The risk of high-grade lesions and carcinoma is known to correlate with specific high-risk HPV genotypes. The distribution of HPV types varies between countries and little is known about HPV distribution in our country. Therefore, the purpose of this study was to determine the range and frequency of HPV genotypes in studied group of women in Montenegro. HPV genotypes were determined using the method of enzyme restriction of PCR products amplified with group-specific primers MY09/MY11 and restricted with seven different restriction endonucleases. Out of the total number of women HPV infection was found in 1/5 of participants (20%). Genotyping performed in HPV DNA positive women shows that the HPV genotype 16 is dominant and present in more than 1/3 of the participants (36.8%). The second most frequent HPV infection is with HPV genotype 58 and it is found in 10.5% of participants. HPV 31 and HPV 6 infections are present in 7.9% of women, while infections with other genotypes were demonstrated individually by 2.6%. Cíl: Cílem práce je zhodnotit endovaskulární léčbu iatrogenních poranění renálních tepen. Metodika: Od listopadu 2005 do listopadu 2013 jsme na našem pracovišti léčili jedenáct pacientů s iatrogenním poraněním renálních tepen. Jednalo se o pět žen a šest mužů ve věku od 45 do 71 let, věkový průměr byl 57, 8 roků. K iatrogennímu poranění došlo u šesti pacientů při perkutánní extrakci konkrementu, u dvou nemocných při parciální nefrektomii, u jedné nemocné při perkutánní nefrostomii a u dvou pacientů při biopsii transplantované ledviny. U sedmi pacientů bylo příčinou hematurie pseudoaneuryzma, u tří nemocných pseudoaneuryzma s arteriovenózní pištěli a u jednoho nemocného arteriovenózní pištěl. Vždy byly poraněny periferní větve renální tepny. U všech nemocných byly k embolizaci použity spirály. Výsledky: Technická úspěšnost v našem souboru byla 100%. U všech nemocných došlo k zástavě krvácení a u nemocných s transplantovanou ledvinou se zlepšila funkce. Nedošlo k žádné závažné komplikaci. Závěr: Endovaskulární léčba iatrogenních poranění renálních tepen je jednoduchá, bezpečná a velmi účinná., Aim: To evaluate the outcome of endovascular treatment of iatrogenic injury of renal arteries. Method: From November 2005 to November 2013 we treated 11 patients with iatrogenic injury of renal arteries. There were 5 women and 6 men, aged from 45 to 71; the average age was 57.8 years. Cause of injury was percutaneous extraction of concrements in 6 patients, partial nephrectomy in 2 patients and biopsy of transplant kidney in 2 patients. All patients had hematuria. Source of hematuria was pseudoaneurysm in 7 patients, pseudoaenurysm and arteriovenous fistula in 3 patients and arteriovenous fistula in one patient. Peripheral branches were injured in all patients. Renal injury was managed by coil embolization in all cases. Results: Technical success was achieved in 100%. Clinical success (stop hematuria, hemodynamic stability, improved function of transplant kidneys) was 100%. No major complication was found. Předpoklad: Studie IMProved Reduction of Outcomes: Vytorin Efficacy International Trial (IMPROVE-IT) hodnotila potenciální prospěch na snížení velkých kardiovaskulárních (KV) příhod při přidání ezetimibu vs placebo k léčbě 40 mg simvastatinu u nemocných s akutním koronárním syndromem a nízkou hladinou LDL-cholesterolu ≤ 125 mg/dl. Metodika: Randomizovaná dvojitě slepá studie u nemocných po akutním koronárním syndromu s nízkou hladinou cholesterolu. Ve větvi se simvastatinem v monoterapii byl cílový LDL-cholesterol < 70 mg/dl, druhá větev byla kombinace simavastatin + ezetimib. Předpokladem bylo, že ezetimib sníží hladinu LDL-cholesterolu o dalších 15 mg/ml s efektem léčby kolem 8–9 %. Primární smíšený cíl byl KV úmrtí, nefatální infarkt myokardu (MI), rehospitalizace pro nestabilní anginu pectoris (NAP) a koronární revaskularizace po 30 dnech. Cílový počet