Antifosfolipidový syndrom (APS) je autoimunitně podmíněné onemocnění s celou řadou potenciálně život ohrožujících manifestací. Vyskytuje se samostatně v primární podobě nebo jako sekundární doprovázející řadu chorobných stavů (autoimunitní, nádorová onemocnění atd.). Diagnostická kritéria zahrnují jak klinickou, tak laboratorní složku. Mezi klinické manifestace patří výskyt trombóz arteriálních či žilních a dále komplikace v těhotenství. Laboratorně musí být opakovaně prokázána přítomnost antifosfolipidových protilátek v minimálním odstupu 12 týdnů. Klinická manifestace APS může být velmi pestrá a často vyžaduje multidisciplinární přístup. Obávanou, ale vzácnou komplikací je tzv. katastrofický APS spojený s vysokou morbiditou a mortalitou. Včasná diagnóza spolu s terapií výrazně ovlivňuje prognózu pacientů s APS. APS occurs in a primary form with no associated conditions, or in a secondary form associated with various other illnesses (autoimmune, neoplastic and others). APS is diagnosed according to clinical and laboratory criteria. It is characterized by recurrent arterial or venous thrombosis and/or pregnancy‑related complications and laboratory findings of antiphospholipid antibodies in a minimum time interval of 12 weeks between individual laboratory tests. Clinical manifestations of APS can vary and a multidisciplinary approach is needed. Catastrophic APS is a very serious and life‑threatening condition associated with high mortality and morbidity, although it is rare. Cíl studie: zhodnocení efektivity cerkláže dle McDonalda ve Fakultní nemocnici Ostrava v období 1/2007 – 3/2014. Typ studie: retrospektivní studie Název a sídlo pracoviště: Porodnicko–gynekologická klinika, Fakultní nemocnice Ostrava Metodika: retrospektivní analýza provedených cerkláží ve Fakultní nemocnici Ostrava a rozbor výsledků takto ošetřených gravidit. Kritériem úspěšnosti byl stanoven porod po ukončeném 34. týdnu gravidity. Závěr: ve sledovaném období bylo indikováno devět cerkláží – 7 terapeutických a 2 záchranné. Výkony byly provedeny mezi 18. a 25. týdnem gravidity. Tři pacientky již měly v minulosti cerkláž provedenu. 57 % takto ošetřených těhotných porodilo po ukončeném 34. týdnu gravidity. Pouze v jednom případě (11,1 %) byla cerkláž neúspěšná a těhotenství skončilo potratem. Průměrná doba mezi provedením cerkláže a porodem činila 69 dnů., Objective: To evaluate the efficacy of cerclage according to McDonald at the University Hospital Ostrava in the period 1/2007 - 3/2014. Design: A retrospective study Obstetrics and Gynecology: Obstetrics - Gynecology, University Hospital Ostrava Methods: A retrospective analysis of cerclage performed at the University Hospital Ostrava and analysis of the results thus treated pregnancies. The criterion for success was determined after childbirth completed 34 weeks of preg - nancy. Conclusion: In the reporting period, nine indicated cerclage - 7 of therapeutic and 2 emergency. The procedures were performed between the 18th and 25th week of pregnancy. Three patients have had in the past cerclage performed. 57% of the treated pregnant women gave birth after 34 completed weeks of gestation. Only in one case (11.1 %) cerclage was unsuccessful and the pregnancy ended in a miscarriage. The aim of our study was to determine the significance of lymphocyte-platelet adhesion (LPA), interleukins, transforming included 139 pregnant women aged between 17 and 27 years (21.3±4.22 years). GH was diagnosed in 119 women after 20 weeks of pregnancy. 20 patients (control group) were with physiological course of pregnancy. The distribution of patients by groups was carried out according to the level of blood pressure (BP) in accordance with ICD-10 (Geneva, WHO, 2002). The survey was conducted at the moment of detection pregnancy from 7 to 10 weeks and in dynamics of I, II and III trimesters of gestation. In the dynamics of gestation, were studied the number of desquamated endothelial cells circulating in the systemic circulation (CECs), nitrates levels, the adhesion of platelets by estimation their ability to form co-aggregates with lymphocytes by determining the percentage of lymphocytes aggregates with thrombocytes (lymphocyte-platelet plugs), at risk of hypertensive disorders, especially after 20-22 weeks of gestation and later, lymphocytes ability to platelets adhesion is rose, the concentrations of pro-inflammatory cytokines and NO level are increased. Nejlépe dokumentován je význam podávání folátů, které snižují výskyt defektů neurální trubice, vrozených srdečních vad, vrozených vad uropoetického traktu, plodů s nižší porodní váhou, patologií placenty a spontánních potratů. Suplementaci foláty minimálně měsíc před otěhotněním a v 1. trimestru doporučuje 17 německých odborných společností. Metylfolát [(6S)-5methyltetrahydrofolát] je biologicky