The authors present the case of a young woman with newly diagnosed Takayasu’s arteritis. This woman, with arterial hypertension, was investigated for the unspecific symptoms at the beginning. Afterwards, the transthoracic echocardiography showed dysfunction of the left ventricle and the abdominal sonography showed a stenosis of the right renal artery. PET/CT scan showed chronic modification after inflammatory processes on the wall of the thoracic and abdominal aorta. Cíl studie: Metabolismus železa musí být velmi pečlivě regulován. Klíčovým systémovým regulátorem je peptidový hormon hepcidin, který způsobuje “uzamčení” železa uvnitř buňky; to pak nemůže být utilizováno. Dialyzovaní pacienti často trpí anémií, která je způsobena mnoha faktory, mj. stavem mikrozánětu a také zřejmě zvýšenou retencí hepcidinu v důsledku snížené glomerulární filtrace. Naším cílem bylo popsat vztahy mezi hepcidinem a dalšími parametry metabolismu železa, zánětu a erytropoezy u těchto pacientů. Typ studie: Observační Název a sídlo pracoviště: Ústav klinické biochemie a hematologie LF UK a FN v Plzni, Alej Svobody 80, 304 60 Plzeň Materiál a metody: Do studie bylo zařazeno 164 chronicky dialyzovaných pacientů (věk 66 ± 13, 25 - 92 let; 101 mužů a 63 žen) a 37 zdravých kontrol (věk 55 ± 20, 21 - 92 let; 16 mužů a 21 žen), u kterých byl stanoven jejich kompletní krevní obraz, parametry metabolismu železa, zánětu a výživy. Výsledky: Koncentrace železa, transferrinu a hemoglobinu byla významně nižší u dialyzovaných pacientů (p<0,0001), naopak ferritin (p<0,0001), solubilní transferrinové receptory(p<0,05), hepcidin (p=0,0003), CRP a IL-6 (p<0,0001) byly u těchto pacientů významně vyšší než u zdravých kontrol. Hepcidin slabě koreloval pozitivně s CRP a ferritinem a negativně s transferrinem u dialyzovaných pacientů, u zdravých kontrol nebyla nalezena žádná korelace hepcidinu s ostatními parametry. Závěr: Zdá se, že vliv zánětu na hladiny hepcidinu není u hemodialyzovaných pacientů zcela zásadní a na jeho koncentraci se podílí i jiné faktory, např. jeho retence., Objective: Peptide hormone hepcidin is a key systemic regulator of the iron metabolism. Hepcidin binds to the iron cell exporter ferroportin so iron is kept in the cells unavailable for erythropoiesis. The synthesis of hepcidin is up-regulated by high iron stores and inflammation. Dialyzed patients have very often impaired iron management – they suffer from anemia, which is caused by many factors including the state of micro inflammation and hepcidin retention due to decreased glomerular filtration rate. Our aim was to describe the relationship of hepcidin and other parameters of iron metabolism, erythropoiesis and inflammation. Design: Observation Settings: Institute of Clinical Biochemistry and Hematology, Charles University Hospital in Pilsen, Alej Svobody 80, 304 60 Plzeň Material and Methods: Complete blood cell count, iron, ferritin, transferrin, CRP, albumin, creatinine, hepcidin, soluble transferrin receptors (sTfR) and IL-6 were measured in samples from 164 patients included in chronic hemodialysis program (age 66 ± 13, 25 - 92 years), 63 women and 101 men and 37 control healthy volunteers (age 55 ± 20, 21 - 92 years), 21 women and 16 men. Results: Iron, transferrin and hemoglobin were significantly lower in the patients group (p<0.0001) while ferritin (p<0.0001), sTfR (p<0.05), hepcidin (p=0.0003), CRP and IL-6 (p<0.0001) were significantly higher in the patients group. Hepcidin weakly positively correlated with CRP and ferritin and weakly negatively correlated with transferrin in hemodialyzed patients. No correlation of hepcidin with other biochemical parameters in controls was found. Conclusion: It seems that the influence of inflammation on hepcidin levels in hemodialyzed patients is not crucial and other factors including its retention in end-stage renal disease participate., T. Lokální chemoprofylaxe neinvazivních nádorů močového měchýře je používána více