Cíl. Posoudit přínos 18F-FDG-PET/CT k volbě léčebné strategie u renálního karcinomu. Metoda. Byla provedena analýza souboru 40 nemocných (28 mužů a 12 žen s průměrným věkem 67,8 let v rozpětí 53-86 let), kteří podstoupili 18F-FDG-PET/CT. U všech nemocných bylo na základě ultrasonografie podezření na lokálně objemný nebo pokročilý renální karcinom. 18F-FDG-PET/CT vyšetření byla provedena protokolem, který zahrnoval dvoufázovou CT angiografii se zachycením ledvin v arteriální a venózní fázi. U nemocných byla porovnána úroveň akumulace 18F-FDG s histologicky stanoveným gradingem a dále byl sledován vývoj onemocnění po dobu 12 měsíců od provedení 18F-FDG-PET/CT. Výsledky. Celková mortalita v souboru byla 45%, nejvyšší úroveň akumulace dosahovaly nádory 4. stupně gradingu (průměrná hodnota SUV 10,4, rozpětí 5-22) , nejvyšší mortalita byla u nádorů s výší akumulace přesahující 10 SUVmax (63, 6%). Nové informace přineslo 18F-FDG-PET/CT ve srovnání s nálezem CT u 87,5%. Závěr: 18F-FDG-PET/CT přispívá k rozhodnutí o terapii, zejména odhalením vzdálené diseminace, ale má potenciál i k odhadu prognózy pomocí stanovení maximálních hod not akumulace 18F-FDG., Aim. To assess the impact of 18F-FDG-PET/ CT on the treatment decision in renal cell carcinoma. Method. The analysis of the sample of 40 patients was made; it consists of 28 males, 12 females, mean age 67.8 years, ranging 53-86 years. All patients underwent 18F-FDG-PET/ CT including two-phase CT-angiography of the kidneys, locally advanced or generalized renal cell carcinoma was suspected in all patients. The level of the 18F-FDG accumulation within the tumor was compared with the histological grading and the development of the disease was surveyed twelve months after 18F-FDG-PET/CT. Results. Overall mortality reached 45%, the highest 18F-FDG accumulation showed tumor of the grade 4 (mean SUV 10.4, ranging 5-22), the highest mortality was present in tumors exceeded the SUVmax value of 10 (mortality 63.6%). New information was brought by 18F-FDG-PET/CT in comparison to CT in 87.5%. Conclusion. 18F-FDG-PET/CT is useful tool to make decision about the way of treatment especially when the generalization is confirmed or excluded; the method is bearing the potential to set the prognosis according the reached value of 18F-FDG accumulation measured in SUVmax., Jiří Ferda, Eva Ferdová, Milan Hora, Ondřej Hes, and Lit.: 16
Hodgkinův lymfom (HL) je jedním z nejlépe léčitelných nádorových onemocnění. Moderními léčebnými strategiemi je vyléčeno 85–95 % pacientů. Do popředí začaly vystupovat pozdní následky léčby, především kardiotoxicita a sekundární malignity, na které 15 let po skončené terapii umírá více pacientů než na HL. Cílem výzkumu je najít takovou terapii, která by zajistila maximální kontrolu tumoru při minimalizaci akutní a dlouhodobé toxicity. German Hodgkin Study Group (GHSG) prokázala ve studii HD10 pro počáteční stadia HL možnost redukce chemoterapie i radioterapie, bez snížení účinnosti léčby. Studie pro pokročilá stadia HD9 prokázala výborné výsledky chemoterapie BEACOPP eskalovaný oproti konvenční terapii ABVD, ale s vyšší akutní a dlouhodobou toxicitou. Cílem dalších studií GHSG pro pokročilá stadia (HD12, HD15 a nyní aktivní studie HD18) je snížit intenzitu chemoterapie a posoudit, zda je nutná následná radioterapie. Pozitivní výsledky přináší hodnocení studie HD15, kde je testován význam FDG – PET při indikaci radioterapie. Současná studie HD18 sleduje pomocí FDG – PET i časnou odpověď tumoru po 2 cyklech chemoterapie. Další naděje jsou vkládány do nových biologických léků, které cílí přímo na receptory maligních buněk (monoklonální protilátky, inhibitory)., Hodgkin lymphoma (HL) is one of the best curable malignant diseases. Modern therapeutic strategies can cure 85–95 % of patients. Late effects have become increasingly important, especially cardiotoxicity and second tumors, that cause more deaths than HL 15 years after completion of treatment. The goal of the research is to find the therapy that maximizes tumor control and minimizes acute and longterm toxicity. German Hodgkin Study Group (GHSG) proved the possibility to reduce chemotherapy and radiotherapy without reducing the effectiveness of treatment in the HD10 study for early stages of HL. HD9 study for advanced stages demonstrated excellent results of BEACOPP escalated chemotherapy compared to the conventional therapy ABVD, but BEACOPP escalated induced higher acute and long-term toxicity. The goal of subsequent GHSG studies for advanced stages (HD12, HD15 and ongoing active HD18 study) is to reduce the intensity of chemotherapy and to evaluate the significance of subsequent radiotherapy. The HD15 study tests the significance of FDG-PET for indication of radiotherapy. Current HD18 study tests by FDG-PET the early response of tumor after 2 cycles of chemotherapy.
