Úvod: Posouzení významu PET/CT pro iniciální staging karcinomu jícnu s důrazem na metastatické postižení uzlin a průkaz vzdálených metastáz. Dále bylo cílem práce ověření významu PET/CT vyšetření při hodnocení efektu neoadjuvantní léčby. Metody: V prospektivní studii byl sledován soubor 354 nemocných s karcinomem jícnu, kteří byli vyšetřeni na I. chirurgické klinice LF UP a FN Olomouc v letech 2006–2012. Vstupní PET/CT vyšetření bylo provedeno u 349 nemocných. Analyzovali jsme přínos vyšetření ke stanovení stagingu onemocnění a v závislosti na něm jsme stanovovali strategii léčby. Na základě vstupního PET/CT byl u 102 nemocných zjištěn různý stupeň generalizace a byli indikovaní k paliativní či symptomatické terapii. U 247 pacientů bylo zjištěno omezení nádoru na jícen ev. regionální uzliny. Po posouzení celkového stavu a dle přání pacienta byla dále u 188 nemocných indikovaná neoadjuvantní radiochemoterapie (RCHT), léčbu nedokončilo 32 nemocných. U 156 pak bylo v průměrném odstupu 8,4 týdnů od ukončené neoadjuvantní terapie provedeno kontrolní vyšetření PET/CT. Na základě tohoto vyšetření bylo kompletní vymizení známek nádoru (complete response- CR) zjištěno u 38 vyšetřených (24,4 %), regrese u 89 (57,0 %), stacionární nález u 10 (6,4 %) a progrese u 19 (12,2 %). K chirurgické léčbě bylo indikováno 97 pacientů, z toho resekci jícnu bylo možné provést u 85 a u 12 byl výkon pouze paliativní resp. šlo o exploraci. Výsledky: Vstupní PET/CT vyšetření u 349 pacientů u naprosté většiny správně posoudilo rozsah onemocnění v souhlase s histologicky ověřenou diagnózou. Falešně negativní výsledek byl jen u 5 nemocných (1,43 %). Při hodnocení efektu neoadjuvantní léčby byli pacienti rozděleni do skupin dle nálezu při kontrolním PET/CT po neoadjuvanci a bylo vyhodnocováno jejich přežívání. Byl zjištěn signifikantní rozdíl (p=0,0004) v přežívání mezi skupinami s rozdílnou reakcí na neadjuvantní léčbu (CR (n=38), regrese (n=89), stacionární nález (n=10), progrese (n=19)) bez ohledu na další léčbu po neoadjuvanci, ve prospěch pacientů s lepší reakcí na neadjuvantní léčbu. Signifikantní rozdíl v přežívání byl zaznamenán i mezi skupinou pacientů po neoadjuvanci radikálně operovaných (n=85) a neoperovaných (n=59) ve prospěch operovaných (p=0,003). Nejdelšího průměrného přežívání 38,6 měsíců (medián 29,0 měsíců) bylo dosaženo ve skupině operovaných po neoadjuvanci, kde kontrolní PET/CT ukázalo CR. Nebyl však prokázán signifikantní rozdíl (p=0,587) ve výsledcích mezi skupinami operovaných s rozdílnou reakcí na neadjuvantní léčbu (regrese, stacionární nález). Počet případů v jednotlivých hodnocených skupinách není zatím natolik velký, abychom získané výsledky mohli považovat za jednoznačně průkazné a ve sledování a zařazování dalších nemocných do studie budeme dále pokračovat. Závěr: V práci je dokumentován význam a přínos PET/CT v iniciálním stagingu karcinomu jícnu, zejména v průkazu metastatického onemocnění, a to jak v postižení uzlin, tak v odhalení vzdálených metastáz. PET/CT má velký význam pro stanovení léčebné strategie. Dále byl ověřen význam PET/CT vyšetření při hodnocení efektu neadjuvantní léčby., Introduction: To evaluate the significance of PET/CT for the initial staging of esophageal cancer with emphasis on metastatic lymph node affection and detection of distant metastases. Furthermore, the aim of the work was to analyze the significance of PET/CT examination when evaluating the effect of neoadjuvant therapy. Methods: A set of 354 patients with esophageal cancer treated at the 1st Department of Surgery, University Hospital Olomouc and Medical Faculty at Palacky University in Olomouc between the years 2006−2012 were analyzed in a prospective study. The initial PET/CT examination was performed in 349 patients. We analyzed the benefit of this examination in regard to disease staging and based on the result, therapeutic strategy was determined. The initial PET/CT showed varying degrees of disease generalization in 102 patients, these patients were indicated for palliative or symptomatic therapy. In 247 patients, the disease was limited only to the esophagus and /or regional lymph nodes. After considering the patient’s overall condition and taking into account the wishes of the patient, 188 patients were