Poruchy spánku patria k najčastejším non‑motorickým prejavom u pacientov s Parkinsonovou chorobou (PCh), v populačných štúdiách je uvádzaný ich výskyt až u 60 % pacientov. Štúdie tiež uvádzajú rozdielnosť prejavov PCh medzi mužmi a ženami. Cieľ: Zistiť, či sú u pacientov s PCh na Slovensku prítomné medzi pohlaviami rozdiely v klinických prejavoch PCh a výskyte porúch spánku. Materiál a metódy: Zisťovanie bolo robené dotazníkovou metódou s účasťou pacientov a neurológov z jednotlivých regiónov Slovenskej republiky. Dotazníky boli vyplňované počas rutinných kontrolných návštev. Boli zisťované socioekonomické premenné ako pohlavie, vek, vek pri začiatku ochorenia, stupeň poškodenia hodnotený škálou Hoehna a Yahrovej, dominujúci fenotyp ochorenia a používaná medikácia. Pri hodnotení porúch spánku bola použitá slovenská verzia dotazníka PDSS. Výsledky: Získali sme dáta od 1 067 pacientov. Pri porovnávacích analýzach neboli zistené podstatné rozdiely vo väčšine hodnotených parametrov. U mužov bol o niečo vyšší podiel pacientov s hodnotou škály Hoehna a Yahrovej 4 a boli zistené signifikantne vyššie dávky levodopy v monoterapii (p < 0,01). Významné rozdiely boli zistené v skóre škály PDSS pre hodnotenie rušivých halucinácií počas noci – skóre u mužov bolo 8,22, u žien 8,48 (p < 0,05). Podobný výsledok sme zaznamenali v skóre hodnotiacom nutnosť močenia počas noci – u mužov bolo skóre 5,90, u žien 6,53 (p < 0,01). Differences in symptoms between men and women have been reported. Aim: In the present study, we assessed whether there are gender differences in clinical presentation of PD and prevalence of sleep disturbances in individuals diagnosed in the Slovak Republic. Material and method: Questionnaires were distributed to participating neurologists and patients in outpatient practices across the Slovak Republic. Sociodemographic variables – gender, age, age at onset, disease severity according to Hoehn and Yahr stage, phenotype of the dominating symptom of Parkinson’s disease and type of medication – were collected. The Slovak language version of the PDSS was used in a questionnaire to test sleep disturbances. Results: Data from 1,067 outpatients with PD were collected. Comparative analyses showed males and females not to be significantly different on the majority of the demographic and medical characteristics collected. Males had a slightly higher proportion of individuals with Hoehn and Yahr score 4 and, among those taking levodopa medication as monotherapy, males took significantly higher levodopa dose than females (p < 0.01). A significant difference in the distribution of PDSS subscores between males and females was observed on item 7 (distressing hallucinations at night) – score for males 8.22, for females 8.48, p < 0.05. Gliomy limbického a paralimbického systému představují anatomicky a tím pádem i operačními přístupy unikátní skupinu mozkových nádorů. Mikrochirurgie v kombinaci s moderními perioperačními postupy zvyšuje šance na radikální odstranění těchto gliomů. Z hlediska lokalizace je dělíme do tří základních skupin: 1. gliomy inzuly, 2. amygdalohippokampálního komplexu a 3. cingula. V minimonografii se věnujeme anatomii i funkci limbického a paralimbického systému a v návaznosti na ni představujeme ucelený pohled na jednotlivé neurochirurgické přístupy k těmto oblastem. Maximální resekce za zachování funkční integrity mozku je předpokladem následné smysluplné onkologické léčby. Taktéž prezentujeme výsledky operací našeho pracoviště ve smyslu morbidity a radikality za posledních sedm let., Limbic and paralimbic gliomas represent a unique group of brain tumors both from an anatomic and surgical point of view. Microsurgery, together with modern perioperative procedures, enables a wider extent of resection of these gliomas. Based on localization, we divide gliomas into three basic groups: 1. insular, 2. amygdalohippocampal complex and 