Vznik a progrese symptomatické diabetické neuropatie (SDN) se váže k hyperglykemii. Při terapii je proto velmi důležitá snaha o zlepšení kompenzace diabetes mellitus. Z tohoto důvodu je třeba kooperace v terapii mezi neurologem a diabetologem. Farmakologická terapie symptomatické senzitivní periferní diabetické neuropatie je svízelná a relativně málo účinná. K symptomatické terapii je užívána celá řada léčivých látek určených primárně k intervenci jiných patologických stavů. V doporučených postupech jsou uváděna antidepresiva, antikonvulziva, opiáty a jejich deriváty. Tato terapie je však zatížena relativně vysokou incidencí nežádoucích účinků, které snižují adherenci pacientů k léčbě. Výborných výsledků dosahuje terapie kyselinou tioktovou., The development and progression of symptomatic diabetic neuropathy (SDN) is linked to hyperglycemia. The effort to improve compensation of diabetes mellitus during therapy is therefore very important. This is where the cooperation between the diabetologist and neurologist within therapy plays an important role. The pharmacological therapy of symptomatic sensitive peripheral diabetic neuropathy is difficult and with a less than satisfactory effect. A variety of active substances is used in symptomatic therapy, primarily designed for intervention in other pathological conditions. The recommended guidelines include antidepressants, anticonvulsants, opiates and their derivatives. However this therapy brings with it a relatively high incidence of adverse effects which detract from patients’ adherence to treatment. Lyellův syndrom (syndrom toxické epidermální nekrolýzy) je raritně se vyskytující onemocnění, které poprvé popsal skotský lékař Alan Lyell v roce 1956. Je charakterizováno rozsáhlou kožní a slizniční nekrolýzou postihující minimálně 30 % tělesného povrchu a systémovými projevy. Podle četnosti výskytu se jedná o extrémně vzácnou nozologickou jednotku s incidencí 0,5–1,5/1 milion obyvatel a rok. Leyllův syndrom je považován za toxoalergickou reakci, spouštěčem je nejčastěji některé léčivo. Syndrom je spojen s vysokou letalitou (knižně udávanou v rozmezí 30–90 %). Neodkladná lokální i celková léčba na jednotkách intenzivní péče nebo na specializovaném pracovišti může zlepšit celkově špatnou prognózu pacientů. V naší kazuistice popisujeme vzácný případ pacienta s rozvojem Lyellova syndromu po práci s běžným fungicidním prostředkem, který je častou součástí vybavení zahrádkářů a zemědělců. It is characterized by huge skin and mucosa necrolysis, which affects at least 30 % of body surface, and systemic symptoms. According to the frequency of the occurrence it is an extremely rare condition, with an incidence of 0.5–2 cases per million residents per year. Leyll’s syndrome is considered as a toxoallergic reaction, triggered mostly by some medication and it is associated with a very high mortality rate (in the literature reported between 30 to 90 %). Adequate and timely local and systemic treatment at the Intensive Care Unit or at the specialized clinic can improve the overall poor prognosis of the patients. In our case report we describe a very rare case of the Lyell’s syndrome after exposure to the antifungal organosulfur compound, which is widely used by the homegardners and farmers. Toxická epidermální nekrolýza je vzácné, akutní, život ohrožující onemocnění projevující se rozsáhlým odlučováním epidermis a slizničních povrchů. Jde o závažnou nežádoucí reakci na podávané léky, nejčastěji antibiotika, antikonvulziva či nesteroidní antirevmatika. Jako první ji popsal skotský dermatolog Alan Lyell v roce 1956, proto je též nazývána Lyellův syndrom. Mortalita tohoto onemocnění je vysoká, proto je klíčová včasná diagnostika a neodkladné zahájení adekvátní péče o pacienta. Jedná se o vzácné onemocnění s velmi malou incidencí a závažnou prognózou, což značně ztěžuje provedení velkých randomizovaných klinických studií. Je tedy obtížné hodnotit efektivitu jednotlivých terapeutických možností. Jako nejperspektivnější se prozatím jeví nitrožilní podávání imunoglobulinů se zřejmým zlepšením klinického stavu pacienta, dobrou tolerancí a minimálními vedlejšími nežádoucími účinky. Nové experimentální postupy se snaží hledat cestu terapie cíleným ovlivněním granulyzinu jakožto hlavního cytotoxického mediátoru. It is a serious adverse response to administered drugs, mostly antibiotics, anticonvulsants or NSAIDs. First described by Scottish dermatologist Alan Lyell in 1956, it is also known as Lyell’s syndrome. Mortality of the disease is high, therefore its early diagnosis is crucial and immediate initiation of appropriate patient care necessary. It is a rare disease with a very low incidence and serious prognosis, which is a considerable hindrance to undertaking large randomized clinical studies. It is therefore difficult to evaluate the effectiveness of various therapeutic options. As the most promising so far appears the administration of intravenous immunoglobulins, apparently reaching improvement in the clinical condition of the patient, with a good tolerances and minimal side adverse effects. New experimental techniques endeavour to seek a way to the therapy through targeted influencing of granulysin as the major cytotoxic mediator.
