Cíl: předvést praktické použití softwarové techniky určené k analýze variance - one-way ANOVA - ke dvěma účelům verifikačního procesu: k odhadu nejistoty analytické metody dle dokumentu CLSI (Clinical and Laboratory Standards Institute) C51-A Vol. 32 No. 4 „Expression of Measurement Uncertainty in Laboratory Medicine, Approved Guideline“, 2012 [1] a současně ke zjednodušení postupu verifikace preciznosti měřicího postupu dle CLSI EP15-A2 Vol. 25 No.17 „User Verification of Performance for Precision and Trueness; Approved Guideline, Second Edition“ [2]. Výsledky: aplikace obou normativních postupů je předvedena v praktickém příkladu odhadu nejistoty jedné koncentrační hladiny látkové koncentrace urey v lidském séru (měřená veličina) a současně ke zjednodušenému postupu verifikace preciznosti tohoto měřicího postupu. Verifikační experiment odpovídá dokumentu EP15-A2 a měření se prováděla na jedné koncentrační hladině měřené veličiny vždy ve třech opakováních v průběhu pěti dní. Cíl - diskuse: předvést možnost zjednodušení verifikačního postupu. Závěr: Pomocí techniky one-way ANOVA lze současně provést výpočet nejistoty měření i mezilehlé (celkové) preciznosti., Objective: To demonstrate the practical use of the „one-way ANOVA“ technique for the expression of measurement uncertainty in laboratory medicine as well as to simplify the process of the user verification of performance for precision according, according to documents CLSI (Clinical and Laboratory Standards Institute), titled as C51-A Vol.32 No.4 „Expression of Uncertainty in Measurement Laboratory Medicine, Approved Guideline“, 2012 [1], next only the C51-A, and according to CLSI EP15-A2 Vol. 25 No.17 document titled as „User verification of Performance for Precision and trueness; Approved Guideline, Second Edition“ [2], hereinafter EP15-A2. Results: The application of both normative practices is demonstrated in the practical example use for the uncertainty method estimation on one concentration level of the urea molar concentration in the human serum (measurand), together with the simplified procedure of the user verification of the method performance for the precision. The verification experiment matches the document EP15-A2, i.e. one measurand level running in 3 replicates on each of the 5 days. The calculation of the measurement uncertainty and the intermediate precision is processed by analyzing components of the variability in the „one-way ANOVA“ technique. Obrovskobuněčné arteritida je řazena mezi vaskulitidy velkých cév. Jejím prvním projevem může být horečnatý stav s elevací markem zánětu (FW a CRP), v rámci vyšetřování takových nemocných lze využít PET (PET/CT) vyšetření. Cíl. Ověření, zda vysoká akumulace FDG ve stěnách velkých tepen odpovídá obrovskobuněčné arteritidě. Metoda. Zařazeno bylo 39 nemocných s podezřením na vaskulitidu velkých tepen na základě PET (25 pacientů) a PET/CT (14 pacientů), jednalo se o 30 žen a devět mužů ve věku 45-81 let, průměr 64,9 let, medián 65 let. K potvrzení byl zvolen přímý průkaz histologickým vyšetřením z excize tepny (4 pacienti) nebo nepřímý - terapeutický test poklesem FW a CRP při kortikoterapii v imunosupresivních dávkách (39 pacientů). Akumulace FDG byla měřena v sedmi cévních oblastech, pokud přesahovala akumulaci referenčního jaterního parenchymu, byla oblast hodnocena jako pozitivní. Kontrolním souborem byl vzorek 100 pacientů bez vaskulitidy. Porovnávány byly výsledky dalších zobrazovacích vyšetření. Výsledky. Obrovskobuněčné arteritis byla ověřena u tří ze čtyř histologických vyšetření. Nepřímým testem byla potvrzena vaskulitida u všech 39 pacientů. U pacientů s vaskulitidou před nasazením kortikoterapie byla nalézána pozitivita alespoň tří cévních oblastí, zatímco v kontrolním souboru byla nalézána pozitivita 0-2 cévních oblastí. Další zobrazovací vyšetření jasné známky vaskulitidy nedetekovala, mohla