In order to study cardiac pathology in different subtypes of ischemic stroke we examined 165 stroke patients. The 1st group included 90 (54.5%) patients with hemodynamic stroke, the 2nd one - 75 (45.5%) patients with cardioembolic stroke. Control group consisted of 45 individuals without cerebrovascular diseases. Cardiac pathologies with the prevalence of various types of ischemic heart disease were observed in all groups. The parameters of left ventricular stroke volume and ejection fraction were lower in patients with hemodynamic stroke than in other groups, but were within normal limits. Potential sources of cardiogenic embolism were found in all groups: in the 1st group at 74.4%, in the 2nd one at 100%. Cíl studie: Metabolismus železa musí být velmi pečlivě regulován. Klíčovým systémovým regulátorem je peptidový hormon hepcidin, který způsobuje “uzamčení” železa uvnitř buňky; to pak nemůže být utilizováno. Dialyzovaní pacienti často trpí anémií, která je způsobena mnoha faktory, mj. stavem mikrozánětu a také zřejmě zvýšenou retencí hepcidinu v důsledku snížené glomerulární filtrace. Naším cílem bylo popsat vztahy mezi hepcidinem a dalšími parametry metabolismu železa, zánětu a erytropoezy u těchto pacientů. Typ studie: Observační Název a sídlo pracoviště: Ústav klinické biochemie a hematologie LF UK a FN v Plzni, Alej Svobody 80, 304 60 Plzeň Materiál a metody: Do studie bylo zařazeno 164 chronicky dialyzovaných pacientů (věk 66 ± 13, 25 - 92 let; 101 mužů a 63 žen) a 37 zdravých kontrol (věk 55 ± 20, 21 - 92 let; 16 mužů a 21 žen), u kterých byl stanoven jejich kompletní krevní obraz, parametry metabolismu železa, zánětu a výživy. Výsledky: Koncentrace železa, transferrinu a hemoglobinu byla významně nižší u dialyzovaných pacientů (p<0,0001), naopak ferritin (p<0,0001), solubilní transferrinové receptory(p<0,05), hepcidin (p=0,0003), CRP a IL-6 (p<0,0001) byly u těchto pacientů významně vyšší než u zdravých kontrol. Hepcidin slabě koreloval pozitivně s CRP a ferritinem a negativně s transferrinem u dialyzovaných pacientů, u zdravých kontrol nebyla nalezena žádná korelace hepcidinu s ostatními parametry. Závěr: Zdá se, že vliv zánětu na hladiny hepcidinu není u hemodialyzovaných pacientů zcela zásadní a na jeho koncentraci se podílí i jiné faktory, např. jeho retence., Objective: Peptide hormone hepcidin is a key systemic regulator of the iron metabolism. Hepcidin binds to the iron cell exporter ferroportin so iron is kept in the cells unavailable for erythropoiesis. The synthesis of hepcidin is up-regulated by high iron stores and inflammation. Dialyzed patients have very often impaired iron management – they suffer from anemia, which is caused by many factors including the state of micro inflammation and hepcidin retention due to decreased glomerular filtration rate. Our aim was to describe the relationship of hepcidin and other parameters of iron metabolism, erythropoiesis and inflammation. Design: Observation Settings: Institute of Clinical Biochemistry and Hematology, Charles University Hospital in Pilsen, Alej Svobody 80, 304 60 Plzeň Material and Methods: Complete blood cell count, iron, ferritin, transferrin, CRP, albumin, creatinine, hepcidin, soluble transferrin receptors (sTfR) and IL-6 were measured in samples from 164 patients included in chronic hemodialysis program (age 66 ± 13, 25 - 92 years), 63 women and 101 men and 37 control healthy volunteers (age 55 ± 20, 21 - 92 years), 21 women and 16 men. Results: Iron, transferrin and hemoglobin were significantly lower in the patients group (p<0.0001) while ferritin (p<0.0001), sTfR (p<0.05), hepcidin (p=0.0003), CRP and IL-6 (p<0.0001) were significantly higher in the patients group. Hepcidin weakly positively correlated