Úvod: Dynamická scintigrafie jícnu (DSJ) je funkční vyšetřovací metoda, která analyzuje tranzit bolusu radiofarmaka (RF) jícnem do žaludku. Hodnocení motility jícnu je přínosné po vyloučení organické příčiny onemocnění v diagnostice řady patologických stavů u dospělých i dětí. Nejčastější indikací je podezření na poruchu jícnové peristaltiky a funkce dolního jícnového svěrače (LES) u jícnové i mimojícnové manifestace refluxní choroby jícnu (RCHJ), méně často u osob se systémovým onemocněním pojiva, diabetiků apod. Metodika: 3 – 6hodinové lačnění, 48 hodin před vyšetřením vysadit prokinetika. Poloha vleže na zádech, detektor kamery nad hrudníkem. Laborant (sestra) provede nácvik polknutí příslušného objemu roztoku RF – dospělí 10 ml, děti 5 ml. Poté pacient bolus RF (20–50 MBq 99mTc-MAA) polkne a zahajujeme dynamickou studii: 1. grupa – 240 snímků á 120 ms, 2. grupa – 120 snímků á 15 s. Vyznačujeme oblasti zájmu (ROI) nad jednotlivými třetinami jícnu a žaludkem. Po sestrojení křivek hodnotíme v rámci první fáze studie tranzitní čas jícnem (MTT), přítomnost antiperistaltiky či retence RF v jícnu. Z druhé fáze vyšetření hodnotíme známky gastroezofageálního refluxu (GER). Výsledky: Uvádíme příklady patologických nálezů antiperistaltiky, prodlouženého MTT, retence v jícnu a známek GER u nemocných s RCHJ, bronchiálním astmatem, chronickým kašlem, stavem po fundoplikaci pro RCHJ, sklerodermií a achalázií. Závěr: DSJ je jednoduchá, funkční, neinvazivní vyšetřovací technika s nízkou radiační zátěží, která je vysoce citlivá v detekci poruch jícnové motility. Po vyloučení organicity onemocnění podává doplňující informaci o funkci jícnu. Vyšetření se osvědčilo také u dětí. Úvod: Dynamické metody kvantitativního testování senzitivity (Quantitative Sensory Testing; QST) jsou relativně novou nadstavbovou aplikací QST umožňující objektivizovat funkci centrálních modulačních mechanizmů algické percepce, jejichž individuální nastavení může přispívat k rozvoji chronických bolestivých stavů. Testování těchto mechanizmů umožňuje např. predikci rozvoje pooperační bolesti. K nejčastěji hodnoceným dynamickým QST metodám patří tzv. podmíněná modulace bolesti (Conditioned Pain Modulation; CPM) a časová sumace percepce bolestivých podnětů (Temporal Summation; TS). Soubor a metodika: Vyšetření dQST (dynamic methods of Quantitative Sensory Testing) metod s využitím termoalgických podnětů a modulace bolestivým chladem bylo provedeno u 77 zdravých dobrovolníků (47 žen testovaných v ovulační fázi menstruačního cyklu, 30 mužů, věk 19–73 let). Hodnocen byl také vliv věku a pohlaví na výsledky obou dQST testů. Výsledky: Uspokojivá funkce podmíněné modulace bolesti byla prokázána u 84 % testovaných dobrovolníků. Časová sumace bolestivých podnětů byla zachycena u 92, resp. 98 % jedinců v závislosti na použité teplotě podnětů. Nefunkčnost mechanizmů u zbylé části souboru byla ve většině případů podmíněna nesprávně nastavenými technickými parametry testu. U žen testovaných v ovulační fázi menstruačního cyklu jsme prokázali signifikantně vyšší efektivitu CPM mechanizmu ve srovnání s muži. Funkce TS se mezi oběma pohlavími významně nelišila. Významný vliv věku na efektivitu CPM ani TS mechanizmu nebyl ve sledovaném souboru prokázán. Závěr: Provedená studie ověřila použitelnost zvolených algoritmů dynamického testování senzitivity a prokázala antinociceptivní nastavení žen v ovulační fázi menstruačního cyklu ve srovnání s muži, zatímco významný vliv věku na funkci centrálních modulačních mechanizmů algické percepce nebyl zachycen., Background: So called “dynamic quantitative sensory methods” (dQST) represent relatively new QST application which enables to objectify certain mechanisms of pain processing, e.g. central integration or descending control. Their dysfunction may contribute to the development of chronic painful conditions. dQST methods proved the potential to predict the development of chronic postoperative pain. Conditioned pain modulation (CPM) and temporal summation (TS) are the most commonly used