3. cingulum gliomas. In our review, we describe the anatomy and function of the limbic and paralimbic system and in relation to that, we present a description of the specific neurosurgical approaches to these regions. Maximal resection with brain function preservation represents the basis for subsequent oncologic therapy. We also present the surgical results of our department in the last seven years with regard to morbidity and radicality. Cíl studie: Na základě soudobých studií podat přehled o hlavních biologických funkcích lipoproteinů o vysoké hustotě (HDL), zejména ve vztahu k reverznímu transportu cholesterolu a k jejich protizánětlivému působení. Popsat pravděpodobné mechanismy vzniku dysfunkčních prozánětlivých HDL a uvést některé parametry asociované s tímto procesem. Podat přehled o laboratorních metodách stanovení kvantitativních, strukturních a funkčních vlastností HDL, včetně moderních metod založených na hmotové spektrometrii. Typ studie: přehledový článek Závěr: V případě systémového zánětu a/nebo oxidačního stresu dochází ke vzniku dysfunkčních HDL akumulujících oxidanty. Probíhá strukturní modifikace apolipoproteinu AI, což vede mimo jiné k inhibici reverzního transportu cholesterolu a tvorbě prozánětlivých HDL. Na ztrátě příznivých vlastností HDL se podílí modifikace proteinové i lipidové složky HDL. Izolované měření HDL cholesterolu nemusí u některých syndromů bezvýhradně korelovat s mírou kardiovaskulárního rizika a v některých situacích je vhodné stanovit markery mapující strukturu a funkci HDL., Objective: On the basis of recent studies to make an overview concerning the crucial biological functions of high density lipoproteins (HDL), with emphasis on the role in reverse cholesterol transport and their antiinflammatory traits. The aim was to describe probable mechanisms of dysfunctional proinflammatory HDL formation with introduction some of associated parameters. To present laboratory methods for determination of quantitative, structural and functional qualities of HDL, including advanced mass spectrometry techniques. Study design: review Conclusion: In the case of systemic inflammation and/or oxidative stress, the formation of dysfunctional HDL accumulating oxidants takes place, including apo AI structural modifications. This process can lead to the inhibition of reverse cholesterol transport and proinflammatory HDL generation, among others. Loss of beneficial qualities results from modification of both lipid and protein components of HDL. Cíl: Autoři prezentují první zkušenosti s hemihypoglossofaciální anastomózou při řešení postoperačních plegií lícního nervu u neurochirurgických pacientů. Soubor a metodika: V průběhu let 2009–2014 byli na našem oddělení operováni tři pacienti, u kterých byla provedena hemihypoglossofaciální anastomóza dle Cushimana a Sekhara. Důvodem plegie nervus facialis byl u dvou pacientů objemný vestibulární schwannom (v jednom případě operován po ozáření gama nožem) a u jednoho pacienta objemný schwannom nervus facialis pod bazí lební. Výsledky: U dvou pacientů bylo dosaženo reinervace ve stupni III a u jednoho pacienta bylo dosaženo reinervace ve stupni IV dle House a Brackmanna. U všech pacientů se rozvinula pouze lehká hemilinguální atrofie, která u žádného z nich neměla žádné funkční následky ve smyslu poruchy žvýkání, polykání či fonace. Průměrný follow‑up pacientů byl 39,3 měsíce. Závěry: Naše první zkušenosti ukázaly u tří pacientů dobrou reinervaci nervus facialis po hemihypoglossofaciání anastomóze. Metodu považujeme spolu s dalšími variantami hemihypoglossofaciálních spojek (Darrouzet, LeClerc) za efektivní způsob neurotizace plegického lícního nervu při malém riziku vzniku nežádoucích příznaků., Aim: Authors present their early experience with a hemihypoglossofacial anastomosis in the treatment of neurosurgical patients with complete postoperative facial palsy after removal of tumors in the