with CRP and ferritin and weakly negatively correlated with transferrin in hemodialyzed patients. No correlation of hepcidin with other biochemical parameters in controls was found. Conclusion: It seems that the influence of inflammation on hepcidin levels in hemodialyzed patients is not crucial and other factors including its retention in end-stage renal disease participate., T. Hyperviskózní syndrom u pacientů s monoklonálními gamapatiemi vzniká jednak na podkladě patologicky zvýšené hladiny monoklonálního imunoglobulinu, jednak vlastnostmi monoklonálního imunoglobulinu. Vysoká hodnota paraproteinu způsobí nárůst viskozity séra, což vede k rozmanitým klinickým projevům, které označujeme jako hyperviskózní syndrom. Tento stav řadíme mezi akutní stavy v hematologii, neboť pacienta ohrožuje fatálními komplikacemi (např. cévní mozková příhoda, kardiální léze, respirační selhání, ...). Po jeho potvrzení by měla být co nejdříve provedena výměnná plazmaferéza, současně je nutné zahájení léčby základního onemocnění. Víno pijeme nejméně 10 000 let nebo déle, v našem regionu již od doby keltské a germánské. Pokus používat víno k léčbě začíná v dávnověku, avšak teprve v současné době byl prokázán jeho příznivý účinek v prevenci infarktu myokardu, cévní mozkové příhody, diabetes mellitus a hypertenze. Pití velmi dobře působí také v pokročilém věku. Nejlepší účinky má každodenní pití při večeři, u mužů 20–40 g alkoholu, u žen polovina této dávky., We have been drinking wine at least for 10 000 years or longer, in our region already from Celtic and Germanic times. Favorable effects on prevention of myocardial infarction, stroke, diabetes and hypertension were demonstrated recently, following attempts to use alcohol for treatment in ancient times. Favorable effects of wine drinking could be seen also in aging population. The best results for men are reached by daily drinking of 20–40 g alcohol at dinner. According to our research, Uzbek individuals with Parkinson’s disease have high prevalence of 0/0 polymorphisms of GSTT1 and GSTM1 genes, as well as combinations of GSTT1 (0/0)/GSTM1/(0/0) genotypes. These mutations are associated with an earlier debut of the disease, its mixed form, and rapid rate of progression. Purpose: To investigate the protective action of Ceruloplasmin (Cp) on the lysis of erythrocytes in men with prostate tumors. Material and Methods: The blood erythrocytes of the patients with benign hyperplasia of prostate (BHP) and prostate adenocarcinoma (CaP) were studied. Patients at the age 60-75 with early stages of a cancer have been investigated. The control group was consisted of apparently healthy males with the compatible age. n=15 for each group. The clinical stage of the disease was diagnosed by means of rectal, histological and echographic examination of the prostate gland. Photometric methods were applied to register lysis dynamics in order to test out the protective action of Cp. Results: The nonspecific protective function of Cp preparation on BHP and CaP erythrocytes as well as on the control group erythrocytes was revealed. CaP erythrocytes have shown more sensitivity to the lysis provocative factor than BHP and the control group erythrocytes, that was presumably attributed to the structural and functional changes of the erythrocytes developed in the presence of malignant tumor., N. V klinické kazuistice je prezentována role cetuximabu ve 2. linii terapie metastatického kolorektálního karcinomu s následnou udržovací monoterapií, kdy byla u pacienta dosažena kompletní remise nádorového onemocnění. Uvedený případ potvrzuje účinnost léčby cetuximabem u předléčených pacientů s metastazujícím kolorektálním karcinomem za předpokladu nemutovaného genu K-RAS. Jedná se tak o