aktivní folát, který není závislý na aktivitě MTHFR. Backround: Worldwide, approximately 1 billion people, from which 200250 million are women, smoke. Based on the Estonian Medical Birth Register data, in 2010, 7,5% of pregnant women were smokers, and in 2012, 7% of pregnant women were smoking during their pregnancy. Thus, smoking rates among pregnant women fell only 0,5%. The objective of the research was to find out the views of pregnant women who were smoking during pregnancy of the effect of smoking on the health of them and their babies, and experiences in quitting smoking, changing indicators, e.g. smoking and socioeconomical status (initial exposure to smoking, employment, longterm partnerships etc.) in 20092013. Methods: This research is a phenomenological study, which was being carried out from October 2009 until January 2013. Data collection methods were semistructured interviews with 45 pregnant women smoking during their pregnancy and being registered for antenatal care in three health care institutions of Republic of Estonia. An inductive approach for qualitative analysis was used. Results: Most women smoking during pregnancy started smoking in their teens, their parents smoked, they were under 30 years old housewives or unemployed. On an average of 1120 cigarettes were consumed each day, the pregnancy of the research group did not influence frequency and tobacco intake. The participants in the research group reported that tobacco consumption helped them relax, gave them a chance to spend time in a good company, and it was not regarded as a bad habit but a social addiction, being caused by availability of tobacco products. Although most of them had an opinion about harmful effects of smoking, e.g. a cough, asthma and rapid fatigue, on the health of them and less on their expected babies, they continued smoking. Conclusions: The smoking and socioeconomical status of women smoking during pregnancy in 20092013 has not changed, health awareness of them is low and has not improved. Support groups with close relatives are needed to motivate cessation. In the first place, prevention needs a good example, purposeful cooperation, which starts in daily life context at homes, in educational institutions and in the community. Limiting the availability of tobacco products, postponing and preventing an initial exposure to smoking are thought to be of great importance. Erytrocytární aloimunizace (izoimunizace, hemolytické onemocnění plodu a novorozence – HON) se v současnosti díky rutinní imunoprofylaxi anti-D imunoglobulinem vyskytuje sporadicky. Výskyt atypických aloimunizací non-RhD-antigeny zůstává neměnný, poněvadž není známa profylaxe a ani prevence nezajišťuje stoprocentní účinnost. Pokud se imunoprofylaxe anti-D imunoglobulinem neprovede nebo selže, hrozí vznik hemolytického onemocnění plodu a novorozence. Každý případ je potom unikátní (specifita a typ protilátky, blokující protilátky v séru matky, antigenní výbava erytrocytů plodu, gestační stáří při začátku onemocnění atd). Pokroky v genové diagnostice, imunohematologii a dopplerovské ultrasonografii významně snižují frekvenci nutných invazivních zákroků v diagnostice a léčbě senzibilizovaného těhotenství. Intraumbilikální přísně intravenózní transfuze pod ultrasonografickou kontrolou je první metodou volby léčby případů s těžkou anémií plodu i při rozvinutém hydropsu. Konzultace a léčba se soustřeďují do center s perfektní porodnickou a neonatologickou péčí a nepřetržitou operační pohotovostí. The occurrence of atypical alloimmunizations related to non-RhD-antigens remains unchanged, as there is no prophylaxis known and even the prevention does not provide one hundred percent efficiency. When anti-D immunoglobulin immunoprophylaxis is not undertaken or fails, the threat of hemolytic disease of the fetus and newborn arises. Every case is then unique (specificity and type of antibody, blocking antibodies in maternal serum, antigenic make-up of fetal erythrocytes, gestational age at the disease onset etc). The advancements in genetic diagnostics, immunohematology and Doppler ultrasonography significantly reduce the frequency of necessary invasive interventions in the diagnostics and treatment of sensibilized pregnancy. Intraumbilical strictly intravenous transfusion under ultrasound guidance is the first-line method of therapy for cases of severe fetal anemia as well as advanced hydrops. Consultations and treatment are provided in the centres for comprehensive obstetrical and neonatal care with continuous intervention preparedness. Podmínkou zdraví, nejenom reprodukčního, je zdravý způsob života vycházející z kvalitní informovanosti. Výsledky současných výzkumů ukazují, že nezdravý způsob života matky/rodičů již před početím dítěte a následně prenatálně hraje roli v etiologii různých poruch