příhod byl 5 250. Výsledky: Bylo zařazeno 18 144 nemocných s ST elevacemi IM (STEMI, n = 5 192) nebo non-ST elevacemi IM či NAP (NAP/nonSTEMI, n = 12 952) od října roku 2005 do července roku 2010. Primární cíl se vyskytl u 2 742 nemocných (34,7 %) na léčbě simvastatinem v monoterapii a u 2 572 nemocných (32,7 %; p = 0,016) léčených kombinací. Pacienti léčení kombinací simvastatin + ezetimib vs pacienti léčení simvastatinem + placebo měli o 6,4 % nižší kombinovaný cíl kardiovaskulární úmrtí, nefatální infarkt myokardu, nefatální cévní mozková příhoda, hospitalizace pro nestabilní anginu pectoris a revaskularizace po 30 dni. Srdeční infarkty byly sníženy o 13 %, nefatální CMP o 20 %. Úmrtí z kardiovaskulární příčiny bylo v obou skupinách stejné. Průměrná doba sledování byla 6 let, nejdelší 8,5 roku. Průměrně 2 pacienti ze 100 předešli kardiovaskulární příhodě za 7 let (Number Needed to Treat – NNT) 50/7 let. Závěr: Studie prokázala jasný prospěch z kombinační léčby simvastatinem a ezetimbem u nemocných po akutním koronárním syndromu a nízkým LDL-cholesterolem., Background: The IMProved Reduction of Outcomes: Vytorin Efficacy International Trial (IMPROVE-IT) is evaluating the potential benefit for reduction in major cardiovascular (CV) events from the addition of ezetimibe versus placebo to 40 mg/d of simvastatin therapy in patients who present with acute coronary syndromes and have low-density lipoprotein cholesterol (LDL-C) ≤ 125 mg/dl. Methods: Randomized double blind clinical trial in patients with acute coronary syndrome and low cholesterol level. The simvastatin monotherapy arm’s LDL-C target was < 70 mg/dl, the comparison arm was simvastatin + ezetimibe. Ezetimibe was assumed to further lower LDL-C by 15 mg/dl and produce an estimated ~ 8 % to 9 % treatment effect. The primary composite end point was CV death, nonfatal myocardial infarction (MI), nonfatal stroke, rehospitalization for unstable angina (UA), and coronary revascularization (≥ 30 days postrandomization). The targeted number of events was 5,250. Results: 18,144 patients were enroled with either ST segment elevation MI (STEMI, n = 5,192) or UA/non-ST segment elevation MI (UA/NSTEMI, n = 12,952) from October 2005 to July 2010. Primary endpoint occured in 2 742 patients (34.7 %) treated with simvastatin in monotherapy and in 2 572 patients (32.7 %) (p = 0.016) treated with combination. Compared to patients with coronary heart disease given the drug simvastatin plus a placebo, those given both simvastatin and the non-statin drug, ezetimibe, had a 6.4 % lower combined risk of subsequent heart attack, stroke, cardiovascular death, rehospitalization for unstable angina and procedures to restore blood flow to the heart. Heart attacks alone were reduced by 13 %, and non-fatal stroke by 20 %. Deaths from cardiovascular disease were statistically the same in both groups. Patients were followed an average of approximately six years, and some as long as 8.5 years. Approximately 2 patients out of every 100 patients treated for 7 years avoided a heart attack or stroke [Number Needed to Treat (NNT) = 50/7years]. Imunostimulace polybakteriálním lyzátem (Urivac®) v prevenci recidivujících infekcí dolních močových cest. Infekce močových cest (IMC) zaujímají v evropských zemích i u nás dlouhodobě druhé místo ve výskytu infekčních onemocnění v populaci a představují tak významný socioekonomický problém. Spolu s respiračními infekty se rozhodující měrou podílejí na preskripci antimikrobiálních přípravků. Extenzivní antimikrobiální léčba je jednou z nejvýznamnějších příčin nárůstu bakteriální rezistence. Určitou možnost prevence recidiv infekcí dolních močových cest a snížení spotřeby antimikrobiálních preparátů představuje imunostimulace. Prospektivní multicentrická studie hodnotila klinický účinek, snášenlivost a přínos k léčbě recidivujících zánětů při podávání imunostimulačního přípravku Urivac®. Počet