Pro hemodialyzované pacienty, u kterých byl diagnostikován diferencovaný karcinom štítné žlázy, může mít zásadní význam léčba 131I. Diagnóza nádorového onemocnění tyto pacienty vyřazuje z registru čekatelů pro transplantaci ledviny do doby, než bude prokázána 3-5letá remise. Jelikož je 131I vylučován z těla převážně ledvinami, je efektivní poločas 131I u pacientů s renálním selháním prodloužen. To je důvod, proč by měla být redukována podávaná léčebná aktivita, aby došlo ke snížení radiační zátěže především pro krvetvornou kostní dřeň. Prezentujeme kazuistiku 48letého muže v terminálním renálním selhání, léčeného 131I pro papilární karcinom štítné žlázy. Terapeutické aktivity jsme snížili o 30 %. Hemodialýza byla prováděna po podání léčebné aktivity 131I na lůžkové části kliniky nukleární medicíny. 131I je dialyzovatelný a po každé dialýze klesla zbytková aktivita o 33-67 %. Radiační zátěž personálu hemodialyzačního centra byla minimální. Průběh hemokoagulace můžeme všeobecně rozdělit do následujících fází: iniciace, amplifikace, propagace a stabilizace trombu. Nejzásadnější změny dosud akceptovaného schématu hemokoagulace se týkají postavení a vůbec existence zevní a vnitřní větve koagulačního sytému, centrální úlohy tkáňového faktoru v iniciaci hemokoagulaci a nově chápané úlohy faktorů XI a XII. Nově byla i rozpoznána aktivní úloha monocytů, leukocytů, erytrocytů a endoteliálních buněk a jejich receptorů v hemokoagulaci, stejně jako úloha cirkulujících mikropartikulí sloužících k přenosu některých působků a informací. Změněné schéma hemokoagulace reflektují i nově se objevující možnosti antikoagulace s výhodnějším poměrem antikoagulačních vlastností k riziku krvácení a trombotické příhody. Dlouhodobě je stále více zřetelné, že jak vnitřní, tak zevní větev koagulačního systému je prvotně spouštěna uvolněním aktivního tkáňového faktoru, vznikem komplexu tkáňového faktoru s faktorem VIIa a následnou aktivací faktoru X na Xa. Současně je zpochybňována úloha faktoru XII v iniciaci tzv. vnitřního koagulačního systému, jeho role byla nově experimentálně prokázána společně s faktorem XI při propagaci a s faktorem XIII při konečném zpevňování trombu a při hledání nových antitrombotik by právě jeho blokáda mohla splňovat požadavek vysoké účinnosti s minimem krvácivých komplikací., The process of hemocoagulation can be generally divided into the following stages: initiation, amplification, propagation and stabilization of thrombus. The most important changes of the hemocoagulation scheme accepted so far concern the position and generally the existence of the coagulation system external and internal branches, the central role of a tissue factor in initiation of hemocoagulation and the newly understood roles of factors XI and XII. Newly an active role of monocytes, leukocytes, erythrocytes and endothelial cells and their receptors in hemocoagulation has been identified, as well as the role of circulating microparticles which transfer some agents and information. The changed scheme of hemocoagulation also reflects the emerging possibilities of anticoagulation with a more favourable proportion of anticoagulation properties to the risk of bleeding and thrombotic events. Over the long term it is increasingly evident that both the internal and external branches of the coagulation system are initially launched through release of an active tissue factor, formation of a complex of a tissue factor with factor VIIa and ensuing activation of factor X on Xa. At the same time the role of factor XII in the intrinsic coagulation system is questioned, its role has now been experimentally proven together with factor XI in propagation and with factor XIII in the final strengthening of thrombus and when seeking new antithrombotic drugs it might be just its blocking which could meet the requirement on high effectiveness with minimum bleeding complications., and Jan F. Úvod: Správné zhodnocení jaterních pseudolézí pomocí multidetektorového CT či MR je velmi