Analyzuje příčiny, které k reformě vedly a seznamuje s reformní strategií a jejím postupem. S odstupem několika let mapuje dopady reformy na dostupnost zdravotní péče, chování poskytovatelů i zdravotních pojišťoven a na pacienty. Nastiňuje ekonomické důsledky reformy a některé etické souvislosti., The article is focused on the long-planned health care reform in the Netherlands completed in 2006 in a broader historical context. The aim of the text is: firstly, to analyse the causes of the reform and secondly, to discuss reform strategy as well as its progress. We studied the relationship of connective tissue state with magnesium and hydroxyproline levels in blood and urine of young women with primary dysmenorrhea. Study showed that in girls with dysmenorrhea and phenomena of connective tissue dysplasia hydroxyproline level in urine was increased which was associated with increased degradation of collagen and decreased level of magnesium in blood serum. Pectus excavatum je najčastejšou vrodenou deformitou hrudníka. Deformitu ťažkého stupňa musíme považovať za absolútnu indikáciu na operačnú korekciu. Závažnosť deformity určená s pomocou rentgenových zobrazovacích vyšetrení je najdôležitejším indikačným kritériom. Štandardné RTG vyšetrenie hrudníka v dvoch projekciách, natívne špirálové MDCT vyšetrenie ponúkajú možnosť zhodnotiť rôzne základné ukazovatele závažnosti deformity, ako sú napríklad Halierov index, korekčný vertebrálny a fronto-sagitálny index. Rozvoj minimálne invazívnych operačných techník a narastajúci počet pacientov s asymetrickou deformitou liečených touto technikou priniesol potrebu nových indexov, ktoré operatérovi umožnia individuálne predtvarovať dlahu aj v tejto skupine pacientov. Autori práce sa v práci zamerali aj na komplikácie operačnej korekcie a typické nálezy, ktoré umožnia rentgenológovi tieto komplikácie zachytiť., Pectus excavatum is the most common inborn deformity of the chest. Severe deformity is considered to be an absolute indication for surgical correction. Indication criterias are dependent on multiple factors, but the severity of the malformation based on rádiológie assessment is the most important. Standard chest X-ray and native MDCT examination offer the evaluation of the severity of the deformity with the use of basic indices like Halier, correctional, vertebral or fronto-sagittal index. The evolution of minimally invasive pectus excavatum repair (MIRPE) and high number of asymmetric deformities treated with this procedure has brought the necessity for the creation of new indices. These indices help surgeons to individually preshape implanted bar in complex cases. Účinná léčba nádorových onemocnění často způsobuje trvalou ztrátu reprodukčních schopností pacienta. Jsou představeny moderní ukazatele ovariální rezervy, které mohou v běžné klinické praxi významně pomoci při zhodnocení míry rizika poškození gonadálních buněk v důsledku onkologické léčby. Práce dále popisuje zavedené metody reprodukční ochrany – kryokonzervace embryí a oocytů, kryokonzervace ovariální tkáně včetně možnosti její autotrasplantace a metody ochrany ovariální funkce aplikací analog gonadoliberinu. V diskuzi je pozornost věnována zejména výhodám a limitům popsaných metod ovariální protekce. V závěru jsou formulována stručná doporučení pro konzultace s pacientkami v adolescentním a reprodukčním věku před onkologickou léčbou, která významně ohrožuje jeho fertilitu., Effective treatment of cancer often causes patients irreversible damage of reproductive abilities. Cíl: Studie se zaměřuje na deskripci současného stavu poznání o resilienci rodičů sluchově postižených dětí. Metody: V rámci výzkumného přístupu EBN (Evidence Based Nursing) byl za výzkumnou metodu zvolen systematický přehled (systematic review). Byly sledovány tyto proměnné: místo výzkumu, metoda výzkumu, velikost vzorku, složení vzorku, cíl výzkumu, sledované proměnné, navrhované intervence, ošetřovatelské diagnózy NANDA-International. Výsledky: Z celkového počtu 280 nalezených studií bylo na základě vyřazovacích kritérií vybráno 26 studií kvantitativního charakteru. Bylo zjištěno, že žádná ze studií přímo s konceptem resilience nepracuje ani ve svých deklarovaných cílech ani v rámci zkoumaných proměnných. Závěry: Zdroje resilience bude možno odhalit pouze na základě kvalitativní sekundární analýzy obsahů studovaných textů., Aim: The objective of the research is to identify the state of knowledge about the resilience of parents of hearingimpaired children in relevant researches published up-to-day. Methods: The basic framework of the presented research is evidence-based nursing. The chosen technique is a systematic review. The following variables were monitored: location of the research, the research method, sample size, sample design, the objective of the research, variables, proposed interventions, NANDA-Int. nursing diagnosis. Results: The core sample consisted of 280 articles. On the basis of elimination criteria, most of them were rejected. Twenty six articles with quantitative methods fulfi lled the set criteria and were analysed. It was found that none of the studies works directly with the concept of resilience, neither in its declared objectives nor in the variables. V současnosti neexistuje standardní léčba cílená na inhibici HER2 pro pacientky s metastatickým HER2-pozitivním