Karcinóm žalúdka je smrteľná choroba a je stále druhá najčastejšia príčina úmrtí na rakovinu na celom svete. Chirurgický zákrok s kompletnou resekciou tumoru, ktorá má kuratívny potenciál, zostáva hlavnou terapiou pre skoré štádiá. Bohužiaľ, väčšina pacientov s rakovinou žalúdka je diagnostikovaná v pokročilom štádiu, čím paliatívna systémová liečba zostáva jedinou liečebnou možnosťou. Najčastejšia kombinácia chemoterapie ako liečby prvej línie na liečbu pokročilého karcinómu žalúdka (PKŽ) zahŕňa zlúčeniny platiny a fluoropyrimidínov. Fluoropyrimidíny sú chrbtovou kosťou chemoterapeutických režimov pre liečbu karcinómu žalúdka. Teysuno® (S-1) je perorálny štvrtogeneračný fluoropyrimidín, v ktorom sa kombinuje tegafur, čo je prekurzor 5-fluorouracilu (5-FU) a dva biochemické modulátory: (1) 5-chlór-2,4-dihydroxypyridín, čo je silný, ale reverzibilný inhibítor dihydropyrimidíndehydrogenázy, ktorá bráni degradácii 5-FU, a (2), oxonát draselný, ktorý znižuje gastrointestinálnu (GI) toxicitu inhibíciou 5-FU fosforylácie v gastrointestinálnej sliznici. Teysuno® (S-1) ako monoterapia je štandard na adjuvantnú liečbu resekovaného karcinómu žalúdka v Japonsku a v kombinácii s cisplatinou v pokročilom ochorení na základe úrovne 1 dôkazu v klinických štúdiách fázy III. S-1 je teraz široko dostupný a bol schválený v európskych krajinách pod obchodným názvom Teysuno® ako liečba prvej línie pri PKŽ. S-1 je všeobecne dobre znášaný a vykazuje nízky toxický profil. Je to nový liek, ktorý poskytuje výhodu pohodlného a bezpečného perorálneho podávania proti intravenóznemu fluoropyrimidínu v PKŽ., Gastric cancer is a lethal disease and continues to be the second leading cause of cancer death worldwide. Surgical resection remains the main treatment for early stages with complete resection having the potential for a cure. Unfortunately, most patients with gastric cancer are diagnosed in advanced stages, rendering palliative systemic therapy as the only choice of treatment. The most common chemotherapy combination as a first-line treatment in advanced gastric cancer (AGC) includes a platinum compound and fluoropyrimidine. Fluoropyrimidines have been the backbone in the chemotherapy regimens for the treatment of gastric cancer. Teysuno® (S-1) is a peroral fourth-generation fluoropyrimidine that combines tegafur, which is a prodrug of 5-fluorouracil (5-FU), and two biochemical modulators: 5-chloro-2,4-dihydroxypyridine, a powerful but reversible inhibitor of dihydropyrimidine dehydrogenase that prevents 5-FU degradation, and potassium oxonate, which reduces gastrointestinal (GI) toxicity by inhibition of 5-FU phosphorylation in the GI mucosa. Teysuno® (S-1) is considered, as a single agent as the standard of care in Japan for the adjuvant treatment of resected gastric cancer and in combination with cisplatin in the advanced setting based on level 1 evidence in Phase III trials. S-1 is now widely available and has been approved in European countries with trade name Teysuno® as a first-line treatment for AGC. S-1 is generally well tolerated with a low toxicity profile. Úvod: Cílem prospektivní randomizované studie bylo porovnání tahové cerkláže a hákovité dlahy při léčbě AC luxací, především z hlediska funkčních a radiologických výsledků. Metoda: Soubor tvořilo 80 pacientů s akutní akromioklavikulární (AC) luxací typu 3, 4 a 5 Rockwoodovy klasifikace. Diagnóza byla stanovena na základě klinického (defigurace a instabilita) a rtg vyšetření (AP a zátěžové rtg). Tahovou cerkláží (TC) bylo ošetřeno 40 pacientů a hákovitou dlahou (HD) rovněž 40 pacientů. Hodnocení bylo prováděno v průběhu jednoho roku po operaci na základě rtg snímků a Constant score. Výsledky: Constantovo skóre 3 měsíce od operace činilo průměrně u TC 84 bodů a u HD 88 bodů. Po 1 roce od operace byl výsledek shodně 93 bodů u obou skupin. V souboru HD se hodnota skóre zvýšila mezi 2 a 4 týdny od operace z 56 na 78 bodů. V 71 případech bylo pooperační postavení AC kloubu a implantátu hodnoceno jako správné. U 6 případů jsme nalezli malpozici Kirschnerových drátů a ve 3 případech horizontální rozšíření AC kloubu. Redislokace v rozsahu 50–100 % šíře kosti akromia byla