Hlavní stanovisko práce: Retrospektivní hodnocení výsledků aplikace rázové vlny u 81 pacientů s induratio penis plastica (Mb. Pyeronie) po předchozí neúspěšné konzervativní terapii na elektrohydraulickém litotryptoru MEDILIT 7. Cíl: Cílem naší práce je retrospektivní zhodnocení skupiny 81 mužů s induratio penis plastica před a po aplikaci rázových vln do oblasti fibrózních plátů. Soubor pacientů a metoda: U skupiny 81 mužů s průměrným věkem 57,6 roku (33–75 let) s IPP jsme aplikovali rázové vlny do oblasti fibrózních plátů. Všichni pacienti podstoupili předchozí neúspěšnou konzervativní terapii. Délka onemocnění byla průměrně 21,4 měsíce (12–37 měsíců). Kritériem k zařazení do souboru bylo onemocnění trvající minimálně 12 měsíců. U všech pacientů se plát nacházel na dorzu penisu v oblasti kořene a deformoval penis. Všichni muži pociťovali subjektivní obtíže při sexuálním styku. Tyto obtíže jsme hodnotili IIEF-5 dotazníkem, který dosahoval před výkonem průměrně 11,7 bodu (3–17 bodů). Velikost plátu byla průměrně 12,5 mm (8–30 mm) dorzální angulace při erekci průměrně 55 stupňů (20–90 stupňů). Rázové vlny jsme aplikovali ve třech dobách. Rázové vlny jsme generovali na elektrohydraulickém litotryptoru MEDILIT 7. Výsledky: Ze sledovaného souboru 81 pacientů došlo ke zlepšení u 56 mužů. Deviace penisu při erekci se u nich průměrně zmenšila o 66% a velikost plátu o 28,2%. Skóre v dotazníku IIEF-5 vzrostlo o 67%. Všichni zlepšení pacienti dosáhli uspokojivého sexuálního styku. Vážné komplikace jsme nezaznamenali. Závěr: Podle našich sedmiletých zkušeností se jedná o konzervativní, bezpečnou metodu, která u části pacientů, kteří podstoupili předchozí neúspěšnou konzervativní terapii, může zlepšit sexuální život. Jako prognostický faktor úspěchu léčby v našem souboru se zdá být věk pacienta., Retrospective evaluation of shock wave therapy (ESWT) results in 81 patients with Peyronie´s disease after unsuccessful conservative treatment. ESWT was performed using electrohydraulic lithotriptor MEDILIT 7. Aim: Authors retrospectively evaluate results of extracorporal shock waves therapy (ESWT) in 81 patients with Peyronie´s disease after unsuccessful conservative treatment. Methods: In a group of 81 men with an average age of 57.6 years (33–75 years) with induratio penis plastica (Peyronie’s disease) we applied shock waves into the fibrous sheets. All patients had failed conservative treatment. The length of illness was an average of 21.4 months (12–37 months). The criteria for inclusion in the group were disease lasting at least 12 months. All patients had plaque on the dorsum of the penis root and a deformed penis. All men felt subjective symptoms during sexual intercourse. These difficulties were evaluated using the IIEF-5 questionnaire, which scored an average 11.7 points (3–17 points) prior to treatment. The size of the plaque was approximately 12.5 mm (12–37 mm), dorsal angulation during erection averaged 55 degrees (20–90 degrees). Shock waves were applied in three sessions. ESWT was performed using the electrohydraulic lithotriptor MEDILIT 7. Results: From the group of 81 patients an improvement was observed in 56 men. Deviation during erection improved by an average of 66%. The size of plaque was reduced by 28.2%. IIEF-5 questionnaire score increased by 67%. All patients had satisfactory improvement of sexual intercourse. We recorded no serious complications. Conclusion: According to our seven year experience this is a safe, non-invasive method which can improve the sexual life of a proportion of patients who had previously failed conservative therapy. Chromý a kol. v r. 2009 zjistili, že stanovení celkové sérové bílkoviny biuretovým činidlem za použití albuminových kalibrátorů, vypracované v r. 1981 Doumasovou pracovní skupinou jako kandidáta na referenční metodu, poskytuje výsledky s neakceptovatelnou systematickou pozitivní chybou, způsobenou zejména endogenním sérovým bilirubinem a lipidy. Zjistili též, že k nápravě stačí užít bíkovinné standardy na bázi séra nebo plasmy, které jsou komutabilní