Frustrace z neuspokojivých výsledků léčby vedla odbornou veřejnost k odstartování celosvětové kampaně za přežití sepse. Ischemická choroba srdeční (ICHS) a další degenerativní kardiovaskulární choroby včetně chlopenních vad se ve vyšších věkových skupinách vyskytují podstatně častěji než u mladších nemocných. Současná široká dostupnost srdečních operací přispívá ke zvyšování jejich počtu u seniorů. Cílem naší práce bylo zhodnotit časné výsledky srdečních operací u nemocných starších 70 roků. V letech 2003- 2007 bylo v Centru kardiovaskulární a transplantační chirurgie operováno 2 379 nemocných této věkové kategorie. Pro ICHS bylo operováno 1 326 pacientů, pro onemocnění srdeční chlopně (ChloV) 654 pacientů a 399 pacientů mělo kombinovanou vadu (KombV). Průměrný věk nemocných byl 75,1 +- 3,7 roků a průměrná funkční třída byla 2,9. Ve skupině s ICHS byly implantovány průměrně 2,8 bypassu/ pacienta, u 139 nemocných byla revaskularizace provedena na bijícím srdci a alespoň jeden tepenný štěp byl použit u 255 nemocných. Ve skupině nemocných s chlopenní vadou nebo kombinovaným postižením byl zákrok na jedné chlopni proveden u 883 nemocných, zákrok na dvou chlopních u 153 a na třech chlopních u 17 nemocných. Hospitalizační mortalita byla ve skupině ICHS 3,8 %, ve skupině ChloV 4,3 % a ve skupině KombV 6,8 %., Coronary artery disease and other degenerative cardiovascular diseases including valve defects occur significantly more frequently in elderly patients than in younger age groups. The current broad availability of cardiac operations contributes to the increasing numbers of seniors undergoing such operations. The aim of our study was to assess the early results for patients over the age of seventy. In the years 2003-2007 2 379 patients in this age group underwent operations in the Centre of cardiovascular surgery and transplantations in Brno. 1 326 patients were operated on for coronary artery disease (CAD), 654 patients were treated for valve disease (VAD) and 399 patients underwent a combined procedure (COD). The average age was 75.1 +- 3.7 years and the average functional class was 2.9. In the group of patients undergoing CAD 2.8 bypasses per patient were implanted, 139 were operated on off pump and at least one arterial graft was used in 255 patients. In the group of VAD and COD patients one valve was replaced or repaired in 883, two valves in 153 patients and three valves in 17 patients. V úvodu sdělení jsou připomenuty základní změny imunity ve stáří dané involucí složek samotného imunitního systému, jako je snížená humorální i buněčná odezva, ale i predisponujícími orgánovými změnami - sníženou žaludeční kyselostí, dysfunkcí řasinkového epitelu, zpomalením odtoku moči, sníženou kožní trofikou apod. Dále jsou infekce starších nemocných rozděleny na infekce komunitní a nozokomiální. Z komunitních infekcí je věnována pozornost specifikům respiračních, močových, gastrointestinálních epizod a infekcím kožních defektů. Podstatná část sdělení se zabývá nozokomiálními infekcemi ve vyšším věku - jejich epidemiologií, nejčastějšími původci, možnostmi preventivních opatření. Podrobněji jsou diskutovány nozokomiální pneumonie a močové infekce, ranné infekce a infekční komplikace spojené s invazivními monitorovacími a léčebnými metodami. V tomto přehledovém sdělení je stručně nastíněna problematika fyziologie přenosu vzruchu senzitivními a viscerálními nervovými vlákny a patofyziologie viscerální bolesti přenášené z útrobních orgánů do Haedových zón na kůži. Text pojednává o viscerálních receptorech, o aferentních dráhách a centrálních strukturách, o mediátorech i o možnostech ovlivnění viscerální bolesti. V posledních letech se objevují stále častější upozornění na nedostatečno u diagnostiku a léčbu depresivních stavů u seniorů, udává se výskyt deprese u 15 % seniorů žijících ve vlastním prostředí a u 30 % seniorů žijících v institucích. Dále jsou opakovaně referovány nepříznivé důsledky dlouhodobě neléčené deprese na celkový vývoj zdravotního stavu seniorů. Cílem naší práce bylo zjistit aktuální situaci v záchytu deprese v běžné seniorské populaci a o jejím vlivu na zdravotní a funkční stav seniorů. Soubor nemocných a metodika. Soubor nemocných tvoří 161 seniorů ve věku nad 65 let žijících ve vlastním prostředí v péči praktických lékařů a geriatrické ambulance. Tito nemocní byli vyšetřeni testy funkčního geriatrického vyšetření a základním laboratorním souborem. Byl vyhodnocen výskyt diagnostikované a nediagnostikované deprese a srovnání celkového zdravotního a funkčního stavu seniorů s depresí a bez deprese. Výsledky. Ve sledovaném souboru byla deprese diagnostikována u 30 (18,6 %) seniorů, léčeni byli pouze 4 (2,5 %). U seniorů s depresí byl prokázán významně nižší kognitivní výkon (test Mini Mental- State Examinati on - MMSE), významně horší výsledky testů soběstačnosti (Activities if Daily Living - ADL, Instrumental ADL Scale - IADL), vyšší počet užívaných léků a vyšší počet diagnóz, pro které byli tito nemocní léčeni. Závěry: 1. Deprese je v běžné seniorské populaci poddiagnostikovaná, a tudíž nedostatečně léčená. 2. Neléčená deprese výrazným způsobem zhoršuje vývoj zdravotního stavu seniorů a jejich soběstačnost. 