Úvod: Cerebrovaskulární rezerva (CVR) je jedním z důležitých parametrů k realizaci a monitoraci úspěšnosti angiochirurgické intervence u pacientů po ischemické cévní mozkové příhodě, tranzitorní ischemické atace nebo k upřesnění typu demence. Cíl: Cílem této práce je seznámit s provedením vazodilatačního testu s dipyridamolem jako možné alternativy farmakologické zátěže, s výskytem vedlejších účinků, získanými výsledky, jeho přednostmi a nedostatky. Soubor a metody: Byla provedena retrospektivní analýza 20 pacientů (8 žen, 12 mužů ve věkovém rozmezí 37–78 let) – 6 po TIA, 14 po ischemické cévní mozkové příhodě vyšetřených v průběhu dvou let. Snímání proběhlo na gamakameře Infinia Hawkeye s kolimátory fan beam technikou SPECT standardním způsobem za bazálních podmínek a v odstupu 1–13 dnů po farmakologické zátěži dipyridamolem (v 4minutové i.v. infuzi v dávce 0,84 mg/kg do 50 ml F1/1) s následnou i.v. aplikací 99mTc-HMPAO v 10. minutě. K potlačení vedlejší účinků zaznamenaných po dipyridamolu byl jako antidotum použit Syntophyllin i.v. Rozdíly bazální a pozátěžové studie byly hodnoceny vizuálně i semikvantitativně pomocí programu NeuroGamTMSegami a statistického parametrického mapování. Výsledky: 9 (45 %) pacientů mělo normální CVR, redukovaná se vyskytla u 11 pacientů (55 %). 8 pacientů (6 mužů, 2 ženy) s jednostrannou těsnou stenózou nebo okluzí vnitřní krkavice (ACI) bylo následně léčeno neurochirurgicky (karotická endarterektomie nebo arterio-arteriální extra-intrakraniální bypass). 6 (75 %) z těchto pacientů bylo s redukovanou CVR, 2 (25 %) měli normální CVR. Zbývajících 12 pacientů bylo léčeno konzervativně, 5 (42 %) z nich mělo redukovanou CVR (mírná redukce CVR nebo pacient rizikový k chirurgickému výkonu) a 7 (58 %) pacientů mělo normální CVR. Vedlejší účinky dipyridamolu (hypotenze, kašel, tlak na hrudi, bolest hlavy) byly zaznamenány u 5 pacientů, k ústupu potíží došlo během 2–5 minut. Závěr: Zátěžová perfuzní scintigrafie mozku s podáním dipyridamolu je vhodným testem k posouzení vaskulární rezervy mozku. Úvod: Zhoršení výživy u seniorů, bývá popisováno ve spojitosti s institucionální péčí. Jako vhodný nástroj pro posouzení stavu výživy se jeví Škála pro hodnocení stavu výživy – MNA. Cíl výzkumu: Zjistit stav výživy u seniorů ve vybraných institucích prostřednictvím Škály pro hodnocení stavu výživy – MNA a následně analyzovat hodnoty Body mass index (BMI) u sledované populace. Metodika: Výzkumný soubor tvořilo 200 osob ve věku nad 60 let v institucionální péči. Statistické testy byly hodnoceny na hladině významnosti 5 %. Výsledky: Z 200 seniorů v institucionální péči dle Škály pro hodnocení stavu výživy (MNA) bylo ve stavu dobré výživy 32 %, rizikem podvýživy bylo ohroženo 38 % seniorů a podvýživou trpělo 30 %. Byl zjištěn statisticky významný rozdíl ve stavu výživy v závislosti na druhu instituce (p=0,001). Byl zjištěn statisticky významný rozdíl mezi sledovanými institucemi v hodnotách BMI (p=0,0084). Hodnoty BMI vyšší než 23 byly u 65 % seniorů, hodnoty BMI nižší než 19 byly u 16 % seniorů. Závěr: Z výsledku studie vyplývá, že pouze jedna třetina seniorů byla ve stavu dobré výživy, což vede k nutnosti včasné detekce stavu výživy prostřednictvím MNA., Introduction: Nutritional disorders in the elderly are described in relation to institutional care. Mini Nutritional Assessment (MNA) was used for assessing nutritional status. Objective: To assess the nutritional status of the elderly in selected institutions through Mini Nutritional Assessment (MNA) and to analyze the Body Mass Index (BMI) of the studied population. Methods: The research sample consisted of 200 people aged over 60 years in institutional care. Statistical tests were evaluated at a significance level of 5%. Results: Of the 200 seniors in institutional care according to MNA, 32% were in good nutrition condition, 38% of respondents were at risk of malnutrition and 30% of seniors suffered from malnutrition. A statistically significant difference in nutritional status was revealed