Cíl. Posoudit přínos 18F-FDG-PET/CT k volbě léčebné strategie u renálního karcinomu. Metoda. Byla provedena analýza souboru 40 nemocných (28 mužů a 12 žen s průměrným věkem 67,8 let v rozpětí 53-86 let), kteří podstoupili 18F-FDG-PET/CT. U všech nemocných bylo na základě ultrasonografie podezření na lokálně objemný nebo pokročilý renální karcinom. 18F-FDG-PET/CT vyšetření byla provedena protokolem, který zahrnoval dvoufázovou CT angiografii se zachycením ledvin v arteriální a venózní fázi. U nemocných byla porovnána úroveň akumulace 18F-FDG s histologicky stanoveným gradingem a dále byl sledován vývoj onemocnění po dobu 12 měsíců od provedení 18F-FDG-PET/CT. Výsledky. Celková mortalita v souboru byla 45%, nejvyšší úroveň akumulace dosahovaly nádory 4. stupně gradingu (průměrná hodnota SUV 10,4, rozpětí 5-22) , nejvyšší mortalita byla u nádorů s výší akumulace přesahující 10 SUVmax (63, 6%). Nové informace přineslo 18F-FDG-PET/CT ve srovnání s nálezem CT u 87,5%. Závěr: 18F-FDG-PET/CT přispívá k rozhodnutí o terapii, zejména odhalením vzdálené diseminace, ale má potenciál i k odhadu prognózy pomocí stanovení maximálních hod not akumulace 18F-FDG., Aim. To assess the impact of 18F-FDG-PET/ CT on the treatment decision in renal cell carcinoma. Method. The analysis of the sample of 40 patients was made; it consists of 28 males, 12 females, mean age 67.8 years, ranging 53-86 years. All patients underwent 18F-FDG-PET/ CT including two-phase CT-angiography of the kidneys, locally advanced or generalized renal cell carcinoma was suspected in all patients. The level of the 18F-FDG accumulation within the tumor was compared with the histological grading and the development of the disease was surveyed twelve months after 18F-FDG-PET/CT. Results. Overall mortality reached 45%, the highest 18F-FDG accumulation showed tumor of the grade 4 (mean SUV 10.4, ranging 5-22), the highest mortality was present in tumors exceeded the SUVmax value of 10 (mortality 63.6%). New information was brought by 18F-FDG-PET/CT in comparison to CT in 87.5%. Cíl: Posoudit vlastní zkušenosti s prováděním PET/CT s fluorocholinem (18F-FCH-PET/CT) u karcinomu prostaty. Metodika: Byla provedena analýza indikací a výsledků 172 vyšetření 18F-FCH-PET/ CT u 139 mužů, průměrný věk nemocných byl 62,5 v rozpětí 48-81 let. Věnovali jsme se výskytu metastáz, vztahu hladiny PSA u biochemického relaps a přítomnosti nádorové tkáně po radikální léčbě a také porovnání bi-optického nálezu s nálezem PET/CT. Výsledky: U nemocných s hladinou PSA 2-5 ng/1 jsme nalezli metastázy nebo lokální rekurenci v 77,4 %, u hladin nad 5 ng/1 ve 100 %. Při porovnání nálezů biopsie s nálezy PET/CT jsme dosáhli senzitivity 96 %, specificity 83,3 %. Závěr: 18F-FCH-PET/CT je, jak ukazují naše výsledky, efektivním nástrojem v posuzování stagingu i restagingu karcinomu prostaty., Aim: To evaluate our own experience with fluorocholine 18F-FCH-PET/CT in prostatic carcinoma. Method: the analysis of the indications was performed in sample of 172 18F-FCH-PET/CT in 139 male patients, with mean age of 62.5 years in range 48-81 years. We evaluated number and site of metastases, the relation of PSA level in biochemical relaps with the presence of FCH accumulation in tumorous tissue and finally we correlate the findings of biopsy with results of PET/CT. Results: IN patients with PSA level 2-5 ng/1 we found in cases of biochemical relapse the accumulating tumorous tissue of local recurrence or metastases in 77.4%, in those patients with PSA level above 5 ng/1 in 100% of cases. In comparison of biopsies with PET/CT we found sensitivity 96% and specificity 83.3%. Uroteliální karcinomy vyžadují komplexní přístup ke stagingu, jelikož jejich typickou vlastností je multifokalita - možnost lokálně agresivního chování i vzdálené diseminace. V komplexním zobrazení je možné využít MDCT, MRI i 18F-FDG-PET/CT. Při hodnocení nádorů je zásadní odlišit invazi nádoru do okolní tukové tkáně nebo do parenchymu ledviny, eventuálně do okolních orgánů. Lokální šíření je možné posoudit poměrně spolehlivě pomocí MDCT i MRI. Pro posouzení vzdálené diseminace onemocnění je optimální metodou 18F-FDG-PET/CT, která nabízí odhalení metastáz v uzlinách ve skeletu, játrech i plicním parenchymu. In the assessment of the tumors, it is most important to distinguish the invasive spread of the tumor to the surrounding fatty tissue or into renal parenchyma or into neighboring organs respectively. Local invasion could be evaluated by MDCT or MRI with the relatively high accuracy. 18F-FDG-PET/CT is the optimal method able to search on the remote metastases. It offers depiction of the metastases in lymph nodes, in the skeleton, liver or lung parenchyma. Renální karcinom jako nejvýznamnější solidní nádor ledvin dosahuje v České republice nejvyšší incidence v celosvětovém měřítku. Odlišit jej mezi ostatními solidními a cystoidními lézemi je nejdůležitější úlohou zobrazovacích metod. Základní metodou je ultrasonografie, kterou je možné rozšířit pomocí aplikace kontrastní látky na metodu vysoce spolehlivou v diferenciální diagnostice solidních útvarů. K předoperačnímu zobrazení nádorů ledvin je nezbytné zobrazení cévního zásobení ledviny i vlastního tumoru pomocí CT angiografie nebo MR angiografie. Magnetickou rezonancí lze blíže specifikovat především hypovaskularizované nádory, u kterých se difuzní vážení ukazuje jako velmi důležité při odlišení papilárních renálních karcinomů. PET/CT s podáním fluorodeoxyglukózy je významnou metodou umožňující na základě míry akumulace fluorodeoxyglukózy posoudit agresivity renálního karcinomu., Renal cell carcinoma reaches the highest incidence in the Czech Republic world wide. The most important role of the imaging methods is to distinguish it in between the all solid and cyst-like renal lesions. Even the ultrasound (sometimes including application of contrast material) remains the basic imaging method, CT-angiography and MR-angiography is the most important recent methods to display its own tumorous tissue including the vessels supplying the tumor and the whole kidney. Magnetic resonance is able to improve the differential diagnosis in hypo-vascular (especially papillary renal cell carcinoma) when the diffusion weighted imaging is used.