na rtg snímku patrná u 4 pacientů (10 %) ze skupiny TC a u 5 pacientů (13 %) ze skupiny HD. Viditelnou osteolýzu distální plochy akromia jsme nalezli u 83 % pacientů s HD. Komplikace jsme zaznamenali u 30 % pacientů s TC a u 5 % pacientů s HD. Závěr: Hákovitá dlaha i tahová cerkláž jsou srovnatelnými metodami při léčbě AC luxace hodnocené radiologicky a klinicky po 3 měsících a 1 roce od operace. Hákovitá dlaha má nižší počet komplikací a umožňuje časnější plnou zátěž a hybnost než TC. U TC doporučujeme odstranění implantátu za 8 týdnů, dobu 6 týdnů považujeme za příliš krátkou pro zhojení měkkých tkání. U HD je vhodné odstranění do 3 měsíců vzhledem k subakromiálnímu dráždění a tlakové osteolýze., Introduction: The aim of the prospective randomized study was to compare tension wire cerclage and hook plate in the treatment of AC dislocation, primarily from the viewpoint of functional and radiological results. Method: The cohort comprised 80 patients with acute acromioclavicular (AC) dislocation of types 3, 4 and 5 of Rockwood classification. The diagnosis was based on the clinical (disfiguration and instability) and radiographic examination (AP and stress radiograph). Forty patients were treated with tension band wiring (TBW) and another 40 with a hook plate (HP). Evaluation was performed during one year after the surgery based on radiographs and the Constant score. Results: The mean Constant score 3 months after the surgery was 84 points for TBW and 88 points for HP. One year after the surgery, the result was the same in both groups: 93 points. In HP group the score increased from 56 to 78 points between 2 and 4 weeks from the surgery. In 71 cases the postoperative position of the AC joint and implant was assessed as correct. Malposition of Kirschner wires was recorded in 6 cases and horizontal widening of the AC joint in 3 cases. Redislocation of up to 50−100% of the width of acromion was shown by radiograph in 4 TBW patients (10%) and in 5 HP patients (13%). A visible osteolysis of the distal surface of acromion was found in 83% of patients with HP. Complications were recorded in 30% of TBW patients and in 5% of HP patients. Conclusion: Based on radiological and clinical results assessed 3 months and 1 year after the surger, the hook plate and tension band wiring are comparable treatment methods for AC dislocation. The hook plate is associated with a lower complication rate and allows earlier full weight bearing and mobility than tension wire cerclage. In TBW we recommend to remove the implant after 8 weeks; 6 weeks are in our view too short a period for the healing of soft tissues. Od roku 1999 do roku 2003 bylo na Neurochirurgické klinice 1. LF UK, IPVZ a ÚVN Praha operováno celkem 277 nemocných s adenomem hypofýzy. Z tohoto počtu bylo 108 pacientů s hormonálně aktivními adenomy hypofýzy. K normalizaci nebo k dostatečnému poklesu hormonální hladin došlo u 61 pacientů po první operaci. V 47 případech nebyl hormonální pokles po první operaci dostatečný. Celkem v 18 případech byla indikována sekundární operace, pooperační medikamentózní léčba byla indikována u 17 pacientů, na Leksellově gama noži bylo ozářeno 34 pacientů. Po transsfenoidální operaci byla mortalita nulová, morbidita byla zaznamenána u 2 pacientů (1× trvalé zhoršení zraku, 1× likvorea). Trvalá substituce adiuretinem je nutná u 7 pacientů., There had been treated 277 patients with pituitary adenoma in period from 1999 to 2003 at Department of Neurosurgery, Central Military Hospital, Prague. Altogether there had been treated 108 patients with hormone-secreting adenomas. Normalization or sufficient hormone level decrease was observed in 61 patients after the first surgery. Hormone level decrease was not sufficient in remaining cases. Second surgery was performed in 18 cases, medicamental treatment was indicated in 17 cases, radiosurgery was applied in 34 cases. There was no mortality after transsphenoidal surgery, morbidity was observed in 2 cases (visual impairment in 1 case, CSF leak in 1 case). Autoři prezentují sestavu 76 nemocných radikálně operovaných pro karcinom hlavy pankreatu a analyzují pooperační komplikace a přežívání. Materiál