresívnej symptomatológie sa u nich spájala s vyššou psychickou záťažou a nižšou subjektívnou pohodou. Významným a zároveň alarmujúcim zistením bolo, že len 28 % sestier uvádzalo nevýznamnú depresívnu symptomatológiu. Až u 50 % sestier bola podľa PHQ 9 prítomná ľahká depresia. Štatisticky významné korelácie boli identifikované medzi všetkými tromi faktormi Meisterovho dotazníka, ako aj jeho hrubým skóre a PHQ 9. Najsilnejšie korelácie boli medzi Faktorom I (preťaženie) a PHQ 9. Záver: Výsledky poukazujú na skutočnosť, ţe práve aspekty pracovného preťaţenia (časová tieseň, problémy a konflikty, vysoká zodpovednosť) môžu u sestier viesť ku častejšiemu prežívaniu jednotlivých symptómov depresie (predovšetkým k únave a zníženému záujmu). V ďalšom výskume navrhujeme sa zamerať na sledovanie vzťahu troch premenných: depresia, burnout a psychická záťaž sestier., Aim: The study has a design of descriptive cross-sectional survey. The aim of this study was to investigate relationship between psychological load, depressive symptomatology and subjective well-being of nurses. Methods: Sample of respondents consisted of 78 nurses from two public hospitals (Levoča and Spišská Nová Ves). To measure the psychological load was used the Meister questionnaire of neuro-psychological load. To measure the incidence of depression was used the screening tool Patient Health Questionnaire 9 (PHQ 9). To measure subjective well-being was used Personal Well-Being Index (PWI) for a healthy adult population. Results: The results indicated that there is a significant correlation between psychological load, depressive symptomatology and subjective well-being of nurses. The higher frequency of depressive symptomatology was associated with higher level of psychological load and lower subjective well-being level. The depressive symptomatology has a significant relation with neuropsychological strain and subjective well-being. Important and at the same time alarming finding was that only 28% of nurses reported insignificant depressive symptomatology. According to PHQ 9 was present mild depression in up to 50% of nurses. Statistically significant correlations were identified between all three factors of Meister questionnaire, as well as his gross score and the PHQ 9. The strongest correlations were between Factor I (overload) a PHQ 9. Conclusion: The results show that mainly aspects of work overload (time constraints, problems and conflicts, high accountability) can lead in nurses to more frequent survival of individual symptoms of depression (especially the fatigue and reduced interests). Po dva roky trvajících bolestech bederní páteře s narůstajícími klaudikacemi dolních končetin byl u 61leté ženy diagnostikován rozsáhlý tumor v oblasti těla L3 a retroperitonea. Nádor byl totálně odstraněn a provedena náhrada těla L3 a stabilizace. Po operaci přetrvávaly výrazné bolesti, oslabení dolních končetin a instabilita stoje a chůze. Po léčebné rehabilitaci došlo k vymizení bolesti, ústupu nestability i parézy levé dolní končetiny. Vývoj pooperačního stavu dokumentuje stabilometrické vyšetření., After 2-year period of persistent pain of lumbar spine with increasing intensity of lower extremities claudication a vast tumor of L3 vertebra extending to the retroperitoneum was diagnosed in a 61-year old woman. The total L3 vertebrectomy as well as complete tumor removal has been performed and situation was reconstructed and stabilized. Pronounced pain, lower extremities paresis and instability persisted after the surgery. The rehabilitation therapy led to the relief of the pain improvement of standing and gait instability and paresis. Latitude gradients and secular trends in Europe and North America have been found in the male-female ratio at birth (M/F: male births divided by total births) which is expected to be 0.515. Annual national data for Czechoslovakia and the post-Czechoslovakian (Czech Republic and Slovakia) countries for male and female live births were obtained from the World Health Organisation and analysed with contingency tables. This study analysed 13,123,538 live births. An overall decreasing trend in M/F was found (p < 00001). No latitude gradient was noted. There was an overall deficit of 15,232 male births based on an M/F of 0.515. M/F is declining in this region, despite well developing economies that have resisted the worldwide slowdown. Cíl: Práce se zaměřuje na zjištění vlivu sociální opory jako protektivního faktoru zdraví na míru deprese, úzkosti a životní spokojenosti u pacientů léčených pro lékařskou diagnózu F10.2 Závislost na alkoholu a F32 Depresivní epizoda na odděleních psychiatrie. Metody: Pro účely výzkumu byl zvolen kvantitativní přístup. Ke sběru dat byla použita dotazníková baterie. Baterii tvořily čtyři standardizované