Nadhmotnosť a obezita u detí v školskom veku a adolescencii predstavujú v súčasnosti celosvetový zdravotný problém pútajúci pozornosť nielen odbornej, ale aj laickej verejnosti. Nárast nadhmotnosti a obezity u školskej mládeže je daný nesprávnymi stravovacími návykmi a tiež zníženou pohybovou aktivitou. Projekt Health Behaviour in School-Aged Children (HBSC) je jedna z prvých medzinárodných prierezových štúdií zameraných na zdravie adolescentov a ich správanie súvisiace so zdravím. Cieľom príspevku je prezentovať výskyt nadhmotnosti a obezity u detí v školskom veku a adolescencii v oblasti stredného Slovenska. Respondenti udali výšku a hmotnosť a na základe týchto údajov bol vypočítaný body mass index (BMI). Zber údajov bol realizovaný na 25 základných školách a dvoch osemročných gymnáziách v Žilinskom a Banskobystrickom kraji v období máj až jún 2010. Súbor respondentov tvorilo 1187 žiakov v troch vekových kategóriách: 11-ročných (313 respondentov), 13-ročných (442 respondentov) a 15-ročných (432 respondentov). Na zber údajov bol použitý štandardizovaný dotazník vytvorený pre potreby HBSC štúdie. Zber údajov obsahoval aj demografické údaje: pohlavie, ročník, vek. Signifikantná interakcia dvoch premenných bola hodnotená pomocou Chí-kvadrát testu. Výskyt nadhmotnosti a obezity u školákov v oblasti stredného Slovenska nepresiahol jednu desatinu u dievčat. S vekom klesá výskyt nadváhy a obezity u dievčat (z 8 % u 11-ročných na 6 % u 15-ročných) u chlapcov (z 19 % u 11-ročných na 9 % u 15-ročných). U 13-ročných chlapcov sme zistili štatisticky významne vyšší výskyt obezity (p = 0,003). V skupine 11 a 15-ročných respondentov neboli tieto rozdiely štatisticky signifikantné (11-roční: p = 0,011; 15-roční: p = 0,284). Počet detí v školskom veku a adolescencii s nadhmotnosťou a obezitou na Slovensku, ale aj vo svete neustále stúpa a prináša mnoho zdravotných a psychosociálnych dôsledkov. Dodržiavanie zásad správnych stravovacích návykov a dostatok pohybovej aktivity u školskej mládeže má mimoriadny význam v prevencii vzniku obezity., Overweight and obesity among school-age and adolescent children currently pose a worldwide health problem drawing attention not only of the professional but also general public. The Health Behaviour in School-Aged Children (HBSC) project is one of the first international cross-sectional studies focused on teenagers' health and their health-related behaviour. The aim of this article is to demonstrate the occurrence of overweight and obesity in school-age children and adolescents in Central Slovakia. The body mass index (BMI) of respondents was calculated on the basis of given height and weight. Data collection was performed in 25 primary schools and two eight-year grammar schools in the Žilina and Banská Bystrica regions from May to June 2010. The group of respondents consisted of 1187 students in three age categories, 11-year-olds (313 respondents), 13-year-olds (442 respondents) and 15-year-olds (432 respondents). A standardized questionnaire developed for the needs of the HBSC study. Data collection included demographic data: gender, school class and age. Significant interaction of two variables was assessed by using the chi-square test. The occurrence of overweight and obesity among schoolchildren in Central Slovakia did not exceed one tenth amongst schoolgirls. With higher age, the occurrence of overweight and obesity among girls decreases from 8% among 11-year-olds to 6% in 15-year-olds, and from 19% among 11-year-olds to 9% among 15-year-olds in the group of boys. Among 13-year-old boys we found a significantly higher prevalence of obesity (p = 0.003). In the group of 11 and 15-year-olds these differences were not statistically significant (11-year-old: p = 0.011, 15-year-old: p = 0.284). The number of school age children and adolescents with overweight and obesity in Slovakia, but also worldwide, is constantly increasing and brings many health