BACKGROUND: Occurrence of gastric metastasis as the first symptom of breast carcinoma with a long period of latency before presentation of the primary breast carcinoma is rare. CASE REPORT: A patient with gastric metastasis as the first symptom of lobular breast carcinoma, treated by neoadjuvant preoperative chemoradiotherapy and total gastrectomy, with complete local control. Fourteen months after presentation of the gastric metastasis a primary lobular breast carcinoma was discovered, treated by radiotherapy, chemotherapy and hormonal treatment with complete local response. Twenty-three months after diagnosis of breast cancer multiple colorectal metastases from the breast cancer occurred, which were treated by chemotherapy and hormonal treatment. Eighty-six months after diagnosis of gastric metastasis the patient died due to progression of cancer. CONCLUSIONS: Metastases to gastrointestinal or gynaecological tracts are more likely in invasive lobular carcinoma than invasive ductal cancer. The pathologist should determine whether or not they check estrogen and progesterone receptor status not simply by signet ring cell morphology but also by consideration of clinic-pathological correlation of the patient, such as the presence of a past history of breast cancer, or the colorectal localization of poorly differentiated carcinoma, which may occur less frequently than in the stomach. and D. Endometrióza znamená přítomnost tkáně endometria mimo dutinu děložní. Hlubokou infiltrující endometriózu (deep infiltrating endometriosis – DIE) představují rektovaginální léze, infiltrující onemocnění střev, močového měchýře, močovodu a DIE se vzácně objevuje i v dalších lokalizacích. Negynekologické komplikace DIE rozdělujeme do dvou hlavních kategorií: samotné onemocnění zahrnuje pánevní a břišní bolest, střevní striktury, opakující se krvácení z konečníku, dyschezii, chronickou anémii, dysurii, hematurii, stenózu ureterů, ztrátu ledvinných funkcí a komplikace léčby, ty jsou charakterizovány vznikem fistul, dysfunkcí močového měchýře a opakujícími se záněty urinárního traktu. DIE přináší riziko pro vznik malignit zasažených orgánů. DIE může být léčena konzervativně, nebo chirurgicky. Předkládaná práce je přehledem gastrointestinálních a urologických komplikací DIE a předkládá také kazuistiku pacientky centra pro léčbu endometriózy v nemocnici ve Znojmě (ČR)., Endometriosis is the presence of endometrial tissue outside the endometrial cavity. Deep infiltrating endometriosis (DIE) includes rectovaginal lesions as well as infiltrative disease to the bowel, urinary bladder, ureter and other rare locations. Non gynecological complications of DIE can be divided into two major categories: complications of the disease itself which include pelvic and abdominal pain, bowel stricture, cyclic rectal bleeding, dyschezia, chronic anemia, dysuria, hematuria, ureteral stenosis as well as loss of kidney function and complications of the treatment which include fistula formation, bladder void dysfunction and recurrent urinary tract infections. DIE has potential for malignant transformation in the affected organ. DIE may be managed conservatively or surgically. However definitive treatment is surgical. V tomto článku je prezentován stručný přehled možných zdrojů gastrointestinálního krvácení s riziky a úskalími jejich diagnostiky a terapie u osob vyššího věku. Článek poukazuje zejména na ty, které se vyskytují u starších významně častěji a mohou své nositele ohrožovat bezprostředně na životě. Dále je diskutována problematika průjmu u nemocných v kritickém stavu a pseudomembranózní kolitidy. Autoři poukazují na kritické situace, se kterými se může lékař pečující o seniory setkat běžně v každodenní klinické praxi. Rozebírají v tomto přehledném článku uvedenou problematiku z hlediska etiopatogeneze, klinického obrazu, průběhu, diagnostiky, léčby i prognózy s ohledem na zvláštnosti vyššího věku., The article provides a brief overview of possible sources of gastrointestinal bleeding in elderly patients and of the risks and issues related to their diagnosing and therapy. Among such