posterior fossa or skull base. Patients and method: In three patients undergoing an open surgery for complete facial palsy between 2009 and 2014, the hemihypoglossofacial anastomosis according to Cushimano and Sekhar was performed. Facial palsy was due to a large vestibular schwannoma in two patients and a facial nerve schwannoma in one patient. Results: Functional reinnervation of grade III according to the House and Brackmann scale was achieved in two patients and reinervation of grade IV in one patient. All patients had mild hemilingual atrophy only that did not lead to any functional disturbances in chewing, swallowing or speech. The mean follow-up period was 39, 3 months. Conclusions: Our early experience showed very good functional reinervation in patients after hemihypoglosofacial anastomosis. We consider this method together with other modifications (Darrouzet, LeClerc) to be very effective in the treatment of facial plegia with low risk of side effects. Cíl studie: Metabolismus železa musí být velmi pečlivě regulován. Klíčovým systémovým regulátorem je peptidový hormon hepcidin, který způsobuje “uzamčení” železa uvnitř buňky; to pak nemůže být utilizováno. Dialyzovaní pacienti často trpí anémií, která je způsobena mnoha faktory, mj. stavem mikrozánětu a také zřejmě zvýšenou retencí hepcidinu v důsledku snížené glomerulární filtrace. Naším cílem bylo popsat vztahy mezi hepcidinem a dalšími parametry metabolismu železa, zánětu a erytropoezy u těchto pacientů. Typ studie: Observační Název a sídlo pracoviště: Ústav klinické biochemie a hematologie LF UK a FN v Plzni, Alej Svobody 80, 304 60 Plzeň Materiál a metody: Do studie bylo zařazeno 164 chronicky dialyzovaných pacientů (věk 66 ± 13, 25 - 92 let; 101 mužů a 63 žen) a 37 zdravých kontrol (věk 55 ± 20, 21 - 92 let; 16 mužů a 21 žen), u kterých byl stanoven jejich kompletní krevní obraz, parametry metabolismu železa, zánětu a výživy. Výsledky: Koncentrace železa, transferrinu a hemoglobinu byla významně nižší u dialyzovaných pacientů (p<0,0001), naopak ferritin (p<0,0001), solubilní transferrinové receptory(p<0,05), hepcidin (p=0,0003), CRP a IL-6 (p<0,0001) byly u těchto pacientů významně vyšší než u zdravých kontrol. Hepcidin slabě koreloval pozitivně s CRP a ferritinem a negativně s transferrinem u dialyzovaných pacientů, u zdravých kontrol nebyla nalezena žádná korelace hepcidinu s ostatními parametry. Závěr: Zdá se, že vliv zánětu na hladiny hepcidinu není u hemodialyzovaných pacientů zcela zásadní a na jeho koncentraci se podílí i jiné faktory, např. jeho retence., Objective: Peptide hormone hepcidin is a key systemic regulator of the iron metabolism. Hepcidin binds to the iron cell exporter ferroportin so iron is kept in the cells unavailable for erythropoiesis. The synthesis of hepcidin is up-regulated by high iron stores and inflammation. Dialyzed patients have very often impaired iron management – they suffer from anemia, which is caused by many factors including the state of micro inflammation and hepcidin retention due to decreased glomerular filtration rate. Our aim was to describe the relationship of hepcidin and other parameters of iron metabolism, erythropoiesis and inflammation. Design: Observation Settings: Institute of Clinical Biochemistry and Hematology, Charles University Hospital in Pilsen, Alej Svobody 80, 304 60 Plzeň Material and Methods: Complete blood cell count, iron, ferritin, transferrin, CRP, albumin, creatinine, hepcidin, soluble transferrin receptors (sTfR) and IL-6 were measured in samples from 164 patients included in chronic hemodialysis program (age 66 ± 13, 25 - 92 years), 63 women and 101 men and 37 control healthy volunteers (age 55 ± 20, 21 - 92 years), 21 women and 16 men. Results: Iron, transferrin and hemoglobin were significantly lower in the patients group (p<0.0001) while ferritin (p<0.0001), sTfR (p<0.05), hepcidin (p=0.0003), CRP and IL-6 (p<0.0001) were significantly higher in the patients group. Hepcidin weakly positively correlated with CRP and ferritin and weakly negatively correlated with transferrin in hemodialyzed patients. No correlation of hepcidin with other biochemical parameters in controls was found. Conclusion: It seems that the influence of inflammation on hepcidin levels in hemodialyzed patients is not crucial and other factors including its retention in end-stage renal disease participate., T. BACKGROUND: In the cholesteatoma surgery ossicles can be replaced to reconstruct middle ear function. It is important that these ossicles are free of squamous epithelium, to prevent residual disease. This study focuses on the histological findings of the malleus and incus harvested during cholesteatoma surgery. MATERIALS AND METHODS: Eighty middle ears ossicles were examined in vivo and histologically to consider the relationship of cholesteatoma to ossicles, grade of bone destruction and invasion of cholesteatoma to deeper layers of bone. RESULTS: Serious ossicular destruction was observed more frequently in incus compared to malleus (p=0.0065). Difference of ossicles destruction between children and adults was not significant (p=0.3032). Deep invasion of cholesteatoma into the vascular spaces or inner core of the bone was not observed. CONCLUSIONS: Autograft ossicles from cholesteatomatous ears should not necessarily be rejected for reconstruction of the ossicular chain. V léčbě uveálních melanomů se v některých případech používá radiochirurgie CyberKnifem. Histologickými a imunohistologickými změnami v nádorové tkáni po terapii se zabývá poměrně málo autorů. V tomto sdělení uvádíme výsledky vyšetření dvou takových případů. U obou pacientů byla provedena enukleace bulbu pro bolestivý glaukom. U prvého pacienta šlo o recidivující melanom, který byl ošetřen gama nožem před deseti lety. Pro lehkou progresi ložiska melanomu bylo provedeno ošetření CyberKnifem celkovými dávkami 45Gy ve 3 frakcích. Po necelých pěti měsících po ozáření bulbu došlo k regresi recidivujícího nádoru z původní prominence 1,4 mm v ploše 6 mm × 0,6 mm na prominenci 0,3 mm v ploše 3 mm × 0,3 mm. Histologicky byla zaznamenána rezidua řídce celulárního ložiska regresivně změněných nádorových buněk a přítomnost reaktivních změn optického nervu. Druhý pacient byl měsíc po radioterapii CyberKnifem s následnou komplikovanou operací katarakty a s enukleací bulbu bez regrese – tedy zániku nádorových buněk. Velikost tumoru 11,1 mm × 15mm s prominencí 9,3mm přetrvávala i po ozáření dávkou 40Gy/1Fr. V histologickém vyšetření byly nádorové buňky bez patologických změn. V imunohistochemickém vyšetření jsme pozorovali u prvého pacienta vysokou difuzní expresi proteinu bcl-2 až do výše 75 % a méně než 1% pozitivitu ve vyšetření s proliferačním markerem Ki-67. U druhého případu byla situace odlišná, s výraznou až 75% pozitivní expresí bcl-2 proteinu v nádorových buňkách v povrchových vrstvách tumoru a s klesajícím počtem pozitivní exprese v hlubokých vrstvách nádoru, kde dosahovala maxima do 25 %. Proliferační aktivita s pozitivní jadernou expresí v Ki-67 vzrůstala od povrchu, kde se pohybovala v rozmezí 1–3 % do hloubky nádoru a k jeho bázi, kde dosahovala až 5 %. Naše pozorování ukazuje na výraznou redistribuci genetické aktivity uvnitř nádoru. Ta se projevuje „overexpresí“ bcl-2 proteinu a u druhého případu je patrná rozdílná pozitivita bcl-2 proteinu v různých vrstvách nádoru. Vztah tohoto fenomenu k radioterapii není dosud jasný., Radiosurgery with CyberKnife is used in the treatment of uveal melanomas in some cases. Histological and immunohistological changes in tumour tissue following treatment have been addressed by relatively few authors. This paper presents the results of investigations of two such cases. Enucleation of the eye was performed for painful glaucoma in both patients. Patient 1 had a recurrent