indicated for neoadjuvant chemoradiotherapy (CRT); 32 patients did not complete this treatment. In 156 patients a follow-up PET/CT scan was performed after an average of 8.4 weeks following completion of neoadjuvant therapy. Based on this examination, a complete response- CR, was observed in 38 patients (24.4%), regression of the tumor in 89 (57.0%), stationary findings were seen in 10 (6.4%), and progression in 19 (12.2%). Ninety-seven patients were indicated for surgical resection; however, esophagectomy was only possible in 85 patients, in the remaining 12 patients only an explorative laparotomy was performed due to disease progression. Results: The initial PET/CT examination performed in 349 patients correctly described the extent of the disease in accordance with the histologically confirmed diagnosis in virtually all patients. A false positive result was seen in only 5 patients (1.43%). When evaluating the effect of neoadjuvant therapy, the patients were divided into groups based on the findings of the follow-up PET/CT after neoadjuvant therapy and their overall survival was evaluated. A significant difference (p=0.0004) in survival was observed between the groups based on the different reactions to neoadjuvant therapy (CR (n=38), regression (n=89), stationary findings (n=10), progression (n=19)) without taking into account the following treatment the patient received after neoadjuvant therapy. Patients who had a better response to neoadjuvant therapy had better survival results. There was also a significant difference in survival between the group of patients who completed neoadjuvant therapy and underwent radical surgical resection (n=85) versus those patients who completed neoadjuvant therapy but did not undergo subsequent surgery (n=59). The operated group had a significantly higher overall survival (p=0.003). The longest mean survival, 38.6 months (median 29.0 months), was achieved by the group of patients who completed neoadjuvant therapy, showed a complete response on the follow-up PET/CT, and underwent surgical resection. However, a significant difference was not observed (p=0.587) between the groups who underwent surgical resection and whose follow-up PET/CT results differed (regression or stationary findings). To date, the number of cases in the individual groups is not great enough to consider the obtained results conclusive, and we will continue to include more patients into the study and continue with the analysis. Conclusion: The work documents the significance and benefit of PET/CT in the initial staging of esophageal cancer, especially in detecting metastatic disease- positive lymph nodes as well as distant metastases. The aim of this study was to determine the prevalence of musculoskeletal problems among Czech dentists and to analyze the factors that affect these disorders. Information was gathered through questionnaire completed by 581 physicians. The questionnaire ascertained general information about physicians including their work habits and the characteristics of their work environment along with the occurrence of musculoskeletal problems as well as their intensity. In the past year the occurrence of at least mild difficulties associated with the locomotive system was reported by 96.9% of dentists surveyed (n = 557), with 66.3% of respondents (n = 381) reporting difficulties ofa moderate or major nature. Most respondents of both sexes indicated neck pain. A statistically significant correlation with the occurrence of musculoskeletal complaints of medium and major intensity was demonstrated for the following factors: sex, age, running a private practice, past injury or musculoskeletal diseases, and the perception of work as psychologically demanding. Musculoskeletal disorders in dentists in Czech Republic are relatively frequent and serious health problem. The causes of these diseases must be identified and appropriate preventive interventions undertaken that will contribute to a reduction in the incidence of these problems. and Z. Sustová, L. The aim of this study is to evaluate the results of total hip arthroplasty in patients with Parkinson's disease during a period of five years, focusing on the assessment of the risks and benefits of surgery. During this period we