3. Nemocní s neléčenou depresí jeví výraznou tendenci k vyšší potřebě zdravotnické péče. 4. V praxi je nutno seniory s depresí aktivně vyhledávat a jejich depresi řešit., The words of caution in relation to insufficient diagnoses and treatment of depression in seniors had become more frequent over the recent years; the prevalence of depression is estimated to be ar und 15% in seniors living in their own environment and 30% in seniors living in institutions. Furthermore, negative consequences of long-term untreated depression on the overall health status of seniors are repeatedly being reported. The aim of our research was to ascertain the current situation in detection of depression in a common senior population and to ascertain the effect of depression on the seniors' health and functional status. Patient sample and methods. The patient sample comprised of 161 seniors over 65 years of age, living in their own environment and being in the care of general practitioners and geriatrists. These patients were assessed using geriatric functional assessment tests and the core laboratory assessments battery. We analyzed the prevalence of diagnosed and undiagnosed depression and compared overall health and functional status of seniors with and without depression. Results. Depression was diagnosed in 30 (18.6%) seniors in the researched sample; only 4 (2.5%) were treated. Seniors with depression had significantly lover cognitive performance (Mini Mental- State Examinati on test - MMSE), significantly worse results of the self-sufficiency test (Activiti es of Daily Living - ADL, Instrumental ADL Scale - IADL), used higher number of drugs and had higher number of diagnoses for which they were treated. Trastuzumab is a humanized monoclonal antibody directed against the HER-2 receptor. Trastuzumab-based therapy significantly improves response rate (RR), time to progression (TTP) and overall survival (OS) for women with HER-2 positive metastatic breast cancer. Despite its initial efficacy, acquired resistance to trastuzumab develops in a majority of patients with MBC, and a large subset never responds, demonstrating primary resistance. The purpose of this retrospective study was to determine prognostic factors applicable to clinical practice. METHODS: We enrolled 112 women with metastatic breast cancer, who started the trastuzumab-based therapy at Masaryk Memorial Cancer Institute until January 2007. Clinical and laboratory factors, such as: patients conditions, character ofmetastatic spread, histology, estrogen, progesterone and Her-2 receptor status, Her-2/neu gene amplification, and serum tumor markers CEA, CA 15-3 and extracellular domain of Her-2 receptor (S-HER-2 ECD) were monitored. The association of all factors to response to therapy, time to progression (TTP) and overall survival (OS) was assessed. RESULTS: In 95% patients, the trastuzumab was combined with cytostatics (83% taxanes), 88,4% of patients started the trastuzumab as the first or second-line anticancer treatment. The median TTP was 284 days (9,3 months) and the median OS was 612 days (20,1 months) for all patients, RR was 54,5%. The highest RR was associated with the first-line treatment (p<0.0001) and with HER-2 gene/Chromosome 17 ratio > 2,2 (p=0,0092). Eleven patients (9,8%) discontinued the treatment because of toxicity, 7 patients did it as a result of cardiotoxicity (6,2%). CNS metastases occurred in 31 patients (27,7%). The S-HER-2 ECD was the most frequently elevated serum marker at the time of the treatment initialization (72,5%) and at the time of the progression (55,9%). Cox regression analysis identified S-HER-2 ECD levels at the beginning and between day 90 and 130 of the trastuzumab therapy as the best predictors of TTP. On the other hand the best predictor of OS was level of CEA before the treatment started and level of S-HER-2 ECD between day 90 and 130 of the trastuzumab therapy. Zlomeniny krčku stehenní kosti u seniorů představují medicínský, sociální a ekonomický problém. V předkládané práci je vyhodnocen soubor 411 pacientů, kterým byla v období 1. 1. 1996 až 31. 12. 2006. implantována endoprotéza kvůli zlomenině krčku stehenní kosti. V souboru bylo 71 % žen a 29 % mužů, byl patrný meziroční nárůst počtu pacientů, zejména v dekádě mezi 70 a 80 roky věku. V 91 % byla implantována totální, v 9 % cervikokapitální endoprotéza. Autoři se zaměřili na vyhodnocení komplikací léčby. 85 % pacientů bylo bez komplikací, v 11 % byly komplikace interního stavu, ve 4 % ortopedické komplikace (3 % luxací, 0,5 % povrchových infektů, 0,5 % aseptického uvolnění). 56 % pacientů bylo propuštěno do domácí péče, na Rehabilitační kliniku bylo přeloženo 24 % pacientů a 20 % pacientů bylo propuštěno do LDN. Diskutovány jsou možnosti konzervativní a operační léčby. V prevenci se zdůrazňuje nejen terapie osteoporózy, ale zejména nutnost komplexní péče o seniory, z hlediska léčby pak odpovědná indikace s vědomím možných rizik jak konzervativního, tak operačního postupu. Nezbytná je dobře fungující následná zdravotní a sociální péče., Femoral neck fractures in seniors have their own medical, social and economic problems. We evaluated 411 patients (71 % women and 29 % men) who were treated of arthroplasty due to femoral neck fracture in period 1. 1. 1996?31. 12. 2006. We observed a rising number of patients each year, especially in the decade between 70 and 80 years of patient?s age. 91 % patients were treated of a total hip arthroplasty, 9 % patients were treated of a hemiarthroplasty. We evaluated the complications of our treatment. 85 % patients were without any complications, 11 % patients had complications of their internal status, 4 % patients had the orthopaedics complications (3 % dislocations, 0.5 % superficial infections, 0.5 % aseptic loosenings). 56 % patients were discharged home, 24 % patients were discharged to the Department of Rehabilitation and 20 % patients had a sequential therapy in medical institutions. We discussed the conservative and operative possibilities of treatment. We emphasised not only the treatment of osteoporosis but especially the necessity of the comprehensive care of seniors. As for the treatment we emphasised the necessity of the responsible indication of both conservative and operative treatment, possible complications were kept in mind.
