Our case-based review focuses on limb salvage through operative management of Charcot neuroarthropathy of the diabetic foot. We describe a case, when a below-knee amputation was considered in a patient with chronic Charcot foot with a rocker-bottom deformity and chronic plantar ulceration. Conservative treatment failed. Targeted antibiotic therapy and operative management (Tendo-Achilles lengthening, resectional arthrodesis of Lisfranc and midtarsal joints, fixation with large-diameter axial screws, and plaster cast) were performed. On the basis of this case, we discuss options and drawbacks of surgical management. Our approach led to healing of the ulcer and correction of the deformity. Two years after surgery, we observed a significant improvement in patient's quality of life. Buddův-Chiariho syndrom je soubor onemocnění s velice heterogenním spektrem příčin vzniku podmíněný blokádou odtoku krve z jater. V tomto kazuistickém sdělení je popsán případ 32leté pacientky se symptomatickou stenózou jaterní žíly idiopatického původu, která byla léčena na našem pracovišti v letech 2014 a 2015. Žena trpěla refrakterním fluidothoraxem, přičemž po neúspěšné diuretické léčbě byla odeslána na angiointervenční oddělení naší nemocnice. Do stenotické pravé jaterní žíly, při hypoplastické střední a levé, byl implantován balonexpandibilní stent. Po 6 měsících bez obtíží se znovu objevila dušnost na podkladě fluidothoraxu, při provedené angiografii byla diagnostikována restenóza ve stentu a byla provedena dilatace balónkem. K dosažení regrese nebylo nutné vytvoření transjugulární portosystémové spojky (TIPS), ale postačilo pouze zavedení balonexpandibilního stentu, což se ukázalo jako dobré řešení pro tento vzácný typ Buddova-Chiariho syndromu., Budd-Chiari syndrome is a group of rare disorders with heterogenous spectrum of causes based on liver outflow blocation. There is a case report of a 32 year old female patient with syptomatic hepatic vein stenosis of idiopathic origin. She was treated in our department in 2014 and 2015. She suffered from refractory hydrothorax and after diuretic therapy was sent to angiointerventional unit of our hospital. The left and middle hepatic veins were hypoplastic. The stenotic right hepatic vein was treated with implantation of balloon expandable stent. After 6 months without symptoms she suffered from shortness of breath with hydro-thorax, on angiography in-stent restenosis was found and balloon dilatation was performed. Cíl práce: Zhodnotit bezpečnost a účinnost léčby bazálním inzulinem glargin v režimu bazál/bolus v podmínkách běžné klinické praxe v České republice. Soubor a metodika: Projekt LINDA byl neintervenční, multicentrický (n = 255), národní observační projekt, který probíhal v podmínkách běžné klinické praxe. Do sledování bylo zařazeno celkem 4 998 pacientů s diabetes mellitus 1. a 2. typu (DM1T, DM2T) s převážně inzulinovou terapií (99,7 %), po následném zahájení léčby inzulinem glargin v režimu bazál/bolus. Pacienti byli sledováni po dobu 6 měsíců od zahájení této terapie. Primárním cílem projektu bylo sledování četnosti výskytu závažných hypoglykemických příhod při léčbě bazálním inzulinovým analogem glargin (Lantus?) v podmínkách běžné klinické praxe. Sekundárními sledovanými parametry byla změna glykovaného hemoglobinu (HbA1c), glykemie nalačno (FPG), změna tělesné hmotnosti, změna dávek inzulinu, změna počtu hypoglykemických příhod ve srovnání s předchozí léčbou a sledování četnosti nežádoucích účinků. Výsledky: Závažné hypoglykemie byly pozorovány během léčby inzulinem glargin u 0,8 % pacientů. Při srovnání výskytu hypoglykemií s předchozí terapií bylo prokázáno klinicky a statisticky významné snížení jejich četnosti. Procento pacientů s hypoglykemickou příhodou (17,6 %), závažnou (0,8 %) a závažnou noční hypoglykemií (0,3 %) bylo za poslední měsíc léčby s inzulinem glargin v režimu bazál/bolus konzistentně menší ve srovnání s posledním měsícem léčby před zahájením této terapie (42,5 %; 17,6 %; 13,8 % pacientů). U pacientů s DM1T došlo k poklesu všech hypoglykemií z hodnoty 37,80 ? 15,95 příhod/pacient/rok na 8,76 ? 4,38 příhod/pacient/rok (p < 0,001), závažných hypoglykemií z hodnoty 5,64 ? 3,27 příhod/pacient/rok na 0,0396 ? 0,012 příhod/pacient/rok (p < 0,001) a závažných nočních hypoglykemií z hodnoty 3,84 ? 2,04 příhod/pacient/rok na 0,0096 ? 0,003 příhod/pacient/rok (p < 0,001). U pacientů s DM2T poklesla četnost všech hypoglykemií z hodnoty 12,48 ? 7,57 příhod/pacient/rok na 1,68 ? 