Bisphosphonates (BP) are potent inhibitors of bone resorption used mainly in the treatment of metastatic bone disease and osteoporosis. By inhibiting bone resorption, they prevent complications as pathological fracture, pain, tumor-induced hypercalcemia. Even though patient’s benefit of BP therapy is huge, various side effects may develop. Bisphosphonate-related osteonecrosis of the jaws (BRONJ) is among the most serious ones. Oncologic patients receiving high doses of BP intravenously are at high risk of BRONJ development. BPs impair bone turnover leading to compromised bone healing which may result in the exposure of necrotic bone in the oral cavity frequently following tooth extraction or trauma of the oral mucosa. Frank bone exposure may be complicated by secondary infection leading to osteomyelitis development with various symptoms and radiological findings. In the management of BRONJ, conservative therapy aiming to reduce the symptoms plays the main role. In patients with extensive bone involvement resective surgery may lead to complete recovery, provided that the procedure is correctly indicated. Since the treatment of BRONJ is difficult, prevention is the main goal. Therefore in high risk patients dental preventive measures should be taken prior to bisphosphonate administration. Cíl: Cílem příspěvku je literární analýza dosavadních nejvíce využívaných nefarmakologických metod tlumících bolest u novorozencůpro nejlepší praxi založenou na důkazech. Metodika: Pro získání dat za období 2000–2012 byly použity elektronické licencované a volně přístupné databáze. Hodnocení nalezených důkazů bylo realizováno dle metodiky Fineout-Overholt, Johnstona (2005), kteří řadí studie dle specifi ckých kritérií do sedmi kategorií. Do výběru předkládaného příspěvku byly zařazeny důkazy úrovně I, II, III. Zvolená kritéria splňovalo 42 studií (4 meta-analýzy, 6 systematických přehledů, 1 multicentrická studie a 31 randomizovaných kontrolovaných studií). Výsledky: Nefarmakologické metody využívané k tlumení bolesti u novorozenců jsou mnohem více efektivní, pokud jsou použity v kombinaci s dalšími nefarmakologickými metodami, jako jsou muzikoterapie, zavinutí, usnadněné zasunutí, multisenzorická stimulace, klokánkování a non-nutritivní a nutritivní sání. Závěr: Nefarmakologické postupy jsou účinné a vedou k efektivnímu zmírnění bolesti především při drobných procedurálních výkonech jako je odběr krve z patičky pomocí lancety, venepunkce, apod. Léčba bolesti a řízení její prevence by měla být nezbytnou součástí standardní i intenzivní zdravotní péče o novorozence., Aim: The aim of study is a literary analysis of the current most used non-pharmacological methods of relieving pain in neonates for the best evidence-based practice. Methods: For obtain data for the period 2000–2012 were used e-licensed and freely accessible database. Evaluation of the evidence found was carried out according to the methodology Fineout-Overholt, Johnston (2005), who put the study according to specifi c criteria in seven categories. The selection of the present study were classifi ed evidence level I, II, III. Selected criteria met the 42 studies (4 meta-analyzes, 6 systematic reviews, 1 multicenter study and 31 randomized controlled study). Results: Non-pharmacological methods used to control pain in neonates are much more eff ective when used in combination with other non-pharmacological methods, such as swaddling, facilitated tucking, multisensory stimulation, kangaroo care and non-nutritive and nutritive sucking. Cieľ: Cieľom štúdie bolo posúdiť bolesť, funkčný stav, úzkosť, depresiu, sociálne aktivity a sociálnu oporu u seniorov s reumatoidnou artritídou (RA) a ich vzájomné vzťahy, posúdiť možné rozdiely vzhľadom na pohlavie, rodinný stav, vek, trvanie ochorenia a komorbiditu. Metódy: U 60 seniorov s RA bola posudzovaná intenzita bolesti vizuálnou analógovou škálou (VAS), funkčná disabilita Stanfordským dotazníkom hodnotiacim zdravie indexom