cílenou biologickou léčbu, která se stává standardem personalizované medicíny nemocných s pokročilým nádorovým onemocněním., The clinical case report presents the role of cetuximab in the second-line treatment for metastatic colorectal cancer with subsequent maintenance monotherapy in which the patient achieved a complete remission of a cancerous disease. The present case confirms the efficacy of treatment with cetuximab in pretreated patients with metastatic colorectal cancer in the presence of the unmutated K-RAS gene. Spontaneous rupture is a rare and dramatic complication of hepatocellular carcinoma (HCC), burdened by a high mortality. Here we describe a case of a 73-year-old man, who arrived at the ER because of syncope, and acute epigastric and right upper quadrant abdominal pain. He had a history of hepatitis C-related liver cirrhosis and HCC in treatment with sorafenib. The physical examination showed a state of hemorrhagic anemia with the presence of blood in the peritoneal cavity. The patient underwent an urgent liver resection. Thirty days after surgery, he was in good general condition. Sorafenib is a multikinase inhibitor recently introduced in the therapy of patients with advanced HCC. Among the various side effects reported in patients treated with sorafenib, there is a higher risk of bleeding. V letech 1959–2008 počet zhoubných novotvarů centrální nervové soustavy (ZN CNS) z dat registru nádorů ČR dosáhl 26 548, z toho 14 263 (53,7 %) u mužů a 12 285 (46,3 %) u žen, v roce 2008 představující u mužů 412 (8,1/100 tis.), u žen 418 (7,9/100 tis.) onemocnění. Jejich prevalence v letech 1989–2005 vzrostla z 1169 na 2 565 žijících případů s očekávaným počtem 3 430 v roce 2015. Z 962 vícečetných ZN CNS, evidovaných v letech 1976–2005, bylo 150 primárních onemocnění u mužů a 162 u žen, 327 následných ZN CNS u mužů a 323 u žen tj. 5,1 % ze všech nově hlášených u mužů a 6,2 % u žen. Za toto období vzrostl v pětiletých intervalech počet primárních případů dvakrát, následných téměř třicetkrát. U primárních a následných ZN CNS převažovaly ve věku do 49 let ženy nad muži, v 50–69 letech muži nad ženami. Ze 14 krajů se vyskytlo 47 % ZN CNS ve čtyřech krajích (Moravskoslezský, Jihomoravský, Praha, Středočeský). Pouze 6,2 % multiplicit bylo evidováno v časných a 7,4 % v pokročilých klinických stadiích; 86,4 % stadií nebylo nahlášeno. Průměrný interval mezi 150 primárními ZN CNS a 170 jinými následnými novotvary u mužů byl pět let, mezi 162 primárními a 170 následnými u žen byl šest let, ze kterých byly u obou pohlaví nejpočetnější následné karcinomy trávicího ústrojí a kůže. Před 327 následnými ZN CNS bylo u mužů 390 primárních novotvarů, u žen před 323 následnými 362 primárních, ze kterých byly nejpočetnější karcinomy kůže, rodidel, prsů, prostaty a trávicího ústrojí. V říjnu 2007 žilo v době analýzy z 962 ZN CNS jen 91 a zemřelo 871 nemocných s převahou žijících žen nad muži., The number of malignant neoplasms of central nervous system (MN CNS), based in the Czech Cancer Registry, reached 26,548 diseases, of which there were 14,263 (53.7 %) of males and 12,285 (46.3 %) of females in 1959–2008, presented 412 (8.1 per 100,000 males) and 418 (7.9 per 100,000 females) diseases in 2008. Their prevalence increased from 1,169 to 2,565 survived cases between 1989–2005, with 3,430 cases expected in 2015. A total of 962 registered multiple MN CNS there were 150 primary diseases in males and 162 in females, 327 subsequent MN CNS in males and 323 in females, i. e. 5.1 % in males and 6.2 % in females of all newly registered MN CNS in period 1976–2005. The primary and subsequent MN CNS prevailed in the age group till 49 years in females over males, in the age group 50–69 years in males over females. A