než 40 let. Využíváno je několik agens, mezi nejčastější patří mitomycin C (MMC) a epirubicin. Názor na léčebná schémata není jednotný. Avšak na podkladě EBM je zřejmé, že adjuvantní instilace cytotoxických látek do močového měchýře prodlužuje dobu beznádorového intervalu. V práci předkládáme dlouhodobé zkušenosti s intravezikální chemoprofylaxí povrchových nádorů močového měchýře., Local chemoprophylaxis of noninvasive bladder tumors has been used for more than 40 years. Nádory ledvin se vyskytují v každém věku. Existují věkově specifické rozdíly nejen v histologickém typu nádoru, ale i epidemiologii, etiologii, patogenezi, biologických studiích a klinickém chování. Tyto věkově specifické rozdíly jsou rovněž patrné u stejného typu nádoru a ovlivňují prognózu a přežívání pacientů. Otázkou zůstává, jestli jsou současně platná doporučení léčebné strategie skutečně vhodná pro všechny věkové skupiny., Renal tumors occur accross the age. The age specific differences are described not only on histology type of tumor, but also on epidemiology, ethiology, pathogenesis, biology studies and clinical behavior. These age specific differences are noticed also in the same type of the tumor and have an effect on prognosis and survival of the patients. Cieľ: Osobnostné charakteristiky môžu významným spôsobom ovplyvňovať individuálne schopnosti jedinca zvládať vyhorenie. Cieľom tejto výskumnej štúdie bolo preskúmať vzťah medzi osobnostnými charakteristikami sebaúcty a zmyslu pre integritu a syndrómom vyhorenia u študentov pomáhajúcich profesií. Metódy: Výskumu sa zúčastnilo 109 univerzitných študentov denného bakalárskeho štúdia odborov ošetrovateľstva, pôrodnej asistencie, verejného zdravotníctva a psychológie (priemerný vek 20,64±2,74; 86,1 % žien). Na meranie úrovne syndrómu vyhorenia bola použitá Škála syndrómu vyhorenia v škole (SBI); ďalej boli použité Antonovského škála zmyslu pre integritu (SOC) a Rosenbergova škála sebaúcty (RSS). Na štatistické spracovanie údajov bola použitá korelačná analýza (r) a viacnásobná lineárna regresná analýza. Výsledky: Bol zistený významný negatívny vzťah medzi syndrómom vyhorenia a osobnostnými charakteristikami sebaúcty (r = -0,33, P≤0,01) a zmyslom pre integritu (r = -0,40, P ≤ 0,01); teda vysoká sebaúcta a vysoká úroveň zmyslu pre integritu bola spojená s nižším vyhorením u študentov. Ako významný prediktor syndrómu vyhorenia v rámci regresnej analýzy sa ukázal najmä zmysel pre integritu (β = -0,36; 95 % konfidenčný interval: -0,50, -0,06). Záver: Osobnostné faktory sa ukázali ako významne prepojené so syndrómom vyhorenia u študentov. Keďže predispozície využívané pri zvládaní nárokov štúdia majú tendenciu pretrvávať aj neskôr počas profesionálnej praxe, je možné predpokladať, že zlepšovanie zručností zvládania stresu u študentov je užitočné v prevencii neskoršieho výskytu vyhorenia., Aim: Personality characteristic might be important factors influencing the individual abilities to cope with the burnout syndrome. Aim of the study was to examine the relationship between personality factors such as self-evaluation and sense of coherence and burnout syndrome among university students of helping professions. Methods: Together 109 university students of nursing, obstetrics, public health and psychology were included in the study (average age 20.64 ±2.74, 86.1% females). Scale Burnout Inventory (SBI) in the school was used for assessing the burnout levels. Sense of Coherence Questionnaire (SOC) and Rosenberger’s Self Esteem Scale (SES) were used as well. Correlation analysis (r) and multiple linear regression analysis were used for statistical analysis of results. Results: Significant negative relationship was found between burnout syndrome and self esteem (r = -0.33, P ≤ 0.01), as well as sense of coherence (r = -0.40, P ≤ 0.01); it means that higher levels of self esteem and sense of coherence was associated with lower burnout syndrome levels among students. In the linear