Nádory v oblasti tureckého sedla tvoří 10–15 % všech nitrolebních nádorů. Nejčastěji se zde vyskytují adenomy hypofýzy, které tvoří 80 % a jejichž incidence je udávána 1,5–2/100 000/rok, prevalence 20/100 000. Jedná se v naprosté většině o nezhoubné nádory. Komplexní léčba adenomů hypofýzy vyžaduje spolupráci neurochirurga, endokrinologa, radiochirurga, radiologa a oftalmologa. Základní léčebná modalita je s výjimkou prolaktinomů operace. Využití nových neurochirurgických technik umožnilo zvýšit radikalitu operačního výkonu bez zvýšení rizika pro pacienta. Cílem léčby je takové ovlivnění nádoru, které nebrání pacientovi žít normální život bez nutnosti hormonální substituce a bez ovlivnění zraku., Pituitary tumours constitute 10–15 % of all intracranial tumours. Adenomas are the most common and account for 80 % of tumours of the sella turcica. Their incidence is 1.5–2/100,000 / year and prevalence 20/100 000. The vast majority of adenomas have a benign course. Comprehensive treatment of pituitary adenomas requires cooperation among a neurosurgeon, an endocrinologist, a radiosurgeon, a radiologist and an ophthalmologist. The main treatment modality with the exception of prolactinomas is surgery. Using of new neurosurgical techniques have permitted to increase the extent of surgery without increasing the risk to the patient. Meningeomy jsou zpravidla pomalu rostoucí nádory, které vznikají z arachnoideálních buněk. Tvoří přibližně třetinu všech primárních intrakraniálních nádorů. Vyskytují se častěji u žen. Klinická manifestace meningeomů je závislá především na lokalizaci a velikosti tumoru. V terapii se uplatňuje hlavně chirurgická léčba, jejíž úspěšnost je spojena s radikalitou resekce. Další léčebnou modalitou je radiochirurgie, alternativou spíše experimentální zůstává hormonální terapie. Materiál a metodika: Autoři popisují zkušenosti svého pracoviště se záchytem, diagnostikou, terapií a dalším sledováním pacientů s meningeomem (n = 552). Výsledky a závěr: Navzdory převážné histologické benignitě meningeomů, recidivy mohou být poměrně časté a oscilují mezi 10 až 30 %, vždy s ohledem na stupeň radikality operace. Proto je nutná dispenzarizace těchto pacientů a pravidelné kontroly pomocí MR, Meningiomas are usually slow-growing tumors, which arise from arachnoideal cells and represent about one third of all primary intracranial tumors. They occur more frequently in women. Clinical manifestations of meningiomas depend on location and tumor size. In therapy, surgical treatment is mostly applied; the success rate is associated with the amount of the resection. Another treatment modality is radiotherapy, hormonal therapy remains on experimental field. Material a methods: Authors describe their experiences with the diagnosis, the therapy and with the follow up of the patients with meningioma (n = 552). Results and conclusion: Despite of mostly benign histology of meningiomas, their recurrence may be relatively frequent and oscillates between 10 to 30 % often depends of the extension of the operation. Gliomy patří mezi nejčastější primární mozkové intraaxiální nádory. Jedná se o širokou skupinu nádorů lišících se svojí typickou lokalizací, věkovou predispozicí, morfologií, stupněm malignity a sklonem k progresi. Na základě WHO klasifikace a gradingu se dělí na dvě základní skupiny: gliomy nízkého stupně malignity (low grade glioma – LGG) a gliomy vysokého stupně malignity (high grade glioma – HGG). Tyto dvě skupiny se mezi sebou značně liší svými biologickými vlastnostmi, a tím i celkovou prognózou pacienta. Mezi základní možnosti jejich terapie patří chirurgická resekce s následnou radioterapií a chemoterapií. Ve sdělení jsou rozebrány možnosti a limitace těchto jednotlivých modalit léčby pro LGG i HGG., Glioma is a common type of primary brain intraaxial tumors. This is a broad group of tumors that differ in their typical localization, age predisposition, morphology, degree of malignancy and the tendency to progression. On the basis of the WHO classification with grading system are divided into two basic groups: glioma low grade malignancy (low grade glioma – LGG) and glioma high grade of malignancy (high grade glioma – HGG). These two groups in between vary considerably in their biological characteristics and thus the overall prognosis of the patient. Among the basic capabilities of their therapies include surgical resection followed by radiation therapy and chemotherapy.