disability (HAQ-DI), úzkosť Beckovým inventárom úzkosti (BAI), depresia Zungovým sebahodnotiacim dotazníkom depresie (SDS) a sociálna interakcia škálou sociálnych aktivít (SA) a sociálnej opory (SO) nástroja Škály merania vplyvu chorôb kĺbov 2 (AIMS2). Výsledky: U seniorov s RA bolo zistené pociťovanie priemerne silnej bolesti, stredná až ťažká funkčná disabilita, prežívanie stredne silnej úzkosti a depresie, obmedzovanie v spoločenských aktivitách. Vo vnímaní sociálnej opory bolo zistené najlepšie skóre. Medzi sledovanými premennými boli Pearsonovou koreláciou zistené väčšinou signifikantné (P ≤ 0,05) kladné slabé až silné vzťahy. Medzi pohlaviami neboli zaznamenané rozdiely. Horšie skóre (P ≤ 0,05) bolo zistené v oblasti prežívania úzkosti a vo vnímanej sociálnej opore u osamelo žijúcich seniorov oproti seniorom žijúcim v manželskom zväzku. Štatisticky významný pozitívny vzťah (P ≤ 0,05) sa potvrdil medzi vekom a prežívaním úzkosti, medzi trvaním ochorenia a depresiou, medzi komorbiditou a funkčným stavom a depresiou. Záver: Ako naznačujú výsledky, prioritou v starostlivosti o túto skupinu seniorov by nemalo byť iba zmierňovanie bolesti a zlepšovanie/udržiavanie funkčných schopností, ale aj redukcia úzkosti a depresie, a to o to viac, čím sú seniori starší, čím u nich trvá ochorenie dlhšie, s komorbiditou a tiež u osamelo žijúcich., Aim: The aim of this study was to assess pain, functional status, anxiety, depression, social activity, and social support and their interactions in older adults with rheumatoid arthritis (RA), and to assess possible differences by gender, marital status, age, duration of disease, and co-morbidity. Methods: Pain intensity was assessed by the Visual Analogue Scale (VAS), functional disability by the Stanford Health Assessment Questionnaire – Disability Index (HAQ-DI), anxiety by the Beck Anxiety Inventory (BAI), depression by the Zung Self-rating Depression Scale (SDS), social activity by the Arthritis Impact Measurement Scales 2 (AIMS2) with scale of Social Activity (SA), and social support (SO) by AIMS2 with scale Support from family and friend in 60 elderly with RA. Results: There were found average strong pain, moderate to severe functional disability, moderate anxiety and moderate-to-marked depression, and limitation in social activity in elderly with RA. The best score was found in scale of social support. Significant (p ≤ 0.05) mild to severe relations were confirmed between explored objectives by Pearson’s correlations. Between genders no significant differences were recorded. Worse score (p ≤ 0.05) was detected in anxiety and social support in elderly living alone compared to married elderly. Between age and anxiety, duration of disease and depression, co-morbidity and functional disability, and depression were confirmed significant (p ≤ 0.05) positive relationships. Brachyterapie je metoda léčby zářením, při které se radioaktivní zdroje zavádějí přímo do oblasti nádoru. Umožňuje aplikaci vysoké dávky záření v krátkém čase a dosažení vysokého stupně lokální kontroly nádoru. Největší klinická zkušenost byla získána s brachyterapií s kontinuálním nízkým dávkovým příkonem (0,4–2,0 Gy/hod), kdy se zářiče zaváděly manuálně. V současně době byla tato metoda nahrazena na většině pracovišť automatickými afterloadingovými přístroji využívajícími vysoký dávkový příkon (> 12 Gy/hod) a frakcionaci. U časných ORL nádorů lze užít brachyterapii jako samostatnou léčebnou metodu. Její výhodou v porovnání s chirurgickou léčbou je většinou lepší kosmetický a funkční efekt, v porovnání se zevním ozařováním lepší kontrola nádoru a eliminace postradiační xerostomie. U rozsáhlejších nádorů je brachyterapie indikována k navýšení dávky po zevní radioterapii. Lze ji použít i pro léčbu recidiv v ozářeném terénu. Klasickými indikacemi jsou nádory dutiny ústní, orofaryngu a nasofaryngu. Přes současný technologický pokrok metod zevní radioterapie zůstává brachyterapie důležitou léčebnou modalitou u nádorů