Předmětem této práce je objektivně posoudit dostupná výzkumná data týkající se bezpečnosti biologické terapie v těhotenství a kojení pacientek s idiopatickými střevními záněty. Biologická léčba se zdá být v graviditě bezpečná bez zvýšeného rizika vrozených malformací. Dostupné klinické údaje naznačují, že účinnost infliximabu a adalimumabu v dosažení klinické odpovědi a udržení remise těhotných pacientek převažuje teoretické riziko ohrožení plodu těmito léky. Terapie anti-TNFα preparáty by měla být, pokud je to možné, ukončena na konci 2. trimestru pro transplacentární přestup a z toho vyplývající riziko pro plod. Užití infliximabu a adalimumabu je pravděpodobně slučitelné s kojením. Biological therapies appear to be safe in pregnancy, as no increased risk of malformations has been demonstrated. Available clinical results suggest that the efficacy of infliximab and adalimumab in achieving clinical response and maintaining remission in pregnant patients might outweigh the theoretical risks of drug exposure to the fetus. If possible, anti-TNF therapy should be stopped by the end of the second trimester due to transplacental transfer and potential risk for the fetus. The use of infliximab and adalimumab is probably compatible with breastfeeding. Denosumab is a human monoclonal antibody representing a novel therapy of osteoporosis. Contrary to always other antiosteoporotic drugs, it is not contraindicated in advanced chronic kidney disease, as its pharmacokinetic does not differ from patients with normal kidney function. However, published case reports in chronic kidney disease (CKD) patients stopped the therapy after single dose because of hypocalcemia. V současné době je věnována familiární hypercholesterolemii (FH) po letech opět zvýšená pozornost. Je tomu tak především proto, že se toto geneticky podmíněné onemocnění stává cílem velmi účinných nových hypolipidemik, PCSK-9 inhibitorů, lomitapidu a mipomersenu. Předložené sdělení se zabývá popisem 2 souborů nemocných (vždy ve své době z největších na světě), před 50 lety a nyní. I když přímé statistické srovnání není možné, lze přeci jen najít v klinickém obrazu FH jisté vývojové změny. Faktem však je, že základní charakteristika FH se zásadně nezměnila. V biochemickém obrazu dominuje těžká izolovaná hypercholesterolemie s hodnotami celkového cholesterolu 9–10 mmol/l, s hodnotami LDL-cholesterolu 7–8 mmol/l a s normálními hodnotami triglyceridů. Je zajímavé, že právě triglyceridy se při srovnání s dobou před 50 lety významně zvyšují a dosahují hraničních hodnot. Hladiny HDL-cholesterolu jsou normální. Manifestace ICHS ve věku od 40 let u mužů a od 50 let u žen není výjimkou (ojediněle se vyskytují i případy infarktu myokardu již ve 3. dekádě života). Klasickou klinickou manifestací FH je xantomatóza. Pravděpodobně v důsledku časné detekce nemocných s FH a účinné agresivní léčby se se šlachovými xantomy, s xanthelasma palpebrarum a arcus lipoides setkáváme méně často než před desítkami let. Obezita, diabetes mellitus (DM) ani hypertenze nepatří mezi typické klinické příznaky FH., Currently, the familial hypercholesterolemia (FH) rises the interest. The reason is that this genetic disorder is targeted by newly emerged and highly effective hypolipidemic agents, PCSK-9 inhibitors, lomitapid and mipomersen. Present paper discusses 2 patient study groups, before 50 years and nowadays. Although direct statistical analysis is impossible some changes in clinical features of FH might be found over the course of the time. In fact, the basic FH characteristic has not changed dramatically. Severe isolated hypercholesterolemia with total cholesterol 9–10 mmol/l, LDL-cholesterol 7–8 mmol/l and normal values of triglycerides dominates in laboratory analysis. Interestingly, the values of triglycerides increase and almost reach the pathological range in comparison to the values from the period 50 years ago. The values of HDL-cholesterol are normal. Manifestation of CHD in male patients over 40 years of age and in female patients over 50 years of age is not exceptional (rarely occur cases of myocardial infarction in third decade of age). Typical clinical manifestation of FH is xanthomatosis. The early detection and aggressive treatment in FH patients cause that xanthoma tendinosum, xanthelesma and arcus lipoides are less frequent as decades ago. Endometrióza znamená přítomnost tkáně endometria mimo dutinu děložní. Hlubokou infiltrující endometriózu (deep infiltrating endometriosis – DIE) představují rektovaginální léze, infiltrující onemocnění střev, močového měchýře, močovodu a DIE se vzácně objevuje i v dalších lokalizacích. Negynekologické komplikace DIE rozdělujeme do dvou hlavních kategorií: samotné onemocnění zahrnuje pánevní a břišní bolest, střevní striktury, opakující se krvácení z konečníku, dyschezii, chronickou anémii, dysurii, hematurii, stenózu ureterů, ztrátu ledvinných