důležité z důvodu odlišení těchto benigních změn od primárních či sekundárních malignit v jaterním parenchymu. Metoda: Autoři provedli systémový přehled literatury v databázi PubMed s vyhledáváním slov játra, pseudoléze v angličtině a češtině od roku 2000 do roku 2014. Výsledky: Autoři předkládají přehled literatury. Dále autoři retrospektivně zhodnotili soubor pacientů léčených pro jaterní onemocnění ve FN Ostrava, kdy se setkali s touto abnormalitou celkem v 7 případech. U 3 pacientů byla provedena diagnostická laparoskopie s vizuální kontrolou léze, s upřesněním diagnostiky pomocí peroperační ultrasonografie (IOUS) a s excizí části ložiska pro následné histologické vyšetření. Histologický nález u všech 3 pacientů odpovídal steatotickému postižení jater. Další 2 pacienti byli operováni pro jiná ložiska metastazujícího kolorektálního karcinomu a revize suspektní léze byla provedena během operačního výkonu otevřenou cestou. Kromě vizuální kontroly a peroperačního IOUS byla provedena excize k histologickému vyšetření, které prokázalo známky velkokapénkové i malokapénkové steatózy a nepřítomnost maligních změn. Poslední dva pacienti jsou dále sledováni v hepatologické poradně a jsou v rámci dispenzarizace kontrolováni v 6měsíčních intervalech. Závěr: Autoři prezentují vlastní zkušenosti získané v rámci mezioborové spolupráce na multidisciplinárních konferencích a připojují literární údaje o této neobvyklé problematice. Správným vyhodnocením nálezů této variety zejména u onkologických pacientů je možno zabránit zbytečným biopsiím, duplicitním vyšetřením a diagnostickým laparoskopiím či explorativním laparotomiím., Introduction: Accurate detection of hepatic pseudolesions using multi-detector CT and MRI examinations is crucial for the differentiation of benign alterations from primary and secondary malignant lesions in hepatic parenchyma. Method: The authors conducted a systematic literature review in PubMed. “Liver“ and “pseudolesion“ were used as keywords in English and Czech, and papers/articles published from 2000 to 2014 were retrieved. Results: The authors presented a literature review. In addition, the authors performed a retrospective evaluation of a group of patients treated for liver disease at University Hospital Ostrava where this anomaly was encountered in 7 cases. In 3 of the patients, diagnostic laparoscopy was done, with visual examination of the lesion accompanied by intraoperative ultrasound exam (IOUS) and partial excision, to establish the diagnosis. Subsequent histological assessment of the specimens confirmed the diagnosis of a steatotic lesion in each of these 3 patients. Additional 2 of the 7 patients underwent liver surgery for concurrent metastatic lesions of colorectal cancer and an open-access revision of the suspected lesions was performed. Visual inspection and intraoperative ultrasound (IOUS) was followed by excisional biopsy. The histology revealed macro- and micro-vesicular steatosis and excluded malignant changes. The last 2 patients still continue to be followed–up regularly on a 6-month routine check-up basis at our hepatology unit. Conclusion: The authors presented their own experience gained through inter-disciplinary cooperation at Multidisciplinary conferences. A literature overview of this unusual subject is also included. Hepatorenální syndrom je život ohrožující komplikace jaterních chorob. Jedná se o funkční postižení ledvin, které se vyskytuje jak u akutních (typ I), tak i chronických (typ II) onemocnění jater a je spojeno s vysokou mortalitou. Léčba má omezené možnosti, ale podávání vazopresorů (terlipresin), albuminu a portosystémový shunt dokážou prognózu zlepšit. Transplantace jater je léčbou nejefektivnější. Autoři prezentují aktuální data o této problematice včetně doporučení pro léčebnou praxi., Hepatorenal syndrome (HRS) is a life-treating complication of liver diseases. This functional kidney impairment is classified into acute (type I) and chronic (type II) types and is connected