ORL., Brachytherapy is a method of radiation treatment, when the radioactive sources are directly applicated into the tumor region. Brachytherapy allows to deliver high doses to the tumor in a short time with excellent local tumor control. The largest experience was achieved with continual low dose rate brachytherapy (0.4–2Gy/hr) and manual insertion of sources. Recently this method was mostly replaced by automatic afterloading devices and high dose rate (> 12Gy/hr) fractionated treatment. Brachytherapy can be used in early stages of head and neck cancer as a sole treatment method. The advantage in comparison with surgery is a better functional and cosmetic outcome and in comparison with external beam radiotherapy a better tumor control and absence of xerostomia. In locally advanced tumors brachytherapy can be used for dose escalation of external beam radiotherapy. Brachytherapy is also useful for salvage treatment of recurrences after previous radiotherapy. Classical indications are oral cancer, oropharyngeal and nasopharyngeal tumors. The problem of prophylaxis of incompetence of bronchial stump and bronchial fistula is many sided. Notwithstanding the development of new technologies, perfection of surgical technique, its frequency still remains on high level that persistently requests the search of new ways for resolving this problem, which preserved its topicality till nowadays. We have found that: The most efficiency of BFS is achieved during treatment of bronchial fistulas with diameter up to 3 mm and with effectiveness at 83.2% patients after rightside PE and at 92.3% patients after leftside PE. In case of BPF with diameter more than 3 mm the number of satisfactory results of BFS reaches only 33.3%. but, it is necessary to mention that among patients with BPF sized over 3 mm after leftside PE the effectiveness of BFS significantly exceeds as general index (50.0% against 33.3%) so after rightside PE (50.0% against 25.0%)., F. Cíle studie: 1. Zhodnocení buněčností likvoru (CSF, z angl. cerebrospinal fluid = mozkomíšní mok), koncentrací glukózy v CSF, hodnot glukózového kvocientu (Q glu. ), koncentrací laktátu v CSF a hodnot koeficientu energetické bilance (KEB) coby ukazatelů intenzity zánětlivého procesu v centrálním nervovém systému (CNS) ve skupinách pacientů bez postižení CNS, pacientů s mírnými serózními záněty CNS neinfekční etiologie, pacientů se serózními záněty CNS infekční etiologie a pa- cientů s purulentními záněty CNS s extracelulárními bakteriemi v patogenezi. 2. Porovnání informačního potenciálu parametrů energetického metabolismu glukózy v likvorovém kompartmentu v soubo - ru vyšetřených pacientů, tzn. koncentrací glukózy v CSF, hodnot Q glu. , koncentrací laktátu v CSF a hodnot KEB. Typ studie: Retrospektivní studie. Materiál a metody: Vyšetřili jsme 133 vzorků CSF pacientů bez postižení CNS, 227 vzorků CSF pacientů s mírnými serózními záněty CNS s intrathekální syntézou imunoglobulinů neinfekční etiologie, 208 vzorků CSF pacientů se serózními záněty CNS infekční příčiny a 140 vzorků CSF pacientů s purulentními záněty CNS s extracelulárními bakteriemi v patogenezi. Oblastí našeho zájmu byly buněčnost CSF, koncentrace glukózy v CSF, hodnoty Q glu. , koncentrace laktátu v CSF a hodnoty KEB. Pro statistické hodnocení vybraných parametrů jsme použili D’Agostinův omnibus test, Kruskalův-Wallisův test s následnou post hoc analýzou Dunnovou metodou, Spearmanův korelační koeficient a multinomiální logistickou regresní analýzu. Výsledky: V likvorovém kompartmentu pacientů bez postižení CNS jsme nezjistili žádné změny na úrovni buněčností CSF ani změny energetické. V likvorovém kompartmentu pacientů s mírnými serózními záněty CNS neinfekční etiologie byly pouze v některých případech patrny zvýšené buněčnosti CSF a jisté odchylky hodnot Q glu. , koncentrací laktátu v CSF a hod - not KEB od normálu. V likvorovém kompartmentu pacientů se serózními záněty CNS infekční etiologie byly pozorovány projevy