Introduction: Ankylosing spondylitis (AS) is an inflammatory rheumatic disease characterized by the development of osteoproductive changes in the spine which could possibly result in ankylosis. Treatment with tumour necrosis factor alpha (TNFα) inhibitors has proved to be an important step forward in the treatment of this disease, but for the time being it is not clear whether it favourably influences radiographic progression of the disease. Vascular endothelial growth factor most probably plays a role in the development of osteoproductive changes and recently its predictive influence on radiographic progression has been demonstrated. Bone morphogenic protein 2 (BMP-2) participates in the regulation of bone proliferation and its increased serum level has been demonstrated in patients with advanced AS and correlated with the degree of radiographic changes. Aim: The study aims to evaluate the VEGF and BMP-2 levels in patients with ankylosing spondylitis and how these levels relate to the concurrent treatment with TNFα inhibitors. Methods: Sera were evaluated from patients at the Rheumatologic Clinic of the Hradec Králové Faculty Hospital who fulfilled the modified New York Criteria for AS (n = 55). In these patients, the parameters of the activity of the disease (BASDAI = Bath Ankylosing Spondylitis Disease Activity Index, CRP = C-reactive protein) and the concurrent therapy (TNFα inhibitors, n = 21, vs. non-anti TNFα, n = 34) were recorded. The levels of VEGF and BMP-2 were analyzed using the ELISA method. Results: In patients treated with TNFα inhibitors, a significantly lower VEGF level was found when compared to untreated patients (140.3 (109.4; 262.2) vs. 261 (172.4; 396.6) pg/ml; p = 0.02). No difference was found between BMP-2 levels in both groups (treated vs. untreated patients) (254.8 (2301; 267.3) vs. 261.1 (248.6; 273.5) pg/ml; p = 0.24). A correlation analysis did not reveal any relationship between VEG F and BMP-2 (r = 0.057; p = 0.68). Serum levels of VEGF correlated with serum levels of CRP (r = 0.56; p = 0.00001) and the BASDAI value (r = 0.33; p = 0.015). Conclusion: Significantly lower VEGF levels were found in patients treated with TNFα inhibitors versus the untreated patients. These findings are in harmony with some hitherto published analyses and may give evidence of a favourable effect of TNFα inhibitors on radiographic progression. BACKGROUND: The current treatment of hereditary hemochromatosis (HH) consists of performing periodic whole blood phlebotomies. Erythrocytapheresis (EA) can remove up to three times more red blood cells per single procedure and could thus have a clinical benefit. A prospective study of 30 consecutive cases of HH were included in a periodic EA program. METHODS AND PATIENTS: EA were performed using a discontinuous flow cell separators. The protocol consisted of a bimonthly EA until normalization of the serum ferritin was reached. The aim was to reduce the total erythrocyte volume by 25-35%, eventually, to adjust the amount so that hematocrit would not drop below 0.25. RESULTS: 530 +/- 101 ml of erythrocytes were removed (median 517, range 116-761 ml). Iron depletion (ferritin < 20 microg/l) was achieved in all patients after a mean 6.9 +/- 7.6 months, median 5 months, range 1-36 months and a mean 14 EA sessions. The procedures were well tolerated and there were no severe side-effects. CONCLUSIONS: We conclude that HH patients treated with EA achieved iron depletion quickly under good conditions of tolerance. Úvod: Efekt kyanoakrylátových lepidel byl opakovaně hodnocen u resekcí jater, plic a dalších orgánů, ale minimálně u resekcí ledvin. Cílem bylo zhodnocení užití kyanoakrylátového tkáňového lepidla u otevřené a laparoskopické resekce ledviny. Metoda: Byl hodnocen soubor 32 pacientů, kteří podstoupili resekci ledvin s ošetřením resekované plochy kyanoakrylátovým lepidlem. Dvacet tři pacientů prodělalo laparoskopickou resekci a 9 pacientů otevřenou. Byly hodnoceny operační výsledky, komplikace a onkologické výsledky. Výsledky: Medián doby sledování byl 46 měsíců. Průměrná doba byla u otevřené resekce 104 minut a u laparoskopické 157 minut, krevní ztráty byly 250 a 184 ml. Nebyla zaznamenána žádná závažná operační komplikace. Ve sledovaném souboru nebylo zaznamenáno úmrtí ani recidiva nádoru v tříletém pooperačním sledovaném období. Závěr: Hemostáza kyanoakrylátovým lepidlem Glubran II se jevila jako účinná a bezpečná u resekce ledvin. Na druhou stranu zvyšuje náklady na operaci a není nezbytná. Klíčová slova: parciální nefrektomie − laparoskopická resekce ledvin − kyanoakrylátové tkáňové lepidlo, Introduction: The effect of cyanoacrylate glues was repeatedly evaluated in resections of liver, lungs and others organs, but minimally in kidney resection. The aim of our study was to evaluate the use of cyanoacrylate tissue glue in open and laparoscopic partial nephrectomy. Method: We evaluated a cohort of 32 patients who underwent partial nephrectomy with treatment of the resected area using cyanoacrylate glue. Laparoscopic resection was done in 23 patients, while 9 patients underwent an open procedure. Surgical results, complications and oncologic results were assessed. Results: The median follow-up duration was 46 months. The mean time of open partial nephrectomy was 104 minutes and that of laparoscopic resection was 154 minutes; blood losses reached 250 ml and 184 ml, respectively. No serious complications occurred, and all patients survived the 3-year follow-up without any tumor recurrence. Conclusion: Hemostasis using the cyanoacrylate glue Glubran II was effective and safe. Autoři referují o vzácném případu traumatické totální avulze očního bulbu s optickým nervem v úrovni chiazmatu. Kazuistické sdělení retrospektivně popisuje průběh ročního sledování mladé ženy, která byla zraněna při hasičských závodech. Avulze bulbu a optiku, kombinovaná s poraněním chiazmatu, rezultovala v hemianopický defekt druhostranného zorného pole. Při traumatu vzniklo subarachnoidální krvácení do oblasti cisteren chiazmatu, jež spontánně ustoupilo. Nedošlo k rozvoji vaskulárních spazmů mozku. Celková kortikoterapie a neuroprotekce neměla vliv na průběh onemocnění. Během sledovací doby objektivně došlo k descendentní atrofii korespondujících axonů a retinálních gangliových buněk. V současné době neexistuje efektivní terapie poranění v oblasti chiazmatu., The authors report a rare case of complete traumatic avulsion of the eyeball with the optic nerve at the level of chiasm. The case report retrospectively describes a year-long monitoring of a young woman who was injured in fire-fighter races. Avulsion of the globe and optic nerve combined with injured chiasm resulted in contralateral hemianopic visual field defect. The trauma led to subarachnoid hemorrhage in cisterns of chiasm, which spontaneously resolved. There was no development of vascular spasms in the brain. Systemic corticosteroid therapy and neuroprotection did not affect the course of the disease. During the observation period, descending atrophy of the corresponding axons and retinal ganglion cells developed. There is currently no effective therapy of chiasm injuries., and L. Hejsek, V. Úvod: Cílem tohoto článku je prezentovat naše zkušenosti s transanální endoskopickou mikrochirurgií (TEM) jako jedním ze základních operačních přístupů k selektovaným pacientům s nádory konečníku. Metody: Retrospektivně jsme vyhodnocovali soubor 80 pacientů, u kterých byla pro nádor konečníku provedena operační léčba pomocí TEM. U těchto pacientů jsme sledovali a statisticky vyhodnocovali konkrétní předoperační a pooperační ukazatele, které bychom chtěli prezentovat: předoperační bioptické vyšetření a pooperační výsledek histologického vyšetření, velikost nádoru a jeho uložení v konečníku, hloubku resekce a hodnocení základních demografických ukazatelů. Výsledky: Z našich zkušeností u zmíněných pacientů vyplývá, že není významný rozdíl mezi věkem operovaných mužů a žen ani při rozlišení na benigní a maligní nádory. Nejčastěji operovanými pacienty byli muži, průměrně ve věku 64 let, kteří byli primárně vyšetřeni pro krvácení z konečníku. Nejčastějším nálezem byl low-grade adenom, který byl potvrzen i v definitivním resekátu. Celkem u 17 pacientů byl nalezen v definitivním histologickém vyšetření maligní tumor, který nebyl zachycen předoperačně. Závěr: V rukách zkušeného chirurga představuje TEM reálnou možnost provedení radikální resekce i maligního, lokálně nepokročilého nádoru konečníku, pokud je v dosahu operačního rektoskopu. TEM je metodou bezpečnou, bez negativního vlivu na funkce análních svěračů. Vzhledem k možnosti falešně negativního předoperačního histologického vyšetření preferujeme radikální odstranění nádoru s celou šířkou stěny konečníku. I když optimální lokalizace pro použití operačního rektoskopu je na zadní straně konečníku, téměř u poloviny našich pacientů se tyto nádory vyskytovaly na stěně přední. Vzhledem k technické proveditelnosti sutury stěny rekta je vhodné, aby báze tumoru nepřesahovala polovinu obvodu konečníku. Technika TEM je vhodná zejména pro nízce uložené, nestenotizující, lokálně nepokročilé nádory konečníku. Naše dobré výsledky