Colonic polyps and polyposis are studied in 183 patients. It has been found, that sensitivity of rectoscopy is 66.7%, irrigoscopy 61.1%, virtual colonoscopy computeraided tomography of colon (VCCATC) 79.1%. Endoscopic polypectomy was the main treatment method used in patents with colonic polyps and polyposis (18.6%). Cíl: Práce shrnuje naše první zkušenosti se stanovením obsahu tuku a železa v játrech na 3T přístroji pomocí softwaru využívajícího současně techniku two-point Dixon, techniku multiechoDixon kombinovanou s T2* relaxometrií a single voxel 1H spektroskopii v kombinaci s T2 relaxometrií. Metodika: Do studie bylo zahrnuto 55 MR vyšetření jater provedených na 3T přístroji za použití softwarového balíku Liver-Lab (Siemens, Erlangen, Německo), který obsahuje sekvence a posprocessingové nástroje umožňující stanovit obsah tuku a železa pomocí výše uvedených technik. Jako referenční metody jsme použili 1H single voxel spektroskopii a sérovou hladinu feritinu. Výsledky: Technika two-point Dixon dosáhla při rozhodování o přítomnosti steatózy ve srovnání se spektroskopií senzitivity (SE) 72,4 % a specificity (SP) 80,7 %. Technika multiecho Dixon v oblasti zájmu, která se shodovala s oblastí zájmu pro spektroskopii, měla při rozhodování o přítomnosti steatózy SE 72,4 % a SP 92,3 %. Při kvantifikaci dosáhla velmi vysoké shody se spektroskopií r = 0,93. Při rozhodování o přítomnosti jaterní siderózy měla technika two-point Dixon SE 69,2 % a SP 60,0 %. T2* relaxometrie měla při rozhodování o přítomnosti siderózy SE 92,3 % a SP 100 %. Při kvantifikaci dosáhla vysoké shody se sérovými hladinami feritinu, r = 0,74. U T2 relaxometrie byla při rozhodování o přítomnosti siderózy SE 61,5 % a SP 80,0 %. Shoda se sérovými hladinami feritinu byla nízká, r = 0,28. Při srovnání T2 a T2* relaxometrie byla shoda obou metod nízká, r = 0,38. Závěr: Ve studii se ukazuje, že ze srovnávaných metod pro současné stanovení koncentrace tuku a železa v játrech je z praktického hlediska nevýhodnější technika multiecho Dixon spojená s T2* relaxometrií. Klíčová slova: magnetická rezonance, two-point Dixon, Multiecho Dixon, 1H MR spektroskopie, T2* relaxometrie, T2 relaxometrie, Aim: Our study deals with our first experience with quantification of liver fat and iron concentration at 3T MRI using two-point Dixon technique, multiecho Dixon technique combined with T2* relaxometry and single voxel 1H spectroscopy in combination with T2 relaxometry. Methods: 55 MR examinations were included into the study. The examinations were performed using a software package Liver-Lab (Siemens, Erlangen, Germany), which contains the sequence and posprocessing tools enabling evaluation using techniques that are mentioned above. As reference methods we used 1H single voxel spectroscopy and serum ferritin. Results: In decision on the presence of steatosis two-point Dixon reached sensitivity (SE) 72.4% and specificity (SP) 80.7% compared to MR spectroscopy. Multiecho Dixon with measurement in the regio of interest that coincide with the area of interest for spectroscopy, had in decision on the presence of steatosis SE 72.4% and 92.3% SP. In quantification it reached a very high agreement with spectroscopy, r = 0.93. When deciding on the presence of hepatic siderosis two-point Dixon compared to serum feritine level reached SE 69,2% and SP 60.0%. T2* relaxometry had in decision on the presence of siderosis SE 92.3% and SP 100%. In quantification it reached a high agreement with serum ferritin, r = 0.74. At T2 relaxometry was when deciding on the presence siderosis SE 61.5% and 80.0% SP. Agreement with serum ferritin was low, r = 0.28. When comparing T2 and T2* relaxometry the agreement of both methods was low, r = 0.38. Conclusion: Our study shows that multiecho Dixon technique combined with T2* relaxometry is from practical point of view the most suitable technique for simultaneous estimation of the liver steatosis and siderosis.