atak infekcí dolních močových cest se po imunostimulaci významně snížil jak v jejím průběhu, tak po jejím ukončení (p = 2,2x10-16). V průběhu imunostimulace bylo bez příznaků 85 % nemocných a při dalším sledování 70,8 %. Snížil se také počet aplikovaných antimikrobiálních přípravků. Subjektivní zlepšení přetrvávalo ve více než 57 % i po ukončení léčby. Mikrobiologická odpověď dosáhla 77 %, v odstupu od ukončení léčby se snižuje. U exacerbací byla uropatogenní flóra detekována v 61 %. Nežádoucí účinky u dvou nemocných vedly k předčasnému ukončení terapeutického režimu., In European countries, including the Czech Republic, urinary tract infections (UTIs) have long occupied the second place in the incidence of infectious diseases in the population, thus representing a major socioeconomic problem. Along with respiratory infections, they are the major contributors to the prescription of antimicrobial agents. Extensive antimicrobial therapy is one of the most important causes of increased bacterial resistance. Immunostimulation is a possible way of preventing recurrent lower urinary tract infections and reducing the consumption of antimicrobial agents. A prospective multicentre study evaluated the clinical effi cacy, tolerability, and benefi t to treatment of recurrent infections with the immunostimulatory drug Urivac®. The number of attacks of lower urinary tract infections following immunostimulation reduced signifi cantly both during its course and after its cessation (p = 2.2 x 10–16). During immunostimulation, 85 % of patients were symptom-free, and during subsequent follow-up it was 70.8 %. There was associated with a reduction in the number of the antimicrobial agents administered. Subjective improvement persisted in more than 57 % even after treatment cessation. Microbiological response reached 77 %; it decreases with increased time from treatment cessation. In the case of exacerbations, uropathogenic fl ora was detected in 61 %. Úvod: Axilární disekce lymfatických uzlin (ALND) je dnes jednoznačně doporučována pouze u 3 a více pozitivních sentinelových uzlin (SLN). Není indikována u negativní sentinelové uzliny a u přítomnosti pouze izolovaných tumorózních buněk. Přínos ALND je dále zkoumán u pacientek s méně než 3 pozitivními sentinelovými uzlinami a/nebo pouze s mikrometastatickým postižením. Cíl: Posouzení indikace axilární disekce uzlin při nálezu mikrometastatického postižení sentinelové uzliny u karcinomu prsu. Metoda: Biopsie sentinelové uzliny s periareolární aplikací Sentiscintu. Soubor 265 pacientek, u 82 pozitivní sentinelová uzlina, u 17 prokázáno pouze mikrometastatické postižení, u 4 byla nalezena kombinace mikrometastatického a makrometastatického postižení dvou různých sentinelových uzlin. U 7 pacientek s pouze mikrometastatickým postižením a u všech 4 pacientek s kombinací mikrometastatického a makrometastatického postižení sentinelových uzlin byla provedena ALND. Pacientky byly následně sledovány v průměru 41 měsíců. Výsledky: U všech pacientek byly vyjmuty při SLNB alespoň dvě sentinelové uzliny. U 7 pacientek s izolovaným mikroskopickým postižením SLN a alespoň jednou další vyjmutou a negativní uzlinou při SLNB byly po ALND nalezeny v axile pouze negativní lymfatické uzliny, u zbylých 10 pacientek nebyla prokázána axilární rekurence v průběhu sledování. U poloviny pacientek s kombinovaným mikrometastatickým a makrometastatickým postižením různých SLN bylo nalezeno po ALND metastatické postižení i dalších axilárních uzlin. Závěr: Při vyjmutí pouze jedné sentinelové uzliny s nálezem izolovaného mikrometastatického postižení není možno predikovat postižení dalších axilárních uzlin a upustit od ALND. Základní ideou dekompresivní kraniektomie (DK) je chirurgické řešení konfliktu vzrůstajícího nitrolebečního obsahu s pevným objemem kostní schránky. Zvětšený nitrolebeční obsah je většinou představován edémem mozku při nejrůznějších