0,78 příhod/pacient/rok (p < 0,001), závažných hypoglykemií z hodnoty 2,04 ? 0,94 příhod/pacient/rok na 0,0132 ? 0,005 příhod/pacient/rok (p < 0,001) a závažných nočních hypoglykemií z hodnoty 1,32 ? 0,77 příhod/pacient/rok na 0,0048 ? 0,0008 příhod/pacient/rok (p < 0,001). Bylo prokázáno statisticky významné zlepšení metabolické kompenzace při užití inzulinu glargin. U DM1T došlo ke snížení hladiny glykovaného hemoglobinu (HbA1c) z hodnoty 7,74 ? 1,71 % na 6,43 ? 1,39 % (∆ -1,31 ? 0,32 %; p < 0,001) a u DM2T z hodnoty 8,13 ? 1,56 % na 6,72 ? 1,40 % (∆ -1,41 ? 0,28 %; p < 0,001). Statisticky významně (p < 0,001) se zvýšil počet pacientů s hodnotami HbA1c < 5,4 %. Statisticky významný byl také pokles glykemie nalačno (FBG) a hodnot 6bodového glykemického profilu, a to u obou typů diabetu (p < 0,001). Změna terapie a následná léčba inzulinem glargin byla lékaři a pacienty vnímána pozitivně. Závěr: Za podmínek běžné klinické praxe zahájení léčby inzulinem glargin v režimu bazál/bolus u pacientů s předchozí inzulinovou terapií vedlo ke snížení četnosti výskytu hypoglykemických příhod včetně závažných a závažných nočních hypoglykemií a ke zlepšení metabolické kompenzace pacientů s diabetem (snížení glykovaného hemoglobinu, glykemie nalačno a hodnot 6bodového glykemického profilu). Byla zaznamenána větší spokojenost se stávající léčbou ze strany pacientů a lékařů. Klíčová slova: diabetes mellitus – inzulin glargin- observační projekt – režim bazál/bolus, Objective: To evaluate the safety and efficacy of basal insulin glargine using a basal-bolus regimen in a common clinical practice setting in the Czech Republic. Patients and methods: The LINDA project was a non-interventional, multicenter (n = 255), national, observational project. A total of 4,998 patients with Type 1 and 2 diabetes mellitus (T1DM, T2DM) with predominantly insulin therapy (99,7 %), after switch on insulin glargine at basal-bolus regimen, were enrolled in this project. The patients were followed up for 6 months after initiation of the therapy with insulin glargine. The primary objective of the project was to investigate the incidence of severe hypoglycemic episodes during the treatment with basal insulin analogue glargine (Lantus?) in a common clinical practice setting. The secondary endpoints were changes in glycosylated hemoglobin (HbA1c) levels, fasting plasma glucose (FPG), body weight, insulin dose, change of number of hypoglycemic episodes in comparison the previous therapy and the frequency of adverse effects. Results: Severe hypoglycaemia were observed during treatment with insulin glargine at 0.8 % patients. When comparing the incidence of hypoglycemia with the previous therapy, we demonstrated a clinically and statistically significant reduction in their frequencies. The percentage of patients with hypoglycemic episodes (17.6 %), severe hypoglycemia (0.8 %) and severe nocturnal hypoglycemia (0.3 %) over the last month of treatment with insulin glargine using the basal-bolus regimen was consistently lower compared to the last month of treatment before initiation of this therapy (42.5 %, 17.6 %, and 13.8 % of the patients, respectively). In patients with T1DM, the incidence of hypoglycemia decreased from 37.80 ? 15.95 episodes/patient/year to 8.76 ? 4.38 episodes/patient/year (p < 0.001) for all hypoglycemic episodes; from 5.64 ? 3.27 episodes/patient/year to 0.0396 ? 0.012 episodes/patient/year (p < 0.001) for severe hypoglycemia; and from 3.84 ? 2.04 episodes/patient/year to 0.0096 ? 0.003 episodes/patient/year (p < 0.001) for severe nocturnal hypoglycemia. In patients with T2DM, the incidence of hypoglycemia decreased from 12.48 ? 7.57 episodes/patient/year to 1.68 ? 0.78 episodes/patient/year (p < 0.001) for all hypoglycemic episodes; from 2.04 ? 0.94 episodes/patient/year to 0.0132 ? 0.005 episodes/patient/year (p < 0.001) for severe hypoglycemia; and from 1.32 ? 0.77 episodes/patient/year to 0.0048 ? 0.0008 episodes/patient/year (p < 0.001) for severe nocturnal hypoglycemia. A statistically significant improvement in the metabolic control was demonstrated when using insulin glargine. The glycated hemoglobin (HbA1c) decreased from 7.74 ? 1.71 % to 6.43 ? 1.39 % (∆ -1.31 ? 0.32 %, p < 0.001) in patients with T1DM, and from 8.13 ? 1.56 % to 6.72 ? 1.40 % (∆ -1.41 ? 