performed total hip arthroplasty in 14 patients (15 hips) with Parkinson's disease. Patients were evaluated by subjective symptoms and objective findings, with a focus on the use of support while walking and walking distance, severity of Parkinson's disease before surgery and at the time of the last follow-up. During the postoperative period, the following parameters were assessed: length of ICU stay, mobilization, complications, the total duration of hospitalization and follow-up care after discharge. Of the 11 patients (12 hips) followed-up 1-5 years with an average of 3 years after operation 8 cases showed progression of neurological disability. 5 patients (6 hips) showed an increased dependence on the use of support when walking and reduced distance that the patient was able to walk. Subjectively, 10 hip joints were completely painless and 2 patients complained of only occasional mild pain in the operated hip. Complications that were encountered were urinary tract infection (5 patients), cognitive impairment (3 patients) and pressure ulcer (2 patients). We did not observe any infection or dislocation of the prosthesis. Three patients fell and fractured the femur and 3 patients in our cohort died during follow up. Implantation of total replacement is possible with judicious indication after careful evaluation of neurological finding in patients with minimal or mild functional impairment of the locomotor system. Prerequisite for a good result is precise surgical technique and optimal implant position with balanced tension of the muscles and other soft tissues around the hip. and PG. Mathew, P. Sponer, T. Úvod: Cílem tohoto článku je prezentovat naše zkušenosti s transanální endoskopickou mikrochirurgií (TEM) jako jedním ze základních operačních přístupů k selektovaným pacientům s nádory konečníku. Metody: Retrospektivně jsme vyhodnocovali soubor 80 pacientů, u kterých byla pro nádor konečníku provedena operační léčba pomocí TEM. U těchto pacientů jsme sledovali a statisticky vyhodnocovali konkrétní předoperační a pooperační ukazatele, které bychom chtěli prezentovat: předoperační bioptické vyšetření a pooperační výsledek histologického vyšetření, velikost nádoru a jeho uložení v konečníku, hloubku resekce a hodnocení základních demografických ukazatelů. Výsledky: Z našich zkušeností u zmíněných pacientů vyplývá, že není významný rozdíl mezi věkem operovaných mužů a žen ani při rozlišení na benigní a maligní nádory. Nejčastěji operovanými pacienty byli muži, průměrně ve věku 64 let, kteří byli primárně vyšetřeni pro krvácení z konečníku. Nejčastějším nálezem byl low-grade adenom, který byl potvrzen i v definitivním resekátu. Celkem u 17 pacientů byl nalezen v definitivním histologickém vyšetření maligní tumor, který nebyl zachycen předoperačně. Závěr: V rukách zkušeného chirurga představuje TEM reálnou možnost provedení radikální resekce i maligního, lokálně nepokročilého nádoru konečníku, pokud je v dosahu operačního rektoskopu. TEM je metodou bezpečnou, bez negativního vlivu na funkce análních svěračů. Vzhledem k možnosti falešně negativního předoperačního histologického vyšetření preferujeme radikální odstranění nádoru s celou šířkou stěny konečníku. I když optimální lokalizace pro použití operačního rektoskopu je na zadní straně konečníku, téměř u poloviny našich pacientů se tyto nádory vyskytovaly na stěně přední. Vzhledem k technické proveditelnosti sutury stěny rekta je vhodné, aby báze tumoru nepřesahovala polovinu obvodu konečníku. Technika TEM je vhodná zejména pro nízce uložené, nestenotizující, lokálně nepokročilé nádory konečníku. Naše dobré výsledky jsou dány především přísnou selekcí pacientů, technickým zvládnutím operačního postupu a letitými zkušenostmi., Introduction: The purpose of this paper is to present our experience with transanal endoscopic microsurgery (TEM) as an operative approach in a selected group of patients with rectal tumours. Methods: We present a retrospective study of 80 patients operated for rectal tumour using TEM. Specific preoperative and postoperative indicators were gathered and statistically evaluated. We assessed histological examination before the surgery and definitive histology of the resected tumour, resection depth and basic demographic data of our