Wound healing is a complex physiological process important for tissue homeostasis. An acute injury initiates massive cell migration, proliferation and differentiation, synthesis of extracellular matrix components, scar formation and remodelling. Blood flow and tissue oxygenation are parts of the complex regulation of healing. Higher organisms utilize molecular oxygen as a terminal oxidant. This way of gaining energy for vital processes such as healing leads to the production of a number of oxygen compounds that may have a defensive or informatory role. Cíl: Cílem práce bylo vyhledat důkazy na podporu intervence žvýkaní žvýkačky v pooperační péči u pacientů podstupujících operační výkon v břišní dutině ke snížení rizika paralytického pooperačního ileu, a to snížením času do prvního odchodu větrů a času do první stolice. Metodika: Za účelem nalezení důkazů pro použití žvýkaní žvýkačky v pooperační péči u pacientů podstupujících operační výkon v břišní dutině jako strategie na snižení rizika paralytického pooperačního ileu (snížením času do prvního odchodu větrů a času do první stolice) byl proveden přehled vybraných metaanalýz a randomizovaných kontrolovaných studií (RCT). Vyhledávání probíhalo v následujících databázích: OVID Medline®, CINAHL, and PubMed. Vyhledávání se zaměřilo na publikace v anglickém jazyce v recenzovaných časopisech v období 2002 až 2012. Z metaanalýz byl abstrahován rozdíl vážených průměrů (weighted mean difference) jako míra účinku (effect size) žvýkaní žvýkačky na čas do prvního odchodu větrů a na čas do první stolice. Z výsledků randomizovaných kontrolovaných studií bylo vypočítáno Cohenovo d (effect sizes) na určení míry účinku (effect size) žvýkání žvýkačky na čas do prvního odchodu větrů a na čas do první stolice. Na znázornění míry účinku (effect size) z RTC byly vytvořeny grafy (forest plots). Výsledky: Ke kritickému zhodnocení byly vybrány čtyři randomizované kontrolované studie a dvě metaanalýzy. Všech šest studií prokázalo statisticky významný a velmi velký až střední učinek na čas do prvního odchodu větrů a na čas do první stolice. Na základě míry ůčinků (effect sizes) byl vyvozen závěr, že žvýkaní žvýkačky v pooperačním období klinicky významně zkracuje čas do prvního odchodu větrů a čas do první stolice. Závěr: Silné důkazy podporují žvýkaní žvýkačky v pooperační péči u pacientů podstupujících operační výkon v břišní dutině, co vede ke snížení rizika paralytického pooperačního ileu, a to snížením času do prvního odchodu větrů a času do první stolice., Aim: The aim of the paper was to search for evidence that the use of gum chewing in the postoperative care of patients who had undergone abdominal surgery decreases the risk of paralytic postoperative ileus by significantly reducing the time to first flatus and time to first bowel movement. Methods: A literature review of selected meta-analyses and randomized control trials (RCT) was conducted to find the evidence that the use of gum chewing in the postoperative care of patients who had undergone abdominal surgery decreases the risk of paralytic postoperative ileus expressed as the time to first flatus and time to first bowel movement. The following databases were searched: OVID Medline®, CINAHL, and PubMed. The search focused on material published in English in peer-reviewed journals between the years 2002 and 2012. Weighted mean difference was the effect size abstracted from the meta-analyses of gum chewing on time to first flatus and time to first bowel movement. From the RCT, Cohen’s d effect sizes were calculated to determine the strength of the gum chewing intervention on time to first flatus and time to first bowel movement. Forest plots were created to present the effect sizes from the RCT. Results: Four randomized controlled trials and two meta-analyses were selected and critically appraised. All six studies concluded that gum chewing has a statistically significant and very large to medium effect on time to first flatus and time to first bowel movement. Based on the effect sizes, the conclusion was drawn that chewing gum during the postoperative period leads to a clinically significant reduction in time to passage of first flatus