se vzorky pacientů, a jsou certifikovány Kjeldahlovou metodou. V našich dvou předchozích publikacích, (Chromý a kol., 2015 a Vinklárková a kol., 2015), jsme ověřili Kjeldahlovy postupy užívané v laboratorní medicíně. Zvolili jsme přímou analýzu ze sraženiny bílkovin promyté kyselinou trichloroctovou a prověřili jsme její analytické provozní parametry. V této práci popisujeme zjednodušenou analýzu s modifikovanou Kjeldahlovou digesční baňkou, kterou lze přímo připojit k tomu přizpůsobené Parnes-Wagnerově destilační aparatuře a uvádíme její validační parametry. Zjednodušenou Kjeldahlovu metodu navrhujeme jako standardní operační postup pro stanovení celkové bílkoviny v referenčních materiálech užívaných v klinické chemii., Chromý et al. found in 2009, that albumin-calibrated analysis of serum total protein with biuret reagent according to a candidate reference method developed by Doumas working group in 1981, provide results with unacceptable positive bias, caused namely by endogeneous serum bilirubin and lipids. They also found, that a simplest way to rectify this defect, is the use of serum and/or plasma-based protein standards, commutable with patients samples, which are certified by the Kjeldahl method. In our foregoing two articles, (Chromý et al., 2015, and Vinklárková et al., 2015), we reviewed Kjeldahl methods adopted by laboratory medicine, selected a direct analysis on washed protein precipitates with trichloroacetic acid and verified its preliminary analytical performance parameters. In this article, we describe a simplified Kjeldahl analysis with modified digestion flask, now directly connectable to accordingly adapted Parnas and Wagner distillation unit, and present its re-validation. Early diagnosis of optic neuritis (ON) is essential to prevent or limit the structural damage and permanent loss of visual function. The aim of this study was to estimate prognostic importance of the neuron-specific enolase (NSE) in blood serum and lachrymal fluid in optic neuritis. The clinical-diagnostic examination was performed on patients with optic neuritis, including optical coherence tomography (OCT) and Visually Evoked Potentials (VEP), MR – tractography, as well as analysis of NSE content in blood serum and in lachrymal fluid. Decreased level of NSE in lachrymal fluid in patients with acute stage of optic neuritis and increased level of NSE in patients with optic neuritis at the stage of transition into atrophy of optic nerve disc were detected. Cíl studie: Cílem metodického sdělení bylo zavedení a validace dostupné a praktické enzymatické metody stanovení Dlaktátu v krevní plazmě a moči. Zavedená metoda byla využita ke stanovení koncentrací D-laktátu v plazmě v souboru 117 pacientů v perioperační péči. Typ studie: Jedná se o metodické sdělení s uvedením výsledků prospektivní observační klinické studie. Název a sídlo pracoviště: Centrum pro výzkum a vývoj, Fakultní nemocnice Hradec Králové Materiál a metody: Krevní plazma je deproteinována ultrafiltrací, moč je analyzována přímo. Vlastním principem metody jsou dvě spřažené enzymové reakce katalyzované D-laktát dehydrogenázou při redukci nikotinamid-adenin-dinukleotidu a Dglutamát- pyruvát transaminázou. Měřena je konečná koncentrace redukovaného nikotinamid-adenin-dinukleotidu při 340 nm. Soubor pacientů studie byl tvořen 117 pacienty podstupujícími resekci tlustého střeva nebo konečníku pro operabilní karcinom kolorekta. Odběry byly prováděny předoperačně, dvě hodiny po operaci a první až čtvrtý pooperační den. Výsledky: V plazmě bylo dosaženo preciznosti CV 1,5 - 4,9 % podle koncentrace. Kvantifikační limit metody je na koncentraci 10 μmol/l. Detekční limit metody je na koncentraci 5 μmol/l. Metoda je lineární v rozsahu 10 μmol/l až 1 mmol/l. Bazální předoperační hodnoty 33,4 (26,4; 39,7) μmol/l stouply již za dvě hodiny po ukončení operace na koncentraci 90,2 (78; 101) μmol/l, která dosáhla vrcholu v první pooperační den a následně došlo v průběhu dalších tří dní k návratu k původním hodnotám. Závěr: Uvedená metoda může být vzhledem ke své