nápadně zvýšené intenzity zánětlivého procesu v likvorovém kompartmentu především na úrovni buněčností CSF, koncentrací laktátu v CSF a hodnot KEB. Vysoká intenzita purulentního zánětlivého procesu bakteriální etiologie v CNS se výrazně projevila na úrovni všech sledovaných parametrů. Nejtěsnější vztah jsme v našem souboru pacientů zjistili mezi buněčnostmi CSF a hodnotami KEB ( ρ = -0,770), následoval vztah mezi buněčnostmi CSF a koncentracemi laktátu v CSF ( ρ = 0,734), hodnotami Q glu. ( ρ = -0,676) a koncentracemi glukózy v CSF ( ρ = -0,544). Využitelnost uvedených parametrů k predikci intenzity zánětlivého procesu v CNS jsme ověřili pomocí multinomiální logistické regresní analýzy. Nejvyšší pre - dikční potenciál měly buněčnost CSF se 71,9 % a KEB se 71,6 % správně zařazených pacientů. Následovaly koncentrace laktátu v CSF s 64,7 %, Q glu. s 58,8 % a koncentrace glukózy v CSF s 54,7 % správně klasifikovaných pacientů. Závěr: Získané výsledky podpořily vhodnost využití úrovní buněčností CSF, koncentrací glukózy v CSF, hodnot Q glu. , kon - centrací laktátu v CSF a hodnot KEB k diagnostice postižení a monitorování intenzity zánětů v CNS. Navíc umožnily vytvořit pořadí energetických parametrů v likvorovém kompartmentu pacientů našeho souboru podle úrovně jejich informačního potenciálu. Nejpříznivějších výsledků pro hodnocení intenzity zánětlivého procesu v CNS tak bylo dosaženo v případě hod - not KEB, následovaly je koncentrace laktátu v CSF a hodnoty Q glu. a nejméně příznivých výsledků jsme dosáhli v případě koncentrací glukózy v CSF., Objective: 1. Compare the information potential of the used parameters of the glucose energy metabolism in the CSF compartments in our group of the investigated patients, i.e. concentrations of glucose in the CSF, values of the Q glu. , concentrations of lactate in the CSF and values of the KEB. Design: Retrospective study. Material and Methods: We examined 133 CSF specimens in patients without CNS impairment, 227 CSF specimens in pa - tients with slight serous inflammation with intrathecal synthesis of immunoglobulins of non-infectious aetiology in the CNS, 208 CSF specimens in patients with serous inflammation of infectious aetiology in the CNS and 140 CSF specimens in patients with purulent inflammation in the CNS with extracellular bacteria in pathogenesis. The objects of our interest were numbers of cells in the CSF, concentrations of glucose in the CSF, values of the Q glu. , concentrations of lactate in the CSF and values of the KEB. The D’Agostino Omnibus test, the Kruskal-Wallis test with follow-up post hoc analysis using the Dunn’s method, the Spearman correlation coefficient and the multinomial logistic regression analysis were used for statis - tical analysis of the examined parameters. Results: We did not find any changes in the numbers of cells in the CSF and in energy ratios in the CSF compartment of patients without CNS impairment. We found raised numbers of cells in the CSF and slight alterations of the glucose quo - tients, lactate concentrations in the CSF and the values of the KEB only in some patients with slight serous inflammations of non-infectious aetiology in the CNS. We observed manifestations of conspicuously increased intensity of inflammation in the numbers of cells in the CSF, lactate concentrations in the CSF and the values of the KEB in patients with serous inflam - mations of infectious aetiology in the CNS. Very high intensity of purulent inflammation in the CNS of bacterial aetiology was well apparent in all the evaluated parameters. Concerning the relationship, either direct or indirect, between the number of cells in the CSF and the other parameters, we found the highest correlation between the number of cells in the CSF and the values of the KEB ( ρ = -0.770), followed by the lactate concentrations in the CSF ( ρ = 0.734), the Q glu. ( ρ = -0.676) and the glucose concentrations in the CSF ( ρ = -0.544). We verified the applicability of