0.28 %, p < 0.001) in patients with T2DM. A statistically significant (p < 0.001) increase in the number of patients with HbA1c < 5.4 % was further demonstrated. The decrease in fasting blood glucose (FBG) and 6-point blood sugar profile was also statistically significant in both types of diabetes (p < 0.001). Changes in therapy and subsequent treatment with insulin glargine were perceived positively by both physicians and patients. Conclusion: In the common clinical practice setting, the initiation of treatment with insulin glargine using the basal-bolus regime in patients with previous insulin therapy resulted in a reduction in the incidence of hypoglycemic events, including severe hypoglycemia and severe nocturnal hypoglycemia, and improved metabolic control in patients with diabetes (reduced glycated hemoglobin, fasting glucose values and 6-point blood glucose profile). Greater satisfaction with the current treatment was reported by both patients and physicians. Introduction: Ankylosing spondylitis (AS) is an inflammatory rheumatic disease characterized by the development of osteoproductive changes in the spine which could possibly result in ankylosis. Treatment with tumour necrosis factor alpha (TNFα) inhibitors has proved to be an important step forward in the treatment of this disease, but for the time being it is not clear whether it favourably influences radiographic progression of the disease. Vascular endothelial growth factor most probably plays a role in the development of osteoproductive changes and recently its predictive influence on radiographic progression has been demonstrated. Bone morphogenic protein 2 (BMP-2) participates in the regulation of bone proliferation and its increased serum level has been demonstrated in patients with advanced AS and correlated with the degree of radiographic changes. Aim: The study aims to evaluate the VEGF and BMP-2 levels in patients with ankylosing spondylitis and how these levels relate to the concurrent treatment with TNFα inhibitors. Methods: Sera were evaluated from patients at the Rheumatologic Clinic of the Hradec Králové Faculty Hospital who fulfilled the modified New York Criteria for AS (n = 55). In these patients, the parameters of the activity of the disease (BASDAI = Bath Ankylosing Spondylitis Disease Activity Index, CRP = C-reactive protein) and the concurrent therapy (TNFα inhibitors, n = 21, vs. non-anti TNFα, n = 34) were recorded. The levels of VEGF and BMP-2 were analyzed using the ELISA method. Results: In patients treated with TNFα inhibitors, a significantly lower VEGF level was found when compared to untreated patients (140.3 (109.4; 262.2) vs. 261 (172.4; 396.6) pg/ml; p = 0.02). No difference was found between BMP-2 levels in both groups (treated vs. untreated patients) (254.8 (2301; 267.3) vs. 261.1 (248.6; 273.5) pg/ml; p = 0.24). A correlation analysis did not reveal any relationship between VEG F and BMP-2 (r = 0.057; p = 0.68). Serum levels of VEGF correlated with serum levels of CRP (r = 0.56; p = 0.00001) and the BASDAI value (r = 0.33; p = 0.015). Conclusion: Significantly lower VEGF levels were found in patients treated with TNFα inhibitors versus the untreated patients. These findings are in harmony with some hitherto published analyses and may give evidence of a favourable effect of TNFα inhibitors on radiographic progression. The aim of this study is to evaluate the results of total hip arthroplasty in patients with Parkinson's disease during a period of five years, focusing on the assessment of the risks and benefits of surgery. During this period we performed total hip arthroplasty in 14 patients (15 hips) with Parkinson's disease. Patients were evaluated by subjective symptoms and objective findings, with a focus on the use of support while walking and walking distance, severity of Parkinson's disease before surgery and at the time of the last follow-up. During the postoperative period, the following parameters were assessed: length of ICU stay, mobilization, complications, the total duration of hospitalization and follow-up care after discharge. Of the 11 patients (12 hips) followed-up 1-5 years with an average of 3 years after operation 8 cases showed progression of neurological disability. 