patients. Results: Our experience suggests that there is no significant difference between the age of patients according to sex, even if malignant and benign tumours are assessed separately. The majority of our operated patients were male patients, mean age of 64 years who were primarily referred to our department because of bleeding low-grade adenomas of the rectum. In 17 patients a malignant tumour was diagnosed based on final histopathologic examination. None of them had signs of malignancy before the surgery. Conclusion: In the hands of an experienced surgeon, TEM is a feasible option of radical removal of rectal, locally not advanced malignancies if within reach of TEM rectoscope. TEM is a safe procedure that does not result in any alteration of rectal sphincter functions. Possible false negative preoperative benign diagnosis leads us to approach radical removal of the tumour with full wall thickness resection. Although the posterior aspect of the rectum is an optimal location for the rectoscope, anterior tumours were found in almost one half of our patients. Considering technical feasibility of the rectal wall suture, the base of the tumour should not extend beyond one half of the rectal circumference. Low non-stenotising noninvasive tumours of the rectum are suitable for TEM. Úvod: Tato práce referuje první zkušenosti s laparoskopicky asistovanou transanální totální excizí mezorekta u pacientů s karcinomem středního a dolního rekta. Metody: V období od října 2013 do května 2014 bylo na dvou pracovištích (CHK FN Brno, CHO KOC Nový Jičín) operováno 17 pacientů technikou transanální totální mezorektální excize. V souboru bylo 11 mužů a 6 žen ve věku 49–81 let. Vzdálenost dolního okraje tumoru od análního okraje byla 30–80 mm, medián 60 mm. U všech pacientů byla konstruována ručně šitá kolo-anální anastomóza. Výsledek: U všech operovaných byl dosažen negativní distální resekční okraj. Cirkumferentní resekční okraj byl pozitivní ve dvou případech. Kvalita totální mezorektální excize byla hodnocena v 8 případech jako kompletní a v 9 případech jako téměř kompletní. Dehiscence kolo-anální anastomózy byla zaznamenána ve dvou případech (12 %). Operační čas se pohyboval v rozsahu 212–375 minut (medián 280 minut). Medián pooperační hospitalizace byl 9 dnů v rozsahu 6 až 30 dnů, v případě řešení anastomotické a rané komplikace. Závěr: Na základě prvních získaných zkušeností hodnotíme výkon jako realizovatelný a bezpečný v podmínkách našich chirurgických pracovišť s uspokojivými patologickými výsledky. Přesné indikace zatím nelze jednoznačně doporučit. Předpoklad je, že k transanální totální mezorektální excizi by měli být indikováni pacienti s nízce uloženým karcinomem rekta, event. s úzkou pánví, objemným mezorektem a objemnou prostatou., Introduction: First experiences with laparoscopic low anterior resection with transanal mesorectal excision in patients with low and mid rectal cancer are referred. Methods: During a 9 month period (between October 2013 and May 2014), 17 laparoscopically assisted rectal resections with transanal total mesorectal excision, hand-sewn colo-anal anastomosis and protective ileostomy were performed at two institutions (CHK FN Brno, CHO KOC Nový Jičín). There were 11 males and 6 females, with a mean age 68 years (range 49–81 years) in our cohort. The tumor was located 30–80mm from the anal verge (median 60 mm). Results: A negative distal resection margin was achieved in all patients. The circumferential resection margin was positive in two cases. The TME quality was described as complete in 8 cases and nearly complete in 9 cases. Mean operative time was 280 min (range 212–375 min) with no intra-operative complications. Anastomotic leakage was observed in two patients (12%). Median length of postoperative hospital stay was 9 days with a range of 6–30 days, in case of anastomotic and wound complications. Conclusion: According to our first experience we evaluated this method as feasible and safe with satisfactory pathological outcomes. We cannot recommend exact indications yet. Cil: Klasická neuralgie trigeminu (CTN) je charakterizována epizodickou ostrou neuropatickou obličejovou bolestí a s výjimkou přítomnosti neurovaskulárního konfliktu (NVC) nebyla v našem souboru podmíněna