depending on the type of institution (p=0.001) and a statistically significant difference between the observed values of institutions in terms of BMI (p=0.0084). Values of BMI in excess of 23 were identified in 65% of the elderly, values of BMI below 19 were found in 16% of elderly. Conclusion: The results show that only one third of the elderly were in good nutritional status. Úvod: Observační, prospektivní a neintervenční projekt SOLOSTAR měl za cíl získat informace z klinické praxe v České republice o spokojenosti pacientů s používáním předplněného inzulinového pera SoloStar. Metodika: 1 805 pacientů s diabetem 1. nebo 2. typu bylo po zahájení používání pera SoloStar sledováno 3–4 měsíce. Spokojenost pacientů s perem SoloStar a s jeho jednotlivými parametry byla posuzována ve skupině všech pacientů a v podskupinách rozdělených dle demografických parametrů včetně zdravotního omezení. Pacienti také porovnávali pero SoloStar s pery používanými před vstupem do projektu. Výsledky: 98,3 % pacientů ohodnotilo celkovou spokojenost s perem SoloStar jako „výbornou“ a „dobrou“ (nejlepší a druhé nejlepší hodnocení na 5stupňové škále). Na celkovou spokojenost s perem SoloStar nemělo statisticky významný vliv žádné z demografických kritérií, a to ani zrakový handicap či omezení manuální zručnosti. Parametry pera SoloStar byly u více než 90 % pacientů hodnoceny jako „výborné“ a „dobré“. Lépe byly některé parametry hodnoceny zejména pacienty bez předchozí zkušenosti s aplikací inzulinu. Pacienti, kteří používali jiný typ aplikátoru před zařazením do projektu, byli více spokojeni s perem SoloStar než s předchozím aplikátorem a většina z nich ohodnotila používání pera SoloStar (93,1 %) a aplikaci inzulinu perem SoloStar (83,5 %) v porovnání s původním perem jako „mnohem jednodušší“ a „jednodušší“ (nejlepší a druhé nejlepší hodnocení na 5stupňové škále). Závěr: Používání pera SoloStar je v běžné klinické praxi v České republice spojeno s vysokou spokojeností pacientů, a to včetně pacientů se zdravotním omezením. Pacienti navíc pero SoloStar upřednostňovali před inzulinovými pery používanými před vstupem do projektu. Methodology: 1 805 patients suffering from type 1 or 2 diabetes who began using the pen SoloStar were observed for 3 to 4 months. Satisfaction of patients with SoloStar and with its particular features was evaluated in the group of all patients and in subgroups defined by demographic parameters including the type of handicap. Results: 98.3 % of patients rated overall satisfaction with the pen SoloStar as “excellent” and “good” (the highest and second highest rating on a 5-point scale). Demographic criteria, including visual handicap and manual dexterity handicap, did not have any statistically significant effect on the rating of the overall satisfaction with SoloStar. Features of SoloStar were rated as “excellent” and “good” by more than 90% of patients. Higher rating of some of the features of SoloStar was awarded mostly by patients without prior experience with insulin application. Patients who used other insulin applicators before entering into the survey were more satisfied with the pen SoloStar than with the previously used applicator and most of them rated the use of SoloStar (93.1 %) and insulin application with SoloStar (83.5 %) as “much easier” and “easier” (the highest and second highest rating on a 5-point scale) in comparison with the previously used pen. Conclusion: The use of the pen SoloStar in a clinical practice setting in the Czech Republic is associated with high levels of satisfaction of patients, including handicapped patients. In addition, patients preferred SoloStar over insulin pens used before the entry into the survey. Our case-based review focuses on limb salvage through operative management of Charcot neuroarthropathy of the diabetic foot. We describe a case, when a below-knee amputation was considered in a patient with chronic Charcot foot with a rocker-bottom deformity and chronic plantar ulceration. Conservative treatment