a metodika: V období 1/2006–9/2010 podstoupilo 76 nemocných potenciálně kurativní hemipankreatoduodenektomii pro adenokarcinom hlavy pankreatu. Pacienti jsou prospektivně sledováni, diskutovány jsou komplikace a přežití. Výsledky: 30denní pooperační mortalita je 2,6 %. Na základě TNM klasifikace 11 % mělo stadium I, 78 % stadium II a 3 % stadium III. Nejzávažnější komplikací byla insuficience pankreatojejunální anastomózy (6,5 %) a opožděná evakuace žaludku (11,8 %). Ve stadiu I přežívá 1., 2., 3. a 4. rok 82 %, 52 %, 35 % a 35 % operovaných. Závěr: Resekce hlavy pankreatu je bezpečná operace s nízkou mortalitou, ale neuspokojivým dlouhodobým přežíváním. Toto je zřejmě způsobeno agresivní biologickou aktivitou nádoru a pozdní diagnostikou., The authors refer seventy six patients radically operated for pancreatic head carcinoma and analyzed postoperative complications and survival. Material and Methods: During 1/2006–9/2010 76 patients underwent potentially curative hemipancreato-duodenectomy for adenocarcinoma of the head of pancreas. The patients are prospectively followed, complications and survival are discussed. Results: The 30-days postoperative mortality was 2,6 %. According to TNM classification 11 % of patients had stage I, 78 % stage II and 3 % stage III. Most severe complications were leak from pancreato-jejunal anastomosis (6,5 %) and delayed gastric emptying (11.8 %). The survival analyses showed 1,2,3 and 4 years survival as follows: 82 %, 52 %, 35 %, 35 % respectively. Conclusion: Resection of the pancreatic head is a safe procedure with low mortality but disappointing long-term survival. Úvod: Perkutánní punkční cholecystostomie je považována za výkon v nouzi při selhávání konzervativní léčby těžké akutní cholecystitidy u starých a kriticky nemocných pacientů. Otázkou je, nakolik je v současnosti toto řešení aktuální či obsoletní. Metody: Retrospektivně byla analyzována data pacientů, kteří podstoupili v období 1/2010−1/2015 punkční cholecystostomii pod kontrolou počítačové tomografie (CT). Sledována byla data pacientů, úspěšnost a komplikace výkonu, krátkodobé a dlouhodobé výsledky. Výsledky: Punkční cholecystostomie pod CT navigací byla na Chirurgické klinice 2. LF UK a FNM za sledované období provedena u 30 pacientů. V souboru bylo 21 žen (70 %), 9 mužů (30 %), průměrný věk 78 let (SD?12,3), medián 82 let (34–93 let). Indikací výkonu byla těžká akutní cholecystitida, respektive empyém žlučníku nereagující na konzervativní léčbu u rizikových pacientů aktuálně neúnosných pro operační výkon. Primárně byla punkce úspěšná u 23 pacientů (77 %), selhala u 7 pacientů (23 %) s nutností opakované punkce u jedné pacientky a urgentní cholecystektomie u 6 nemocných. Průměrná doba hospitalizace byla 16,5 dne (SD?8,2), medián 15 dnů (7–49 dnů). Celková 30 denní mortalita byla 17 %, mortalita v souvislosti s primární indikací k výkonu byla 10 %. Recidivu onemocnění jsme zachytili u 3 pacientů (10 %). U jednoho byla řešena opakovanou drenáží, u druhého konzervativní terapií a u třetího akutní cholecystektomií. U jednoho pacienta (3 %) byla provedena odložená laparoskopická cholecystektomie bez komplikací. Závěr: Perkutánní cholecystostomie pod CT kontrolou u pacientů neúnosných k operaci představuje efektivní a bezpečnou terapeutickou možnost., Introduction: Percutaneous cholecystostomy is considered to be an emergency treatment option when conservative treatment of acute cholecystitis fails in elderly and critically ill patients. The question is: to what extent is this technique still up-to-date or obsolete. Methods: We retrospectively reviewed data of patients who underwent a computer tomography (CT) guided percutaneous cholecystostomy between 1/2010−1/2015. We analyzed the patient data, the success rate, complications of the procedure, short- and long-term outcomes. Results: 30 patients undergoing CT-guided percutaneous cholecystostomy at the Department of Surgery, 2nd Faculty of Medicine, Charles University in Prague and Motol University Hospital during the study period were enrolled. The study group included 21 females (70%) and 9 males (30%) with mean age of 78 years (SD?12.3), median 