patologických stavech. Přestože DK zažívá v posledních letech renesanci a objevily se důkazy o její účinnosti, její použití není dosud paušálně akceptováno. Nejčastější indikací DK je kraniocerebrální poranění. Důkazy o její efektivitě jsou však limitované. Jsou očekávána data o účinnosti sekundární DK u nemocných s refrakterní nitrolebeční hypertenzí po selhání konzervativní léčby. Provedení bifrontální sekundární dekomprese ještě před vyčerpáním konzervativní léčby nepřináší nemocným s difuzním poraněním benefit. Postavení primární DK u akutního subdurálního hematomu je testováno v randomizované studii. Nejsilnější důkazy o efektivitě DK jsou u nemocných s maligním hemisferálním infarktem, přesto frekvence operace stále neodpovídá incidenci maligního hemisferálního infarktu. DK by měla být zvážena u nemocných s trombózou mozkových splavů a refrakterní nitrolebeční hypertenzí, kteří mohou z operace profitovat i při oboustranné mydriáze. DK s evakuací nebo bez evakuace hematomu může snížit mortalitu nemocných s expanzivním supratentoriálním intracerebrálním krvácením, kteří jsou v bezvědomí nebo mají refrakterní nitrolebeční hypertenzi. Dekomprese může přinést benefit selektované podskupině nemocných se subarachnoidálním krvácením, její použití však není v současných doporučeních specifikováno., The fundamental idea behind decompressive craniectomy (DC) is a surgical solution to the conflict of an increasing intracranial content within the fixed-volume intracranial cavity. The expanding intracranial content is caused by brain edema caused by a variety of pathologies. In spite of DC renaissance over the recent years and some evidence of its effectiveness, its use is not yet generally accepted. Traumatic brain injury is by far the most frequent indication for DC. However, the evidence of its effectiveness is limited. Data on the role of secondary decompression in patients with refractory intracranial hypertension after conservative treatment failure are expected to be released soon. Bifrontal early DC is not superior to medical management in patients with diffuse traumatic injury. Primary decompression in patients with acute subdural haematoma has been investigated in a randomized trial. The most conclusive evidence is in patients with malignant middle cerebral artery infarction. In spite of this, the surgery is still underutilized and the frequency of its use does not correspond with the incidence of malignant infarction. When decompression is performed within 48 hours of stroke onset in patients younger than 60 years, it reduces mortality and improves functional outcome. DC is also a lifesaving procedure in patients over 60 years of age for whom it improves chances of survival without total dependency. Decompression should be considered in patients with cerebral venous thrombosis that causes intractable intracranial hypertension. Furthermore, patients with bilateral mydriasis can also profit from the procedure. DC with or without hematoma evacuation might reduce mortality in patients with large supratentorial intracerebral hemorrhage who are in coma or have refractory intracranial hypertension. Even though the DC can be effective in selected subgroups of patients with subarachnoid hemorrhage, current guidelines, do not specify the role of decompression in these patients. Cíl: Zjistit průměrnou incidenci infekční endokarditidy (IE) v okrese Tábor za sledované období a srovnat získané údaje s dříve publikovanou celorepublikovou studií. Metodika: Retrospektivní monocentrická observační studie sledující výskyt a charakteristiky infekční endokarditidy v definované spádové oblasti za roky 2009–2013. Zařazeni byli pouze pacienti s prokázanou a léčenou infekční endokarditidou a pacienti, u nichž byla diagnóza stanovena až pitvou. Výsledky: Ve sledovaném období bylo zachyceno celkem 39 případů infekční endokarditidy, což odpovídá průměrné incidenci 7,8 případů na 