Úvod: Dynamické metody kvantitativního testování senzitivity (Quantitative Sensory Testing; QST) jsou relativně novou nadstavbovou aplikací QST umožňující objektivizovat funkci centrálních modulačních mechanizmů algické percepce, jejichž individuální nastavení může přispívat k rozvoji chronických bolestivých stavů. Testování těchto mechanizmů umožňuje např. predikci rozvoje pooperační bolesti. K nejčastěji hodnoceným dynamickým QST metodám patří tzv. podmíněná modulace bolesti (Conditioned Pain Modulation; CPM) a časová sumace percepce bolestivých podnětů (Temporal Summation; TS). Soubor a metodika: Vyšetření dQST (dynamic methods of Quantitative Sensory Testing) metod s využitím termoalgických podnětů a modulace bolestivým chladem bylo provedeno u 77 zdravých dobrovolníků (47 žen testovaných v ovulační fázi menstruačního cyklu, 30 mužů, věk 19–73 let). Hodnocen byl také vliv věku a pohlaví na výsledky obou dQST testů. Výsledky: Uspokojivá funkce podmíněné modulace bolesti byla prokázána u 84 % testovaných dobrovolníků. Časová sumace bolestivých podnětů byla zachycena u 92, resp. 98 % jedinců v závislosti na použité teplotě podnětů. Nefunkčnost mechanizmů u zbylé části souboru byla ve většině případů podmíněna nesprávně nastavenými technickými parametry testu. U žen testovaných v ovulační fázi menstruačního cyklu jsme prokázali signifikantně vyšší efektivitu CPM mechanizmu ve srovnání s muži. Funkce TS se mezi oběma pohlavími významně nelišila. Významný vliv věku na efektivitu CPM ani TS mechanizmu nebyl ve sledovaném souboru prokázán. Závěr: Provedená studie ověřila použitelnost zvolených algoritmů dynamického testování senzitivity a prokázala antinociceptivní nastavení žen v ovulační fázi menstruačního cyklu ve srovnání s muži, zatímco významný vliv věku na funkci centrálních modulačních mechanizmů algické percepce nebyl zachycen., Background: So called “dynamic quantitative sensory methods” (dQST) represent relatively new QST application which enables to objectify certain mechanisms of pain processing, e.g. central integration or descending control. Their dysfunction may contribute to the development of chronic painful conditions. dQST methods proved the potential to predict the development of chronic postoperative pain. Conditioned pain modulation (CPM) and temporal summation (TS) are the most commonly used dQST tests. Patients and methods: The CPM and TS testing was performed in a group of 77 healthy volunteers (47 women tested in the ovulatory phase of the menstrual cycle, 30 men, age range 19–73) using thermal pain stimuli. Efficiency of CPM and TS mechanism including age and gender influence was evaluated. Results: Efficient CPM was found in 84% of healthy volunteers. In remaining 16%, the inefficiency was mainly due to low intensity of testing or modulatory stimuli. Based on the temperature used, efficient TS mechanism was found in 92% or 98% of healthy individuals. Women in the ovulatory phase of the menstrual cycle showed significantly more efficient CPM comparing to men. No sex differences of TS testing were found. Age had no significant effect on the CPM or TS mechanism in our group of healthy voluteers. Conclusions: Our study has verified the applicability of CPM and TS algorithms used. Women in ovulatory phase of the menstrual cycle show increased anti-nociceptive setting comparing to men. No significant effect of age on central modulation of pain perception has been found in pain-free healthy individuals. Na nedávném kongresu Evropské urologické asociace (EAU) ve švédském Stockholmu byla prezentována data, která jednoznačně ukazují na to, že se lékaři při léčbě příznaků syndromu dolních cest močových u mužů až příliš soustřeďují na evakuační (obstrukční) symptomy a opomíjejí příznaky jímací (iritační), které zřejmě ještě negativněji ovlivňují kvalitu života jejich pacientů. Výsledky studie NEPTUNE II naopak demonstrují, že kombinovaná léčba tohoto onemocnění je účinná a dobře tolerovaná při krátkodobém i dlouhodobém použití., At recent EAU meeting in Stockholm (Sweden) new data were presented, clearly demonstrating that urologists treating symptoms associated with lower urinary tract syndrome focus too much on voiding (obstructive) symptoms while neglecting filling (irritating) symptoms, which might have substantially larger negative impact on patient quality of life. Cíl: Správný pooperační režim je důležitou součástí rekonvalescence po dekompresi karpálního tunelu. Cílem této prospektivní randomizované studie je zjistit, zda pooperační dočasné dlahování má klinicky významný efekt, nebo zda není naopak zbytečné. Výsledky dosud publikovaných prací nedávají jasné, prokazatelné a jednoznačné doporučení. Soubor a metodika: Do souboru bylo zařazeno celkem 72 pacientů s klinicky a elektromyograficky prokázaným syndromem karpálního tunelu, kteří byli randomizováni do dvou skupin. V první