regression model, burnout syndrome was significantly predicted by sense of coherence (β = -0.36; 95% Confidence Interval: - 0.50, -0.06). Conclusion: Personality characteristics were found to be significantly associated with the burnout levels in our study. Osteosarkom (OS) je nejčastější zhoubný nádor skeletu, představuje asi 35 % primárních kostních nádorů. Úspěšná léčba osteosarkomu vyžaduje úzkou spolupráci odborníků multidisciplinárního týmu, kam patří onkolog, chirurg, patolog a radiolog. Nejlepší péči zajišťují specializovaná centra s odpovídajícími zkušenostmi a možností komplexní péče. Léčebný režim musí zahrnovat radikální odstranění primárního nádoru, případně metastáz v kombinaci se systémovou chemoterapií. Z cytostatik je to zejména doxorubicin, vysoko-dávkovaný metotrexát, cisplatina a ifosfamid. Cílem rozdělení chemoterapie na část předoperační a pooperační je redukce nádorové masy a lepší ohraničení tumoru vůči okolním strukturám. Tím je usnadněna chirurgická radikalita s možností náhrady postižené části skeletu bez mutilujícího výkonu. Důležitá je také znalost histopatologické odpovědi na předoperační chemoterapii, která ovlivňuje intenzitu pooperační léčby. Dosud není dostatečně vyřešena léčba metastatického onemocnění a případné recidivy. Zlepšení výsledků je možno očekávat až po zavedení nových léčebných přípravků a modalit., Osteosarcoma (OS) is the most common malignant tumour of the skeleton accounting for approximately 35 % of primary bone tumours. Successful treatment of osteosarcoma requires a close cooperation between the specialists of a multidisciplinary team consisting of an oncologist, surgeon, pathologist and radiologist. The best care is provided by specialized centres with appropriate experience in which comprehensive care is available. The therapeutic regimen must include radical removal of the primary tumour and/or metastases in combination with systemic chemotherapy. The cytostatic drugs primarily used include doxorubicin, high-dose methotrexate, cisplatin and ifosfamide. The goal of dividing chemotherapy into preoperative and postoperative parts is a reduction in the tumour mass and improved delineation of the tumour from the surrounding structures. This improves the surgical radicality with the affected part of the skeleton being replaced without the need for a mutilating procedure. Also of importance is the knowledge of the histopathological response to preoperative chemotherapy which affects the intensity of postoperative treatment. The management of metastatic disease and possible recurrence has not been addressed adequately as yet. Only a few cases of pneumocystis pneumonia (PCP) in Cushing’s syndrome have been published in the literature so far. In the majority of these patients, the pneumonia occurred after reduction of the hypercortisolism with medicamentous treatment. We report two cases of PCP during conservative treatment of hypercortisolism. We describe clinical, imaging and laboratory findings in two patients and review published cases of pneumocystits pneumonia in Cushing’s syndrome. A 60-year-old woman and 20-year-old man with Cushing’s syndrome due to ectopic ACTH syndrome were treated at our department. Both developed pneumocystis pneumonia early after treatment with ketoconazole and ethomidate bromide had been introduced and the levels of cortisol rapidly decreased. PCP prophylaxis in patients with high cortisolemia should be started before treatment of hypercortisolism in current practice. Mediterranean spotted fever (MSF) in Bulgaria shows increasing severity, especially in patients over sixty years. As T lymphocytes are the primary effector cells in MSF and are altered by the ageing process we aimed to specify the changes in their numbers in elderly patients. We performed a clinicoepidemiological study, haematological and biochemical analysis of 132 patients of 60+ and 30 patients between 19 and 57 years with MSF. We investigated the cell