100 000 obyvatel ročně. Medián věku pacientů v době diagnózy byl 67 let. V souboru nemocných výrazně převažovali muži, kteří byli postiženi 3krát častěji než ženy. Přibližně stejně četně byla postižena mitrální (36 %) a aortální (33 %) chlopeň a elektroda kardiostimulačního systému (31 %). Ve srovnání s referenční prací bylo zachyceno více infekcí u pacientů s kardiostimulátory. Dvě třetiny infekcí byly způsobeny stafylokoky, což je proti pilotní studii výrazný nárůst. Naopak mortalita se podstatně neměnila. Závěr: Charakteristiky infekční endokarditidy se vyvíjejí, hospitalizační mortalita pacientů s IE je vysoká, kolem 30 %. Samo onemocnění IE je z hlediska roční úmrtnosti špatným prognostickým faktorem. Klíčová slova: incidence – infekční endokarditida, Aim: To find out an average incidence of infective endocarditis (IE) in Tabor district in a monitored period of time and compare the obtained data with previously published national study. Methods: Retrospective monocentric observational study monitoring occurence and characteristics of infective endocarditis in a defined region in years 2009–2013. Only patients with proved and treated infective endocarditis and patients whose diagnosis was revealed by autopsy were involved. Results: In the monitored time 39 cases of infective endocarditis occured, which means that the average incidence was 7.8 cases on 100 000 people per year. Median age in the time of diagnosis was 67. The group of patients was strongly dominated by men, who were affected three times more often than women. Almost the same rates of infective endocarditis were localized on mitral (36 %) and aortic (33 %) valve and electrode of a pacemaker (31 %). In comparison with the reference study there were more pacemaker-related infections detected. Two thirds of infections were caused by staphylococci. That means a prominent growth when compared to the pilot study. On the contrary, there was no significant change in mortality. Conclusion: Characteristic features of infective endocarditis are evolving, patients’ in-hospital mortality rate is high, about 30 %. The disease by itself is a poor prognostic factor in a one-year mortality rate. Key words: incidence infective – infective endocarditis, and Rudolf Koubek
Úvod: Kombinace monoklonální antiCD20 protilátky rituximabu s chemoterapií je vysoce účinná v léčbě chronické lymfocytární leukemie (CLL). Hlavním nežádoucím účinkem rituximabu jsou infuzní reakce, např. syndrom uvolnění cytokinů (cytokine release syndrome – CRS), který zpravidla doprovází první podání rituximabu. Cílem naší retrospektivní analýzy bylo zhodnotit četnost výskytu infuzních reakcí v rutinní klinické praxi, jejich vztah k parametrům nádorové nálože a zjistit možnou spojitost infuzních reakcí s účinností léčby. Dále byla posouzena bezpečnost rychlé infuze s rituximabem u nemocných s CLL. Charakteristika souboru: Hodnoceno bylo 108 nemocných s CLL léčených chemoimunoterapií s rituximabem od května roku 2005 do března roku 2011 v našem centru. V rámci 1. linie (n = 66, 71 % mužů, věkový medián 63 let) byly použity tyto léčebné režimy: fludarabin, cyklofosfamid, rituximab – FCR (n = 43) a nízkodávkovaný FCR (n = 13); 10 nemocných bylo léčeno jinými protokoly. 42 nemocných (76 % mužů, věkový medián 65 let) podstoupilo 2. linii: rituximab-dexametazon (n = 18), FCR/nízkodávkované FCR (n = 14) a 10 nemocných jiný režim. Součástí premedikace před infuzí rituximabu byl metylprednisolon 80 mg i.v., paracetamol 1 000 mg p. o. a bisulepin 1 mg i. v., všichni nemocní měli nitrožilní hydrataci minimálně 2 000 ml/den a alopurinol 300–600 mg/den. V 1. cyklu byl rituximab podáván frakcionovaně v celkové dávce 375 mg/m2 a od 2. cyklu formou rychlé infuze v dávce 500 mg/m2. Výsledky: K rozvoji infuzní reakce po rituximabu došlo u 32 % nemocných v 1. linii a 19 % v 2. linii. Jednalo se převážně o mírné reakce a k závažné toxicitě (stupeň III/IV