and psychosocial consequences. U primárně neresekabilních jaterních metastáz může neoadjuvantní konverzní systémová léčba při použití moderních režimů chemoterapie v kombinaci s cílenou biologickou léčbou umožnit radikální chirurgickou resekci metastatických ložisek na játrech. Optimální konverzní režim by měl být co nejméně toxický. Tato kritéria splňuje monoklonální protilátka cetuximab. Následující klinická kazuistika demonstruje využití cetuximabu v konverzní léčbě jaterních metastáz, která vedla k jejich radikální resekci., In primarily unresectable liver metastases, neoadjuvant conversion systemic therapy with the use of modern chemotherapy regimens in combination with targeted biological therapy may allow for a radical surgical resection of metastatic foci in the liver. Cíle studie: 1. Seznámení biochemické obce s využitím mikrodialýzy v neurochirurgii, resp. v neurointenzivní péči. 2. Před - stavení hypotézy o kreatinfosfátu coby alternativním zdroji energie pro mozkovou tkáň. Typ studie : Přehledná práce Materiál a metody: Sběr vzorků extracelulární tekutiny umožňuje ve standardním protokolu vyšetřit metabolity glukózové - ho metabolismu s cílem včasného záchytu rozvíjející se ischemie mozku. Mikrodialýzu využíváme převážně u pacientů po kraniocerebrálním poranění či subarachnoidálním krvácení z prasklé mozkové výdutě. Nicméně v odebraných vzorcích je možno vyšetřit rozsáhlou škálu dalších metabolitů a biologicky aktivních látek a rozšířit tak naše poznání o procesech pro - bíhajících v mozku. Výsledky: Shrnujeme poznatky o mikrodialýze v neurointenzivní péči neurochirurgických pacientů, možnosti vlivu na aktuální terapii stejně jako význam prediktivní. V naší experimentální práci se zaměřujeme na metabolismus kreatinfosfátu, který dle našich prvních pozorování může být klíčovou energetickou alternativou kyslíkového metabolismu, jak demonstrujeme na dvou kazuistikách. Závěr: Mikrodialýza nabízí možnost přímé, průběžné monitorace metabolismu mozku v běžné klinické praxi. Standardní diagnostiku lze rozšířit o spektrum dalších metabolitů a biologicky aktivních látek. Nonetheless, the samples may be examined for a wide scale of metabolites and biological active substances and extend our knowledge of brain metabolism. Results: We summarize the current knowledge on the use of microdialysis in neurointensive care of neurosurgical patients, its influence on the therapy and its predictive meaning. In the experimental work we focused on the metabolism of creatine phosphate, which, according to our first observations, can represent alternative energy to glucose oxidative metabolism, as demonstrated on 2 case reports. Conclusions: Microdialysis provides direct and continuous online monitoring of brain metabolism in everyday clinical prac - tice. This cross sectional study focused on how Postural Stability (PS) indicators: body sway deviation (BSD) and body sway velocity (BSV), change with age and their association with levels of social and physical activity. Observational study: 80 older adults (aged: 60-96) were purposefully recruited from two sources: the University of the Third Age (TAU) (n = 35) and a residential care home (CH) (n = 45). Differences in the indicators of PS, approximated through Centre of Pressure (COP) measurements, were assessed by the Romberg Stance Test (Test A) subsequently repeated on 10 cm foam surface (Test B), using a Kistler Dynamometric Platform. The RCH Group was older, had higher BMI and was less socially and physically active, showed more body sway in all indicators compared to TAU group. For all participants body sway velocity (BSV) was significantly correlated with age. The strength of correlation of body sway deviation (BSD) with age was also significant but not as strong. The findings indicate in line with previous studies that deterioration in BSV is associated with poor PS more than deterioration in BSD. and J. Jančová Všetečková, N. Drey
