V kazuistice prezentujeme případ objemného AAA a jeho operačního řešení., Abdominal aortic aneurysm (AAA) is related with higher age and poses a serious threat to the patient´s life. Aneurysms larger than 5.5 cm in diameter are indicated for open or endovascular repair. Giant aneurysms are rarely encountered due to a high risk of rupture. We present a case report of such a giang aneurysm and its open repair., and V. Opatrný, V. Třeška, J. Moláček, R. Tupý
Gliomy limbického a paralimbického systému představují anatomicky a tím pádem i operačními přístupy unikátní skupinu mozkových nádorů. Taktéž prezentujeme výsledky operací našeho pracoviště ve smyslu morbidity a radikality za posledních sedm let., Limbic and paralimbic gliomas represent a unique group of brain tumors both from an anatomic and surgical point of view. Microsurgery, together with modern perioperative procedures, enables a wider extent of resection of these gliomas. Based on localization, we divide gliomas into three basic groups: 1. insular, 2. amygdalohippocampal complex and 3. cingulum gliomas. In our review, we describe the anatomy and function of the limbic and paralimbic system and in relation to that, we present a description of the specific neurosurgical approaches to these regions. Maximal resection with brain function preservation represents the basis for subsequent oncologic therapy. We also present the surgical results of our department in the last seven years with regard to morbidity and radicality. Key words: limbic system – insula – hippocampus – cingulum – glioma – resection The authors declare they have no potential conflicts of interest concerning drugs, products, or services used in the study. Autoři na dvou předložených kazuistikách představují širokou diferenciální diagnostiku granulomatózních procesů hepatobiliárního traktu. Izolovaný granulomatózní zánět v této oblasti nacházíme velmi zřídka. Diferenciální diagnostika těchto procesů zasahuje od infekční etiologie až po systémová onemocnění či chemickou iritaci. Granulomy často imitují maligní tumory v jaterní krajině, a chirurg tak může být lékařem, který dané onemocnění odhalí jako první při resekčním výkonu či cholecystektomii., The authors present two case reports to demonstrate wide differential diagnosis of granulomatous disorders of the hepatobiliary tract. Isolated granulomatous disorders are very rare in this area. Differantial diagnosis includes infectious and systemic diseases or chemical irritation. Granulomas often immitate malignant tumors and the surgeon could be first to diagnose this disease during liver resection or cholecystectomy., and J. Brůha, V. Liška, V. Třeška, T. Skalický, J. Cíl studie: Na základě soudobých studií podat přehled o hlavních biologických funkcích lipoproteinů o vysoké hustotě (HDL), zejména ve vztahu k reverznímu transportu cholesterolu a k jejich protizánětlivému působení. Popsat pravděpodobné mechanismy vzniku dysfunkčních prozánětlivých HDL a uvést některé parametry asociované s tímto procesem. Podat přehled o laboratorních metodách stanovení kvantitativních, strukturních a funkčních vlastností HDL, včetně moderních metod založených na hmotové spektrometrii. Typ studie: přehledový článek Závěr: V případě systémového zánětu a/nebo oxidačního stresu dochází ke vzniku dysfunkčních HDL akumulujících oxidanty. Probíhá strukturní modifikace apolipoproteinu AI, což vede mimo jiné k inhibici reverzního transportu cholesterolu a tvorbě prozánětlivých HDL. Na ztrátě příznivých vlastností HDL se podílí modifikace proteinové i lipidové složky HDL. Izolované měření HDL cholesterolu nemusí u některých syndromů bezvýhradně korelovat s mírou kardiovaskulárního rizika a v některých situacích je vhodné stanovit markery mapující strukturu a funkci HDL., Objective: On the basis of recent studies to make an overview concerning the crucial biological functions of high density lipoproteins (HDL), with emphasis on the role in reverse cholesterol transport and their antiinflammatory traits. The aim was to describe probable mechanisms of dysfunctional proinflammatory HDL formation with introduction some of associated parameters. To present laboratory methods for determination of quantitative, structural and functional qualities of HDL, including advanced mass spectrometry techniques. Study design: review Conclusion: In the case of systemic inflammation and/or oxidative stress, the formation of dysfunctional HDL accumulating oxidants takes place, including apo AI structural modifications. This process can lead to the inhibition of reverse cholesterol transport and proinflammatory HDL generation, among others. Loss of beneficial qualities results from modification of both lipid and protein components of HDL. In some cases, an isolated quantitative measurement of HDL cholesterol may not fully correlate with cardiovascular risk and it is eligible to determine some of structural and/or functional markers of HDL., Novotný D., Karásek D., Vaverková H., Malina P., and Literatura
Průměrný follow‑up pacientů byl 39,3 měsíce. Závěry: Naše první zkušenosti ukázaly u tří pacientů dobrou reinervaci nervus facialis po hemihypoglossofaciání anastomóze. Metodu považujeme spolu s dalšími variantami hemihypoglossofaciálních spojek (Darrouzet, LeClerc) za efektivní způsob neurotizace plegického lícního nervu při malém riziku vzniku nežádoucích příznaků., Aim: Authors present their early experience with a hemihypoglossofacial anastomosis in the treatment of neurosurgical patients with complete postoperative facial palsy after removal of tumors in the posterior fossa or skull base. Patients and method: In three patients undergoing an open surgery for complete facial palsy between 2009 and 2014, the hemihypoglossofacial anastomosis according to Cushimano and Sekhar was performed. Facial palsy was due to a large vestibular schwannoma in two patients and a facial nerve schwannoma in one patient. Results: Functional reinnervation of grade III according to the House and Brackmann scale was achieved in two patients and reinervation of grade IV in one patient. All patients had mild hemilingual atrophy only that did not lead to any functional disturbances in chewing, swallowing or speech. The mean follow-up period was 39, 3 months. Conclusions: Our early experience showed very good functional reinervation in patients after hemihypoglosofacial anastomosis. We consider this method together with other modifications (Darrouzet, LeClerc) to be very effective in the treatment of facial plegia with low risk of side effects.