sources, the authors lay stress on those with a significantly higher incidence in elderly patients and with imminent risk for the patients' life. Further discussed are issues of diarrhoea in critically ill patients and of pseudomembranous colitis. The authors bring attention to critical situations the physician providing medical care to senior patients will face in everyday clinical practice. Gastrointestinální stromální tumory (GIST) jako nejčastější mezenchymální tumory trávicího traktu vycházejí z Cajalových intersticiálních buněk. Ke konfirmaci morfologické diagnózy jsme použili panel imunohistochemických protilátek CD117, desmin, S100 a DOG1. Při hodnocení biologických vlastností jsme vycházeli z Miettinenovy klasifikace pro GIST, rozdělující tumory do tří skupin podle rizika agresivního chování. Klasifikace tumoru sleduje tři parametry: lokalizaci, velikost a mitotický index. Mitotický index je definován jako počet mitóz v nádorové tkáni na celkové ploše 5 mm2. Provedli jsme srovnání metod odběru tenkojehlové, navigované aspirační cytologie – EUS FNA a klešťové endoskopické biopsie či resekátu nádorové tkáně z pohledu patologa se zaměřením na stanovení všech potřebných morfologických a prognostických parametrů této neoplázie. Naše závěry a zkušenosti ukazují, že výsledky cytologického vyšetření jsou pro diagnostiku GIST srovnatelné s bioptickým vyšetřením. Hlavní limitací cytologické metody je nemožnost v 75 % případů stanovit mitotický index z důvodů malého množství nádorové tkáně. Tento nedostatek lze nahradit vyšetřením nukleárního proteinu asociovaného s buněčnou proliferací Ki-67, který se dle literárních zdrojů ukazuje jako významný prognostický parametr progrese onemocnění., Gastrointestinal stromal tumors (GIST) belong to the most common mesenchymal tumors of digestive tract deriving from Cajal interstitial cells. A panel of immunohistochemical antibodies CD117, desmin, S100 and DOG1 is used to confirm the morphological diagnosis. When evaluating the biological characteristics we used a Miettinen classification for GIST which divides tumors into three groups according to a risk of aggressive behaviour. This classification observes three parameters – tumor localisation, its size and mitotic index. Mitotic index is defined as a number of mitoses in the tumor tissue on the overall area of 5 mm2. We performed a comparison of sampling methods – biopsy forceps, resection versus endoscopic ultrasound-guided fine-needle aspiration cytology (EUS FNA) from the point of view of a pathologist, focusing on the assessment of all necessary morphological and prognostic parameters of neoplasia. The study conclusions and experience show that when diagnosing GISTs, results of cytological examination of a sample using EUS FNA are comparable to those using forceps endoscopic biopsy. The main limitation of EUS FNA method is that in 75 % of cases it is impossible to state the mitotic index due to insufficient amount of tumor tissue. Jsou diskutovány další vlivy, které vývoj daný prostým stárnutím dále urychlují a zhoršují. V každém aspektu je zdůrazněn rozdíl v symptomatologií, diagnostice i terapii oproti mladší geníraci. Hypotalamický hamartom může způsobovat rozvoj katastrofické a extrémně farmakorezistentní epilepsie s epileptickou encefalopatií. První epileptický záchvat je typicky gelastický, posléze následují další typy záchvatů. Diagnostika malého hypotalamického hamartomu pomocí magnetické rezonance mozku může být obtížná. Neurochirurgická léčba včetně radiochirurgie je vzhledem k uložení komplikovaná s rizikem vedlejších účinků. Odstranění hypotalamického hamartomu však velmi účinně ovlivňuje epileptické záchvaty a brání rozvoji encefalopatie. Poruchy spánku patria k najčastejším non‑motorickým prejavom u pacientov s Parkinsonovou chorobou (PCh), v populačných štúdiách je uvádzaný ich výskyt až u 60 % pacientov. Štúdie tiež uvádzajú rozdielnosť prejavov PCh medzi mužmi a ženami. Cieľ: Zistiť, či sú u pacientov s PCh na Slovensku prítomné medzi pohlaviami rozdiely v klinických prejavoch PCh a výskyte porúch spánku. Materiál a metódy: Zisťovanie