dQST tests. Patients and methods: The CPM and TS testing was performed in a group of 77 healthy volunteers (47 women tested in the ovulatory phase of the menstrual cycle, 30 men, age range 19–73) using thermal pain stimuli. Efficiency of CPM and TS mechanism including age and gender influence was evaluated. Results: Efficient CPM was found in 84% of healthy volunteers. In remaining 16%, the inefficiency was mainly due to low intensity of testing or modulatory stimuli. Based on the temperature used, efficient TS mechanism was found in 92% or 98% of healthy individuals. Women in the ovulatory phase of the menstrual cycle showed significantly more efficient CPM comparing to men. No sex differences of TS testing were found. Age had no significant effect on the CPM or TS mechanism in our group of healthy voluteers. Conclusions: Our study has verified the applicability of CPM and TS algorithms used. Women in ovulatory phase of the menstrual cycle show increased anti-nociceptive setting comparing to men. No significant effect of age on central modulation of pain perception has been found in pain-free healthy individuals. Dospívající dívky vyhledávají gynekologa nejčastěji při poruchách menstruačního cyklu. V ideálním případě by se dívka měla obracet na dětského gynekologa, který ovšem není všude dostupný. Proto často řeší tento problém gynekologové specializovaní na dospělé pacientky. Ti většinou problém řeší předepsáním hormonální antikoncepce, jejíž podávání cyklus upraví. To však není kauzální řešení problému tzv. juvenilní metroragie. Její řešení patří do rukou dětského gynekologa, a pokud je nucen tento problém řešit gynekolog dospělých, měl by mu tento článek poskytnout základní informace., Adolescent girls most frequently seek a gynecologist’s advice in relation to menstrual cycle disorders. Ideally, they should visit a pediatric gynecologist, whose care, however, is not always within easy reach. Therefore this problem is often treated by gynecologists specialized in adult patients. In most cases they deal with it by prescribing hormonal contraception, whose administering will regulate the cycle. However this is not a causal solution of the issue of juvenile metrorrhagia. Úvod: Léčba intratékálním baklofenem (ITB) kontinuálně pomocí pumpových systémů je důležitou léčebnou modalitou u nemocných s těžkou spasticitou, která již nezabírá na perorální léky nebo když se objeví výrazné nežádoucí účinky. Tato metoda je relativně velmi bezpečná, přesto se ale mohou vyskytnout komplikace spojené s touto léčbou. Nejčastěji se objevují komplikace spojené s implantací systému. Jde zejména o problémy s katétrem. Rotace pumpy je extrémně vzácnou komplikací a její výskyt je méně než 0,3 %. Kazuistika: U 74leté ženy s těžkou spasticitou po chronickém míšním poranění ve výši Th9 byla úspěšné provedena implantace baklofenové pumpy (Medtronic, Synchromed II). Pacienka měla bezprostřední úlevu se snížením spasticity již na dávce baklofenu 100 ?g/den. Na první ambulantní kontrole se opakovaně nezdařilo doplnit rezervoár pumpy. Bylo vysloveno podezření na její rotaci. Pacientka byla okamžitě odeslána na rentgenologické vyšetření, kde bylo potvrzeno přetočení pumpy o 180 stupňů. Tentýž den byla pacienta v lokální anestezii operována, pumpa byla otočena a důkladně zafixována stehy. Vzhledem k rychlé revizi se u pacientky nerozvinul syndrom z náhlého odnětí baklofenu při prázdném rezervoáru pumpy. Závěr: Rotace pumpy je vzácná komplikace léčby ITB. Je nutné okamžité vyřešení tohoto problému, neboť hrozí rozvoj abstinenčního syndromu z náhlého odnětí baklofenu. Léčbu ITB je třeba provádět v centrech se specializovanou péčí o tyto pacienty. Despite generally favorable outcomes and relative safety of this method, complications associated with ITB can be observed. The most common problems include surgery‑related complications resulting from system implantation, including catheter failure. Pump rotation is a rare complication with incidence less than 0.3%. Case report: Seventy‑four‑years‑old female