with high mortality. Treatment options are limited, but administration of vasoconstrictors (terlipressin), albumin and portosystemic shunt may improve their prognosis. Liver transplantation is the most effective method for these patients. Cíl studie: Metabolismus železa musí být velmi pečlivě regulován. Klíčovým systémovým regulátorem je peptidový hormon hepcidin, který způsobuje “uzamčení” železa uvnitř buňky; to pak nemůže být utilizováno. Dialyzovaní pacienti často trpí anémií, která je způsobena mnoha faktory, mj. stavem mikrozánětu a také zřejmě zvýšenou retencí hepcidinu v důsledku snížené glomerulární filtrace. Naším cílem bylo popsat vztahy mezi hepcidinem a dalšími parametry metabolismu železa, zánětu a erytropoezy u těchto pacientů. Typ studie: Observační Název a sídlo pracoviště: Ústav klinické biochemie a hematologie LF UK a FN v Plzni, Alej Svobody 80, 304 60 Plzeň Materiál a metody: Do studie bylo zařazeno 164 chronicky dialyzovaných pacientů (věk 66 ± 13, 25 - 92 let; 101 mužů a 63 žen) a 37 zdravých kontrol (věk 55 ± 20, 21 - 92 let; 16 mužů a 21 žen), u kterých byl stanoven jejich kompletní krevní obraz, parametry metabolismu železa, zánětu a výživy. Výsledky: Koncentrace železa, transferrinu a hemoglobinu byla významně nižší u dialyzovaných pacientů (p<0,0001), naopak ferritin (p<0,0001), solubilní transferrinové receptory(p<0,05), hepcidin (p=0,0003), CRP a IL-6 (p<0,0001) byly u těchto pacientů významně vyšší než u zdravých kontrol. Hepcidin slabě koreloval pozitivně s CRP a ferritinem a negativně s transferrinem u dialyzovaných pacientů, u zdravých kontrol nebyla nalezena žádná korelace hepcidinu s ostatními parametry. Závěr: Zdá se, že vliv zánětu na hladiny hepcidinu není u hemodialyzovaných pacientů zcela zásadní a na jeho koncentraci se podílí i jiné faktory, např. jeho retence., Objective: Peptide hormone hepcidin is a key systemic regulator of the iron metabolism. Hepcidin binds to the iron cell exporter ferroportin so iron is kept in the cells unavailable for erythropoiesis. The synthesis of hepcidin is up-regulated by high iron stores and inflammation. Dialyzed patients have very often impaired iron management – they suffer from anemia, which is caused by many factors including the state of micro inflammation and hepcidin retention due to decreased glomerular filtration rate. Our aim was to describe the relationship of hepcidin and other parameters of iron metabolism, erythropoiesis and inflammation. Design: Observation Settings: Institute of Clinical Biochemistry and Hematology, Charles University Hospital in Pilsen, Alej Svobody 80, 304 60 Plzeň Material and Methods: Complete blood cell count, iron, ferritin, transferrin, CRP, albumin, creatinine, hepcidin, soluble transferrin receptors (sTfR) and IL-6 were measured in samples from 164 patients included in chronic hemodialysis program (age 66 ± 13, 25 - 92 years), 63 women and 101 men and 37 control healthy volunteers (age 55 ± 20, 21 - 92 years), 21 women and 16 men. Results: Iron, transferrin and hemoglobin were significantly lower in the patients group (p<0.0001) while ferritin (p<0.0001), sTfR (p<0.05), hepcidin (p=0.0003), CRP and IL-6 (p<0.0001) were significantly higher in the patients group. Hepcidin weakly positively correlated with CRP and ferritin and weakly negatively correlated with transferrin in hemodialyzed patients. No correlation of hepcidin with other biochemical parameters in controls was found. Conclusion: It seems that the influence of inflammation on hepcidin levels in hemodialyzed patients is not crucial and other factors including its retention in end-stage renal disease participate., T. Vnitřní břišní hernie jsou méně častou chirurgickou diagnózou, mnohdy zjištěnou jako náhodný peroperační nález. Ileózní stavy na podkladě těchto hernií jsou pak raritními případy. V našem článku prezentujeme případ 69leté zdravé ženy, operované na základě chybně interpretovaného CT nálezu pro suspektní volvulus žaludku, kde pravou příčinou