82 years (range 34−93 years). Percutaneous cholecystostomy was indicated for patients with severe cholecystitis/empyema of the gallbladder not responding to conservative therapy who were poor candidates for operative cholecystectomy. Of these, 23 patients (77%) were successfully treated with initial percutaneous cholecystostomy whereas 7 patients (23%) experienced treatment failure – one was subsequently successfully treated with repeated percutaneous cholecystostomy and six underwent emergency cholecystectomy. The mean length of stay was 16.5 days (SD ?8.2), median 15 days (7–49 days). The total 30-day mortality was 17%, and indication-related mortality was 10%. Three patients (10%) had a recurrence. One patient required repeated percutaneous drainage, the second recovered on conservative treatment and the third patient underwent acute cholecystectomy. Only one patient (3%) underwent delayed laparoscopic cholecystectomy without complications. Metastatický melanom je agresivní onemocnění. Dosud využívané léčebné možnosti jako aplikace dakarbazinu, temozolomidu, high dose interleukinu-2 byly relativně omezené. Vemurafenib je malá molekula, která má schopnost inhibovat mutovanou BRAF proteinkinázu. Na základě provedených klinických studií BRIM-2 a BRIM-3, kdy vemurafenib dokázal prodloužit celkové přežití (OS) a dobu do progrese onemocnění (PFS), byl zaregistrována v EU k léčbě v monoterapii u dospělých pacientů s neresekovatelným nebo metastazujícím melanomem s pozitivní mutací V600 genu BRAF., Metastatic melanoma is an aggressive disease. Riziko CNS progrese u vysoce agresivních lymfomů (lymfoblastový a Burkittův lymfom) dosahovalo v historických souborech až 30%, podařilo se jej však výrazně snížit pomocí intratekální profylaktické terapie a inkorporací systémových cytostatik prostupujících hematoencefalickou bariéru do léčebných protokolů. U ostatních agresivních lymfomů je riziko CNS progrese nižší (pod 10 % v recentních studiích), avšak u vybrané skupiny nemocných se může i zde pohybovat kolem 25–30 %. Bohužel, na rozdíl od lymfoblastového a Burkittova lymfomu se u jiných agresivních histologií nepodařilo profylaktické úspěchy zopakovat. Tento článek se zabývá incidencí, rizikovými faktory a možnostmi profylaxe CNS relapsu u imunokompetentních pacientů s difuzním velkobuněčným B-lymfomem, lymfomem z buněk pláště, periferními T-lymfomy a primárním mediastinálním B-lymfomem. Zmíněny jsou rovněž možnosti terapie CNS progrese, pokud nastane., The risk of CNS progression in highly aggressive lymphomas (lymphoblastic, Burkitt‘s) reached up to 30% in historical cohorts, but it was significantly diminished by intrathecal prophylactic treatment and by incorporation of systemic cytostatics penetrating blood-brain barrier into treatment protocols. In other aggressive lymphomas, the risk of CNS progression is lower (under 10% in recent studies), but in selected patients can even here reach 25–30 %. Unfortunately, unlike in lymphoblastic and Burkitt‘s lymphomas, prophylaxis does not seem to be particularly effective in other histologies. This article discusses incidence, risk factors and prophylactic strategies in immunocompetent patients with diffuse large B-cell lymphoma, mantle-cell lymphoma, peripheral T-cell lymphomas and primary mediastinal B lymphoma. Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. This article presents results of using proton therapy after operation of patient with brain meningioma. After operation brain meningiomas is known to not rarely recur due to certain localization and growth of tumor into main cerebral vessels, nerves and other main brain structures. This creates certain difficulties in operative access and surgical intervention, i.e. tumor resection. The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. During colonoscopical inspection, in 1218 weeks after the first infusion at 80% of the patients had observed disappearance of ulcers and erosion. In terms of 6 months of supervision of relapse of disease was not noted. All new directions in treatment of the IDI, apparently, are perspective, but while are proved only theoretically and had no practical development.