failed. Targeted antibiotic therapy and operative management (Tendo-Achilles lengthening, resectional arthrodesis of Lisfranc and midtarsal joints, fixation with large-diameter axial screws, and plaster cast) were performed. On the basis of this case, we discuss options and drawbacks of surgical management. Our approach led to healing of the ulcer and correction of the deformity. Two years after surgery, we observed a significant improvement in patient's quality of life. Advanced diagnostic and imaging techniques, a better understanding of the biomechanics and biology of Charcot neuroarthropathy, and suitable osteosynthetic material enables diabetic limb salvage. and T. Suspenze přední poševní stěny spočívá v rekonstrukci nativní tkáně vaginální cestou. Cílem operace je korekce stresové močové inkontinence, k níž došlo v důsledku hypermobility močové trubice, a případně souvisejícího prolapsu přední poševní stěny jakéhokoli stupně. Ideálním kandidátem pro tento výkon je pacientka s laterální cystokélou (1. a 2. stadium). Operace může být indikována rovněž u pacientek s cystokélou závažnějšího stupně, v tomto případě je však nezbytná další rekonstrukce v podobě suspenze klenby poševní a korekce posteriorního kompartmentu (enterokéla, rektokéla). V našem článku popisujeme techniku rekonstrukce a uvádíme naše počáteční (dosud příznivé) zkušenosti. Operace je krátká (trvá přibližně jednu hodinu), je spojena s minimální morbiditou a nemá žádný vliv na mikční funkci. Operace neovlivňuje hloubku ani délku pochvy – po zhojení sutury se tedy pacientka může vrátit k sexuální aktivitě. Prof. Zimmern (UT Southwestern Medical Center, Dallas, USA) prezentoval na poslední konferenci SUFU a AUA dlouhodobé výsledky po 15 letech., The anterior vaginal wall suspension is a native tissue repair procedure done vaginally to correct stress urinary incontinence secondary to urethral hypermobility and any associated degree of anterior vaginal wall compartment prolapse. The procedure is ideally suited for women with lateral defect cystocele (stages I and II). The procedure can be applied for more advanced degrees of cystocele, but additional repairs will be necessary including vault suspension and posterior compartment repairs (enterocele, rectocele). The technique of the procedure is reported herein, along with our early experience which has been favorable so far. The procedure is short (around one hour), with limited morbidity and no impact on voiding function. Vaginal depth and size are not compromised; so sexual activity can be resumed once the vaginal suture lines have healed. Cíl práce: Zhodnotit bezpečnost a účinnost léčby bazálním inzulinem glargin v režimu bazál/bolus v podmínkách běžné klinické praxe v České republice. Soubor a metodika: Projekt LINDA byl neintervenční, multicentrický (n = 255), národní observační projekt, který probíhal v podmínkách běžné klinické praxe. Do sledování bylo zařazeno celkem 4 998 pacientů s diabetes mellitus 1. a 2. typu (DM1T, DM2T) s převážně inzulinovou terapií (99,7 %), po následném zahájení léčby inzulinem glargin v režimu bazál/bolus. Pacienti byli sledováni po dobu 6 měsíců od zahájení této terapie. Primárním cílem projektu bylo sledování četnosti výskytu závažných hypoglykemických příhod při léčbě bazálním inzulinovým analogem glargin (Lantus?) v podmínkách běžné klinické praxe. Sekundárními sledovanými parametry byla změna glykovaného hemoglobinu (HbA1c), glykemie nalačno (FPG), změna tělesné hmotnosti, změna dávek inzulinu, změna počtu hypoglykemických příhod ve srovnání s předchozí léčbou a sledování četnosti nežádoucích účinků. Výsledky: Závažné hypoglykemie byly pozorovány během léčby inzulinem glargin u 0,8 % pacientů. Při srovnání výskytu hypoglykemií s předchozí terapií bylo prokázáno klinicky a statisticky významné snížení jejich četnosti. Procento pacientů s hypoglykemickou příhodou (17,6 %), závažnou (0,8 %) a závažnou noční hypoglykemií (0,3 %) bylo za poslední měsíc léčby s inzulinem glargin v režimu bazál/bolus konzistentně menší ve srovnání s posledním měsícem léčby před zahájením této terapie (42,5 %; 17,6 %; 13,8 % pacientů). U pacientů s DM1T došlo k poklesu všech hypoglykemií z hodnoty 37,80 ? 