total of 14 Czech regions there were distributed 47 % of all multiple MN CNS in the four regions (Northern and Southern Moravia, Prague, Central Bohemia). Only 6.2 % multiplicities were registered in early and 7.4 % in advanced clinical stages; 86.4 % stages were unregistered. The average interval between 150 primary MN CNS and 170 other subsequent neoplasms was five years in males, between 162 primary and 170 subsequent cases six years in females, of which the most frequent were following digestive and skin carcinomas in both sex. In 327 subsequent MN CNS preceded 390 primary cancers in males, in 323 subsequent were 362 primary cases in females, of which the most frequent were carcinomas of skin, female genital and breast, prostate and digestive tract. Bortezomib (Velcade) je lék ze skupiny inhibitorů proteasomu. Představuje zcela nový a jedinečný léčebný postup v léčbě mnohočetného myelomu (MM). Tato práce přináší retrospektivní analýzu léčebné účinnosti a nežádoucích účinků u nemocných s MM léčených v referenčních centrech CMG v České republice. Celkem bylo hodnoceno 424 nemocných MM léčených bortezomibem od června 2004 do června 2010. Medián délky sledování byl 12,4 měsíců. Průměrný věk nemocných byl 66 let (34,1–86,5). Jednalo se především o nemocné s relabujícím MM. Celkem 48,3 % (205/424) nemocných mělo bortezomib v rámci druhé léčebné linie. Léčebnou odpověď bylo možné hodnotit u 83,3 % (353/424) nemocných. Léčebné odpovědi (? 50% pokles MIg) bylo dosaženo u 52,7 % (186/353), z toho bylo u 14,2 % (50/353) nemocných dosaženo kompletní remise. Nejzávažnější toxicitou byla periferní neuropatie stupně 3 a vyšší u 14 % (49/358) nemocných. Trombocytopenie stupně 3 a vyšší byla u 28,8 % (103/357) nemocných. Infekční komplikace stupně 2 a vyšší byly u 33,6 % (121/359) nemocných. V průběhu léčby bortezomibem nedošlo v našem souboru nemocných ke zvýšení frekvence výskytu trombo embolických komplikací. Mimořádný přínos bortezomibu v klinické praxi je dán rychlým nástupem léčebné odpovědi a vysokým počtem dosažených kompletních remisí. Většina jeho nežádoucích účinků se zpravidla daří zvládat pomocí vhodných profylaktických opatření, případně krátkodobým přerušením a následným snížením dávky podle platných doporučení., Bortezomib (Velcade) is a representative of class of medicines called proteasome inhibitors. This drug represents quite new and unique medical treatment of the patients with multiple myeloma (MM). This article presents a retrospective analysis of effectiveness and adverse effects in patients with multiple myeloma treated at reference centers of CMG in the Czech Republic. A total 424 patients with multiple myeloma treated with bortezomib from June 2004 to June 2010 were evaluated. The median follow-up was 12,4 months. The mean age of patients was 66 year (34,1–86,5) and majority of them were patients with relapsed multiple myeloma. Of the total 424 patients, 205 patients had bortezomib as second line therapy (48,3 %). Assessment of terapeutic response was possible in 83,3 % of treated patients (353/424). Therapeutic response (defined as a ? 50 % decrease in the serum MIG) was achieved in 52,7 % (186/353). Among mentioned responses, complete response was obtained in 14,2 % (50/353). The most severe toxicity observed was peripheral neuropathy, grade 3 or higher was developed in 14 % (49/358). There was recorded a thrombocytopenia grade 3 or higher in 28,8 % (103/357), further occurance of infections grade 2 or higher in 33,6 % (121/359) and eventually we could observe increased number of thromboembolic disease as well. Rapid onset of therapeutic responses and high percentage of obtained complete responses are the main reasons of extraordinary clinical benefit of bortezomib.