skupině byla pacientům po operaci na dva týdny přiložena dorzální dlaha fixující zápěstí v neutrální poloze a v druhé skupině dlaha použita nebyla. Před operací i po operaci byla hodnocena distální motorická latence, síla stisku a míra subjektivních obtíží pomocí specializovaných dotazníků DASH (Disabilities of Arm, Shoulder and Hand Questionnaire) a Bostonského dotazníku (Boston Carpal Tunnel Questionnaire). Výsledky: Po operaci došlo ke zkrácení distální motorické latence a mírnému snížení síly stisku, ale mezi skupinami nebyly nalezeny statisticky významné rozdíly (p > 0,05). Po operaci došlo také k předpokládanému zmírnění subjektivních obtíží, objektivizovanému dotazníkovými koeficienty, ale mezi skupinami nebyl nalezen statisticky významný rozdíl (p > 0,05). Závěr: Podle našich zjištění dočasné dlahování po otevřené dekompresi karpálního tunelu ve srovnání s nedlahovanou skupinou nezlepšuje výsledky chirurgické léčby. Používání dlahy v této indikaci se jeví jako nadbytečné jak z lékařského, tak z ekonomického pohledu. Na druhou stranu dočasné dlahování po operaci výsledky operační léčby nezhoršuje., Aim: Correct post-operative care is an important part of recovery after carpal tunnel decompression. The aim of this prospective randomized study was to determine whether postoperative temporary splinting has a clinically significant effect, or whether it is unnecessary. The results of published data do not give clear, demonstrable and unambiguous recommendations. Material and methods: A total of 72 patients with clinically and electrodiagnostically established diagnosis of carpal tunnel syndrome were randomized into two groups. The first group included patients with a dorsal splint, fixing the wrist in a neutral position for two weeks after surgery, whereas the second group included patients, in whom the splint was not used after surgery. Distal motor latency, grip strength and degree of subjective symptoms were assessed before and after surgery using specific questionnaires, namely the DASH questionnaire (Disabilities of Arm, Shoulder and Hand) and the Boston questionnaire (Boston Carpal Tunnel Questionnaire). Results: After surgery, there was a reduction of distal motor latency and a moderate reduction of grip strength, but there were no statistically significant differences (p > 0.05) between the two groups. After surgery, the anticipated improvement of subjective symptoms was achieved, objectified by questionnaire coefficients. However, there was no statistically significant difference (p > 0.05) between the two groups either. Conclusion: According to our findings, temporary splinting after open carpal tunnel decompression does not improve the results of surgical treatment compared to the non-splinted group. Using the splint in this indication seems to be redundant from the medical and economic point of view. On the other hand, temporary splinting after surgery does not cause worsening of the results of surgical treatment. Background: Few data are available on the specific care giving-related problems of stroke patient’s caregivers and factors that influence the burden of these caregivers. Aim: To study the influences of the active rehabilitation process on anxiety, depression, care burden and perceived social support level of stroke patients caregivers. Design: A prospective clinical trial. Setting: Patients and caregivers entering a rehabilitation program at a university hospital in Turkey. Populations: Ninety patients with a first episode of stroke and 90 caregivers responsible for their care were recruited for our study. Methods: Patients and caregivers were assessed before and after the active rehabilitation process. The functional disability level of the patients was assessed by Functional Independence Measure (FIM). The Beck Anxiety Scale (BAS) and the Beck Depression Scale (BDS) were used for anxiety and depression assessment, the Zarit Care Burden Scale (ZCBS) for care burden assessment and the Multi-Dimensional Scale of Perceived Social Support (MDSPSS) for perceived social support assessment. Results: A statistically significant rise is observed in the special person sub-assessment of MDSPSS in both female and male caregivers. Also, a significant decrease in care burden, anxiety and depression levels of caregivers was noted after the rehabilitation program (p < 0.05). Conclusion: Caregivers accept the rehabilitation period as important social support in addition to the support provided by family and friends. Also, our positive results were associated with an improvement in the patients’ functional level and an increase in the acquisition of knowledge and skill required of caregivers in order to provide care during rehabilitation. Clinical Rehabilitation Impact: The rehabilitation team should be aware of the fact that the perceived care burden may be greater due to the lack of knowledge