bolo robené dotazníkovou metódou s účasťou pacientov a neurológov z jednotlivých regiónov Slovenskej republiky. Dotazníky boli vyplňované počas rutinných kontrolných návštev. Boli zisťované socioekonomické premenné ako pohlavie, vek, vek pri začiatku ochorenia, stupeň poškodenia hodnotený škálou Hoehna a Yahrovej, dominujúci fenotyp ochorenia a používaná medikácia. Pri hodnotení porúch spánku bola použitá slovenská verzia dotazníka PDSS. Výsledky: Získali sme dáta od 1 067 pacientov. Pri porovnávacích analýzach neboli zistené podstatné rozdiely vo väčšine hodnotených parametrov. U mužov bol o niečo vyšší podiel pacientov s hodnotou škály Hoehna a Yahrovej 4 a boli zistené signifikantne vyššie dávky levodopy v monoterapii (p < 0,01). Významné rozdiely boli zistené v skóre škály PDSS pre hodnotenie rušivých halucinácií počas noci – skóre u mužov bolo 8,22, u žien 8,48 (p < 0,05). Podobný výsledok sme zaznamenali v skóre hodnotiacom nutnosť močenia počas noci – u mužov bolo skóre 5,90, u žien 6,53 (p < 0,01). Redakční rada potvrzuje, že rukopis práce splnil ICMJE kritéria pro publikace zasílané do biomedicínských časopisů., Sleep disturbances are one of the most common non-motor symptoms in patient with Parkinson’s disease (PD) with community‑based studies reporting prevalence data of 60%. Differences in symptoms between men and women have been reported. Aim: In the present study, we assessed whether there are gender differences in clinical presentation of PD and prevalence of sleep disturbances in individuals diagnosed in the Slovak Republic. Material and method: Questionnaires were distributed to participating neurologists and patients in outpatient practices across the Slovak Republic. Sociodemographic variables – gender, age, age at onset, disease severity according to Hoehn and Yahr stage, phenotype of the dominating symptom of Parkinson’s disease and type of medication – were collected. The Slovak language version of the PDSS was used in a questionnaire to test sleep disturbances. Results: Data from 1,067 outpatients with PD were collected. Comparative analyses showed males and females not to be significantly different on the majority of the demographic and medical characteristics collected. Males had a slightly higher proportion of individuals with Hoehn and Yahr score 4 and, among those taking levodopa medication as monotherapy, males took significantly higher levodopa dose than females (p < 0.01). A significant difference in the distribution of PDSS subscores between males and females was observed on item 7 (distressing hallucinations at night) – score for males 8.22, for females 8.48, p < 0.05. Motivation. Our previous study showed differences in the atherosclerosis phenotype between Lithuanian and Swedish men that could be influenced by complementary factors, namely oxidation processes and/or oxidative stress. The goal of this study was to evaluate the mainstream biological pathways inducing and maintaining the atherosclerotic process by analyzing genetic biomarkers particularly in inflammatory and metabolic pathways where the main focus is laid on the oxidation process. Methods. There were 32 families recruited for the study and clinical as well as biochemical analyses were performed. For genetic analysis 150 SNPs in 89 genes were selected in order to construct a microarray based on Arrayed Primer Extension (APEX) genotyping technology. Genotyping was carried out in 28 families and transmission disequilibrium test (TDT), siblingTDT (STDT), and combined analysis were performed. Results. Clinical and biochemical analysis revealed that probands with premature CAD were more likely to have diabetes mellitus, arterial hypertension, dyslipidemia and were male with high body mass index. Genetic analysis showed six SNPs statistically significantly associated with the atherosclerosis phenotype in the candidate genes ITGA2, IL1B, ALOX5A, OR13G1, MMP9 and NFKB1. These genes belong to different biological pathways: trombocyte adhesion and vessel damage, inflammation response, cholesterol and lypoxygenase metabolic pathway and nutrition. Conclusions.
