Introduction: Obstruction of the appendiceal lumen is the primary cause of appendicitis. The most common causes of luminal obstruction are fecaliths and lymphoid follicle hyperplasia. Additionally, bacterial infections or enteric and systemic viral diseases can cause a reaction of the lymphoid follicle. Case presentation: An 11-year-old boy with active phase of chickenpox presented on our Pediatric surgery emergency department under the impression of acute appendicitis. An appendectomy was performed on the same day. An inflamed and edematous retrocecal appendix was removed during surgery. Histological investigation of the appendix revealed transmural acute inflammation, with diffuse proliferation of inflammatory cells, with characteristic intranuclear inclusion surrounded by a clear halo. The PCR analysis of peripheral blood and appendix tissue specimen revealed positive VZV DNA. Akromegalie patří mezi vzácná a závažná onemocnění, základem její léčby je kombinace terapie chirurgické, radiační a medikamentózní. V práci se popisuje případ akromegalie diagnostikované před 10 lety ve stadiu, v němž byla možná již jen parciální resekce hypofyzárního tumoru. Přesto se podařilo dosáhnout dlouhodobé stabilizace stavu kombinací opakovaného chirurgického zákroku, radioterapie lineárním urychlovačem i Leksellovým gama nožem a medikamentózní léčby somatostatinovým analogem oktreotidem a blokátorem receptorů pro růstový hormon pegvisomantem. Dávka oktreotidu i pegvisomantu byla opakovaně měněna podle výsledků vyšetření hladiny IGF1, v současnosti je podáváno somatostatinové analogon Sandostatin LAR 30 mg jednou za 3 týdny a blokátor receptorů pro růstový hormon Somavert 20 mg denně. Přes přítomnost závažné choroby nemocný již po 10 let vede spokojený život. Náklady na finančně náročnou léčbu jsou z etického hlediska plně odůvodněné., Acromegaly is a rare and serious disease. A successful and rational therapy of acromegaly ought to combine surgery, radiotherapy and pharmacotherapy. The submitted article presents a case of acromegaly that was only diagnosed at the stage when total pituitary adenoma removal was impossible. Even so, the long-term stabilisation of the disease was reached by way of repeated surgery through transfenoidal and transcranial approach, by linear accelerator radiation therapy and Leksell Gamma Knife radiotherapy and by pharmacotherapy with somatostatin analogon octreotide and growth hormone receptor antagonist pegvisomant. The octreotide and pegvisomant dosage has been repeatedly changed according to IGF1 levels. The contemporary somatostatin analogon Sandostatin LAR 30 mg is given once every 3 weeks and the growth hormone receptor antagonist Somavert 20 mg is applied daily. Despite this serious disease, the patient has already been living contentedly for 10 years. Úvod: Alveolární echinokokóza je vzácné a závažné zoonotické parazitární onemocnění. Kazuistika: Autoři demonstrují případ mladého nemocného s postižením jater, bránice a plic touto formou infekce. Diagnóza echinokokové infekce byla stanovena na základě anamnézy, klinické symptomatologie v kombinaci s USG, CT, MRI a sérologickými metodami. Byla provedena radikální bloková resekce 7. jaterního segmentu, dolního plicního laloku vpravo a bránice. Definitivní stanovení diagnózy alveolární echinokokózy bylo provedeno histopatologicky a pomocí PCR metody ze vzorku resekované tkáně. Závěr: Nemocný je 8 měsíců po operaci bez potíž, trvale dispenzarizován s long-life terapií albendazolem., Introduction: Alveolar echinococcosis is a life-threatening zoonotic parasitic disease. Its incidence is rare. In some cases, the correct and timely diagnosis can be difficult. Case report: The authors present the case of a young patient with liver, diaphragm and lung involvement. The suspicion of echinococcus infection was made on the basis of medical history, clinical symptoms, and a combination of ultrasonography, computed tomography, magnetic resonance imaging tests and serological methods. The patient underwent multimodal treatment with albendazole and en-bloc resection of the liver, lung and diaphragm. The definitive diagnosis of alveolar echinococcosis was determined from samples of the resected tissues using histopathology and polymerase chain reaction methods. The patient has been followed regularly and is on life-long treatment with albendazole. Plicní aspergilóza patří mezi zánětlivá onemocnění způsobená houbami rodu Aspergillus. V závislosti na stavu imunitního systému a plicního parenchymu nemocného existují invazivní, semi-invazivní a neinvazivní formy onemocnění. V diagnostice tohoto onemocnění hrají hlavní úlohu prostý rentgenový snímek hrudníku a HRCT plic. Pseudocysta pankreatu patří mezi nejčastější cystické léze pankreatu. Pseudocystu lze definovat jako tekutinovou intrapankreatickou, peripankreatickou nebo extrapankreatickou kolekci s definovanou stěnou vznikající nejčastěji na pokladě chronického nebo akutního zánětu pankreatu. Jde o poměrně častou komplikaci akutní nebo chronické pankreatitidy. V diagnostice má zásadní význam UZ pro svou neinvazivitu a dostupnost. Mezi další vyšetření vykazující nejvyšší přesnost, specificitu a senzitivitu patří MDCT a ERCP. Na základě MDCT a ERCP vyšetření lze stanovit klasifikaci a na základě této klasifikace následně