15,95 příhod/pacient/rok na 8,76 ? 4,38 příhod/pacient/rok (p < 0,001), závažných hypoglykemií z hodnoty 5,64 ? 3,27 příhod/pacient/rok na 0,0396 ? 0,012 příhod/pacient/rok (p < 0,001) a závažných nočních hypoglykemií z hodnoty 3,84 ? 2,04 příhod/pacient/rok na 0,0096 ? 0,003 příhod/pacient/rok (p < 0,001). U pacientů s DM2T poklesla četnost všech hypoglykemií z hodnoty 12,48 ? 7,57 příhod/pacient/rok na 1,68 ? 0,78 příhod/pacient/rok (p < 0,001), závažných hypoglykemií z hodnoty 2,04 ? 0,94 příhod/pacient/rok na 0,0132 ? 0,005 příhod/pacient/rok (p < 0,001) a závažných nočních hypoglykemií z hodnoty 1,32 ? 0,77 příhod/pacient/rok na 0,0048 ? 0,0008 příhod/pacient/rok (p < 0,001). Bylo prokázáno statisticky významné zlepšení metabolické kompenzace při užití inzulinu glargin. U DM1T došlo ke snížení hladiny glykovaného hemoglobinu (HbA1c) z hodnoty 7,74 ? 1,71 % na 6,43 ? 1,39 % (∆ -1,31 ? 0,32 %; p < 0,001) a u DM2T z hodnoty 8,13 ? 1,56 % na 6,72 ? 1,40 % (∆ -1,41 ? 0,28 %; p < 0,001). Statisticky významně (p < 0,001) se zvýšil počet pacientů s hodnotami HbA1c < 5,4 %. Statisticky významný byl také pokles glykemie nalačno (FBG) a hodnot 6bodového glykemického profilu, a to u obou typů diabetu (p < 0,001). Změna terapie a následná léčba inzulinem glargin byla lékaři a pacienty vnímána pozitivně. Závěr: Za podmínek běžné klinické praxe zahájení léčby inzulinem glargin v režimu bazál/bolus u pacientů s předchozí inzulinovou terapií vedlo ke snížení četnosti výskytu hypoglykemických příhod včetně závažných a závažných nočních hypoglykemií a ke zlepšení metabolické kompenzace pacientů s diabetem (snížení glykovaného hemoglobinu, glykemie nalačno a hodnot 6bodového glykemického profilu). Byla zaznamenána větší spokojenost se stávající léčbou ze strany pacientů a lékařů. Patients and methods: The LINDA project was a non-interventional, multicenter (n = 255), national, observational project. A total of 4,998 patients with Type 1 and 2 diabetes mellitus (T1DM, T2DM) with predominantly insulin therapy (99,7 %), after switch on insulin glargine at basal-bolus regimen, were enrolled in this project. The patients were followed up for 6 months after initiation of the therapy with insulin glargine. The primary objective of the project was to investigate the incidence of severe hypoglycemic episodes during the treatment with basal insulin analogue glargine (Lantus?) in a common clinical practice setting. The secondary endpoints were changes in glycosylated hemoglobin (HbA1c) levels, fasting plasma glucose (FPG), body weight, insulin dose, change of number of hypoglycemic episodes in comparison the previous therapy and the frequency of adverse effects. Results: Severe hypoglycaemia were observed during treatment with insulin glargine at 0.8 % patients. When comparing the incidence of hypoglycemia with the previous therapy, we demonstrated a clinically and statistically significant reduction in their frequencies. The percentage of patients with hypoglycemic episodes (17.6 %), severe hypoglycemia (0.8 %) and severe nocturnal hypoglycemia (0.3 %) over the last month of treatment with insulin glargine using the basal-bolus regimen was consistently lower compared to the last month of treatment before initiation of this therapy (42.5 %, 17.6 %, and 13.8 % of the patients, respectively). In patients with T1DM, the incidence of hypoglycemia decreased from 37.80 ? 15.95 episodes/patient/year to 8.76 ? 4.38 episodes/patient/year (p < 0.001) for all hypoglycemic episodes; from 5.64 ? 3.27 episodes/patient/year to 0.0396 ? 0.012 episodes/patient/year (p < 0.001) for severe hypoglycemia; and from 3.84 ? 2.04 episodes/patient/year to 0.0096 ? 0.003 episodes/patient/year (p < 0.001) for severe nocturnal hypoglycemia. A statistically significant improvement in the metabolic control was demonstrated when using insulin glargine. The glycated hemoglobin (HbA1c) decreased from 7.74 ? 