funkcí a komplikace léčby, ty jsou charakterizovány vznikem fistul, dysfunkcí močového měchýře a opakujícími se záněty urinárního traktu. DIE přináší riziko pro vznik malignit zasažených orgánů. DIE může být léčena konzervativně, nebo chirurgicky. Předkládaná práce je přehledem gastrointestinálních a urologických komplikací DIE a předkládá také kazuistiku pacientky centra pro léčbu endometriózy v nemocnici ve Znojmě (ČR)., Endometriosis is the presence of endometrial tissue outside the endometrial cavity. Deep infiltrating endometriosis (DIE) includes rectovaginal lesions as well as infiltrative disease to the bowel, urinary bladder, ureter and other rare locations. Non gynecological complications of DIE can be divided into two major categories: complications of the disease itself which include pelvic and abdominal pain, bowel stricture, cyclic rectal bleeding, dyschezia, chronic anemia, dysuria, hematuria, ureteral stenosis as well as loss of kidney function and complications of the treatment which include fistula formation, bladder void dysfunction and recurrent urinary tract infections. DIE has potential for malignant transformation in the affected organ. DIE may be managed conservatively or surgically. However definitive treatment is surgical. LDL-aferéza je extrakorporální eliminační metodika, která usiluje o pokud možno specifické odstranění LDL-cholesterolu z cirkulující krve. V současnosti je používáno 6 účinných metod k selektivnímu odstranění LDL-cholesterolu. Mezi hlavní indikace k léčbě LDL-aferézou patří diagnózy homozygotní familiární hypercholesterolemie, heterozygotní familiární hypercholesterolemie refrakterní nebo při intoleranci k standardní léčbě, a dále pacienti se zvýšením lipoproteinu(a) refrakterní nebo intolerantní k farmakoterapii. Dosud však není jednoty o tom, jaká hladina LDL-cholesterolu je rozhodující pro zahájení LDL-aferézy, názory se liší i v jednotlivých státech. I když neexistují klasické velké randomizované studie, existuje dostatečný počet dobře řízených studií k tomu, aby v celém světě bylo uznáváno, že je efekt LDL-aferézy na kardiovaskulární prognózu u závažné hypercholesterolemie významný., LDL-apheresis is an extracorporeal elimination technique, which specifically removes LDL-cholesterol from the circulation. There are six methods for the selective LDL-cholesterol removal these days. The main indications for LDL-apheresis are the diagnosis of homozygous familial hypercholesterolemia, heterozygous familial hypercholesterolemia which is refractory the standard care and intolerance of routine care, and also patients with lipoprotein(a) increase resistant to the farmacotherapy. There is still debate which LDL-cholesterolemia is indication for LDL-apheresis therapy, and the recommendation differs among various countries. Ochorenia myelínu CNS sa súhrnne označujú ako leukoencefalopatie. Termín zahŕňa poruchy, pri ktorých zmeny v bielej hmote prevažujú alebo sú výlučné, pričom patofyziologický mechanizmus ani histopatologická báza nie sú rozhodujúce. Z tejto širokej skupiny vrodených a získaných porúch sa vyčleňuje skupina leukodystrofií, ktorá označuje primárnu poruchu myelínu a buniek tvoriacich myelín. Klasicky sa do tejto skupiny radí šesť klinických jednotiek: X‑adrenoleukodystrofia, metachromatická leukodystrofia, Krabbeho globoidná leukodystrofia, Canavanovej choroba, Pelizaeus‑Merzbacherova choroba a Alexandrova choroba. Pribúdajúcimi poznatkami sa skupina rozširuje o stále nové primárne poruchy. Pre presnú diagnostiku je dôležitý klinický nález, významné sú zmeny na MR mozgu a laboratórne vyšetrenia. Precíznou analýzou MR obrazu je možné diagnostický proces urýchliť. Jedným zo spoločných znakov leukodystrofií je progresívny priebeh. U detí nastáva zväčša rýchla strata motorických a kognitívnych funkcií. V prípade adolescentov a adultných pacientov je ťažisko zmien v psychickej a mentálnej oblasti, pričom motorický deficit nastupuje neskôr. Priebeh X‑adrenoleukodystrofie, metachromatickej leukodystrofie, Krabbeho globoidnej leukodystrofie sa dá vo včasnom štádiu alebo predklinickom období ovplyvniť transplantáciou kostnej drene alebo kmeňových buniek. V prípade metachromatickej leukodystrofie je v skorých štádiách indikovaná génová a enzýmová terapia. Pre ostatné leukodystrofie je dostupná len symptomatická liečba. Cieľom práce je zhrnutie najdôležitejších poznatkov a liečebných možností., Myelin disorders of the central nervous system are also known as leukoencephalopathies. This term includes diseases, in which changes of the white matter are dominant, or even exclusive, while neither the pathophysiological mechanism nor the histopathological basis are decisive. This broad selection of inborn and acquired disorders includes a group of leukodystrophies characterized by primary dysfunction of myelin and myelin-producing cells. Typically, six separate clinical entities