být hodnocena jako abnormní. Závěr. Nález generalizované vysoké akumulace FDG ve velkých tepnách u nemocných se známkami aktivního zánětlivého onemocnění svědčil pro vaskulitidu velkých tepen. PET (PET/CT) vyšetření detekovalo onemocnění v časné fázi a určovalo i jeho rozsah. Přímé zobrazení postižení spánkových tepen se zdařilo až za použití PET/CT skeneru., Aim. To verify if high FDG uptake in the walls of large vessels is corresponding with giant cell arteritis. Method. Based on PET (25 pts) and PET/ CT (14 pts) examinations, 39 patients with suspicion of large-vessel vasculitis formed a cohort in which were 30 males and 9 females aged 45-81 with a mean age of 64.9 and median of 65 years. For verifying, a direct proof using histologie examination of a vessel excision (4 pts) or an indirect one - a therapeutic test for ESR and CRP decreases during corticotherapy in immunosuppressive doses (39 pts) were opted. FDG uptake was measured in 7 vessel areas with an area evaluated as positive when it surpassed an uptake of the liver parenchyma. As a control group a sample of 100 patients free from vasculitis was used. For comparison, results from further imaging examinations were used. Results. Giant cell arteritis was proved in 3 from 4 histologie examinations. By indirect testing, vasculitis in all 39 patients was confirmed. In patients with vasculitis before the use of corticotherapy, positivity of at least 3 vessel areas was found, whereas in the control group positivity was observed in 0-2 vessel areas. Further imaging examinations did not detect clear symptoms of vasculitis, it could be evaluated as abnormal. Conclusion. The finding of generalized high FDG uptake in large vessels in patients with symptoms of active inflammatory disease gave evidence for large-vessel vasculitis. The PET (PET/CT) examinations detected the disease in its early phase and also determined the extent thereof. Hyperparatyreóza (HPT) patří k onemocněním, jejichž incidence stoupá. Převážnou většinu primárních hyperparatyreóz (asi 80 %) tvoří adenomy. Karcinom příštítných tělísek je velmi vzácný. K lokalizaci adenomů příštítných tělísek se standardně používá ultrasonografie, scintigrafie, nejčastěji 99mTc-MIBI a CT. Při stanovení diagnózy primární hyperparatyreózy je operační výkon standardním léčebným postupem. Nezastupitelné postavení má peroperační ultrasonografie. V průběhu tří let bylo na našem pracovišti operováno celkem 21 pacientů s diagnózou adenom příštítného tělíska, který byl histologicky potvrzen u 19 pacientů. Náš malý soubor operovaných pacientů vykazuje úspěšnost 90,5 %., Hyperparathyreosis is one of the diseases whose incidence has been increasing. Vast majority of hyperparathyreosis (about 80 %) is made up by adenomas. Carcinoma of parathyroid glands is very rare. Ultrasonography, scintigraphy, most frequently 99mTc-MIBI and CT are standardly being used for localization of the adenomas of parathyorid glands. Operation is a standard medical method in the process of determinating the diagnosis of primary hyperparathyreosis. Peroperative ultrasonography has irreplaceable position. 21 patients with diagnosis of adenoma of parathyroid glands which was histologically confirmed in 19 patiens have been operated in our workplace during three years. V roce 2013 vysoké české soudy již vydaly druhé zásadní rozhodnutí ve věci regulačních poplatků podle § 16a zákona č. 48/1997 Sb., o veřejném zdravotním pojištění a o změně a doplnění některých souvisejících zákonů, ve znění pozdějších předpisů. První rozhodnutí vydal jako své usnesení vedené pod sp. zn. Konf 38/2012 zvláštní senát zřízený podle zákona č. 131/2002 Sb., o rozhodování některých kompetenčních sporů. Účelem tohoto rozhodnutí pak bylo rozhodnout o právní povaze regulačních poplatků. Druhé rozhodnutí pak vydal Ústavní soud ČR., In the spring 2013 the special