and time to first bowel movement. Rubber dam is considered an ideal device for tooth isolation. Nevertheless, its usage is quite rare in the Czech Republic. The aim of this study was: firstly, to gather and evaluate information regarding the use of rubber dam by dentists in the Czech Republic and to compare it with other countries; secondly to find out whether there are any influencing factors as to rubber dam usage; and finally to find out frequency of rubber dam use separately in endodontic treatment and in placing fillings of different materials. A questionnaire-based survey was conducted. Dentists filled in the questionnaires during dental conventions, educational events, conferences and congresses. Rubber dam was routinely used by less than eight per cent of the respondents (n = 35); less than twenty-two per cent of the respondents (n = 97) used rubber dam occasionally, and more than seventy per cent of the respondents (n = 317) has never use it. The results showed that rubber dam is not used frequently in the Czech Republic. If rubber dam is used, then it is typically for endodontic treatment or composite fillings. Kontinuální tvorba výpotků jakékoliv etiologie zásadním způsobem ovlivňuje kvalitu života nemocných. Akumulace tekutiny v pleurální dutině postupně komprimuje plicní parenchym. Prvním příznakem bývá progrese námahové dušnosti, později se přidává i klidová dušnost. S nárůstem výpotku dochází ke kompresi plicního parenchymu s rozvojem plicní atelektázy. Nemocný je ohrožen vznikem zánětového onemocnění plic v nevzdušném plicním parenchymu. U diagnózy maligního onemocnění je riziko zánětových komplikací výrazně vyšší vzhledem k předchozí chemoterapii či radioterapii, která zásadním způsobem snižuje imunitu. Opakovaná tvorba výpotku se výrazně podílí na rozvoji terminálního stavu nádorové kachexie. Efektivní metodou léčby je chemická pleurodéza, která se užívá u vybraného spektra nemocných. Principem je navození aseptické pleuritidy. Nejefektivnější cestou aplikace léčiva je videotorakoskopie. V souboru bylo 47 nemocných, 29 (61,7 %) mužů a 18 (38,3 %) žen, s cytologicky prokázaným recidivujícím maligním pleurálním výpotkem. Věkové rozmezí 42–80 let, Ø věk 65 let. Cílem práce bylo objektivizovat průběh celkových a lokálních zánětových změn po talkáži. Efekt léčby nebyl ovlivněn základním typem nádorového onemocnění. U obou skupin nebyly významně odlišné pooperační komplikace, což dokládá bezpečnost zvolené metody a podporuje správně zvolený algoritmus léčby., Continuous pleural effusion production of any aetiology can significantly affect the quality of patients life. Chronic effusion accumulation in the pleural cavity can lead to lung parenchyma compression. The first symptom of this entity is a progressive exertional dyspnea, which can later turn in dyspnea at rest. With the increase of the pleural effusion volume, pulmonary atelectasis can develop due to chronic parenchycha compression. The patient is at risk of inflamatory complications from the territory of non-ventilated parenchyma. Patients with a diagnosis of malignant disease have this risk considerably higher due to their previous chemotherapy or radiation, which substantially affect the immunity system. Repeated pleural effusion formation can significantly take part in the development of the terminal status of cancer cachexia. An effective method of palliative treatment is a chemical pleurodesis, which is used in selected patients. The principle is inducing aseptic inflammation. The most effective way of application is via videothoracoscopy. In a group of 47 patients, there were 29 (61.7 %) men and 18 (38.3 %) women with cytologically diagnosed recurrent malignant pleural. The age ranged between 42 and 80 with average age of 65 years. The aim was to assess the course of local and systemic inflammatory changes after talc application. The effect of treatment was not influenced by the type of malignancy . In both groups there was no significant difference in postoperative complications. Docetaxel je cytostatikum užívané v adjuvantní, neoadjuvatní i paliativní léčbě karcinomu prsu. Je podáván jako monoterapie či jako součást kombinovaných režimů. Taxany