jsou dány především přísnou selekcí pacientů, technickým zvládnutím operačního postupu a letitými zkušenostmi., Introduction: The purpose of this paper is to present our experience with transanal endoscopic microsurgery (TEM) as an operative approach in a selected group of patients with rectal tumours. Methods: We present a retrospective study of 80 patients operated for rectal tumour using TEM. Specific preoperative and postoperative indicators were gathered and statistically evaluated. We assessed histological examination before the surgery and definitive histology of the resected tumour, resection depth and basic demographic data of our patients. Results: Our experience suggests that there is no significant difference between the age of patients according to sex, even if malignant and benign tumours are assessed separately. The majority of our operated patients were male patients, mean age of 64 years who were primarily referred to our department because of bleeding low-grade adenomas of the rectum. In 17 patients a malignant tumour was diagnosed based on final histopathologic examination. None of them had signs of malignancy before the surgery. Conclusion: In the hands of an experienced surgeon, TEM is a feasible option of radical removal of rectal, locally not advanced malignancies if within reach of TEM rectoscope. TEM is a safe procedure that does not result in any alteration of rectal sphincter functions. Possible false negative preoperative benign diagnosis leads us to approach radical removal of the tumour with full wall thickness resection. Although the posterior aspect of the rectum is an optimal location for the rectoscope, anterior tumours were found in almost one half of our patients. Considering technical feasibility of the rectal wall suture, the base of the tumour should not extend beyond one half of the rectal circumference. Low non-stenotising noninvasive tumours of the rectum are suitable for TEM. Transplantace ledviny od žijícího dárce inkompatibilního v krevní skupině představuje nový nástroj, jak zvýšit dostupnost transplantací. Většina evropských protokolů je založena na desenzitizačním protokolu s rituximabem, trojkombinace imunosupresiv, intravenózních imunoglobulinů a specifické imunoadsorpce, při které jsou z plazmy odstraňovány konkrétní hemaglutininy. Program transplantací ledvin od inkompatibilních dárců v krevní skupině byl v našem centru založen v roce 2011. Dosud bylo provedeno 21 těchto transplantací. Nejvyšší titr hemaglutininů před rituximabem byl 1 : 64 a korespondující počty imunoadsorpcí před transplantací od 2 do 9 výkonů. Po transplantaci byly imunoadsorpce prováděny podle aktuálního titru hemaglutininů 1–2krát u 5 nemocných. V posledních 2 letech jsou tyto transplantace nabízeny inkompatibilním příjemcům, kteří nenaleznou uplatnění v systému párových výměn ledvin anebo se účastní párové výměny s cílem dosáhnout vyšší míry shody v HLA antigenech. Hlavní komplikací bylo častější pooperační krvácení a infekce močových cest u rizikových nemocných. Ve většině protokolárních biopsií byla popsána pozitivita C4d barvení. Nebyla pozorována ztráta štěpu ani úmrtí nemocných., Living donor AB0 incompatible kidney transplantation represents a new tool how to improve the access to transplantation. Majority of European protocols are based on desensitization with rituximab, triple drug immunosuppression, intravenous immunoglobulins and specific immunoadsoption (IA) which eliminates isohaemaglutinins. AB0i kidney transplant program was initiated in our centre in 2011 and 21 patients have received grafts from incompatible donors until recently. Highest accepted isohaemaglutinins titers before rituximab were 1 : 64 and corresponding pretransplant immunoadsorption procedures varied from 2 to 9. In 5 patients 1–2 IA procedures were performed also after transplantation. With the advent of paired exchange program the AB0i transplantation is offered to patients with unsuccessful matching run or with aim to improve HLA match between donor and recipient. The main complications were postoperative bleeding and urinary tract infections in patients at risk. Majority of protocol biopsies exhibited positivity of C4d staining. The pilot study deals with the clinical results of the regenerative therapeutical method in intraosseous periodontal defects done by surgical augmentation procedure using a material of a new generation prepared on the basis of a bioactive glass. A group of 10 individuals were treated properly for chronic periodontitis. Some of treated infraalveolar periodontal lesions, both periodontal