suffered from spasticity after chronic spinal cord injury at T9 level. e underwent successful implantation of ITB pump (Medtronic, Synchromed II) with prompt spasticity relief on baclofen dose of 100 ?g/day. During the first outpatient appointment, it was difficult to puncture and fill the reservoir with baclofen. Plain x‑ray revealed rotation of the pump with reservoir in downright position. On the same day, the patient underwent emergency surgical revision with correction of the pump position and refill of the reservoir. No baclofen withdrawal syndrome or other complications occurred and the patient recovered well. Conclusion: Pump rotation is a very unusual and rare complication of ITB treatment. Na nedávném kongresu Evropské urologické asociace (EAU) ve švédském Stockholmu byla prezentována data, která jednoznačně ukazují na to, že se lékaři při léčbě příznaků syndromu dolních cest močových u mužů až příliš soustřeďují na evakuační (obstrukční) symptomy a opomíjejí příznaky jímací (iritační), které zřejmě ještě negativněji ovlivňují kvalitu života jejich pacientů. Cíl: Správný pooperační režim je důležitou součástí rekonvalescence po dekompresi karpálního tunelu. Cílem této prospektivní randomizované studie je zjistit, zda pooperační dočasné dlahování má klinicky významný efekt, nebo zda není naopak zbytečné. Výsledky dosud publikovaných prací nedávají jasné, prokazatelné a jednoznačné doporučení. Soubor a metodika: Do souboru bylo zařazeno celkem 72 pacientů s klinicky a elektromyograficky prokázaným syndromem karpálního tunelu, kteří byli randomizováni do dvou skupin. V první skupině byla pacientům po operaci na dva týdny přiložena dorzální dlaha fixující zápěstí v neutrální poloze a v druhé skupině dlaha použita nebyla. Před operací i po operaci byla hodnocena distální motorická latence, síla stisku a míra subjektivních obtíží pomocí specializovaných dotazníků DASH (Disabilities of Arm, Shoulder and Hand Questionnaire) a Bostonského dotazníku (Boston Carpal Tunnel Questionnaire). Výsledky: Po operaci došlo ke zkrácení distální motorické latence a mírnému snížení síly stisku, ale mezi skupinami nebyly nalezeny statisticky významné rozdíly (p > 0,05). Po operaci došlo také k předpokládanému zmírnění subjektivních obtíží, objektivizovanému dotazníkovými koeficienty, ale mezi skupinami nebyl nalezen statisticky významný rozdíl (p > 0,05). Závěr: Podle našich zjištění dočasné dlahování po otevřené dekompresi karpálního tunelu ve srovnání s nedlahovanou skupinou nezlepšuje výsledky chirurgické léčby. Používání dlahy v této indikaci se jeví jako nadbytečné jak z lékařského, tak z ekonomického pohledu. Na druhou stranu dočasné dlahování po operaci výsledky operační léčby nezhoršuje., Aim: Correct post-operative care is an important part of recovery after carpal tunnel decompression. The aim of this prospective randomized study was to determine whether postoperative temporary splinting has a clinically significant effect, or whether it is unnecessary. The results of published data do not give clear, demonstrable and unambiguous recommendations. Material and methods: A total of 72 patients with clinically and electrodiagnostically established diagnosis of carpal tunnel syndrome were randomized into two groups. The first group included patients with a dorsal splint, fixing the wrist in a neutral position for two weeks after surgery, whereas the second group included patients, in whom the splint was not used after surgery. Distal motor latency, grip strength and degree of subjective symptoms were assessed before and after surgery using specific questionnaires, namely the DASH questionnaire (Disabilities of Arm, Shoulder and Hand) and the Boston questionnaire (Boston Carpal Tunnel Questionnaire). Results: After surgery, there was a reduction of distal motor latency and a moderate reduction of grip strength, but there were no statistically significant differences (p > 0.05) between the two groups. After surgery, the anticipated improvement of subjective symptoms was