dle NCI CTCAE – National Cancer Institute – The Common Terminology Criteria for Adverse Events) došlo výjimečně (3 % nemocných v 1. linii a 2 % v 2. linii). Reakce se nejčastěji projevila třesavkou, zimnicí, dušností, horečkou či hypotenzí. U všech nemocných bylo možné dokončit infuzi rituximabu v plánované dávce ve stejný den. Hodnocení léčebné odezvy (overall response rate – ORR) neprokázalo statisticky významné rozdíly mezi nemocnými s reakcí či bez infuzní reakce. Nemocní s infuzní toxicitou měli vyšší absolutní počet lymfocytů (1. linie, 87 vs 56 × 109/l, p = 0,21; 2. linie, 101 vs 14 × 109/l, p = 0,043). Při mediánu sledování 36 měsíců nebyl prokázán statisticky významný rozdíl v PFS (doba přežití bez progrese – progression-free survival) a OS (celkové přežití – overall survival) mezi oběma skupinami. Závěry: Výskyt infuzních reakcí u nemocných s CLL léčených rituximabem je poměrně běžný (32 %), avšak při pečlivé premedikaci jsou reakce velmi dobře zvladatelné a těžká forma je vzácná. Potvrdili jsme velmi dobrou snášenlivost rychlé infuze rituximabu. Naše data neprokázala statisticky významnou souvislost mezi výskytem infuzních reakcí a efektem léčby., Background and Aims: Rituximab in combination with chemotherapy is an effective treatment of patients (pts) with chronic lymphocytic leukemia (CLL). The most frequent adverse event of rituximab is infusion-related toxicity, e.g. cytokine-release syndrome that occurs usually during the first infusion. However, there is scarce data on feasibility and tolerability of rituximab infusions in CLL outside clinical trials. Therefore, we performed a single-center retrospective analysis of the frequency of rituximab infusion-related adverse events during the first- and the second line CLL treatment administered in the routine practice. We also analyzed its relation to parameters of tumor load and possible association with treatment efficacy. The safety of rapid infusion of rituximab in CLL pts was also evaluated. Patients and Methods: We analyzed 108 pts with CLL treated with rituximab-containing regimens between March 2005 and May 2011 at our institution. The most common first-line regimens (n = 66, 47 males, median age 63 years) were FCR (43 pts) and low-dose FCR (13 pts); 10 pts were treated by other protocols. Forty-two pts (32 males, median age, 65 years) underwent second line treatment: 18 pts rituximab-dexamethasone, 7 pts FCR, 7 pts low-dose FCR and 10 pts other regimens. Intravenous hydration (2 000 ml daily on days 0 and 1 of cycle), allopurinol 300–600 mg p. o. daily and premedication with methylprednisolone 80 mg i. v., acetaminophen 1 000 mg p. o. and bisulepine 1 mg i. v. were administered before rituximab infusion. Rituximab was given by fractionated infusion (100 mg for 2 hours, then if tolerated well, the rest of the dose with infusion rate escalation from 100 mg/hour up to 400 mg/hour) at the dose of 375 mg/m2 in the first cycle. Subsequent doses (500 mg/m2) were administered by rapid-infusion protocol. Results: Rituximab infusion-related toxicity occurred in 32 % pts (n = 21) during the first line treatment and 19% pts (n = 8) during the second line treatment. Adverse events were predominantly mild and NCI CTCAE grade III/IV occurred rarely (3% in the first line, 2% in the second line). Infusion toxicity manifested predominantly as rigors, chills, fever and hypotension. All patients with adverse events could finish rituximab infusion as initially planned on the same day. Treatment response analysis did not demonstrate statistically significant differences between patients with and without rituximab infusion toxicity. Patients who developed rituximab infusion toxicity had higher absolute lymphocyte count (first line, 87 vs 56 × 109/l, p = 0.21; second line, 101 vs 14 × 109/l, p = 0.043). At the median follow-up of 36 months, there were no statistically significant differences in PFS or OS