Jako Bouveretův syndrom je označována obstrukce v oblasti duodena impaktovaným žlučovým kamenem, který vycestuje cholecystoduodenální či cholecystogastrickou píštělí. Onemocnění je vzácné, vyskytuje se zejména u starších žen s četnými komorbiditami a vysokým operačním rizikem. K diagnóze se zpravidla dospěje endoskopicky nebo pomocí zobrazovacích metod. Pokud stav nelze řešit endoskopicky, je indikováno operační řešení. Autoři prezentují případ Bouveretova syndromu u 79leté pacientky, u níž se zdařilo zdokumentovat všechny fáze vyšetřovacího a léčebného procesu. Součástí sdělení je přehled literatury týkající se diagnostiky a terapie tohoto vzácného syndromu., Bouveret syndrome is a gastric outlet obstruction caused by impaction of a gallstone that passes through a cholecystoduodenal or cholecystogastric fistula. It is a rare disease, most common in elderly women with multiple comorbidities and high surgical risk. The diagnosis can be made either radiologically or endoscopically. Endoscopic extraction is the preferred therapeutic option. Surgical intervention is indicated when endoscopic methods fail. We describe a case of Bouveret syndrome in a 79 years old woman. Východisko a cíl: Prognóza inoperabilního karcinomu jícnu je velmi závažná, léčba těchto pacientů je paliativní. Cílem sdělení je retrospektivní hodnocení intraluminální brachyterapie vysokým dávkovým příkonem nádorové stenózy inoperabilního karcinomu jícnu. Pacienti a metody: Od února 1996 do června 2011 bylo intraluminální brachyterapií léčeno 41 pacientů s karcinomem jícnu, z toho v 19 případech se jednalo o dlaždicobuněčný karcinom a ve 22 případech o adenokarcinom. U všech pacientů byla před zahájením brachyterapie přítomna dysfagie. Výsledky: U většiny pacientů došlo po brachyterapii k úlevě polykacích obtíží. Medián doby přežití činil 396 dní (95 % CI: 270–492 dní). Nebyla pozorována mechanická komplikace zavedení brachyterapeutického aplikátoru. Závěr: Intraluminální brachyterapie je účinná a bezpečná paliativní metoda léčby dysfagie způsobené nádorovou stenózou jícnu. Background and purpose: The prognosis of inoperable carcinomas of esophagus is poor, and therapeutic efforts are generally limited to palliation. The aim of this study is to retrospectively evaluate the effectiveness of intraluminal high dose rate brachytherapy in the palliative treatment of tumorous esophageal stenoses. Patients and methods: Between February 1996 and June 2011 intraluminal brachytherapy was performed in 41 patients with esophageal carcinoma (squamous cell carcinoma in 19 cases and adenocarcinoma in 22 cases). All patients had dysphagia at presentation. Brachytherapy was performed using high dose rate afterloading system. Results: Dysphagia was improved in majority of patients. The median survival was 396 days (95 % CI: 270–492 days). No mechanical complication was observed during introduction of the applicator. Diabetes mellitus je významným rizikovým faktorem pro rozvoj ischemické choroby srdeční. Její prevalence se u diabetiků udává v širokém rozmezí 10–55 %. Po přítomnosti ischemie myokardu je nutno u diabetiků aktivně pátrat, neboť čekání na rozvoj symptomů je u těchto pacientů spojeno s nálezem již pokročilé koronární aterosklerózy s horšími výsledky koronárních intervencí chirurgické léčby. Podle výsledků studie BARDOT se zdá, že vhodným nástrojem pro stratifikaci rizika kardiálních příhod u diabetiků by mohla být zátěžová scintigrafie myokardu. Toto vyšetření dokáže velmi dobře odhalit přítomnost ischemie myokardu, zhodnotit její rozsah i lokalizaci a odlišit, zda jde o reverzibilní ischemii. Tyto údaje jsou velmi důležité pro určení správné strategie léčby. U diabetiků často nalézáme pokročilou koronární aterosklerózu, která je vhodnější k chirurgické léčbě. Nicméně v době moderních stentů je možno i diabetiky léčit intervenčně. Každý́ diabetik by však měl mít důsledně intervenovány všechny ovlivnitelné rizikové faktory ischemické choroby srdeční a měl by být léčen tak zvanou optimální farmakoterapií. Jedná se o medikaci s prokázaným vlivem na prognózu pacientů, ovlivnění progrese koronární aterosklerózy a zmírnění ischemie myokardu., Diabetes mellitus is a major risk factor for the development of ischemic heart disease. Its prevalence in diabetic patients is reported to range broadly between 10–55 %. It is necessary to actively search for the presence of myocardial ischemia in patients with diabetes, since waiting for the development of symptoms is associated with the finding of already advanced coronary atherosclerosis in these patients, with less satisfactory outcomes of coronary interventions and surgery. Results of the BARDOT study seem to indicate that the appropriate tool for stratification of the risk of cardiac events in diabetics could be stress myocardial scintigraphy. This test is successful in detecting the presence of myocardial ischemia, assessing its size and location and identifying whether reversible ischemia is involved. Such data is very important with regard to choosing the right treatment strategy. We often find advanced coronary atherosclerosis in diabetic patients for which surgical treatment is more appropriate. Nonetheless in the era of modern stents also the patients with diabetes can be treated with intervention. All the influenceable risk factors for ischemic heart disease should be consistently treated by intervention in each diabetic patient, who should be given optimum pharmacotherapy.