1.71 % to 6.43 ? 1.39 % (∆ -1.31 ? 0.32 %, p < 0.001) in patients with T1DM, and from 8.13 ? 1.56 % to 6.72 ? 1.40 % (∆ -1.41 ? 0.28 %, p < 0.001) in patients with T2DM. A statistically significant (p < 0.001) increase in the number of patients with HbA1c < 5.4 % was further demonstrated. The decrease in fasting blood glucose (FBG) and 6-point blood sugar profile was also statistically significant in both types of diabetes (p < 0.001). Changes in therapy and subsequent treatment with insulin glargine were perceived positively by both physicians and patients. Conclusion: In the common clinical practice setting, the initiation of treatment with insulin glargine using the basal-bolus regime in patients with previous insulin therapy resulted in a reduction in the incidence of hypoglycemic events, including severe hypoglycemia and severe nocturnal hypoglycemia, and improved metabolic control in patients with diabetes (reduced glycated hemoglobin, fasting glucose values and 6-point blood glucose profile). Greater satisfaction with the current treatment was reported by both patients and physicians. Figurální (neboli neverbální) fluence je schopnost exekutivních funkcí, která nám poskytuje informace o divergentním myšlení, rozdělené pozornosti, plánování a mentální flexibilitě. Zhoršený výkon v testech zaměřených na figurální fluenci nacházíme u pacientů s neurologickým i psychiatrickým postižením. Pětitečkový test (Five Point Test, 5TT) je jeden z neuropsychologických testů, jenž slouží ke zhodnocení figurální fluence. Úkolem probanda je vytvořit co nejvíce obrazců v časovém limitu. Cílem této studie bylo vytvořit normy k 5TT pro českou dospělou populaci. Předkládáme normativní data pro dospělé ve věku od 20 do 85 let (n = 503). Hodnotili jsme počet správných odpovědí a počet perseverací. Počet správných odpovědí je ovlivněn věkem a vzděláním (r = –0,3; resp. 0,4; p < 0,0001), proto jsou normy rozděleny na pásma po 10 letech a dále podle ukončeného vzdělání. Počet perseverací s těmito proměnnými souvisí jen slabě (rs = 0,1; resp. –0,1; p < 0,05). Pohlaví nemá vliv na počet správných odpovědí ani perseverací (t = 0,09; p > 0,9 pro oba skóry)., Figural (or nonverbal) fluency is the ability of executive functions to provide information about divergent reasoning, divided attention, planning and mental flexibility. Impairments of figural fluency have been found in individuals with various neurological or psychiatric diseases. Five Point Test (5TT) is a neuropsychological test that assesses figural fluency. A participant is asked to generate as many unique designs as possible in a certain time limit. The aim of this study was to create Czech population norms for the Five Point Test. Normative data for adult population aged between 20 and 85 years (n = 503) are presented. We assessed the number of correct answers and the number of perseverations. The number of correct answers is influenced by age and education (r = –0.3 and 0.4, respectively, p < 0.0001); for this reason the norms are stratified into ten age ranges and also according to completed education. The number of perseverations correlates with these variables only weakly (rs = 0.1 and –0.1, respectively, p < 0.05). Gender has no impact neither on the number of correct answers nor on perseverative responses (t-test, p > 0.9 for both scores). The Purpose of this study is to demonstrate the effectiveness of combined treatment for III AB stage of nonsmallcell lung cancer. Nonsmallcell lung cancer spread form treatment is disputable. Operation is not final solution. The combined treatment is being tested (in clinical trials) in this study. This article argues that a combination of treatment is a better option than that of a specific treatment. Objective: to analyze main clinical-statistical indices of spine and spinal cord injuries (SCI) and system of organization of medical aid to the victims. We conducted a cohort retrospective study of materials of medical institutions of Tashkent city. Medical records of patients and records of forensic examinations were analyzed. 