Náš malý soubor operovaných pacientů vykazuje úspěšnost 90,5 %., Hyperparathyreosis is one of the diseases whose incidence has been increasing. Vast majority of hyperparathyreosis (about 80 %) is made up by adenomas. Carcinoma of parathyroid glands is very rare. Ultrasonography, scintigraphy, most frequently 99mTc-MIBI and CT are standardly being used for localization of the adenomas of parathyorid glands. Operation is a standard medical method in the process of determinating the diagnosis of primary hyperparathyreosis. Peroperative ultrasonography has irreplaceable position. 21 patients with diagnosis of adenoma of parathyroid glands which was histologically confirmed in 19 patiens have been operated in our workplace during three years. Our small aggregate of operated patients shows success rate 90,5 %., Luděk Hnízdil, Iva Procházková, Václav Jedlička, Jan Žák, Jan Kalač, Jan Wechsler, Ivan Čapov, and Literatura
Je to nový liek, ktorý poskytuje výhodu pohodlného a bezpečného perorálneho podávania proti intravenóznemu fluoropyrimidínu v PKŽ., Gastric cancer is a lethal disease and continues to be the second leading cause of cancer death worldwide. Surgical resection remains the main treatment for early stages with complete resection having the potential for a cure. Unfortunately, most patients with gastric cancer are diagnosed in advanced stages, rendering palliative systemic therapy as the only choice of treatment. The most common chemotherapy combination as a first-line treatment in advanced gastric cancer (AGC) includes a platinum compound and fluoropyrimidine. Fluoropyrimidines have been the backbone in the chemotherapy regimens for the treatment of gastric cancer. Teysuno® (S-1) is a peroral fourth-generation fluoropyrimidine that combines tegafur, which is a prodrug of 5-fluorouracil (5-FU), and two biochemical modulators: 5-chloro-2,4-dihydroxypyridine, a powerful but reversible inhibitor of dihydropyrimidine dehydrogenase that prevents 5-FU degradation, and potassium oxonate, which reduces gastrointestinal (GI) toxicity by inhibition of 5-FU phosphorylation in the GI mucosa. Teysuno® (S-1) is considered, as a single agent as the standard of care in Japan for the adjuvant treatment of resected gastric cancer and in combination with cisplatin in the advanced setting based on level 1 evidence in Phase III trials. S-1 is now widely available and has been approved in European countries with trade name Teysuno® as a first-line treatment for AGC. S-1 is generally well tolerated with a low toxicity profile. It is a novel agent that provides a convenient and safe peroral advantage over intravenous fluoropyrimidine in AGC., Tomáš Šálek, and Literatura
Expozice faktorům modifikujícím epigenom v prenatálním a časně postnatálním období formuje vnímavost ke vzniku onemocnění, v dalším životě pak modifikuje manifestaci genetické vlohy a potenciálně participuje na patogenezi. Poznatky o interakci genů, chování a prostředí potvrzují opodstatněnost primárně preventivních opatření a mohou přispět ke změně postojů ke zdraví., Non-infectious diseases represent a serious health problem in current society. Their etiology is complex as they result from a time-evolving interaction of genetic and environmental factors, a significant part of which are based on individual behaviour and lifestyle. Lifestyle can modify the impact of genetic load; nonetheless, behaviour and lifestyle are themselves determined by genes to a certain extent. Lifelong environmental influences on the genome are largely mediated epigenetically. Epigenetic mechanisms modify the activity of genes and form a base of the phenotype plasticity. Exposure to epigenetic modificators prenatally and in the early post-natal stage form susceptibility to diseases, modifiying manifestation of a genetic predisposition later in life and potentially participating in pathogenesis. Prenatally induced changes of the epigenome can be passed on to subsequent generations of offspring. Nutritional factors and smoking are major inducers of these epigenetic alterations. The most studied epigenetic mechanisms in relation to environmentally induced modification of gene expression are DNA methylation, post-translational histone modifications, and non-coding RNA regulatory activity. The potential reversibility of epigenetic processes opens up new possibilities for the prevention of complex diseases. Knowledge of the interactions of genes, behaviour and environment confirm the validity of primary preventive measures and may contribute to changing attitudes to health., Šárka Kunzová, Drahoslava Hrubá, and Literatura
Závažným zdravotním problémem rozvinutých zemí jsou chronická neinfekční onemocnění, jejichž etiologie má komplexní charakter. V prevenci i léčbě těchto chorob hrají důležitou roli faktory životního stylu. Životní styl je charakterizován vzorci chování, které komplexně působí na psychické i fyzické zdraví jedince, ovlivňují vznik a rozvoj nemocí, jejich průběh, úspěšnost léčby a kvalitu života. Chování tvořící životní styl je formováno faktory environmentálními, psychosociálními i genetickými, které vzájemně interagují a mají potenciál modifikovat genetické riziko komplexních chorob ovlivněním genové exprese. Tyto poznatky korespondují s bio-psycho-sociálním pojetím zdraví a nemoci a akcentují význam chování a možností jeho změny., Chronic non-communicable diseases with the complete determinants are serious health problems in developed countries. Lifestyle factors are important in prevention and treatment of disease. Lifestyle is characterized by patterns of behaviour and plays a comprehensive role in both mental and physical health and the quality of life. Behavioural, environmental, psychological and genetic elements form the lifestyle, are interactive and can modify the hereditary risk of complex diseases influencing genetic expression. These findings correspond to the bio-psycho-social concept of health and illness and highlight the importance of health-related behaviour and its change., and Šárka Kunzová, Drahoslava Hrubá
Historical treatment options have been limited (e.g. dacarbazine, temozolomide, high dose interleukin-2) and associated with poor outcomes. Vemurafenib is a first-in-class, small molecule BRAFV600 inhibitor. Vemurafenib is approved in the EU as monotherapy in adult patients with BRAFV600 mutation-positive unresectable or metastatic melanoma. Cigaretový kouř obsahuje unikátní množství humánních karcinogenů, včetně tabákově specifických nitrosaminů a polycyklických aromatických uhlovodíků. Tyto dvě skupiny genotoxických karcinogenů hrají společně s nikotinem významnou úlohu v promočních a progresivních stadiích karcinogeneze prostřednictvím epigenetických (ne-genotoxických) mechanismů. Z nich je pravděpodobně nejvýznamnější aktivace některých nikotin-acetylcholinových receptorů (nAChR), β-adrenoreceptorů (β-AR) a aryl-uhlovodíkových receptorů (AhR). Aktivace uvedených receptorů indukuje expresi růstových faktorů a neurotransmiterů, které ovlivňují buněčnou proliferaci, syntézu proteinů a funkci mitochondrií, inhibují apoptózu a podporují angiogenezi, invazi tumoru a tvorbu metastáz., Cigarette smoke contains unique amounts of human carcinogens, including tobacco-specific nitrosamines and polycyclic aromatic hydrocarbons. These two families of genotoxic chemicals and nicotine also play an important role in stages of tumorigenesis promotion and progression through epigenetic (non-genotoxic) mechanisms. Probably the most important pathways are activation of certain nicotinic acetylcholine receptors (nAChR), β adrenoreceptors (β AR) and aryl hydrocarbone receptors (AhR) by these chemicals. The activation of these receptors results in the release of growth factors and neurotransmitters which influence cell proliferation and protein synthesis, inhibition of apoptosis, mitochondrial dysfunction, angiogenesis, tumor invasion and metastasis., Drahoslava Hrubá, and Literatura