tribunal consisting of judges of the Supreme Court and the Supreme Administrative Court resolved a conflict of competencies between civil court and health insurance company. The dispute was who should decide on the obligation to pay regulatory fees under the Public Health Insurance Act. The special tribunal concluded that the competent authority is a civil court. Alzheimerova choroba (ACH) je nejčastější forma demence. Jedná se o degenerativní, nevyléčitelné, terminální onemocnění. Postihuje až 75 % nemocných na celém světě postižených demencí. Prevalence tohoto onemocnění je vysoká, etiologie stále není známá. Je sledována řada rizikových faktorů. Pilotní studie projektu Epidemiologie a genetika Alzheimerovy choroby s 334 případy a 102 kontrolami z psychiatrických léčeben přináší následující výsledky z dotazníkového šetření a stěru z bukální sliznice. Dotazník se skládá z otázek zaměřených na základní identifikační údaje, vzdělání, životní styl, vaskulární rizikové faktory, zájmy, zaměstnání, výskyt nemoci v rodině, výsledky Minimental State Examination (MMSE). Dále jsou uvedeny výsledky analýzy polymorfizmů genu pro angiotenzinkonvertázu (ACE). Ve výsledcích je naznačeno, že u pacientů s ACH se v anamnéze častěji vyskytuje kardiovaskulární onemocnění a úraz hlavy. Výsledky analýz vztahu polymorfizmu I/D genu pro ACE k Alzheimerově chorobě odpovídají dosud publikovaným výsledkům, tzn., že genotypy II a ID mají vztah k riziku ACH, i když zatím nebyl zjištěn statisticky významný rozdíl ve sledovaných skupinách respondentů., Alzheimer's disease (AD) is the most common form of dementia. It is a degenerative incurable terminal disease. AD affects a high percentage (75%) of persons with dementia all over the world. Prevalence is very high. Etiology is still unknown. Numerous etiological factors of AD have already been discovered. Pilot evaluation of 334 cases and 102 controls from mental hospitals in project Epidemiology and Genetics of Alzheimer's Disease provided following the results from questionaires and buccal mucosa smears. The questionnaires contain questions about identification, education, lifestyle, vascular risk factors, hobbies, occupation, incidence of the disease in the family, results of Minimental State Examination (MMSE).Results of the analysis of the gene of I/D polymorphism of ACE are presented. Results suggest that persons with AD have often a cardiovascular disease and head injury in their case history. Úvod: Maligní mezenchymové nádory jsou v orofaciální oblasti daleko vzácnější než karcinomy. Představují pouze 1 % z celkového počtu malignit s více než 50 podtypy. Angiosakomy jsou vysoce maligní nádory vycházející z krevních nebo lymfatických cév. Jejich výskyt je nízký až raritní s nejistou prognózou. Z celkového počtu sarkomů tvoří angiosarkom 5,4 % a v oblasti hlavy a krku se vyskytuje nejčastěji. Rámcový staging a grading se odvíjí dle FNCLC a NCI. Cíl práce: Cílem sdělení je upozornit na vzácné maligní onemocnění v oblasti hlavy a krku, které může být diagnosticky opomenuto či nerozpoznáno s fatálními následky pro nemocného. Materiál, metodika, výsledky: Autoři popisují klinické projevy onemocnění, diagnostiku a následnou terapii angiosarkomu mandibuly u zdravotně vysoce rizikového pacienta. V diagnostice tohoto vzácného onemocnění byly vedle histopatologického obrazu včetně imunohistochemického vyšetření využity i onkologické markery. Terapie byla zaměřena na chirurgickou léčbu a radioterapii. Závěr: Publikované zkušenosti s léčbou malých souborů nemocných v odborné literatuře neumožňují stanovit optimální terapeutický postup. Průměrná doba přežití je rok a půl., Introduction: Malignant mesenchymal tumours of the orofacial region are extremely rare and comprise only 1% of all malignant orofacial tumours with more than 50 subtypes. Angiosarcomas are highly malignant neoplasms that originate