dostupnosti, robustnosti i jednoduchosti snadno zavedena v biochemické laboratoři, kde se vyskytne požadavek na provádění tohoto vyšetření., Objective: The aim of the study was to develop and validate an easily available and robust enzymatic method for determination of D-lactate in blood plasma and urine. The developed method was used for the evaluation of plasmatic D-lactate levels in patients in perioperative care. Design: The article describes the methodology and presents the results of a prospective, observational study. Settings: Center for Research and Development, University Hospital Hradec Králové Material and Methods: Blood plasma was deproteinated by ultrafiltration; urine may be analysed directly. The principle of the method is the coupled reaction of D-lactate dehydrogenase reducing nicotinamide adenine dinucleotide and D-glutamate pyruvate transaminase. The end-point concentration of nicotinamide adenine dinucleotide is determined as absorbance at 340 nm. The patient cohort consisted of 117 patients operated for colorectal cancer. The blood samples were collected preoperatively, 2 hours after operation, and daily from the first to the fourth postoperative day. Data are presented as median (interquartile range). Results: The precision achieved in plasma was 1.5 – 4.9 %. A quantification limit of 10 μmol/L and detection limit of 5 μmol/l were found. The method is linear from 10 μmol/L to 1 mmol/L. Basal preoperative values of 33.4 (26.4; 39.7) μmol/L increased within two hours postoperatively to 90.2 (78; 101) μmol/L, and D-lactate concentration peaked on the first postoperative day. The concentration returned to basal values on the fourth day. Charakteristika nádorů varlat u souboru 84 pacientů a jejich srovnání s daty udávanými v Národním onkologickém registru a literatuře. Cíl: Srovnání charakteristik a výsledků terapie tumorů varlat v našem souboru pacientů s literaturou. Metody: V období let 2006–2010 jsme u všech pacientů, kteří na našem pracovišti podstoupili radikální orchiektomii, vyhodnotili dobu sledování, věk, dobu trvání příznaků, stranové zastoupení, klinické stadium, typ léčby a její výsledky. Nálezy byly srovnány s daty zveřejněnými v Národním onkologickém registru a články ze zemí různých socioekonomických standardů v Evropě i mimo ni. Výsledky: Soubor tvoří 84 pacientů, u kterých bylo provedeno 87 radikálních orchiektomií pro tumor. Průměrná doba sledování byla 44,9 měs ců (rozmezí 18–76 měsíců). Seminom byl nalezen ve 45 (51,7%), nonseminom v 37 (42,5%) a lymfom v pěti (5,7%) případech. Průměrný věk pacientů se seminomem byl 36 let (rozmezí 20–77 let) a průměrná doba trvání příznaků 14,8 týdnů (1 den– 104 týdnů). Ze 45 nádorů (42 pacientů) bylo 40 ve stadiu I (88,9%), čtyři ve stadiu II (8,9%) a jeden ve stadiu III (2,2%). Z 37 u nás adjuvantně léčených či sledovaných pacientů je 36 v remisi (97,3%) a jeden zemřel na interní komorbiditu po dvouleté remisi. Průměrný věk pacientů s nonseminomem byl 32 let (rozmezí 20–59) a průměrná doba trvání pří- znaků 9,5 týdne (1–52 týdnů). Z 37 nádorů bylo 20 ve stadiu I (54,1%), šest ve stadiu II (16,2%), osm ve stadiu III (21,6%). Z 33 u nás pooperačně léčených či sledovaných pacientů je 32 v remisi (96,9%), jeden zemřel na progresi tumoru. Závěr: Výskyt seminomů v našem souboru je mírně častější než výskyt nonseminomů. Nonseminomy byly zachyceny v pokročilejších stadiích. Kurabilita obou skupin nádorů je dobrá a vzájemně srovnatelná. Naše výsledky jsou srovnatelné s vý- sledky udávanými v západoevropských zemích., Characteristics of testicular tumors in a group of 84 patients and a comparison with data form Czech National Cancer Registry and literature. Aim: To compare characteristics and treatment results of testicular tumours in our group of patients. Methods: Time of follow up, age, duration of symptoms, laterality, clinical stage, treatment modality and its results were evaluated in all patients that underwent radical orchiectomy at our department between 2006 and 2010. Our findings were then compared with information