achieved, objectified by questionnaire coefficients. However, there was no statistically significant difference (p > 0.05) between the two groups either. Conclusion: According to our findings, temporary splinting after open carpal tunnel decompression does not improve the results of surgical treatment compared to the non-splinted group. Using the splint in this indication seems to be redundant from the medical and economic point of view. On the other hand, temporary splinting after surgery does not cause worsening of the results of surgical treatment. Activated hepatic stellate cells (HSC) are a major source offibrous proteins in cirrhotic liver. Inducing or accelerating their apoptosis is a potential way of liver fibrosis treatment. Extracellular matrix (ECM) surrounding cells in tissue affects their differentiation, migration, proliferation and function. Type I collagen is the main ECM component in fibrotic liver. We have examined how this protein modifies apoptosis of normal rat HSC induced by gliotoxin, cycloheximide and cytochalasin D in vitro and spontaneous apoptosis of HSC isolated from CCl4-damaged liver. The effect of different host plants on Frankliniella occidentalis (Pergande) (Thysanoptera: Thripidae) body size was investigated. Thrips from three different populations, from the Netherlands, Italy, and USA, achieved greater body sizes when reared on cucumber than on bean. The same thrips grew larger when reared on susceptible than on resistant cucumber. On the latter, reproduction was reduced, suggesting that smaller thrips have a lower reproduction. However, no evidence was found for a correlation between size and reproduction in experiments with thrips from four different populations, from the Netherlands, New Zealand, France, and USA that differed significantly in body size. Also when individual thrips from the four populations were tested, there was no correlation between size and reproduction. It is concluded that resistant cucumber affects both size and reproduction of F. occidentalis. Background: Few data are available on the specific care giving-related problems of stroke patient’s caregivers and factors that influence the burden of these caregivers. Aim: To study the influences of the active rehabilitation process on anxiety, depression, care burden and perceived social support level of stroke patients caregivers. Design: A prospective clinical trial. Setting: Patients and caregivers entering a rehabilitation program at a university hospital in Turkey. Populations: Ninety patients with a first episode of stroke and 90 caregivers responsible for their care were recruited for our study. Methods: Patients and caregivers were assessed before and after the active rehabilitation process. The functional disability level of the patients was assessed by Functional Independence Measure (FIM). The Beck Anxiety Scale (BAS) and the Beck Depression Scale (BDS) were used for anxiety and depression assessment, the Zarit Care Burden Scale (ZCBS) for care burden assessment and the Multi-Dimensional Scale of Perceived Social Support (MDSPSS) for perceived social support assessment. Results: A statistically significant rise is observed in the special person sub-assessment of MDSPSS in both female and male caregivers. Also, a significant decrease in care burden, anxiety and depression levels of caregivers was noted after the rehabilitation program (p < 0.05). Conclusion: Caregivers accept the rehabilitation period as important social support in addition to the support provided by family and friends. Also, our positive results were associated with an improvement in the patients’ functional level and an increase in the acquisition of knowledge and skill required of caregivers in order to provide care during rehabilitation. Clinical Rehabilitation Impact: The rehabilitation team should be aware of the fact that the perceived care burden may be greater due to the lack of knowledge concerning available resources and due to the inability to cope with stress effectively. and A.