in both cohorts. Conclusions: Rituximab infusion-related toxicity in pts with CLL is relatively frequent (32%). However, occurrence of infusion-related symptoms can be reduced by proper premedication and severe adverse events are uncommon. In our experience, all patients were able to receive the planned dose of rituximab. Subsequent doses of rituximab could be safely administered by rapid-infusion protocol. We assessed the intensive care unit (ICU) patients for Invasive aspergillosis (IA) with culture and non-culture based diagnostic methods from Iran. Thirty-six ICU patients with underlying predisposing conditions for IA were enrolled in the study. Sixty eight Bronchoalveolar lavage (BAL) samples were collected by bronchoscope twice a weekly. BAL samples were analyzed by microscopic examination, fungal culture and galactomannan (GM) detection. The Platelia Aspergillus GM EIA was used to quantify GM indices. Samples with a BAL GM index > or = 1 were considered as positive for GM. Patients were classified as having probable or possible IA. Out of 36 suspected patients to IA, 36.1% of cases showed IA which were categorized as: 4 cases of possible IA and 9 of probable IA. 76.2% of BAL samples were positive for GM. From 13 patients with IA, 11 (84.6%) had at least one positive BAL GM index. Of these patients, 9 (81.8%) showed probable IA. The main underlying predisposing conditions were neutropenia (53.8%) and COPD (30.8%). Our study has indicated that COPD must be considered as one of the main predisposing condition for occurrence of aspergillosis in ICU patients. Cíl: Cílem studie bylo zhodnotit přínos elektrofyziologického vyšetření v diferenciální diagnostice lumbální spinální stenózy (LS) a diabetické polyneuropatie (DPN). Soubor a metodika: Elektrofyziologické vyšetření bylo provedeno u 68 pacientů s klinicky symptomatickou LS, u 28 pacientů s DPN a 32 zdravých dobrovolníků. Výsledky: Vyšetřené elektrofyziologické parametry z horních končetin (latence F vlny n. ulnaris a amplituda senzitivního neurogramu n. radialis), všechny hodnocené latence z dolních končetin (F vlny n. tibialis, H reflexu m. soleus, spinální latence MEP) a amplituda senzitivního neurogramu n. suralis signifikantně odlišovaly pacienty s LS a DPN. ROC analýza nicméně prokázala pouze dva elektrofyziologické parametry jako efektivní nezávislé faktory pro odlišení pacientů s LS a DPN: latenci ulnarisové F vlny (optimální diskriminační hodnota je 24,2 ms, senzitivita 82,7 % a specificita 63,9 %) a amplitudu senzitivního neurogramu n. radialis (optimální diskriminační hodnota je 10,5 ?V, senzitivita 75,5 % a specificita 58,2 %). Vícerozměrná diskriminační analýza poskytla kanonické skóre s nejsilnější prediktivní hodnotou. Závěry: Elektrofyziologické vyšetření z horních končetin je užitečné v diferenciální diagnostice LS a DPN, nicméně nejlepší diskriminační efekt vykazuje kanonické skóre, které zahrnuje několik elektrofyziologických parametrů. Methods: Electrophysiological parameters were examined in 68 patients with clinically symptomatic LS, in 28 patients with DPN, and in a group of 32 healthy volunteers. Results: Electrophysiological parameters evaluated from the upper extremities (F-wave latency of the ulnar nerve and radial SNAP amplitude), all the latencies measured in the lower extremities (tibial F-wave, soleus H-reflex and spinal latency of MEP), and the sural SNAP amplitude contributed significantly to distinguishing the LS from DPN patients. ROC analysis, however, disclosed only two electrophysiological parameters as effective in independent discrimination between the LS and DPN patient groups: ulnar F-wave latency (cut-off point at 24.2 ms, sensitivity 82.7% and specificity 63.9%); and radial SNAP amplitude (cut-off point at 10.5 ?V, sensitivity 75.5%, specificity 58.2%). Multivariate discrimination provided a canonical score with the most powerful predictive value of all.
