Brown tumors are bony lesions caused by rapid osteoclastic activity, which rare involved jaws. Renal osteodystrophy (ROD) is associated with different pathogenetic mechanisms--disorder of calcium-phosphate metabolism, impaired metabolism of vitamin D, increased parathyroid activity that lead to extreme concentrations of parathormone. The authors report two cases of jaw enlargement in patients received haemodialysis with excessive increase values of alkaline phosphatase and parathormone in serum. The patients were treated surgically with corrective procedures in maxillo-facial area. ROD of the jaws could be severe complication in dialysis patients with end stage of CKD if no appropriate care aimed at correction or prevention of parathyroid hyperfunction was applied to them. and PF. This article presents results of using proton therapy after operation of patient with brain meningioma. After operation brain meningiomas is known to not rarely recur due to certain localization and growth of tumor into main cerebral vessels, nerves and other main brain structures. This creates certain difficulties in operative access and surgical intervention, i.e. tumor resection. Cíl: Prezentovat zajímavý případ 44leté ženy s metastázou karcinomu prsu v axilární lymfatické uzlině vpravo s negativní mamografií, biopsií a ultrasonografií prsů, kdy 18F-FDG PET/CT odhalil multifokální karcinom prsu vpravo. Materiál a metoda: Pacientka vyhledala lékaře pro hmatnou rezistenci v pravé axile. Následovala exstirpace paketu lymfatických uzlin z pravé axily, histologické vyšetření prokázalo metastázu karcinomu prsu. Mamografie a ultrasonografie obou prsů byla s negativním výsledkem. Vzhledem k pozitivitě metastáz v lymfatických uzlinách v pravé axile následovala ještě biopsie z pravého prsu, také s negativním nálezem. V rámci pátrání po primárním ložisku karcinomu prsu pacientka absolvovala celotělové PET/CT zobrazení v rozsahu od baze lební do proximální třetiny stehen, a to 60 minut po intravenózní aplikaci radiofarmaka 18F-FDG o aktivitě 377 MBq (5 MBq/kg). Akviziční čas PET skenu byl 2,5 minuty na 15 cm projekci, celkem 7 projekcí. Kontrastní (i.v. + per os kontrast) CT sken byl proveden ve venózní fázi. Výsledky: PET/CT odhalil multiložiskový viabilní tumor v laterální části pravého prsu a viabilní metastázy v lymfatických uzlinách v pravé axile a infraklavikulárně vpravo. Závěr: 18F-FDG PET/CT správně detekoval multifokální karcinom prsu u pacientky s pozitivní metastázou tohoto karcinomu v lymfatické uzlině a neznámým primárním ložiskem, čímž zkrátil dobu diagnostického procesu a umožnil zvolit správnou léčbu., Aim: To present a rare case report of the multifocal breast cancer finding on 18F-FDG PET/CT in a 44-year-old woman with the positive right axillary breast metastases and negative mammography, ultrasound and right breast biopsy. Material and Method: The patient underwent 18F-FDG PET/CT for the positive right axillary lymph node breast cancer metastases and negative mammography, ultrasound a right breast biopsy. The whole body PET/CT was obtained 60 minutes after intravenous injection of 18F-FDG (377 MBq – 5 MBq/kg). The PET scan acquisition time was 2.5 minutes per bed position for whole body imaging from base of the skull to mid femur and seven bed positions were necessary. Contrast-enhanced CT (venous phase) provided both full CT evaluation (including intravenous and oral contrast) and PET attenuation correction. Results: The examination revealed PET/CT positive multifocal tumour in the right breast and regional metastases in the right axilla and infraclavicular region without distant metastases. The patient underwent the subsequent neoadjuvant chemotherapy, surgery and adjuvant radiotherapy and the complete remission was induced. Pro hemodialyzované pacienty, u kterých byl diagnostikován diferencovaný karcinom štítné žlázy, může mít zásadní význam léčba 131I. Diagnóza nádorového onemocnění tyto pacienty vyřazuje z registru čekatelů pro transplantaci ledviny do doby, než bude prokázána 3-5letá remise. Jelikož je 131I vylučován z těla převážně ledvinami, je efektivní poločas 131I u pacientů s renálním selháním prodloužen. To je důvod, proč by měla být redukována podávaná léčebná aktivita, aby došlo ke snížení radiační zátěže především pro krvetvornou kostní dřeň. Prezentujeme kazuistiku 48letého muže v terminálním renálním selhání, léčeného 131I pro papilární karcinom štítné žlázy. Terapeutické aktivity jsme snížili o 30 %. Hemodialýza byla prováděna po podání léčebné aktivity 131I na lůžkové části kliniky nukleární medicíny. 131I je dialyzovatelný a po každé dialýze klesla zbytková aktivita o 33-67 %. Radiační zátěž personálu hemodialyzačního centra byla minimální. V léčbě uveálních melanomů se v některých případech používá radiochirurgie CyberKnifem. Histologickými a imunohistologickými změnami v nádorové tkáni po terapii se zabývá poměrně málo autorů. V tomto sdělení uvádíme výsledky vyšetření dvou takových případů. U obou pacientů byla provedena enukleace bulbu pro bolestivý glaukom. U prvého pacienta šlo o recidivující melanom, který byl ošetřen gama nožem před deseti lety. Pro lehkou progresi ložiska melanomu bylo provedeno ošetření CyberKnifem celkovými dávkami 45Gy ve 3 frakcích. Po necelých pěti měsících po ozáření bulbu došlo k regresi recidivujícího nádoru z původní prominence 1,4 mm v ploše 6 mm × 0,6 mm na prominenci 0,3 mm v ploše 3 mm × 0,3 mm. Histologicky byla zaznamenána rezidua řídce celulárního ložiska regresivně