5 patients (6 hips) showed an increased dependence on the use of support when walking and reduced distance that the patient was able to walk. Subjectively, 10 hip joints were completely painless and 2 patients complained of only occasional mild pain in the operated hip. Complications that were encountered were urinary tract infection (5 patients), cognitive impairment (3 patients) and pressure ulcer (2 patients). We did not observe any infection or dislocation of the prosthesis. Three patients fell and fractured the femur and 3 patients in our cohort died during follow up. Implantation of total replacement is possible with judicious indication after careful evaluation of neurological finding in patients with minimal or mild functional impairment of the locomotor system. Toxická epidermální nekrolýza je vzácné, akutní, život ohrožující onemocnění projevující se rozsáhlým odlučováním epidermis a slizničních povrchů. Jde o závažnou nežádoucí reakci na podávané léky, nejčastěji antibiotika, antikonvulziva či nesteroidní antirevmatika. Jako první ji popsal skotský dermatolog Alan Lyell v roce 1956, proto je též nazývána Lyellův syndrom. Mortalita tohoto onemocnění je vysoká, proto je klíčová včasná diagnostika a neodkladné zahájení adekvátní péče o pacienta. Jedná se o vzácné onemocnění s velmi malou incidencí a závažnou prognózou, což značně ztěžuje provedení velkých randomizovaných klinických studií. Je tedy obtížné hodnotit efektivitu jednotlivých terapeutických možností. Jako nejperspektivnější se prozatím jeví nitrožilní podávání imunoglobulinů se zřejmým zlepšením klinického stavu pacienta, dobrou tolerancí a minimálními vedlejšími nežádoucími účinky. Nové experimentální postupy se snaží hledat cestu terapie cíleným ovlivněním granulyzinu jakožto hlavního cytotoxického mediátoru. It is a serious adverse response to administered drugs, mostly antibiotics, anticonvulsants or NSAIDs. First described by Scottish dermatologist Alan Lyell in 1956, it is also known as Lyell’s syndrome. Mortality of the disease is high, therefore its early diagnosis is crucial and immediate initiation of appropriate patient care necessary. It is a rare disease with a very low incidence and serious prognosis, which is a considerable hindrance to undertaking large randomized clinical studies. It is therefore difficult to evaluate the effectiveness of various therapeutic options. As the most promising so far appears the administration of intravenous immunoglobulins, apparently reaching improvement in the clinical condition of the patient, with a good tolerances and minimal side adverse effects. New experimental techniques endeavour to seek a way to the therapy through targeted influencing of granulysin as the major cytotoxic mediator. Úvod: Tato práce referuje první zkušenosti s laparoskopicky asistovanou transanální totální excizí mezorekta u pacientů s karcinomem středního a dolního rekta. Metody: V období od října 2013 do května 2014 bylo na dvou pracovištích (CHK FN Brno, CHO KOC Nový Jičín) operováno 17 pacientů technikou transanální totální mezorektální excize. V souboru bylo 11 mužů a 6 žen ve věku 49–81 let. Vzdálenost dolního okraje tumoru od análního okraje byla 30–80 mm, medián 60 mm. U všech pacientů byla konstruována ručně šitá kolo-anální anastomóza. Výsledek: U všech operovaných byl dosažen negativní distální resekční okraj. Cirkumferentní resekční okraj byl pozitivní ve dvou případech. Kvalita totální mezorektální excize byla hodnocena v 8 případech jako kompletní a v 9 případech jako téměř kompletní. Dehiscence kolo-anální anastomózy byla zaznamenána ve dvou případech (12 %). Operační čas se pohyboval v rozsahu 212–375 minut (medián 280 minut). Medián pooperační hospitalizace byl 9 dnů v rozsahu 6 až 30 dnů, v případě řešení anastomotické a rané komplikace. Závěr: Na základě prvních získaných zkušeností hodnotíme výkon jako realizovatelný a bezpečný v podmínkách našich chirurgických pracovišť s uspokojivými patologickými výsledky. Přesné indikace zatím nelze jednoznačně doporučit. Předpoklad je, že k transanální totální mezorektální excizi by měli být indikováni pacienti s nízce uloženým karcinomem rekta, event. s úzkou pánví, objemným mezorektem a objemnou prostatou., Introduction: First experiences with laparoscopic low anterior resection with transanal mesorectal excision in patients with low and mid rectal cancer are referred. Methods: During a 9 month period (between October 2013 and May 2014), 17 laparoscopically assisted rectal resections with transanal total mesorectal excision, hand-sewn colo-anal anastomosis and protective ileostomy were performed at two institutions (CHK FN Brno, CHO KOC Nový Jičín). There were 11 males and 6 females, with a mean age 68 years (range 49–81 years) in our cohort. The tumor was located 30–80mm from the anal verge (median 60 mm). Results: A negative distal resection margin was achieved in all patients. The circumferential resection margin was positive in two