242 cases of spinal cord injuries were studied. Over the last 10 years in Tashkent the frequency of SCI have been tend to increase, males and working-age people are dominated. In 60.9% of cases the injury was associated, in 36.2% - isolated, and in 2.9% - combined. Fallings from height and road traffic accidents were the main reasons for getting SCI. Lesions at cervical level registered in 45% of cases, thoracic level - 27.3%, at lumbar level - 27.7%, respectively. According to ASIA/IMSOP, full injuries were noted in 40.1% of cases, incomplete injures were in 59.9%. Cíl studie: Cílem studie bylo zjištění vztahů mezi manifestací tinnitu, nálezem na sluchových evokovaných potenciálech a genetickým pozadím u receptoru pro gamaaminomáselnou kyselinu typu A (GABA(A) receptor), podporující desinhibiční hypotézu vzniku tinnitu. Soubor a metodika: Bylo vyšetřeno 131 pacientů z hlediska sluchové ztráty, provedena kvantifikace tinnitu, sluchové evokované potenciály střední latence (MLR) a kmenové sluchové evokované potenciály (BAEP) a dále stanovení genotypu (CA)n repetitivní sekvence pro beta‑3 podjednotku GABA(A) receptoru. Následně byly hledány vztahy mezi jednotlivými výsledky a manifestací tinnitu. Výsledky: Byla nalezena korelace tinnitus skóre s amplitudovým poměrem vln V/III v BAEP (R = 0,22, p < 0,001) a s průměrným sluchovým prahem (R = 0,22, p = 0,17). Rovněž byla nalezena korelace tinnitus skóre s amplitudou vlny PA v MLR (R = 0,31–0,37; p < 0,001). Výsledky MLR neukázaly žádný vztah k průměrnému sluchovému prahu. U skupiny s kratší anamnézou tinnitu (méně než devět měsíců) byl prokázán rozdíl v manifestaci tinnitu na genotypu pro (CA)n repetitivní sekvenci genu pro beta‑3 podjednotku GABA(A) receptoru (p = 0,002). Tento výsledek byl rovněž konzistentní s rozložením amplitudy vlny PA v dané subpopulaci. Závěr: Tyto výsledky svědčí o existenci dvou hlavních regulačních mechanizmů vzniku tinnitu: první, který je závislý na velikosti sluchové ztráty, je na úrovni mozkového kmene, zatímco druhý je na úrovni korové s možnou souvislostí s genotypem (CA)n repetitivní sekvence pro beta‑3 podjednotku GABA(A) receptoru., Study aim:Study objective was to explore associations between manifestation of tinnitus, auditory evoked potentials and genetic background of gamma‑aminobutyric acid type A (GABA(A) receptors) to support the disinhibited feedback hypothesis of tinnitus generation. Materials and methods: A population of 131 patients was assessed for severity of hearing loss, quantification of tinnitus, mid‑latency responses (MLR) and brainstem auditory evoked potentials (BAEP), and (CA)n tandem repeat polymorphism in GABA(A) Beta‑3 subunit gene to establish any correlation with manifestation of tinnitus. Results: It was observed that tinnitus score correlates with V/III amplitude ratio in BAEP (R = 0.22, p < 0.001) and with mean pure tone audiometry (PTA) threshold (R = 0.22, p = 0.017). Analysis of the MLR results showed a significant correlation between the PA wave amplitude and the tinnitus score (R = 0.31–0.37; p < 0.001). MLR result analysis showed no statistically significant correlation between the wave amplitudes and the mean auditory threshold. An analysis of a subgroup with shorter clinical history (less than nine months) revealed a statistically significant difference in the tinnitus score in relation to the genotype of (CA)n tandem repeat of the GABRß3 receptor subunit gene (p = 0.002). This result was also consistent with the distribution of the PA wave amplitude in the given subpopulation. Conclusion: Our findings indicate existence of two main regulatory mechanisms of tinnitus generation: first, the brainstem mechanism is dependent on the severity of the hearing loss; second, the cortical mechanism is likely to be dependent on the genotype of (CA)n tandem repeat in GABA(A) beta‑3 subunit gene., and J.