byla peroperačně zjištěná herniace pravé části kolon do bursy omentalis s dilatací céka a známkami nekrotizace stěny střevní., Internal abdominal hernias present an infrequent surgical diagnosis and are usually encountered accidentally during surgery. Heterotopická mezenteriálna osifikácia (HMO) je zriedkavé ochorenie podmieňujúce patologickú tvorbu kostného tkaniva v dutine brušnej po opakovaných operačných traumách. Autori prezentujú neobvyklý prípad vzniku mezenteriálnej osifikácie u 30ročného muža po operácii pre ileózny stav na podklade tenkočrevnej inkarcerácie. Po prepustení do domáceho liečenia musel byť znovu hospitalizovaný a reoperovaný. V priebehu 6 týždňov bol ešte 3krát reoperovaný pre tenkočrevný ileózny stav a výraznú produkciu z entero-kutánnej fistuly. Napriek intenzívnej parenterálnej hyperalimentácii sa stav pacienta nezlepšil, metabolický rozvrat sa stal pacientovi osudovým. Patofyziológia mechanizmu formovania kostného tkaniva v mäkkých tkanivách nie je známa. The authors present an unusual case of mesenteric ossification formation in a 30 year-old man after surgery due to status ileosus on the basis of intestinal incarceration. After being released into home treatment the patient had to be hospitalized again and reoperated. During the next six weeks, the patient was reoperated three more times due to intestinal status ileosus and significant leaking of the entero-cutaneous fistula. Despite intensive parenteral hyperalimentation the patient‘s condition did not improve and metabolic breakdown became fatal for the patient. Pathophysiology of the mechanism of bone tissue formation in soft tissues is unknown. BACKGROUND: In the cholesteatoma surgery ossicles can be replaced to reconstruct middle ear function. It is important that these ossicles are free of squamous epithelium, to prevent residual disease. This study focuses on the histological findings of the malleus and incus harvested during cholesteatoma surgery. MATERIALS AND METHODS: Eighty middle ears ossicles were examined in vivo and histologically to consider the relationship of cholesteatoma to ossicles, grade of bone destruction and invasion of cholesteatoma to deeper layers of bone. RESULTS: Serious ossicular destruction was observed more frequently in incus compared to malleus (p=0.0065). Difference of ossicles destruction between children and adults was not significant (p=0.3032). Deep invasion of cholesteatoma into the vascular spaces or inner core of the bone was not observed. CONCLUSIONS: Autograft ossicles from cholesteatomatous ears should not necessarily be rejected for reconstruction of the ossicular chain. V léčbě uveálních melanomů se v některých případech používá radiochirurgie CyberKnifem. Histologickými a imunohistologickými změnami v nádorové tkáni po terapii se zabývá poměrně málo autorů. V tomto sdělení uvádíme výsledky vyšetření dvou takových případů. U obou pacientů byla provedena enukleace bulbu pro bolestivý glaukom. U prvého pacienta šlo o recidivující melanom, který byl ošetřen gama nožem před deseti lety. Pro lehkou progresi ložiska melanomu bylo provedeno ošetření CyberKnifem celkovými dávkami 45Gy ve 3 frakcích. Po necelých pěti měsících po ozáření bulbu došlo k regresi recidivujícího nádoru z původní prominence 1,4 mm v ploše 6 mm × 0,6 mm na prominenci 0,3 mm v ploše 3 mm × 0,3 mm. Histologicky byla zaznamenána rezidua řídce celulárního ložiska regresivně změněných nádorových buněk a přítomnost reaktivních změn optického nervu. Druhý pacient byl měsíc po radioterapii CyberKnifem s následnou komplikovanou operací katarakty a s enukleací bulbu bez regrese – tedy zániku nádorových buněk. Velikost tumoru 11,1 mm × 15mm s prominencí 9,3mm přetrvávala i po ozáření dávkou 40Gy/1Fr. V histologickém vyšetření byly nádorové buňky bez patologických změn. V imunohistochemickém vyšetření jsme pozorovali u prvého pacienta vysokou difuzní expresi proteinu bcl-2 až do výše 75 % a méně než 1% pozitivitu ve vyšetření s proliferačním markerem