jinou známou abnormitou či patologií. V poslední době je diskutovaná atrofie trigeminu na straně bolesti. Našim cílem bylo ověřit tyto nálezy a vztáhnout je k dalším klinickým datům. <p align="">Soubor a metodika: Pomocí magnetické rezonance (MR) jsme změřili objemy trojklaného nervu na bolestí postižené a nepostižené straně a vyhodnotili výskyt a typ NVC u 45 nemocných s CTN. Následně jsme provedli korelace mezi objemy postiženého nervu, NVC, klinickými a demografickými daty (trvání nemoci, věk nemocného). <p align="">Výsledky: NVC jsme zjistili u 69 % postižených nervů, 51 % nepostižených nervů a u 42 % nemocných oboustranně. Signifikantně snížený objem trojklaného nervu byl zjištěn na bolestí postižené straně (p < 0,001). Žádné korelace jsme nezaznamenali mezi objemem postiženého nervu, přítomností a typem NVC ani mezi klinickými a demografickými daty. <p align="">Závěr: Naše výsledky naznačují, že NVC může být sice spoušťovým mechanizmem CTN u vnímavých subjektů, ale není spolehlivým ukazatelem nemoci. Snížený objem trojklaného nervu se objevuje nezávisle na NVC a odráží spíše predisponující asymetrii trigeminu než vzniklou atrofii., Aim: Classic trigeminal neuralgia (CTN) is characterized by episodic neuropathic facial pain that, with exception of neurovascular conflict (NVC), was not attributed to any underlying disorder in our sample. Using magnetic resonance imaging, we evaluated volumes of affected and non‑affected nerves and presence and type of NVC in a large group of CTN subjects prior to gamma knife treatment. <p align="">Design: Correlation between affected nerve volume and NVC, clinical data and demographic characteristics (duration of pain, age) were explored in 45 subjects diagnosed with CTN. <p align="">Results: NVC was detected in 69% of affected nerves, 51% of non‑affected nerves, and in 42% of subjects bilaterally. Lower trigeminal nerve volume was detected on the affected side (p < 0.001), however, no correlation between affected nerve volume and the presence and type NVC or demographic data was detected. <p align="">Conclusion: Our results suggest that NVC may trigger CTN in susceptible subjects but is not a reliable disease marker. Úvod: Test 16 slov (T16S) (v originálu Enhanced Cued Recall Test) je skríningový test k diagnostice poruch paměti ve stáří. T16S využívá kategorickou nápovědu během kontrolovaného učení a vybavení. Předpokládá se, že testy vycházející z tohoto paradigmatu jsou schopny identifikovat tzv. hipokampální poruchu paměti, která je charakterizována nízkým skóre ve volném vybavení s minimálním efektem nápovědy a je typická pro Alzheimerovu demenci (AD), a odlišit ji od poruchy paměti frontálního typu, kdy například pacienti s behaviorální variantou frontotemporální demence (bvFTD) navzdory nízkému volnému vybavení relativně profitují z nápovědy. <p align="">Cíl: Ověřit rozlišovací schopnost a využitelnost T16S v diferenciální diagnostice poruch paměti u pacientů s AD a s bvFTD. Metodika: Třicet šest pacientů s diagnózou pravděpodobné AD ve stadiu lehké demence (MMSE 21,7 ? 2,4), 15 pacientů s pravděpodobnou bvFTD (MMSE 23,8 ? 2,4) a 45 kognitivně zdravých seniorů (KS) (MMSE 28,96 ? 1,17) bylo vyšetřeno klinicky, absolvovali komplexní vyšetření neuropsychologickou baterií včetně T16S. <p align="">Výsledky: Skupiny se lišily ve všech třech ověřovaných skórech T16S: ve volném a celkovém vybavení i ve využití nápovědy (p < 0,001). Pro odlišení obou demencí od norem je vhodné zejména skóre volného vybavení (senzitivita a specificita 94 % a 100 % pro AD a 80 % a 91 % pro bvFTD), pro diferenciální diagnostiku obou demencí je nejlepším indexem skór celkového vybavení (senzitivita 86 % a specificita 87 %). <p align="">Závěr: Test paměti T16S je senzitivní a specifický test pro diagnostiku specifické poruchy paměti u AD a bvFTD. Může přispět k diferenciální diagnostice a jako takový ho doporučujeme k praktickému použití v klinické praxi., Introduction: Enhanced Cued Recall (ECR) memory test is especially valuable in screening for cognitive impairment in the elderly. ECR uses categorical cues during controlled encoding and recall; tests based on this paradigm are considered to be able to identify hippocampal memory impairment characterized by poor free recall with minimal effect of cueing, typical of Alzheimer’s disease (AD). The test could aid in differentiating memory impairment in behavioural variant frontotemporal dementia (bvFTD) versus AD – despite the poor free recall bvFTD patients benefit considerably from cueing. <p align="">Objective: To establish the validity of ECR in diagnosing memory impairment in AD and bvFTD. Methods: Thirty six patients with probable mild AD (MMSE 21.7 ? 