from blood or lymphatic vessels. Angiosarcomas form 5,4 % of all sarcomas and in the head and neck region are the most frequent type of sarcoma. Aim: The aim of this case report is to describe a rare malignant neoplasm in the head and neck topography which can be diagnostic difficulty and may be missed or unrecognised with fatal consequences for the patient. Materials, methods, results: The authors describe the clinical manifestations of the disease, the diagnosis and therapy of mandibular angiosarcoma in a seriously ill patient. The diagnosis was made on the basis of the histopathological findings and immunohistochemistry. The following treatment was radical surgery with subsequent radiotherapy and 18 months later, the 84 year old female patient remains alive and apparently healthy. Conclusions: Angiosarcoma has currently been literary described only in small number of patients. Kvalita ţivota je ovlivněna zdravotním stavem, který zahrnuje fyzické, psychické a sociální charakteristiky. Největším zdravotním, ale i společenským problémem současné epochy nejsou nemoci akutní, ale nemoci dlouhodobé, chronické a trvalé. Výzkum se zaměřuje na oblast sexuality u pacientů s hypertenzí, s důrazem na holistické pojetí lidské osobnosti, hlubší vnímání a proţívání sexuálního ţivota, sexuálních potřeb pacientů. Cíl: Cílem kvalitativního šetření bylo zjistit, zda léčba hypertenze ovlivňuje sexuální potřeby pacientů. Metodika: Výzkumný soubor tvořilo 20 respondentů, výběr zkoumaného vzorku byl záměrný, byli to pacienti, kteří se léčili pro hypertenzi minimálně 1 rok ve Fakultní nemocnici Ostrava. Výzkum byl proveden pomocí kvalitativní metody, sběr dat byl proveden rozhovorem, který byl nahráván na diktafon v příjemném, nerušeném prostředí. Ve výzkumu byla pouţita metoda barvení textu s definovanými 4 kategoriemi a podkategoriemi. Výsledky: Z analýzy rozhovorů vyplynulo, ţe u některých pacientů antihypertenzní terapie negativně ovlivnila jejich sexuální potřeby. Byl zjištěn výskyt sexuálních dysfunkcí, u ţen se jednalo o sníţený sexuální apetit, sníţenou lubrikační schopnost, delší interval k vyvrcholení a u muţe byl objeven problém s erekcí. Závěr: V rámci subjektivní spokojenosti s vlastní sexualitou u pacientů, kteří se léčí s arteriální hypertenzí je důleţitá znalost vedlejších účinků a následná edukace s vyuţitím výsledků vědeckých studií domácích i zahraničních odborníků., The quality of life is influenced by health condition, which include physical, psychological and social features. The biggest, not only medical but also social problem nowadays are not acute diseases but long-term diseases, chronical and permanent. The research is concentrated on the sphere of sexuality among patients with hypertension, with stress on holistic concept of human being, deeper perception and experiencing of sexual life and sexual needs of patients. Aim: The aim of qualitative research was to find out how the treatment of hypertension influences sexual needs of patients. Methods: Researched sample was formed by 20 respondents. The choice of researched sample was intentional. They were patients who were treated for hypertension minimally one year in University Hospital Ostrava. The choice of researched sample was intentional. The research was done by qualitative method, collection of datas was done by interview, which was recorded on voice recorder in a pleasant, by nothing disturbed place. In the research was used method of coloring the text with 4 defined categories and subcathegories. Results: From the analysis of interviews was clear that at some patients antihypertension therapy influenced their sexual needs in a negative way. There was found out the occurrence of sexual dysfunctions. Among women it was lower sexual appetite, lower lubricative ability, longer time for climax and among men was found the problem with erection.