obecně spolu s antracykliny patří mezi nejefektivnější chemoterapeutika využívané v léčbě karcinomu prsu. Zařazení docetaxelu do léčebného schématu karcinomu prsu přineslo zlepšení celkového přežití a prodloužení období bez nemoci u pacientek léčených adjuvantně a větší procento léčebných odpovědí, prodloužení doby do progrese a celkového přežití při léčbě paliativní (1, 2, 3, 4, 5). Při využívání docetaxelu v léčbě je potřeba mít na paměti, že se jedná o chemoterapeutikum s nezanedbatelnými nežádoucími účinky. Cílem naší retrospektivní studie bylo vyhodnocení akutní toxicity při léčbě režimy s docetaxelem a zvážení možností léčby akutní toxicity., Docetaxel is a cytostatic drug used in adjuvant, neoadjuvant as well as palliative treatment of breast cancer. It is administered as monotherapy or as part of combination regimens. Taxanes along with anthracyclines are among the most efficacious chemotherapeutic drugs used in the treatment of breast cancer. Inclusion of docetaxel in the therapeutic regimen for breast cancer has resulted in improved overall survival and extended disease-free interval in patients treated with adjuvant therapy and in a greater proportion of therapeutic responses as well as extended time-to-progression and overall survival with palliative therapy (1, 2, 3, 4, 5). It must be kept in mind that, when used for treatment, docetaxel is a chemotherapeutic drug with non-negligible adverse effects. Cíl: Zhodnotit první zkušenosti s prováděním transarteriální radioembolizace (TARE) jaterních nádorů mikročásticemi s obsahem 90-yttria Metodika: V průběhu 6 měsíců jsme provedli u šesti nemocných TARE s aplikací skleněných mikročástic TheraSphere®. U čtyř nemocných šlo o metastatické postižení jater kolorektálním karcinomem, u dvou nemocných hepatocelulárním karcinomem. U nemocných jsme sledovali peri- a postprocedurální průběh, výskyt komplikací, WHO performance status a časný efekt léčby. Výsledky: Ani u jednoho z nemocných jsme nezaznamenali závažné komplikace léčby, při sledování účinků léčby bylo po 6 týdnech zjištěno, že u jedné nemocné došlo k rapidnímu zlepšení WHO performance status o tři kategorie, u ostatních byl status zachován, ani u jednoho nemocného v časném horizontu nebyla zaregistrována jaterní progrese onemocnění. Závěr: První zkušenosti s TARE ukazují i u nemocných s neresekabilním jaterním postižením dobrou toleranci léčby a slibné výsledky terapie., Aim: To evaluate an early experience with transarterial radioembolisation of liver tumors with microspheres with 90-yttrium. Method: During 6 monnths, we performed 6 months TARE with the application of glass-microspheres TheraSphere®. In 4 patients.colorectal carcinoma metastases were treated, in 2 hepatocellular carcinoma respectivelly In patients, the peri- and post-procedural disease development was surveyed according complications, WHO performance status an early effect of therapy. Results: No severe complication was noted, during 6 weeks was noted only one extreme improvement of WHO performance status with decrease by 3 grades, in rest of patients the performance status was unchanged. No progression in liver was noted within 6 weeks. Trombóza hlubokých žil dolních končetin je relativně časté a potenciálně velmi závažné onemocnění. Efektivita tradiční konzervativní systémové antikoagulační léčby je obecně malá a v současnosti se do popředí dostávají metody miniinvazivní terapie spočívající ve farmakologické a mechanické lokální trombolýze. Kromě vyššího počtu rekanalizací a zkrácení doby léčby je zde také nižší výskyt následných komplikací zejména v podobě vzniku posttrombotického syndromu. Svou roli hraje v prevenci závažné plicní embolie v určitých případech i perkutánní implantace kaválních filtrů., Deep vein thrombosis is a a common disease associated with high mortality and morbidity rates. Trombóza ledvinné žíly je vzácné onemocnění novorozeneckého věku. Jeho incidence je nízká, pohybuje se na úrovni 2,2 na 100 000 živě narozených dětí. Následky onemocnění jsou však časté a dosti závažné. Vzhledem k nízkému výskytu onemocnění nemáme v dostupné literatuře jednoznačná a jednotná terapeutická doporučení opírající se o randomizované studie.
