V těhotenství je anafylaktická reakce naštěstí sice relativně vzácná, její důsledky ale mohou vést až k závažnému postižení matky i plodu. Ve většině publikovaných případů těžké anafylaxe v těhotenství bylo těžké postižení dokonce spíše na straně plodu než matky. Vznik anafylaxe se většinou nedá předvídat, první symptomy jsou někdy netypické, pouze lokální, ale rychle mohou přecházet v generalizované postižení celého organismu. Ať už jsou zprostředkovány protilátkami typu IgE (pravá anafylaxe), nebo vznikají na jiném podkladě (anafylaktoidní reakce), klinický obraz bývá obdobný. Je třeba myslet na to, že i klinicky nenápadné počáteční symptomy mohou mít průběh závažný a život ohrožující. Každé opomenutí a prodleva v diagnostice a léčbě anafylaxe může vést k fatálnímu konci. O úspěšném výsledku léčby rozhoduje velkou měrou medicínská zkušenost a včasnost zásahu. Protože je toto téma v současné literatuře zmiňováno spíše jen výjimečně, je cílem tohoto článku shrnout problematiku anafylaxe v těhotenství z hlediska patofyziologie, klinického průběhu, diagnostiky i léčby., Anaphylaxis in pregnancy is a relatively rare reaction but it can lead to serious maternal and fetal impairment. In published cases of severe anaphylaxis in pregnancy there were even more severe disabilities in the fetus than the mother. Development of anaphylaxis is usually unpredictable, the first symptoms are sometimes atypical and only local, but can move quickly in a generalized impairment of the whole organism. Whatever they are mediated by IgE antibodies (right anaphylaxis), or are caused on another basis (anaphylactoid reactions), the clinical picture is similar. It should be kept in mind that even clinically inconspicuous initial symptoms may progress into serious and life-threatening state. Any omission and delay in diagnosis and treatment of anaphylaxis can lead to fatal end. A successful outcome is largely dependent on experience and timeliness of medical intervention. Based on survey of 257 women with nondeveloping pregnancies of various geneses, it was shown that the level of apoptosis marker P53 in the decidual tissue is increased, and antiapoptotic protein Bcl2 simultaneously is reduced, compared with similar data of women with physiological pregnancies. INTRODUCTION: Leptin is an adipokine which has a direct relationship to obesity. Our aim was to measure this hormone in pregnant women at three months intervals throughout their pregnancies to determine the serum value of those who developed preeclampsia. MATERIAL AND METHODS: We followed 19 women (median age 24.8 +/- 5.7 years) with pre-gestational Body Mass Index (BMI) less than 25 kg/m2, 21 (median age 26.1 +/- 4.6 years) with BMI higher than 25 kg/m2 and 16 (median age 30.9 +/- 5.8 years) with Gestational Diabetes Mellitus (GDM) (median age 30.9 +/- 5.8 years), recruited in the 1st trimester of pregnancy. Serum levels of leptin were measured with radioimmunoassay (RIA) technique. RESULTS: In the first trimester of pregnancy leptin levels showed statistically significant differences between normal weight and overweight-obese women (p < 0.001), diabetic women (p < 0.05) and the subgroup of preeclamptic women (p < 0.001). For those women with PGBMI > or = 40 kg/m2 and leptin > or = 40 ng/ml in the second trimester, the Odds Ratio (OR) to develop preeclampsia was of 47.95% CI (4.1-527.2). Analyzing leptin values with ROC curves, the greatest area under the curve (AUC) was for leptin in the second trimester (0.773, CI: 0.634-0.911). CONCLUSION: Women with morbid obesity (BMI > or = 40 kg/m2) had significantly higher levels of serum leptin (p < 0.01) and a value of 40 ng/ml of this hormone seems to be predictive of developing preeclampsia in this group of patients. and H. To study defects and chromosomal abnormalities of the fetus, we retrospectively analyzed results of comprehensive dynamic survey of 26,404 pregnant women aged 1850 years old at 6 to 40 weeks of pregnancy. Of them, 25,956 (98.3%) women had physiological course of pregnancy, 448 (1.7%) women had abnormal pregnancy. For the diagnosis of fetal defects, we carried out ultrasound, biochemical, invasive and cytogenetic studies. The results of study showed that the majority of fetal defects and pathological course of pregnancy was noted in women at the age of 2125 years old, since at this age period women have the highest number of pregnancies. At the older age, we noted a gradual decrease in the number of pregnant women, as well as the number of abnormalities of the fetus. Based on the analysis of the results obtained, we have developed an algorithm for early fetal ultrasound examination. Toxoplasmóza je jedna z nejčastějších zoonóz u nás. Patří mezi onemocnění, která mohou při primoinfekci v graviditě ohrozit vývoj plodu či způsobit mnohočetná poškození, často manifestovaná až po narození dítěte. Závažnost postižení, jeho variabilita a pravděpodobnost vzniku závisí na době, kdy se těhotná žena nakazila. V ČR máme k dispozici několik diagnostických metod. Základem je sérologické vyšetření (protilátky celkové i jednotlivých tříd, avidita IgG, komparativní imunoblot), prováděné z krve či plodové vody. Možný je i přímý průkaz DNA Toxoplasma gondii pomocí PCR z krve, plodové vody či po porodu z placenty. Prenatálně se provádí izolační pokus na myši (inokulací plodové vody). Pomocnou metodou je také ultrazvuk či MRI v průběhu těhotenství, se zaměřením