are included in this group: X-linked adrenoleukodystrophy, metachromatic leukodystrophy, Krabbe’s globoid cell leukodystrophy, Canavan disease, Pelizaeus-Merzbacher disease and Alexander disease. The group is constantly expanding as our knowledge develops. Clinical and laboratory findings, as well as MRI scans, are crucial for exact diagnosis. Accurate analysis of MRI images accelerates the diagnostic process considerably. A common feature of leukodystrophies is their progressive nature. Children exhibit rapid loss of motor and cognitive functions. In adolescent and adult patients, psychological and mental changes are the most prominent, while motor deficit has a later onset. The course of X-linked adrenoleukodystrophy, metachromatic leukodystrophy, and Krabbe’s leukodystrophy can be managed at the early or pre-clinical stages with bone marrow or stem cell transplantation. Gene and enzyme therapy are indicated at the earliest stages of metachromatic leukodystrophy. For the remaining leukodystrophies, only symptomatic therapy is available. Low-grade myofibroblastic sarcoma (LGMS) is a very rare, atypical myofibroblastic tumor with fibromatosis-like features with predilection mostly in head and neck region. LGMS occurs primarily in adult patients with a slight male predominance. Only few cases of LGMS affecting the larynx have been reported in literature to this date. We describe a case of low-grade myofibroblastic sarcoma of the larynx in a 40-year-old male patient. Mezi nejnáročnější součásti klinického neurologického vyšetření patří vyšetření citlivosti vyžadující detailní edukaci pacienta a jeho následnou spolupráci. Při správné metodice vyšetření a jeho exaktním hodnocení lze často odhalit i diskrétní poruchy citlivosti, kterých si pacient nemusí být vědom nebo není schopen je validně popsat. Našim cílem je podat přehled nejčastěji používaných metod vyšetření běžně hodnocených senzitivních a algických modalit (taktilní, vibrační a termické čití, termoalgické čití, hodnocení hluboké tlakové bolesti a dále mechanická algická citlivost a hodnocení dynamické mechanické alodynie), včetně interpretace jejich nálezů a využitelných normativních dat (pokud jsou dostupná), a to od nejjednodušších kvalitativních a semikvantitativních metod po složitější metody kvantitativní. Kvalitativní či semikvantitativní metody jsou v běžné klinické praxi využívány podstatně častěji. Jejich výhodou je rychlost a metodická nenáročnost testování a široká dostupnost používaných pomůcek. Nevýhodou je naopak skutečnost, že poskytují pouze orientační informaci o citlivosti pro danou modalitu a spolehlivě detekují většinou až pokročilé poruchy senzitivity. Absence kvantifikace limituje možnosti těchto metod v longitudinálním sledování pacientů i při srovnání nálezů mezi pacienty či jejich skupinami. Kvantitativní metody souhrnně označované termínem kvantitativní testování senzitivity (Quantitative Sensory Testing, QST) jsou časově a metodicky náročnější a vyžadují použití specializovaných pomůcek. Jejich předností jsou kvantifikované výstupy s lepší diagnostickou validitou, které potenciálně diagnostikují i klinicky méně vyjádřené abnormity a jsou lépe využitelné např. pro longitudinální sledování. Tyto metody se častěji uplatňují v klinických studiích a jsou nezastupitelné v diagnostice neuropatie tenkých vláken., Sensory examination represents one of the most complicated parts of clinical neurological examination that requires detailed instructions to patients and their attentive cooperation. Using correct methodology of testing and precise evaluation of the results, sensory tests frequently disclose even very fine sensory abnormities which patients may not be able to describe or even may not be aware of. The aim of this paper is to provide an overview of methods most frequently used to evaluate common sensory and pain modalities (touch, vibration and thermal sensation, thermoalgic sensation, deep pain pressure, mechanical detection threshold and sensitivity, evaluation of dynamic mechanical allodynia), including interpretation of the findings and recommended normal values (if applicable). In each of the tested modalities, most commonly used methods are described from the easiest qualitative and semiquantitative methods (usually more frequently used in clinical practice) up to more complicated quantitative tests. Main advantages of qualitative and semiquantitative methods include their low methodological and time burden and high availability of the equipment used. Quantitative sensory testing (QST) is more demanding and requires the use of specialized, not widely available, equipment. These methods, however, provide quantified results with better diagnostic validity, may disclose even less pronounced sensory abnormities and are more useful in long-term follow-up as well as in inter-individual comparisons, both at individual and group level.