the parameters mentioned above for prediction of the intensity of inflammation in the CNS via multinomial logistic regression analysis. The number of cells and the KEB, with 71.9 % and 71.6 % respectively, has the highest prediction potential of the correctly classified patients. They were followed by the lactate concentration in the CSF with 64.7 %, the Q glu. with 58.8 % and the glucose concentration with 54.7 % of the correctly classified patients. Conclusion: Our study supports the applicability of the numbers of cells in the CSF, the glucose concentrations in the CSF, the values of the Q glu. , the lactate concentrations in the CSF and the values of the KEB for diagnosing CSF impairment and for monitoring the intensity of inflammation in the CNS. Further, the results enabled determination of the information potential of the energy parameters. The values of the KEB were most suitable for evaluation of the intensity of inflammation in the CNS. Less suitable results were achieved in case of the lactate concentrations in the CSF. Nejzhoubnější gynekologický nádor, karcinom ovaria, je příčinou více než 50 % úmrtí v této skupině nádorů. Více než 60 % případů je diagnostikováno až v pokročilých stadiích s výrazně sníženou pravděpodobností přežití pacientek. Diagnostické nástroje pro efektivní časnou detekci nebo screening zatím neexistují. Terapeutické možnosti (chirurgická operace, chemoterapie) jsou nedostatečně efektivní, neboť karcinom ovaria se vyznačuje silnou tendencí k recidivě spojenou s chemorezistencí. Jedním z nejdéle a nejrozsáhleji používaným markerem pro diagnostiku primárních nádorů i recidivy je CA125/MUC16. Tento glykoprotein je znám již od 80. let 20. století, ale teprve nejnovější studie odhalují jeho biologické funkce v karcinogenezi a interakcích s buňkami imunitního systému. V tomto článku se blíže podíváme na jeho biologický význam a především na současný stav použití CA125 jako významného diagnostického markeru u karcinomu ovaria. V diagnostice primárních nádorů se totiž začínají objevovat nové markery, navíc je význam monitoringu CA125 z hlediska detekce recidivy zpochybňován rozsáhlými klinickými studiemi, které nenalezly přínos pro dlouhodobé přežití pacientek. Také potenciální význam CA125 v imunoterapii karcinomu ovaria se nezdá být tak velký, jak se předpokládalo., The most fatal gynecologic malignancy, ovarian cancer, causes more than 50% deaths in this tumor group. Most of cases (>60%) are diagnosed in advanced stages with poor 5-year survival prognosis. Diagnostic tools for an effective early detection or screening have not been found yet. Treatment possibilities (surgery, chemotherapy) are insufficient while high tendency to recurrence and chemoresistance occurs. CA125/MUC16 has been one of the most extensively used markers for a detection of primary tumors, or recurrence for a long time since its discovery in 80´s. However, the structure and biological functions of CA125 have been discovered relatively recently. CA125 may play an important role in carcinogenesis and interactions with cells of immune system. We reviewed the known biological functions and particularly the current state of using this marker as the diagnostic tool in ovarian cancer. Recently, many new markers emerged ambitiously to replace CA125; moreover, the importance of monitoring CA125 for the recurrence detection has also been questioned in large clinical trials. These studies have not found an impact for overall survival/mortality of patients. The authors present the case of a young woman with newly diagnosed Takayasu’s arteritis. This woman, with arterial hypertension, was investigated for the unspecific symptoms at the beginning. Afterwards, the transthoracic echocardiography showed dysfunction of the left ventricle and the abdominal sonography showed a stenosis of the right renal artery. PET/CT scan showed chronic modification after inflammatory processes on the wall of the thoracic and abdominal aorta.