concerning available resources and due to the inability to cope with stress effectively. and A. BACKGROUND: A few reports demonstrate the occurrence of egg allergy in adolescent and adult patients suffering from atopic dermatitis and the association of this allergy to other food and aeroallergens. AIMS AND OBJECTIVES: The aim of this study is to evaluate the occurrence of egg allergy in patients suffering from atopic dermatitis at the age 14 years and older and to evaluate the relationship between egg allergy or egg sensitisation and the sensitisation to dust, mites, feather, and animal dander. MATERIALS AND METHODS: Complete dermatological and allergological examination was performed. These parameters were examined: food allergy and food sensitisation to egg white and yolk, to mites, animal dander (mixture), feather and dust. The statistical evaluation of the relations among egg allergy, egg sensitisation and sensitisation to mites, animal dander (mixture), feather and dust was performed. Two hundred and eighty eight patients were included in the study (90 men, 198 women, with the average age 25.2). RESULTS AND CONCLUSION: Egg allergy was recorded in 5% and egg sensitisation in 20% of patients; sensitisation to dust is recorded more often in patients with positive results in sIgE for egg white and/or yolk. and J. Autor v článku popisuje šest případů emfyzematózní pyelonefritidy. Průměrný věk nemocných byl 63,5 roku, onemocnění se vyskytovalo častěji u mužů a ani v jednom případě nebyl příčinou konkrement. Dvě třetiny nemocných byli diabetici a nejčastěji byla prokázána jako etiologické agens Escherichia coli a v jednom případě Klebsiella pneumonie. Autor uvádí způsoby léčby jednotlivých případů onemocnění. Pouze v jednom případě vedla emfyzematózní pyelonefritida ke smrti pacienta., The author of this communication describes six cases of emphysematous pyelonephritis. The mean age of patients was 63.5, with the disease seen most frequently in men. None of the cases were associated with stone disease. Two thirds of patients had diabetes mellitus. Escherichia coli was confi rmed in the urine most frequently, and in one case, Klebsiella pneumoniae was identifi ed. The author presents the methods of treatment used in each case. INTRODUCTION: The splenium of the corpus callosum has a specific structure of blood supply with a tendency towards blood-brain barrier breakdown, intramyelinic edema, and damage due to hypoxia or toxins. Signs and symptoms of reversible syndrome of the splenium of the corpus callosum typically include disorientation, confusion, impaired consciousness, and epileptic seizures. CASE REPORT: A previously healthy 32-year-old man suffered from weakness, headache, and fever. Subsequently, he developed apathy, ataxia, and inability to walk, and therefore was admitted to the hospital. Cerebrospinal fluid showed protein elevation (0.9 g/l) and pleocytosis (232/1 ul). A brain MRI showed hyperintense lesions in the middle of the corpus callosum. The patient was treated with antibiotics, and subsequently, in combination with steroids. Two months later, the hyperintense lesions in the splenium and the basal ganglia had disappeared. Almost seven months since his hospitalization in the Department of Neurology, the patient has returned to his previous employment. He now does not exhibit any mental changes, an optic edema and urological problems have improved. In addition, he is now actively engaged in sports. CONCLUSION: We have described a case of a 32-year-old man with confusion, ataxia, and inability to stand and walk. The man developed a febrile meningeal syndrome and a hyperintense lesion of the splenium, which lasted for two months. Neurological changes, optic nerve edema, and urinary retention have resolved over the course of seven months. Typickým patologicko-anatomickým rysem emfyzému je permanentní rozšíření dýchacích cest distálně od terminálních bronchiolů spolu s destrukcí plicního parenchymu, bez výraznější fibrózy. Chirurgická léčba nabízí tři typy intervencí: bulektomii (resekci velkých bul utlačujících zdravější část plíce), volumredukční operaci plic (LVRS) a plicní transplantaci (jedno či lépe oboustrannou). Alternativou LVRS u inoperabilních nemocných mohou být některé bronchoskopické techniky - metody bronchoskopické volumredukce (BVR). Cílem našeho sdělení je ukázat roli radiologa při indikaci bronchoskopické volumredukce., Lung emphysema is characterized as permanently enlarged air space distal to terminal bronchioles accompanied by destruction of lung parenchyma without fibrosis. There are three types of surgical treatment: bullectomy (resection of large emphysematic bulls which oppreses healthy lung parenchyma), volumreduction and lung transplantation (once or double - faced). When surgery is not possible there are some bronchoscopic techniques - bronchoscopic volumereduction.