dotazníky: PSSS (Perceived Social Support Scale) J. A. Blumenthala, BDI-II (Beck Depression Inventory) Becka a kol., SAS (Self Rating Anxiety Scale) Zungova sebeposuzovací stupnice úzkosti a DŽS – Dotazník životní spokojenosti Fahrenberga et al. Výsledky: Z výsledků výzkumného šetření vyplynulo, že existuje statisticky významný rozdíl v míře sociální opory mezi skupinou kontrolní a zkoumanou (pacienti s lékařskou diagnózou deprese a závislost na alkoholu). Potvrdil se předpoklad o existenci signifi kantního negativního vztahu mezi mírou úzkosti a sociální opory u pacientů s lékařskou diagnózou závislost na alkoholu. Výsledek potvrzuje protektivní vliv sociální opory ve vztahu ke zdraví. Nebyla prokázána existence signifi kantního vztahu mezi mírou deprese a mírou sociální opory. Výsledky výzkumu rovněž potvrzují, že existuje statisticky významný rozdíl v míře životní spokojenosti mezi skupinou kontrolní a zkoumanou (osoby se závislostí na alkoholu). Na základě výsledků výzkumu lze konstatovat, že existuje signifi kantní pozitivní vztah mezí mírou sociální opory a mírou životní spokojenosti u osob se závislostí na alkoholu. Závislost na alkoholu vede ke snížení životní spokojenosti. Závěr: Výzkum prokázal význam sociální opory jako protektivního faktoru zdraví. Potvrdilo se, že se zvyšující se mírou sociální opory se snižuje míra úzkosti a naopak se zvyšuje míra celkové životní spokojenosti u osob se závislostí na alkoholu., Aim: The article focuses on the effect of social support as a health protective factor against depression, anxiety and life satisfaction in patients treated for the diagnosis F10.2 Alcohol dependence and F32 Depression on psychiatric wards. Methods: A quantitative approach was chosen for the research. To collect data, a questionnaire battery comprising four standardised questionnaires was used: PSSS (Perceived Social Support Scale) J. A. Blumenthal, BDI-II (Beck Depression Inventory) Beck et al., SAS (Zung’s Self Rating Anxiety Scale) and Questionnaire of Life Satisfaction by Fahrenberg et al. Results: The results of the research show that there is a statistically signifi cant diff erence in the degree of social support between the control and target group (patients with medical diagnosis of depression and alcohol dependence). It confi rmed the assumption about signifi cant negative relation between the degree of anxiety and social support in patients with medical diagnosis of alcohol dependence. The existence of a signifi cant relationship between the level of depression and level of social support was not confi rmed. The research results also confi rm the existence of a statistically signifi cant diff erence in the degree of life satisfaction between the control and target group (people with alcohol dependence). Based on the results, we can claim there is a signifi cant positive relation between the degree of social support and the degree of life satisfaction in people with alcohol dependence. Alcohol dependence leads to lower life satisfaction. Cíl: Stanovit, jak často je SPECT/CT potřebné ke specifikaci abnormálního nálezu na planárním WB kostním skenu, zda je užitečné porovnat scintigrafický nález s předcházejícím vyšetřením, v kolika případech SPECT/CT určí definitivní diagnózu a kolik ložisek abnormální aktivity nacházíme na kostním skenu u benigních a maligních afekcí. Pacienti a metoda: Retrospektivně jsme prostudovali 471 WB skenů u 450 pacientů s karcinomem prsu, plic a prostaty, kteří v roce 2008 podstoupili kostní scintigrafii. SPECT/CT nebo CT jsme použili v 11,9 % vyšetření u 12,4 % pacientů. Opakovaná scintigrafie (319 pacientů po kostní scintigrafii a 36 pacientů po SPECT/CT) sloužila jako referenční vyšetření. Výsledky: Srovnání s předchozí scintigrafií bylo užitečné u pacientů s karcinomem prsu před indikací SPECT/CT. S použitím SPECT/CT jsme mohli specifikovat nejisté scintigrafické léze u 90,9 % vyšetření. Pro detekci kostních metastáz dosáhlo CT ve SPECT/CT vyšetření senzitivitu 72,8 %, specificitu 96 %, pozitivní a negativní prediktivní hodnotu 88,9 %. V pravděpodobnosti rozvoje kostních metastáz do 2 let od vyšetření mezi pacienty s jedním suspektním ložiskem ve skeletu a pacienty bez suspektního ložiska jsme nezaznamenali signifikantní rozdíl (p = 0,4272), ovšem riziko vzrůstalo, jestliže jsme nalezli suspektních ložisek více (p ≤ 0,0001). Závěr: SPECT/CT zvyšuje specificitu kostní scintigrafie. Po porovnání scintigrafického nálezu s předchozím vyšetřením se zdá rozumné uvažovat o použití SPECT/CT v 10-20 % vyšetření. Cieľ: V našej práci sa zameriavame na posúdenie vybraných atribútov spirituality, ako sú ţivotná zmysluplnosť, nádej, spirituálne prejavy a spirituálna pohoda u pacientov so závislosťou od alkoholu, depresívnou poruchou a pacientov so schizofréniou. Metodika: Súbor tvorilo 309 pacientov s diagnostikovanou a liečenou psychickou poruchou. Priemerný vek v súbore bol 40,4 ± 12,5 roka. Z hľadiska diagnóz sa jednalo o 107 pacientov so závislosťou od alkoholu, 109 pacientov s depresívnou poruchou a 93 pacientov so schizofréniou. Na zber údajov boli pouţité Snyderova škála nádeje; Nowotnej škála nádeje; Škála ţivotnej zmysluplnosti; Praţský dotazník spirituality a Škála spirituálnej pohody. Výsledky: Zistili sme výrazné odlišnosti v miere naplnenia spirituálnych premenných v závislosti od psychiatrickej diagnózy. Pacienti so schizofréniou dosahovali signifikantne vyššie priemerné hodnoty ako pacienti so závislosťou od alkoholu a s depresiou a to pri všetkých spirituálnych premenných. Pacienti s depresiou mali najniţšiu mieru nádeje meranej Nowotnej škálou a najniţšiu mieru ţivotnej zmysluplnosti. Pacienti so závislosťou mali najniţšiu mieru nádeje meranej Snyderovou škálou a najmenej spirituálnych prejavov. Spirituálnu pohodu mali pacienti s depresiou a so závislosťou na rovnakej úrovni. Záver: Na základe našich zistení moţno konštatovať, ţe spirituálne premenné ako nádej, zmysluplnosť, vyjadrovanie spirituality v beţnom ţivote, spirituálna pohoda nie sú u psychiatrických pacientov napĺňané v dostatočnej miere. Dôleţitým zistením je aj to, ţe z hľadiska spirituality nepredstavujú psychiatrickí pacienti homogénny súbor, ale vyskytujú sa medzi nimi výrazné rozdiely., Aim: In our work we focus on the assessment of selected attributes of spirituality, such as the meaningfulness of life, hope, spiritual expressions and spiritual well-being in patients with alcohol addiction, depressive disorder and schizophrenia. Methods: The sample consisted of 309 patients with diagnosed and treated psychiatric disorder. The average age in the sample was 40.4 ± 12.5 years. There were 107 patients with alcohol dependence, 109 patients with depressive disorder and 93 patients with schizophrenia. For the data collection was used Snyder Hope Scale; Nowotny Hope Scale; Life Meaningfulness Scale; Prague Spirituality Questionnaire and Spiritual well-being Scale. Results: We found significant differences in the extent of fulfilling spiritual variables depending on the psychiatric diagnosis. Patients with schizophrenia achieved significantly higher mean values than patients with alcohol dependence and depression in all spiritual variables. Patients with depression had the lowest level of hope measured by Nowotny Scale and the lowest rate of life meaningfulness. Patients with addiction had the lowest rate of hope measured by Snyder Scale and the least of spiritual expressions. Spiritual well-being was on the same level in patients with depression and alcohol addiction. Conclusion: Based on our findings we can state that the spiritual variables as hope, meaningfulness, the expression of spirituality in everyday life, spiritual well-being are not in psychiatric patients filled up adequately. Ciele: Cieľom práce bolo posúdiť spirituálnu tieseň u pacientov s depresívnou poruchou a identifikovať vzťah medzi takouto tiesňou a psychickým distresom. Metodika: Spirituálna tieseň bola posúdená Škálou životnej zmysluplnosti a subškálou existenciálna pohoda zo Škály spirituálnej pohody. Psychický distres bol posúdený dotazníkom SCL 90. Výsledky: Zistili sme, že pacienti s depresiou sú rizikoví z hľadiska prežívania spirituálnej tiesne. Zistili sme početné negatívne signifikantné vzťahy medzi znakmi spirituálnej tiesne a dimenziami psychického distresu. Vyššia miera spirituálnej tiesne bola spojená s vyšším psychickým distresom. Záver: Napriek absencii v ošetrovateľských plánoch je spirituálna tieseň závažný problém. Mimo iného prispieva k zintenzívneniu psychických problémov u pacientov s depresiou. Nedostatočná diagnostika a riešenie tejto diagnózy znižuje kvalitu poskytovanej ošetrovateľskej starostlivosti., Aim: The aim of work was to assess spiritual distress in patients with depressive disorder and to identify the relationship between such distress and psychological distress. Methods: To assess spiritual distress, we used Halama′s Life Meaningfulness Scale and as a sub-scale Existential well-being from Spiritual Well-being Scale. Psychological distress was assessed by SCL 90 questionnaire. Results: We found that patients with depression are at risk of experiencing spiritual distress. We found numerous significant negative relationships between variables of spiritual distress and dimensions of psychological distress. Higher level of spiritual distress was associated with higher psychological distress. Conclusion: Despite the absence of diagnosis Spiritual distress in a nursing care plan, it still remains a serious problem, which can lead to increased mental health problems in patients with depression.