melanoma treated with a Gamma Knife ten years ago. Due to slight progression in the melanoma site, treatment with CyberKnife was used with total doses of 45 Gy in 3 fractions. Some five months following eye irradiation, there was regression of the recurrent tumour from the original height of 1.4 mm in an area of 6 mm x 0.6 mm to that of 0.3 mm in an area of 3 mm x 0.3 mm. Histologically, residua of a sparsely cellular locus of regressively altered tumour cells and the presence of reactive changes in the optic nerve were noted. Patient 2 underwent radiotherapy with CyberKnife one month prior, with subsequent complicated cataract surgery and enucleation of the eye without regression, i.e. destruction of tumour cells. The size of the tumour of 11.1 mm x 15 mm with a height of 9.3 mm remained unchanged even after irradiation with a dose of 40 Gy/1 fr. No pathological changes in the tumour cells were seen on histological examination. In patient 1, immunohistochemistry showed high diffusion expression of the bcl-2 protein of up to 75 % and less than 1 % positivity on examination with the proliferation marker Ki-67. In patient 2, the situation was different, with significant, as high as 75 % positive expression of the bcl-2 protein in tumour cells in the superficial tumour layers and a decreasing rate of positive expression in the deep tumour layers where it reached no more than 25 %. The proliferation activity with positive nuclear expression of Ki-67 increased from the surface, where it ranged from 1 % to 3 %, to the depth of the tumour and toward its base, where it reached up to 5 %. Our observation suggests significant redistribution of genetic activity within the tumour. It is manifested by overexpression of the bcl-2 protein and, in patient 2, different bcl-2 protein positivity in the various tumour layers is apparent. Z historického přehledu principů tvorby metod pro odhad výšky postavy člověka na základě skeletu vyplývá, že lze vymezit čtyři základní skupiny metod: metody podílové, regresní, anatomické a metody organické korelace. Každá má svá specifika a úskalí, která bezprostředně vyplývají z jejich matematické konstrukce. Nejčastěji se k odhadu výšky postavy používají některé rozměry dlouhých kostí končetin. Bylo teoreticky prokázáno, že nejpřesnější možné odhady poskytují metody regresní, zejména když k odhadu využijeme kombinaci rozměrů z více kostí. Nutným předpokladem použití lineární regrese je však užití pouze těch rovnic, které byly vytvořeny na základě výběru z populace, z níž pochází i odhadovaný jedinec. V opačném případě, tj. pokud je lineární regrese použita pro odhad jedinců z populace jiné, může dojít k menšímu či většímu vychýlení odhadu. Tato skutečnost byla již mnohokrát empiricky ověřena. Teoretický rozbor lineární regrese ukázal, že jde o důsledek jevu označovaného jako „regrese k průměru“. Metody organické korelace (založené na minimalizaci součtu plochy reziduálních trojúhelníků) jsou méně náchylné k systematické chybě, jsou však také méně přesné. Lze je ale aplikovat na libovolnou populaci a pro jednotlivé případy z neznámé populace jsou nejpřesnější. Anatomické metody jsou aplikovatelné jen v případě dostatečně zachovaného skeletu. Jsou založeny na součtu výšek všech (nebo téměř všech) kostí skeletu, které se na výšce postavy přímo podílejí. Rozdíl mezi výškou postavy za života a součtem délek kostí skeletu od lebky po patní kost se pohybuje v řádu centimetrů a prostor pro chybu odhadu je tak malý. Ať už pak k odhadu výšky ze skeletu v rámci anatomické metody použijeme kterýkoliv ze tří výše uvedených matematických postupů (podílová metoda, lineární regrese, organická korelace), rozdíly mezi jimi pořízenými odhady jsou zanedbatelné a jejich rozlišování pozbývá smyslu. Celkově jsou tedy anatomické metody nejpřesnější a je třeba je doporučit, kdykoliv to stav zachování skeletu dovolí., A historical review of principles