algoritmus léčby. MDCT vyšetření by optimálně mělo předcházet ERCP. K odlišení od maligních cystických lézí pak prokazuje největší význam EUS s FNAB nebo aspirací obsahu se stanovením onkomarkerů a AMS v aspirátu. Funkční klasifikace pseudocyst je důležitým předpokladem pro úspěšný algoritmus léčby. Klasifikace vycházejí zejména z anatomického vztahu pseudocysty k pankreatickému vývodu a morfologii parenchymu pankreatu. V léčbě se uplatňují zejména perkutánní drenáž, endoskopické a chirurgické metody., The pseudocyst of the pancreas is one of the most common cystic lesions of the pancreas. The pseudocysts can be defined as intrapancreatic, peripancreatic or extrapancreatic fluid collections with a defined wall formed mostly on the basis of acute or chronic pancreatitis. This is a relatively common complication of acute or chronic pancreatitis. Sonography is essential for the diagnosis given its noninvasivity and availability. Other assessments that show the highest accuracy, specificity and sensitivity include MDCT and ERCP. The MDCT and ERCP assessments can be used to classify the pseudocyst; the treatment algorithm is then determined based on this classification. MDCT should ideally be preceded by ERCP. EUS-FNAB or aspiration with the determination of oncomarkers and AMS in the aspirate have been shown to be most important to distinguish the pseudocyst from malignant cystic lesions. Functional classification of the pseudocyst is an important prerequisite for successful algorithm of the treatment. The classification is based mainly upon anatomical relations of the pseudocyst to the pancreatic outlet and morphology of the pancreatic parenchyma. Nádory hlavy a krku patří celosvětově mezi karcinomy se vzrůstající incidencí. Hlavním etiologickým faktorem podmiňujícím jejich vznik je kouření, abúzus alkoholu a v poslední době potvrzený HPV. Dvě třetiny pacientů přicházejí v pokročilém stadiu a až třetina pacientů má četné komorbidity. V jejich terapii je nejdůležitější multidiciplinární přístup. Standardem léčby je dle NCCN konkomitantní chemoradioterapie s platinovým derivátem (kategorie 1), nebo radioterapie konkomitantně s biologickou léčbou cetuximabem, nebo chirurgický přístup a dále dle histologie adjuvantní radioterapie či chemoradioterapie (1). V poslední době je zvažována v indikaci léčby také pozitivita HPV. Jedná se o pacienty s lepší prognózou, ale zatím nebyla stanovena standardní terapie. Cetuximab je monoklonální chimerická IgG1 protilátka, která cíleně blokuje extracelulární část EGFR. V České republice je zaregistrován mimo jiné pro léčbu pacientů s lokálně pokročilými nádory hlavy a krku konkomitantně s radioterapií dle Bonnerovy studie fáze III publikované v r. 2006 v N Engl J Med (2), která prokázala pro kombinaci radioterapie a cetuximabu proti radioterapii samotné signifikantní navýšení lokoregionální kontroly 24 měsíců versus 14,9 měsíců, s mediánem OS 54,0 měsíců versus 29,3 měsíců, bez signifikantního navýšení morbidity a toxicity., Worldwide, head and neck tumours are among cancers with an increasing incidence. The main aetiological factors determining their development are smoking, alcohol abuse, and recently detected HPV. Two thirds of patients present in an advanced stage and up to a third of them have numerous comorbidities. A multidisciplinary approach is of utmost importance in the treatment. The standard treatment according to NCCN is concomitant chemoradiotherapy with a platinum derivative (category 1) or radiotherapy concomitantly with biological therapy with cetuximab, or surgical approach, followed by, according to histology, adjuvant radiotherapy or chemoradiotherapy (1). Recently, HPV positivity has also been taken into consideration concerning indications for treatment. This applies to patients with a better prognosis, but no standard treatment has been determined so far. Cetuximab is a chimeric IgG1 monoclonal antibody that targets the extracellular domain of the EGFR. Nádory oblasti hlavy a krku představují v Evropě u mužů čtvrté nejčastější nádory. Nejvyšší výskyt je pozorován v zemích okolo Středozemního moře. Incidence nádorů orofaryngu v České republice je 0,91/100 000 obyvatel s mortalitou 0,65/100 000 obyvatel, nejvíce zastoupeným klinickým stadiem nádoru je klinické stadium IV (58,9 %). U nádorů patra je incidence 0,34/100 000 obyvatel s mortalitou 0,15/100 000 obyvatel. Nejvíce zastoupeným klinickým stadiem je také stadium IV (26,7 %). Postižení žen je výrazně nižší než u mužů, v ČR je poměr mužů k ženám přibližně 4:1. Z hlediska rizikových faktorů hraje rozhodující vliv kouření a konzumace alkoholu. Průměrný věk pacientů se pohybuje mezi 50 a 60 lety. V kazuistikách bude představen pacient s nádorem orofaryngu a pacient s nádorem měkkého a tvrdého patra a patrové tonzily. V obou případech šlo o pokročilé lokální nálezy bez vzdálených metastáz u „typických“ pacientů mužského pohlaví s dlouholetým abúzem alkoholu a tabakizmem., Head and neck tumours are the fourth most common cancer among men in Europe. The highest incidence is in the Mediterranean countries. The incidence rate of oropharyngeal cancer in the Czech Republic is 0.91/100 000 inhabitants with a mortality rate of 0.65/100 000 inhabitants; the most prevalent clinical stage of the tumour is clinical stage IV (58.9%).