karcinomem prsu po progresi na léčbě trastuzumabem a lapatinibem. V retrospektivních studiích se prokázalo, že trastuzumab si může v této situaci zachovávat terapeutickou aktivitu a být validní léčebnou možností pro pacientky v dobrém celkovém stavu., Currently there are no standard therapeutical options for patients with metastatic HER2-positive breast cancer after progression on trastuzumab and lapatinib. Management kvality nabývá stále většího významu, stává se součástí koordinovaných aktivit zdravotnické péče a také vědecko-výzkumným tématem s cílem vymezovat dobrou praxi a přenášet ji dále. Cíl: Cílem výzkumu bylo zmapovat situaci v oblasti řízení kvality perioperační péče a ověřit, zda je rozdíl v úrovni dosahované kvality mezi akreditovanou a neakreditovanou nemocnicí a které atributy kvalitu péče ovlivňují. Metodika: Výzkum se skládal z kvalitativní a kvantitativní části a zahrnoval několik metod sběru dat. Pro vymezení úlohy manažerské funkce, byla zvolena metoda „stínování“ vedoucí sestry centrálních operačních sálů. Pro získání informací o řízení kvality byly zrealizovány tři polo-strukturované rozhovory s odbornicemi na toto téma. Na základě rozhovorů byla sestavena SWOT analýza, z jejichž výsledků byl vytvořen nestandardizovaný dotazník pro kvantitativní ověření identifikovaných bodů. Výsledky: Mezi dva nejdůležitější atributy, které nastavují úroveň kvality péče operačních sálů, a které mají souvislost s existencí akreditace, patří provádění vnějších auditů a sledování indikátorů kvality. V druhé studii se potvrdila pracovní hypotéza, že vyšší úrovně kvality dosáhly akreditované nemocnice. Závěr: Existence certifikátu akreditace má vliv na kvalitu operačních sálů, a nejdůležitějšími atributy jsou nastavení a sledování indikátorů kvality a provádění vnějších auditů., Quality Management is gaining an increasing importance, it is becoming a part of coordinated activities in medical care and also a scientific research topic in order to identify good practice and transfer it further. Aim: The aim of the research was to map the situation in the field of quality management of perioperative care and verify whether there is a difference in the level of quality achieved between accredited and non-accredited hospitals and which attributes affect the quality of care. Methods: The research consisted of qualitative and quantitative sections and included several data collection methods. The method of “shadowing” of head nurse central operating rooms was selected for the definition of the role of manager position. To get information about quality management there have been implemented three semi-structured interviews with the specialists on this topic. Based on the interviews the SWOT analysis was prepared and the results were used to make a non-standard questionnaire for the quantitative verification of the identified points. Results: Carrying out external audits and monitoring of quality indicators are the two most important attributes that set the level of quality of care in operating rooms and that are associated with the existence of accreditation. In the second study the working hypothesis was confirmed, which means that accredited hospitals achieved higher levels of quality. Práce se zabývá řízením péče o nemocné celiakií. Jedná se o modelový případ řízení péče o chronicky nemocné. Analytickým rámcem práce je víceúrovňový inovativní model péče o chronicky nemocné. Klíčová je interakce mezi zdravotnickým týmem, který reprezentuje systém zdravotní péče, mezi komunitními partnery a mezi pacienty a jejich rodinami. V případu celiakie je pak nutno zahrnout také roli zástupců trhu s bezlepkovými potravinami. Interakce těchto aktérů se odehrává v širším politickém kontextu, jehož stav tuto interakci zásadním způsobem ovlivňuje a odráží se tak v konečných výsledcích stavu pacientů a kvalitě jejich života. Pomocí nástroje hodnocení kvality života pod - míněné zdravím SF-36 bylo zjištěno, že kvalita života celiaků je nižší ve srovnání s evropským standardem zdravé populace a že příčiny tohoto problému je nutno hledat a systematicky odstraňovat na všech úrovních systému péče o chronicky nemocné. Zjištění tak potvrzují teoretický předpoklad, že péče o chronicky nemocné je stále značně podceňována a neodpovídá specifickým potřebám chronicky nemocných pacientů, což se odráží právě na celkové kvalitě života., This work deals with coeliac disease management. The analytical framework of this work is the multilevel Innovative Care for Chronic Conditions Framework. Crucial is the interaction between health care team that represents health care system, community partners and patients with their families. In the case of coeliac disease it is necessary to include also the role of representatives of market with gluten free products. The interaction of these participants takes place in wider political context, whose state influences this interaction fundamentally and reflects so in final results state of patients and quality of their live. Using the evaluation of the quality of life tool SF-36, it has been ascertained, that the quality of life of coealiac patients is lower in comparison with European standard of healthy population and that the causes of the problem have to be searched and systematically removed at all levels of the system of chronic health care.