2.4), 15 with probable bvFTD (MMSE 23.8 ? 2.4) and 45 healthy elderly (HE) (MMSE 28.96 ? 1.17) underwent clinical and neuropsychological examination including ECR. <p align="">Results: There was a statistically significant difference between all groups in all three ECR memory performance scores: free recall, total recall and cueing effect (p < 0.001). Free recall was the most effective score for discriminating dementias from healthy elderly (sensitivity 94% and specificity 100% for AD and sensitivity 80% and specificity 91% for bvFTD); total recall was most effective for differential diagnosis of both dementias (sensitivity 86% and specificity 87%). <p align="">Conclusion: ECR is a highly sensitive and specific test for identification of memory impairment in AD and bvFTD, and can contribute to the differential diagnosis. Diabetes mellitus 2. typu představuje závažný medicínský, společenský i ekonomický problém, neboť se stal v posledních letech pandemickým onemocněním, postihuje ve stáří až třetinu české populace. Jde o klinicky nejvýznamnější metabolické onemocnění vyššího věku. Farmakoterapeutický přístup je třeba u seniorů přizpůsobit přítomnosti četných komorbidit, schopnosti pacienta spolupracovat na léčbě. Vzhledem k obávané hypoglykemii, která může přispívat ke zvýšené morbiditě, snižuje kvalitu života a limituje v mnoha případě možnosti léčby. Při rozhodování o léčbě je třeba vycházet z doporučení, která dělí diabetiky z geriatrického hlediska na seniory zdatné nebo křehké. Od toho se odvozují cílové hodnoty kompenzace diabetu. Cílem článku je popsat možnosti bezpečné léčby diabetes mellitus 2. typu u seniorů a upozornit na rizika, která může některá léčba seniorům přinášet., Type 2 diabetes mellitus represents a serious medical, social and economic problem, since it has become a pandemic in recent years, affecting up to 1/3 of the Czech population in old age. It is the clinically most significant metabolic disease in older age. A pharmacotherapeutic approach needs to be adjusted to the presence of multiple comorbidities in the elderly and to the patient’s ability to cooperate with treatment, and owing to the feared hypoglycemia which may contribute to increased morbidity, it detracts from the quality of life and limits the possibilities of treatment in many cases. When taking decisions about the treatment, the recommendations categorizing diabetic patients from the geriatric perspective as fit or fragile should be taken into account. The target values of diabetes compensation are derived from them. Cíl: Cílem studie bylo zhodnocení vlivu dlouhodobého podávání nápoje se zvýšeným obsahem syrovátkových proteinů a sníženým obsahem laktózy vyrobeného podle nové receptury (Nutrisen) seniorům žijícím v institucionalizované péči. Metodika: Studie proběhla u 47 klientů domova pro seniory v Praze starších 65 let v období od května roku 2013 do července roku 2013. Suplementovaná skupina (23 osob) konzumovala denně 200 ml mléčného nápoje ve 3 různých chuťových variantách po dobu 8 týdnů. Kontrolní skupina konzumovala běžnou stravu. V základních charakteristikách se obě skupiny významně nelišily. Byly hodnoceny antropometrické a biochemické ukazatele nutričního stavu a snášenlivost preparátu při dlouhodobém podávání. Výsledky: Snášenlivost a kompliance nutričně definovaného nápoje byla velmi dobrá (86 %). U intervenované skupiny došlo po 8 týdnech ke zvýšení tělesné hmotnosti v průměru o 700 g. Průměrná hladina albuminu a prealbuminu se významně zvýšila (začátek studie vs 8týdenní podávání): 35,5 ? 4,52 g/l vs 36,19 ? 4,1 g/l a 0,160 ? 0,05 vs 0,174 ? 0,04 g/l (p < 0,05), hladina vitaminu D se významně zvýšila z 31,2 ? 16,4 nmol/l na 36,8 ? 17,7 nmol/l (p < 0,001) a došlo k významnému vzestupu HDL-cholesterolu: 1,29 ? 0,33 mmol/l vs 1,35 ? 