guiding the formulation of methods for stature estimation based on measurements of the human skeleton reveals that it is possible to delimit four basic groups of methods: anatomical, constant ratio, linear regression, and organic correlation methods. Each has its own specific features and limits, which are a result of their mathematical constructions. Long bones of limbs are most often used for estimates of stature. It was demonstrated theoretically that linear regression models provide the most precise and accurate estimates of stature from particular skeletal elements, especially when several suitable bones are used in combination. A necessary condition for using the linear regression equations is, however, that we use equations based on the population from which the estimated individual originated. In contrast, when a regression equation is applied to an individual originating from a different population smaller or larger systematic error of the estimate may result. This fact was assessed empirically on numerous occasions and it was demonstrated on the basis of linear regression theory that a phenomenon known as “regression to the mean” can be considered as the cause. Organic correlations (built on the least-triangle area criterion) are less prone to systematic errors, but they are also less precise. They can be applied to any population and are best suited for cases where the population of origin is unknown. Anatomical methods are best applied when the skeleton is adequately preserved as this method requires bone lengths of all (or almost all) bones contributing to body height. The difference between height of the living body and the sum of lengths of relevant bones from head to heel is in the order of centimeters so the estimate error margin is small. Consequently, the differences between estimates obtained from using the different mathematical approaches (constant ratio, linear regression or organic correlation) are insignificant, so there is little benefit in preferring any particular one. Sledování expozice látkám, široce užívaným v průmyslu, a vyhodnocení jejich koncentrací v těle člověka, je jedním z hlavních cílů biologického monitorování obyvatel. V pilotní studii byla sledována hladina metabolitů ftalátu dietylhexylftalátu (DEHP), vybraných metabolitů polyaromatických uhlovodíků a bisfenolu A v moči ve vztahu k možným expozičním zdrojům sledovaných dotazníkovým šetřením. Do studie bylo zařazeno 95 studentů nekuřáků ve věkovém rozmezí 20 až 29 let (44 mužů a 51 žen) z Prahy a Ostravy. Analyty byly stanovovány v moči pomocí metody kapalinové chromatografie s tandemovou hmotnostně-spektrometrickou detekcí typu trojitý kvadrupól (LC-MS/MS). Hodnoty metabolitů DEHP u žádného jedince nepřekročily zdravotně významné limitní hodnoty. Za pomoci dotazníku, který byl součástí studie, nebyly zjištěny žádné skutečnosti, které by významně ovlivňovaly hladiny vybraných látek v moči. Byl zjištěn pouze signifikantní vliv pohlaví u 5-OH-MEHP a sumy 5-OH-MEHP a 5-oxo-MEHP, kdy ostravské ženy mají významně vyšší hladiny metabolitů než muži., The main target of human biomonitoring is monitoring of exposure and effects of substances used in industry and determination of their concentrations in the human body. In this pilot study the metabolite levels of phthalates, polycyclic aromatic hydrocarbons and bisphenol A were determined in urine. Their association with possible exposure sources was analyzed. The study comprised 95 adults (44 males and 51 females) aged 20 and 29 years from Prague and Ostrava. The analytes in urine samples were determined by liquid chromatography with triple quadrupole tandem mass spectrometry detection (LC-MS/MS). Metabolite levels did not exceed limit values for these biomarkers for any participant. The questionnaire survey, as part of the study, revealed no factors that might affect levels of the selected substances in urine.