změněných nádorových buněk a přítomnost reaktivních změn optického nervu. Druhý pacient byl měsíc po radioterapii CyberKnifem s následnou komplikovanou operací katarakty a s enukleací bulbu bez regrese – tedy zániku nádorových buněk. Velikost tumoru 11,1 mm × 15mm s prominencí 9,3mm přetrvávala i po ozáření dávkou 40Gy/1Fr. V histologickém vyšetření byly nádorové buňky bez patologických změn. V imunohistochemickém vyšetření jsme pozorovali u prvého pacienta vysokou difuzní expresi proteinu bcl-2 až do výše 75 % a méně než 1% pozitivitu ve vyšetření s proliferačním markerem Ki-67. U druhého případu byla situace odlišná, s výraznou až 75% pozitivní expresí bcl-2 proteinu v nádorových buňkách v povrchových vrstvách tumoru a s klesajícím počtem pozitivní exprese v hlubokých vrstvách nádoru, kde dosahovala maxima do 25 %. Proliferační aktivita s pozitivní jadernou expresí v Ki-67 vzrůstala od povrchu, kde se pohybovala v rozmezí 1–3 % do hloubky nádoru a k jeho bázi, kde dosahovala až 5 %. Naše pozorování ukazuje na výraznou redistribuci genetické aktivity uvnitř nádoru. Ta se projevuje „overexpresí“ bcl-2 proteinu a u druhého případu je patrná rozdílná pozitivita bcl-2 proteinu v různých vrstvách nádoru. Vztah tohoto fenomenu k radioterapii není dosud jasný., Radiosurgery with CyberKnife is used in the treatment of uveal melanomas in some cases. Histological and immunohistological changes in tumour tissue following treatment have been addressed by relatively few authors. This paper presents the results of investigations of two such cases. Enucleation of the eye was performed for painful glaucoma in both patients. Patient 1 had a recurrent melanoma treated with a Gamma Knife ten years ago. Due to slight progression in the melanoma site, treatment with CyberKnife was used with total doses of 45 Gy in 3 fractions. Some five months following eye irradiation, there was regression of the recurrent tumour from the original height of 1.4 mm in an area of 6 mm x 0.6 mm to that of 0.3 mm in an area of 3 mm x 0.3 mm. Histologically, residua of a sparsely cellular locus of regressively altered tumour cells and the presence of reactive changes in the optic nerve were noted. Patient 2 underwent radiotherapy with CyberKnife one month prior, with subsequent complicated cataract surgery and enucleation of the eye without regression, i.e. destruction of tumour cells. The size of the tumour of 11.1 mm x 15 mm with a height of 9.3 mm remained unchanged even after irradiation with a dose of 40 Gy/1 fr. No pathological changes in the tumour cells were seen on histological examination. In patient 1, immunohistochemistry showed high diffusion expression of the bcl-2 protein of up to 75 % and less than 1 % positivity on examination with the proliferation marker Ki-67. In patient 2, the situation was different, with significant, as high as 75 % positive expression of the bcl-2 protein in tumour cells in the superficial tumour layers and a decreasing rate of positive expression in the deep tumour layers where it reached no more than 25 %. The proliferation activity with positive nuclear expression of Ki-67 increased from the surface, where it ranged from 1 % to 3 %, to the depth of the tumour and toward its base, where it reached up to 5 %. Our observation suggests significant redistribution of genetic activity within the tumour. It is manifested by overexpression of the bcl-2 protein and, in patient 2, different bcl-2 protein positivity in the various tumour layers is apparent. Angiofibromas are rare vascular tumors which originate predominantly in the nasopharynx and occur typically in male adolescents. Extranasopharyngeal sites such as nasal cavity and paranasal sinuses are less frequent. This review article was undertaken to evaluate the incidence, clinical features and management of extranasopharyngeal angiofibromas originating exclusivelly from nasal cavity structures. Our focus of interest was to evaluate the significance of immunohistochemical analysis in diagnosis of such extremely rare neoplasms. In the PubMed and Google Search, we found only 39 cases of nasal angifibroma, 27 males and 12 females from 1980 to 2012. The most prevalent site of origin was nasal septum, followed by inferior and middle turbinate. The commonest symptoms were nasal obstruction and epistaxis. Nasal angiofibromas are clinically distinct from nasopharyneal angiofibromas and can therefore be misdiagnosed. The differential diagnosis includes other vascular lesions, such as lobular capillary hemangioma and sinonasal-type hemangiopericytoma. Although immunohistochemistry is not necessary for differentiation between angiofibroma and capillary hemangioma, that diagnostic procedure may be helpful in distinction from sinonasal hemangiopericytoma. As an ilustration for immunohistochemical analysis, we presented a case of an elderly woman with tumor arising from the middle turbinate, diagnosed as angiofibroma. We report the case of a 52year old man with severe acute pancreatitis. In this case report we discuss the undesirable effects of parenteral nutrition and the importance of nutritional support at patients with severe necrotizing pancreatitis. Severe acute pancreatitis is usually accompanied by systemic inflammatory response syndrome, which results in hypermetabolism with prominent protein catabolism. Providing nutrition to these patients is of paramount importance. An adequate nutritional support is crucial in patients with severe and complicated pancreatitis.