0,35 mmol/l (p < 0,001). Závěr: Nutričně definovaný mléčný nápoj byl v 8týdenní klinické studii seniory dobře snášen a měl příznivý vliv na vývoj hmotnosti, hladinu albuminu, prealbuminu, vitaminu D a HDL-cholesterolu., Aim: The aim of this study was to evaluate the effect of long-term administration of liquid nutritional supplement with increased amounts of whey protein and reduced amounts of lactose, produced in accordance with a new recipe “Nutrisen” on the elderly living in institutionalized care. Methods: The study was carried out from May to July, 2013, on 47 retirement home residents, living in Prague, all of which were 65 years or older. Supplemented group (n = 23) consumed (200 ml) milk drinks with three different flavours on a daily basis for eight weeks. The reference group was on a normal diet. There was no significant difference in baseline characteristics between participants in both groups. Anthropometric and biochemical indicators of nutritional status and tolerance of the nutritional supplement during long-term use were evaluated. Results: Both compliance (daily intake program) and tolerance of the nutritionally defined supplement were very good. For the supplemented group, there was an average weight increased of 700 grams after the 8 week nutritional supplement test period. Average levels of albumin and prealbumin increased significantly (from the beginning to the end of the program), 35.5 ? 4.52 g/l vs 36.19 ? 4.1 g/l and 0.160 ? 0.05 vs 0.174 ? 0.04 g/l (p < 0.05), vitamin D levels increased from 31.2 ? 16.4 nmol/l to 36.8 ? 17.7 nmol/l (p < 0.001) and HDL-cholesterol levels increased from 1.29 ? 0.33 mmol/l to 1.35 ? 0.35 mmol/l (p < 0.001). Conclusion: The specific nutritionally defined milk drink (Nutrisen), used in this study, was well tolerated by the elderly study participants, over the eight-week clinical study. Úvod: Nízkomolekulární inhibitory tyrozinkináz receptoru pro epidermální růstový faktor (EGFR) představují moderní, účinné preparáty užívané k léčbě pacientů s pokročilým nemalobuněčným karcinomem plic (NSCLC). Aktivační mutace genu EGFR predikují dobrý efekt léčby EGFR tyrozinkinázovými inhibitory. Cílem této studie bylo zmapování výskytu mutací genu EGFR u pacientů s NSCLC v České republice. Metodika: Celkem 486 pacientů s pokročilým stadiem NSCLC (stadium IIIB a IV) neskvamózního histologického typu bylo vyšetřeno na přítomnost mutace genu EGFR na třech pneumoonkologických pracovištích. Porovnání zastoupení EGFR mutace dle pohlaví, věku a kouření bylo provedeno pomocí Fisherova-exaktního testu. Výsledky: Mutace genu EGFR byla prokázána u 74 (15,2 %) pacientů. Mutace byly prokázány u 25 (8,8 %) mužů vs. u 49 (24,3 %) žen (p < 0,001), u 12 (6,7 %) kuřáků vs. u 20 (11,2 %) exkuřáků vs. u 38 (37,3 %) nekuřáků (p < 0,001) a u 32 (15,7 %) pacientů do 65 let věku vs. u 42 (14,9 %) pacientů nad 65 let věku (p = 0,898). Závěr: Mutace genu EGFR byly prokázány u 15,2 % vyšetřených pacientů, nejčastěji u žen a nekuřáků. Dosažené výsledky jsou dobře srovnatelné s doposud publikovanými daty., Introduction: Low-molecular-weight tyrosine kinase inhibitors for epidermal growth factor receptor (EGFR) are modern, effective agents used to treat patients with advanced non-small-cell lung carcinoma (NSCLC). Activating EGFR gene mutations predict a good effect of treatment with EGFR tyrosine kinase inhibitors. The goal of this study was to map the occurrence of EGFR gene mutations in patients with NSCLC in the Czech Republic. Methods: A total of 486 patients with advanced stage of NSCLC (stage IIIB and IV) of nonsquamous histological type were investigated for the presence of EGFR gene mutations at three pneumo-oncology centres. A comparison of distribution of the EGFR mutation according to sex, age, and smoking status was performed by means of Fisher's exact test. Results: EGFR gene mutation was demonstrated in 74 (15.2%) patients. Mutations were shown in 25 (8.8%) men vs. 49 (24.3%) women (p < 0.001), in 12 (6.7%) smokers vs. 20 (11.2%) ex-smokers vs. 38 (37.3%) non-smokers (p < 0.001), and in 32 (15.7%) patients under 65 years of age vs. 42 (14.9%) patients over 65 years of age (p = 0.898). Conclusion: EGFR gene mutations were shown in 15.2% of the patients examined, most commonly in female patients and non-smokers.