Ki-67. U druhého případu byla situace odlišná, s výraznou až 75% pozitivní expresí bcl-2 proteinu v nádorových buňkách v povrchových vrstvách tumoru a s klesajícím počtem pozitivní exprese v hlubokých vrstvách nádoru, kde dosahovala maxima do 25 %. Proliferační aktivita s pozitivní jadernou expresí v Ki-67 vzrůstala od povrchu, kde se pohybovala v rozmezí 1–3 % do hloubky nádoru a k jeho bázi, kde dosahovala až 5 %. Naše pozorování ukazuje na výraznou redistribuci genetické aktivity uvnitř nádoru. Ta se projevuje „overexpresí“ bcl-2 proteinu a u druhého případu je patrná rozdílná pozitivita bcl-2 proteinu v různých vrstvách nádoru. Vztah tohoto fenomenu k radioterapii není dosud jasný., Radiosurgery with CyberKnife is used in the treatment of uveal melanomas in some cases. Histological and immunohistological changes in tumour tissue following treatment have been addressed by relatively few authors. This paper presents the results of investigations of two such cases. Enucleation of the eye was performed for painful glaucoma in both patients. Patient 1 had a recurrent melanoma treated with a Gamma Knife ten years ago. Due to slight progression in the melanoma site, treatment with CyberKnife was used with total doses of 45 Gy in 3 fractions. Some five months following eye irradiation, there was regression of the recurrent tumour from the original height of 1.4 mm in an area of 6 mm x 0.6 mm to that of 0.3 mm in an area of 3 mm x 0.3 mm. Histologically, residua of a sparsely cellular locus of regressively altered tumour cells and the presence of reactive changes in the optic nerve were noted. Patient 2 underwent radiotherapy with CyberKnife one month prior, with subsequent complicated cataract surgery and enucleation of the eye without regression, i.e. destruction of tumour cells. The size of the tumour of 11.1 mm x 15 mm with a height of 9.3 mm remained unchanged even after irradiation with a dose of 40 Gy/1 fr. No pathological changes in the tumour cells were seen on histological examination. In patient 1, immunohistochemistry showed high diffusion expression of the bcl-2 protein of up to 75 % and less than 1 % positivity on examination with the proliferation marker Ki-67. In patient 2, the situation was different, with significant, as high as 75 % positive expression of the bcl-2 protein in tumour cells in the superficial tumour layers and a decreasing rate of positive expression in the deep tumour layers where it reached no more than 25 %. The proliferation activity with positive nuclear expression of Ki-67 increased from the surface, where it ranged from 1 % to 3 %, to the depth of the tumour and toward its base, where it reached up to 5 %. Our observation suggests significant redistribution of genetic activity within the tumour. It is manifested by overexpression of the bcl-2 protein and, in patient 2, different bcl-2 protein positivity in the various tumour layers is apparent. Palec je pro funkci ruky nejdůležitějším prstem na ruce. Svou úlohu má nejen z pohledu funkčního, ale také z estetického hlediska zejména proto, že ruka je velmi exponovanou částí těla a ztráta palce může vést k celoživotní fyzické i psychické stigmatizaci. Vzhledem k těmto zkušenostem a snaze navrátit pacienty do běžného způsobu života v co nejkratší době jsou dnes jednoznačně preferovány časné rekonstrukce palce. Při včasné rekonstrukci palce pacient nezíská chybné stereotypy v úchopových manévrech ruky. Práce je koncipovaná jako přehled současných možností nemikrochirurgických a mikrochirurgických postupů při rekonstrukci palce. Its role is not only functional, but also aesthetic, the hand being a very exposed part of the body. Loss of the thumb can lead to life-long physical and mental stigma. With regard to this experience, early reconstruction of the thumb in order to return patients to normal life as soon as possible is clearly preferred. When using early thumb reconstruction, the patient does not gain wrong stereotypes in hand-grip manoeuvres.