pockets and interradicular defects, persisted or recurred in posterior teeth. In order to eliminate them they were indicated for advanced periodontal surgery or regenerative surgical therapy stimulating healing processes of supportive tissues, mostly alveolar bone around treated teeth. Relevant clinical parameters, i. e. values of the pocket depth, gingival recession, and loss of attachment were evaluated before and after the periodontal surgery and compared with each other. Epilepsia je chronické ochorenie, ktoré je často sprevádzané psychiatrickou komorbiditou. Najčastejšou z nich je depresia, ktorá je sprevádzaná subsyndromálnymi, atypickými a chronifikovanými príznakmi depresie. Incidencia depresie je u pacientov s epilepsiou vyššia ako u iných neurologických alebo chronických ochorení. Psychopatológia spojená s epilepsiou obťažuje pacienta častokrát viac ako samotné ochorenie. Cieľom práce je prezentácia a analýza kazuistiky 37-ročného muža s diagnózou epilepsie so sprievodnou psychiatrickou symptomatikou, ktorá znížila kvalitu jeho života oveľa viac ako samotné neurologické ochorenie. V kazuistike popisujeme klinické symptómy neuropsychiatrického ochorenia – epilepsie ako aj životné udalosti, ktoré sú u každého pacienta jedinečné a majú závažný vplyv na priebeh a liečbu epilepsie., Epilepsy is a chronic disease that is often accompanied by psychiatric comorbidity. The most frequent of them is depression accompanied by subsyndromic, chronic and atypical symptoms of depression. The incidence of depression in patients with epilepsy is higher than in other neurological or chronic diseases. Psychopathology associated with epilepsy often bothers the patient more than the disease itself. The aim of this work is to present and analyze a case report of a 37‑year‑old man diagnosed with epilepsy and associated psychiatric symptomatology that reduced his quality of life much more than the neurological disease itself. In this case report, we describe clinical symptoms of neuropsychiatric disease – epilepsy as well as life events that are unique to each patient and have a significant impact on the course and treatment of epilepsy. Úvod: Důležitou součástí diagnostického algoritmu neuropatické bolesti jsou jednoduché dotazníkové nástroje založené na přítomnosti typických popisných charakteristik (tzv. deskriptorů). V českém jazykovém prostředí však není k dispozici validovaná verze žádného z mezinárodně akceptovaných dotazníků. Cíl: Cílem práce bylo vytvoření české jazykové verze jednoho z nejpoužívanějších dotazníků zaměřených na diagnostiku neuropatické bolesti a charakteristiku jednotlivých jejích subtypů, a to tzv. NPSI (Neuropathic Pain Symptom Inventory) a její validace. Soubory a metodika: Dotazník byl přeložen metodou „forward‑backward translation“. Pro potřeby validace byla touto českou jazykovou verzí následně vyšetřena skupina pacientů s neuropatickou bolestí periferního (n = 66) nebo centrálního (n = 30) původu (v rámci polyneuropatie či roztroušené sklerózy) a skupina nemocných s bolestí nociceptivní v důsledku pokročilé artrózy kyčelního nebo kolenního kloubu (n = 70). Za účelem hodnocení opakovatelnosti testu byl u části pacientů (n = 19) dotazník administrován dvakrát s odstupem přibližně tří hod. Výsledky: Srovnání jednotlivých hodnocených položek, dílčích škál i celkového NPSI skóre pomocí ROC analýzy prokázalo vysokou schopnost diskriminace mezi neuropatickou a nociceptivní bolestí (p < 0,001), ale nízkou schopnost odlišení neuropatické bolesti centrálního a periferního původu (p > 0,05). Opakovaná administrace testu potvrdila vysokou opakovatelnost testu (ICC, Intraclass Correlation Coefficient = 0,86 pro sumární skóre). Pomocí clusterové analýzy bylo identifikováno šest odlišných symptomových profilů. Závěr: Vytvořená česká verze dotazníku NPSI je schopna spolehlivě diskriminovat neuropatickou a nociceptivní bolest a identifikovat odlišné symptomové profily bolesti při akceptovatelné opakovatelnosti testu. Dotazník lze tedy doporučit jako vhodný a snadno použitelný nástroj v diagnostickém algoritmu neuropatické bolesti, využitelný i pro longitudinální sledování., Introduction: Simple questionnaires based on so called “neuropathic pain descriptors” are the most frequently used screening tools in the diagnosis of neuropathic pain, especially by non‑specialists. However, no validated Czech version of any of the internationally accepted nueropathic pain questionnaires is available. Aim: The aim of this study was to develop and validate a Czech version of the Neuropathic Pain Symptom Inventory (NPSI), a