na případnou fetopatii. Po narození je dítě z rizikové gravdity podrobně vyšetřeno a dlouhodobě sledováno. Terapii lze podat prenatálně i postnatálně, o jejím efektu se vedou diskuze. Základními preparáty jsou spiramycin, pyrimethamin a sulfadiazin (doplněny kyselinou folinovou). V článku jsou probrány indikace k jednotlivým vyšetřením a doporučeny nejvhodnější postupy v diagnostice, léčbě a také prevenci tohoto onemocnění u těhotných. Text se podrobně věnuje i interpretaci možných výsledků a obsahuje vyjádření k preventivnímu vyšetření těhotných žen u nás., Toxoplasmosis is one of the most common zoonoses in the Czech Republic. Toxoplasma infection during pregnancy can affect fetal development and cause multiple damages that often do not manifest until after the birth. The severity of pediatric disability, its variability and the probability of occurrence depend on the time of maternal infection. Serological diagnosis of toxoplasmosis in pregnant women is performed in two steps: first, total anti-Toxoplasma antibodies are detected, most commonly by the complement fixation test (CFT) combined with IgG ELISA. In the case of a negative result, the woman can be retested 1-3 months later. Low CFT titers show latent infection with a minimal risk of primary infection during pregnancy. When CFT titers of 1:64 or higher are obtained, the second step follows. Positivity in the IgM and IgA tests and low IgG avidity can confirm a recent infection. The diagnosis of fetal infection is based on the detection of anti-Toxoplasma antibodies, and of IgM antibodies in particular, and/or on the detection of Toxoplasma in amniotic fluid by PCR or the mouse test. Other helpful methods are fetal ultrasound or MRI focusing on fetopathy. Each child born from a high-risk pregnancy is thoroughly examined and followed up on a long-term basis. The detection of neonatal anti-Toxoplasma IgM antibodies and a neonatal IgG profile differing from the maternal IgG profile in comparative Western blot are the major markers of congenital infection. Toxoplasma infection can be treated prenatally and postnatally and the therapeutic outcomes are still being considered. The first-line options for congenital toxoplasmosis are spiramycin, pyrimethamine and sulfadiazine with folinic acid. The article discusses the indications for screening and recommends best practices for the diagnosis, treatment and prevention of toxoplasmosis during pregnancy. Diabetes 1. typu (DM1) vzniká jako následek autoimunitní destrukce beta buněk pankreatu. Vliv diabetu na celkový výsledek těhotenství je poměrně dobře prostudován. Mnohem méně se toho však ví o vlivu diabetu matky na vyvíjející se imunitní systém plodu. Riziko rozvoje DM1 u dětí diabetických matek (pokud onemocněly před těhotenstvím) je nižší než např. u dětí diabetických otců. Tato diskrepance podporuje teorii, že by mohlo docházet k určitému navození imunotolerance u dětí DM1 matek. Z tohoto důvodu jsme si pro náš výzkum vybrali T regulační buňky (Tregs), které hrají důležitou roli v imunoregulaci a patogenezi mnoha autoimunitních onemocnění. Studie Tregs pupečníkové krve DM1 matek jsou navíc ojedinělé. Pomocí průtokové cytometrie jsme analyzovali zastoupení Tregs (definovaných jako CD45+ CD3+ CD4+ CD25+ CD127(low/-)) ve vzorcích pupečníkové krve od 17 matek s DM1, 17 s gestačním diabetem a 42 zdravých kontrol. Signifikantní rozdíl jsme nalezli jen v zastoupení Tregs z Th lymfocytů, kde v pupečníkové krvi zdravých matek činil tento poměr 9,25 % a u matek s DM1 6,73 % (p=0,043). Ostatní rozdíly nedosáhly signifikance. Pokud tedy dochází k navození specifické imunotolerance, je tato patrně podmíněna jinak než změnou počtu Tregs. Pokračujeme proto ve funkční analýze Tregs s cílem nalézt vysvětlení pro nižší výskyt diabetu u dětí DM1 matek ve srovnání s ostatními prvostupňovými příbuznými DM1 pacientů. Pochopení tohoto jevu by mohlo přispět např. i k vytvoření imunointervenční terapie DM1., Type 1 diabetes (T1D) develops due to autoimmune pancreatic beta cells destruction. The effect of diabetes on pregnancy outcome is well documented. However, much less is known about the effect of maternal diabetes on the developing foetal immune system. The risk of T1D development in a child of T1D mother who was diagnosed prior pregnancy is lower than e.g. in a child of T1D father. This discrepancy supports the theory that there is some immunoregulatory influence on her baby. For this reason we decided to study T regulatory cells (Tregs) which are important in immunoregulation and for autoimmune diseases pathogenesis. Moreover Tregs cord blood (CB) studies of T1D mothers are limited. Tregs defined as CD45+ CD3+ CD4+ CD25+ CD127(low/-) in CB samples were investigated in our study by flow cytometry and 17 T1D mothers, 17 gestation diabetes mothers and 42 healthy controls were enrolled. We found just that CB from babies of T1D mothers contained 6,73 % Tregs/total Th-lymfocytes in comparison to 9,25 % in controls (p = 0,043). Other differences were not significant. If an establishment of specific immunological tolerance in foetus of T1D mother really exists, it seems to be rather due to other factors than just to changes in Tregs count (which was moreover found to be decreased). We are currently focused on explanation why children of T1D mothers suffer from diabetes less frequently than other first degree relatives by using functional analysis of Tregs.