immunity of 20 patients of 60+ using immunofluocytometry. The control group consisted of 10 younger patients and 10 healthy individuals of 60+. MSF showed more unfavourable course in the elderly.We found T-lymphocytes depletion in all patients with MSF. Activated T lymphocytes were increased in elderly patients with MSF. We found significant differences between the number of activated T lymphocytes in elderly patients and healthy elderly persons. The observed T-lymphocytes depletion in MSF corresponded to the disease severity. Cieľ: V práci sme sa zamerali na vedomosti pacientov s astmou v oblastiach self-manažmentu tohto ochorenia a najmä na vplyv faktoru dĺžky trvania tohto ochorenia na úroveň vedomostí o self-manažmente. Zároveň sme zisťovali vzťah medzi úrovňou vedomostí o self-manažmente ochorenia a kontrolou astmy. Metodika: V tejto prierezovej štúdii bolo do súboru zaradených 297 respondentov. Na testovanie kontroly astmy a úrovne vedomostí pacientov sme použili valídne nástroje a to Dotazník vedomostí o self-manažmente astmy pre dospelých (Measuring asthma self-management knowledge in adults) a Test kontroly astmy (Astma Control Test ACTTM). Na štatistické spracovanie dát boli využité: Pearsonov Chi-kvadrat test nezávislosti a Spearmanov korelačný koeficient. Výsledky: Až 220 z 297 respondentov (74,07 %) preukázalo nedostatočné vedomosti, a to konkrétne v doméne Vedomosti o liečbe astmy. V oblasti Manažment bronchiálnej astmy malo nedostatočné vedomosti 191 respondentov (64,31 %). Početnosť správnych odpovedí zároveň naznačuje, že úroveň vedomostí v doménach self-manažmentu astmy, významných pre kontrolu ochorenia, bola vyššia v skupine pacientov diagnostikovaných v priemere jeden rok ako v skupine, ktorá trpí ochorením dlhšie (6-10 rokov), i keď rozdiel nie je štatisticky významný. Záver: Nedostatky vo vedomostiach astmatikov, vyššia frekvencia chýb v oblastiach self-manažmentu, vrátane self-monitoringu a horšia kontrola astmy pri ochorení diagnostikovanom viac ako 6 rokov naznačujú, že je potrebné v klinickej praxi venovať čas a pozornosť reedukácii, opakovanému posúdeniu vedomostí a praktických zručností (aplikácia metód self-manažmentu, inhalačnej techniky) v skupine už skôr diagnostikovaných astmatikov., Aim: This project is aimed to find out the patient knowledge with asthma in the area of self-management of this disease and particularly the influence of the length of the disease on the knowledge level about self-management. At the same time we tried to find out the relation between the knowledge level of the asthma self-management and control of asthma disease. Methods: The total number of 297 adult asthma patients were included in the cross sectional study. The valid instruments Asthma Control Test and Measuring asthma self management knowledge in adults were used for testing asthma control and the degree of patient knowledge in self management of asthma. The Chi-squared test and the Spearman´s rank correlation coefficient were used for statistical processing of data. Results: A total of 220 out of 279 respondents (74%) showed poor knowledge of selfmanagement of asthma, particularly in “Knowledge of asthma treatment” domain and 191 patients in “Management of bronchial asthma” domain (64%). Patients who had been diagnosed one year ago had better level of knowledge in selfmanagement of asthma than those who had been treated for asthma longer than six years (6-10 years). This difference was not significant, but it was related to those domains which has significant influence on asthma control (domains 2-5). Conclusion: Poor knowledge of asthma patients, higher frequency of errors in self management and worse control of asthma in patients with asthma longer than six years imply the need for better and repeated education, check of patient knowledge and skills for self-management, use of peak flow meter included in this group of patients., Michaela Dingová, Petronela Osacká, and Literatura