questionnaire that allows the user to characterize subtypes of neuropathic pain, is sensitive to change, and has discriminatory power. Patients and methods: The NPSI was translated into the NPSIcz using the forward‑backward translation method and validated in a group of patients suffering from neuropathic pain of peripheral (n = 66) or central origin (n = 30) due to polyneuropathy or multiple sclerosis, and in a group of individuals suffering from nociceptive pain (due to severe osteoarthritis of the knee or hip joint; n = 70). The NPSIcz was repeated within three hours in 19 individuals to determine test‑retest reliability. Results: The NPSIcz showed good diagnostic validity in discriminating between patients with neuropathic and nociceptive pain (p < 0.001) but low ability to discriminate between patients with central and peripheral neuropathic pain (p > 0.05). Repeated administration confirmed acceptable test‑retest reliability (intra‑class correlation coefficient of 0.86 for the NPSIcz sum score). Cluster analysis identified six different pain profiles. Conclusions: The NPSIcz is able to discriminate between neuropathic and nociceptive pain, to identify different pain profiles, and has acceptable test‑retest reliability. It can, therefore, be recommended as an easy‑to‑administer questionnaire suitable for the diagnosis of neuropathic pain, including long‑term follow up. Cíle: Tato studie měla za cíl zjištění diagnostické přesnosti české verze souboru tří otázek ke stanovení diagnózy syndromu neklidných nohou (Restless Legs Syndrome, RLS), které se používají v epidemiologických studiích prováděných dotazníkovou cestou bez asistence investigátora. Originální anglická verze dotazníku byla publikována v roce 2003 Mezinárodní skupinou pro výzkum RLS (International RLS study group) společně s hlavními a vedlejšími diagnostickými kritérii pro klinické nebo telefonické stanovení diagnózy RLS. Soubor a metody: Sada otázek byla přeložena do češtiny a pak zaslepeně zpět do angličtiny, aby nedošlo k posunu významu. Ženy přicházející ke kontrole na Gynekologickou kliniku 1. LF UK a VFN v Praze v 34.–38. týdnu těhotenství byly požádány o vyplnění výše zmíněného dotazníku. Následně lékař vycvičený v diagnostice RLS pacientky kontaktoval na jimi zadaném telefonním čísle k standardnímu diagnostickému rozhovoru k ověření či vyloučení diagnózy RLS. Výsledky obou metod byly porovnávány a byla stanovena senzitivita, specificita, pozitivní a negativní prediktivní hodnoty. Výsledky: Dotazníkovou studii podstoupilo celkem 776 těhotných žen, ve věku 18–49 let. Celkem bylo validováno 199 dotazníků, u 65 osobní pohovor potvrdil diagnózu RLS. Pozitivita všech tří otázek má 89% specificitu, ale jen přibližně 78% pozitivní prediktivní hodnotu a senzitivitu. Za použití pouze první otázky je senzitivita 94 % a negativní prediktivní hodnota 96 %. Ze všech vyšetřených žen na všechny tři otázky odpovědělo pozitivně 28,0 % (95% konfidenční interval je 24,9–31,2 %). Závěr: Studie validovala českou verzi otázek a prokázala jejich relativně vysokou diagnostickou přesnost., Aims: The aim of this study was to estimate diagnostic accuracy of the Czech version of a self-administered minimum question set to diagnose the Restless Legs Syndrome (RLS). The wording of the three questions was published in 2003 by the International RLS study group together with the diagnostic criteria. Material and methods: The question set was translated into Czech, followed by blind translation back to English. Women in 36th–38th week of pregnancy were asked to complete the questionnaire. Subsequently, the women were contacted over a telephone by a physician trained in diagnosing RLS and a standard telephone interview to establish or exclude the diagnosis of RLS was performed. The results of both methods were compared to estimate sensitivity, specificity, positive and negative predictive values of the minimum question set. Results: We surveyed 776 pregnant women (18–49 years old) who came to a prenatal outpatient clinic to consult an obstetrician at the third trimester (36th–38th week of pregnancy). In total, 199 questionnaires were validated; personal interview confirmed the diagnosis of RLS in 65 of them. Positive response to all three questions yields specificity of 89% but only about 78% sensitivity and positive predictive value. Positive response to the first question only has sensitivity of 94% and negative predictive value of 96%. The prevalence of RLS during pregnancy estimated using the three-question questionnaire was 28% (95% confidence interval from 24.9 to 31.2%).