provided in literature Results: A total of 87 radical orchiectomies for tumor were performed on 84 patients. Out of these tumors 45 (51.7 %) were seminomas, 37 (42.5 %) were non seminomas and 5 (5.7 %) were lymphomas. The average time of follow-up was 44,9 months (range 18–76 months). The average age of patients with seminoma was 35 years (range 20–77 years) and the average duration of symptoms was 14,8 weeks (1 day – 104 weeks). Out of 45 seminomas, 40 (88.9%) were in stage I, 4 (8.9%) in stage II, and 1 (2.2%) in stage III. Out of 37 treated or observed patients with seminoma, all reached remission. The average age of patients with non-seminoma was 32 years (range 20–59 years) and the average duration of symptoms was 9,5 weeks (1–52 weeks). Out of 37 non seminomas, 20 (54.1%) were in stage I, 6 (16.2%) in stage II, 8 (21.6%) in stage III. Out of 33 treated patients 32 are in remission (96.9%), 1 patient died of tumor progression. Conclusion: Seminomas were present in a slightly higher number of patients whilst nonseminomas were detected in more advanced stages compared to seminomas. The curability of both groups of tumors is similar and altogether good. The increase in the number of acute respiratory infections in children, accompanied by airway obstruction, often leads to the development of stenotic laryngotracheitis. The primary factors are respiratory viruses, and bacterial flora often joining together, changing the type of the disease which is determined by its outcome. Exposure to infectious agents in the body contribute to the development of chronic infectious diseases of the respiratory tract in children, resulting in damages to the ciliary epithelium and weakening of its connection with the basal cells and the basement membrane. This, in turn, facilitates the penetration of allergens and other inflammation stimulants into submucosal tissue. We have studied the state of the microbial landscape of the mucous membranes in the upper respiratory tract in 275 patients with stenosing laryngotracheitis; they are children aged 6 months to 5 years. All patients were divided into 2 groups: group 1 included 122 children with primary stenosing laryngotracheitis (PSLT); group 2 were 153 patients with recurrent stenosing laryngotracheitis (RSLT). We noted that the most common mucosal lesion of germs in all age groups falls on Staphylococcus aureus. It has been established that the nasopharynx microbial defeat is found twice as often at RSLT than at PSLT. Príspevok pojednáva o teoretických a praktických aspektoch podávania, vybavovania a prešetrovania sťažností a podávania a vybavovania petícií v podmienkach Slovenskej republiky. Zaoberáme sa v jednotlivých častiach príspevku prijímaním sťažností, príslušnosťou orgánu verejnej správy na vybavenie sťažnosti, prešetrovaním a vybavovaním sťažností. Pozornosť venujeme obsahu a výkonu petičného práva ako aj postupu pri podávaní a vybavovaní petícií. Príspevok sme doplnili o grafické spracovanie počtu podaných sťažností a petícií v rezorte zdravotníctva v rokoch 2007–2014., This paper deals with theoretical and practical aspects of submission, handling and investigation of complaints and the submission and processing of petitions in the Slovak Republic. In different parts of the paper we deal with taking complaints, membership of a public authority for dealing with complaints, investigations of complaints. In the paper we pay attention to the content and performance of right of petition, as well as the procedure for filing and processing of petitions. The aim of this research was to investigate the macro- and microelements composition in the cerebrospinal fluid and in the blood serum of patients, with different consequences of TBI before and after complex treatment, with the use of endolumbal and intracystal introduction of ozone and pyracetam in dynamics. Macro- and microelements composition was investigated in the cerebrospinal fluid and serum of 83 patients. State of neurological disorders in patients evaluated by the Glasgow Outcome Scale was extended.