zdraví a abnormalit vývoje jedince, které se mohou manifestovat okamžitě, ale také teprve s dlouhou latencí. Znalosti a návyky žen mohou svým poradenstvím významně ovlivnit porodní asistentky prenatálně v kurzech, v optimálním případě již prekoncepčním poradenstvím. Záměrem práce bylo zjistit, jaká je současná situace v oblasti přípravy žen na těhotenství a mateřství, které faktory ji determinují a jaký je její případný benefit. Užito bylo dotazníkového šetření a hodnocení vybraných antropometrických parametrů. Práce zjistila, že příprava žen na těhotenství a mateřství vykazuje značné rezervy projevené nedostatečnou informovaností i výskytem nevhodného či rizikového chování. Výsledky ve většině sledovaných parametrů korelovaly s dosaženým stupněm vzdělání a byly lepší u žen, které se na své těhotenství a mateřství připravovaly. Prenatální kurzy navštěvuje ale celkově malý počet žen, nejméně ženy s nižšími stupni vzdělání, pro které ale paradoxně kurzy přinášejí vyšší benefit., Healthy lifestyle resulting from well-informed parental knowledge is a condition of both reproductive and general health. Results of current studies show that both pre-conceptual and prenatal unhealthy lifestyles of mothers/parents play an important role in the etiology of different health malfunctions and abnormalities in ontogeny. These malfunctions and abnormalities can be manifested either immediately or with a long latency. The knowledge and habits of women can be significantly influenced by midwives in prenatal courses or, optimally, in a pre-conception advisory capacity. The goal of our work was to ascertain the current situation of pregnanacy and maternity preparedness and associated determining factors. A questionnaire survey and evaluation of chosen anthropometrical parameters were used. The study revealed that existing pregnancy and maternity preparation is insufficient, with associated inadequacies or risks in behaviour. In most cases the results correlated with the educational capacity of mothers and were better in cases where the subjects had prepared for their pregnancy and maternity. Ultrazvukové měření děložního hrdla představuje dobře zavedenou metodu predikce předčasného porodu. Nicméně sensitivita a pozitivní predikční hodnota ultrazvukové cervikometrie zůstává stále relativně neuspokojivá. Proto je snaha upřesnit predikci předčasného porodu využitím dalších metod v kombinaci s ultrazvukovou cervikometrií. Jedna skupina výzkumných prací se zabývá hodnocením různých parametrů z cervikovaginálního sekretu – nejvýznamnější představuje fetální fibronektin (fFN). Pozitivní test na fFN ukazuje relativně nízkou pozitivní predikční hodnotu stran předčasného porodu, nicméně výhodou je vysoká negativní predikční hodnota – bezmála 95%. Data z posledních studií ukazují užitečnost hodnocení fFN spolu s cervikometrií. Nedávné objevy ukázaly, že T regulační lymfocyty (CD4+CD25+CD127 - dále zkráceně Treg lymfocyty) mohou hrát důležitou roli v patofyziologii předčasného porodu. Cílem výzkumu na našem pracovišti je vyhodnotit vztah mezi hladinou Treg lymfocytů a ultrazvukovou délkou děložního hrdla k riziku předčasného porodu. Pilotní data z naší práce ukazují, že kombinované hodnocení uvedených parametrů představuje mnohem lepší možnost predikce předčasného porodu než hodnocení obou parametrů samostatně. Je však potřeba dalšího výzkumu, který bude mít za cíl vyhodnotit korelaci mezi sníženými hladinami Treg lymfocytů a předčasným porodem., Ultrasound measurement of the uterine cervix represents a well-established method in the prediction of preterm labour. However, the sensitivity and positive predictive value of ultrasound cervicometry still remain rather unsatisfactory. Hence, the effort to provide more specific predictions of premature labour using other applicable methods in combination with ultrasound cervicometry. One group of studies have focused on determining various parameters from cervicovaginal secretion - the most important one being fetal fibronectin (fFN). A positive fFN test in the birth canal shows a relatively low positive predictive value in terms of premature birth; however, it does provide the advantage of a high negative predictive value, which is about 95%. Data from latest studies show the usefulness of fFN in combination with cervicometry. Recent discoveries suggest that T regulatory lymphocytes (CD4+CD25+CD127 - Tregs) may play an important role in the pathophysiology of preterm labour. The purpose of our study was to assess the relationship between maternal circulating Tregs and the sonographically measured uterine cervix and the risk of preterm labour. We can show for the first time that the combined assessment of Treg cell count and cervical length provides a much better prediction of preterm delivery that either parameter used on its own.