cases. The TME quality was described as complete in 8 cases and nearly complete in 9 cases. Mean operative time was 280 min (range 212–375 min) with no intra-operative complications. Anastomotic leakage was observed in two patients (12%). Median length of postoperative hospital stay was 9 days with a range of 6–30 days, in case of anastomotic and wound complications. Conclusion: According to our first experience we evaluated this method as feasible and safe with satisfactory pathological outcomes. We cannot recommend exact indications yet. BACKGROUND: Intussusception of the large bowel in adults is a very rare pathological condition. However, it has its clinical importance because intussusception is very often associated with an intraluminal lesion. CASE REPORT: We report two cases of the large bowel intussusception, ileocolic and colorectal. Both intussusceptions were associated with a malignant tumor. However, the clinical presentation was different. One of the intussusceptions was of non-transient character, while the second one resolved spontaneously before operation. Both patients underwent surgery and malignant tumors were found and removed. RESULTS: The purpose of the article is to draw attention to intussusception and emphasize that intussusception, either transient or non-transient, should be further examined. Transplantace ledviny od žijícího dárce inkompatibilního v krevní skupině představuje nový nástroj, jak zvýšit dostupnost transplantací. Většina evropských protokolů je založena na desenzitizačním protokolu s rituximabem, trojkombinace imunosupresiv, intravenózních imunoglobulinů a specifické imunoadsorpce, při které jsou z plazmy odstraňovány konkrétní hemaglutininy. Program transplantací ledvin od inkompatibilních dárců v krevní skupině byl v našem centru založen v roce 2011. Dosud bylo provedeno 21 těchto transplantací. Nejvyšší titr hemaglutininů před rituximabem byl 1 : 64 a korespondující počty imunoadsorpcí před transplantací od 2 do 9 výkonů. Po transplantaci byly imunoadsorpce prováděny podle aktuálního titru hemaglutininů 1–2krát u 5 nemocných. V posledních 2 letech jsou tyto transplantace nabízeny inkompatibilním příjemcům, kteří nenaleznou uplatnění v systému párových výměn ledvin anebo se účastní párové výměny s cílem dosáhnout vyšší míry shody v HLA antigenech. Hlavní komplikací bylo častější pooperační krvácení a infekce močových cest u rizikových nemocných. Ve většině protokolárních biopsií byla popsána pozitivita C4d barvení. Nebyla pozorována ztráta štěpu ani úmrtí nemocných., Living donor AB0 incompatible kidney transplantation represents a new tool how to improve the access to transplantation. Majority of European protocols are based on desensitization with rituximab, triple drug immunosuppression, intravenous immunoglobulins and specific immunoadsoption (IA) which eliminates isohaemaglutinins. AB0i kidney transplant program was initiated in our centre in 2011 and 21 patients have received grafts from incompatible donors until recently. Highest accepted isohaemaglutinins titers before rituximab were 1 : 64 and corresponding pretransplant immunoadsorption procedures varied from 2 to 9. In 5 patients 1–2 IA procedures were performed also after transplantation. With the advent of paired exchange program the AB0i transplantation is offered to patients with unsuccessful matching run or with aim to improve HLA match between donor and recipient. The main complications were postoperative bleeding and urinary tract infections in patients at risk. Majority of protocol biopsies exhibited positivity of C4d staining. Během posledních 30 let se transplantace pankreatu stala rutinním postupem, který je zvláště vhodný pro pacienty s diabetem 1. typu, kteří současně podstupují transplantaci ledviny. Zlepšuje kvalitu života a v porovnání s transplantací samotné ledviny zlepšuje dlouhodobé přežívání. Poskytuje nezávislost na inzulinu s téměř normální glukózovou tolerancí, uvolnění diety, klid od hypoglykemií a možnost zastavení postupu nebo dokonce regresi mikroangiopatických komplikací. Samostatně se transplantace pankreatu provádí zejména u vybraných pacientů s rekurentními epizodami těžkých hypoglykemií při syndromu porušeného vnímání hypoglykemie nebo jako následný výkon u příjemců s funkčním štěpem ledviny od zemřelého či žijícího dárce. 5leté přežívání funkce štěpu pankreatu činí při kombinované transplantaci, transplantaci samotného pankreatu či transplantaci pankreatu následně po ledvině více než 75, 50 a 62 %.
