Feochromocytom se může projevit různými příznaky, symptomy na podkladě setrvalého nebo paroxyzmálního uvolňování katecholaminů. Arteriální hypertenze může být trvalá nebo záchvatovitá a palpitace jsou většinou projevem sinusové tachykardie. V některých případech, dokonce jako první projev feochromocytomu, se mohou vyskytnout závažné kardiovaskulární komplikace, jako je hypertenzní emergentní situace, ischemie myokardu, kardiomyopatie a srdeční selhání, multisystémová krize nebo šok. Vylučování katecholaminů může být doprovázeno i arytmiemi – tachykardiemi (supraventrikulárními a komorovými) a méně často i bradykardiemi (AV blokem nebo junkčními). Incidence karcinomu endometria ve věkové skupině žen mladších 40 let narůstá, a proto roste i zájem o fertilitu šetřící léčbu. Nejčastěji používaná konzervativní léčba progestiny může být zvážena u pečlivě selektovaných pacientek s grade 1, stage IA karcinomem endometria bez myometriální invaze, které si přejí zachovat reprodukční funkce, a jsou ochotny podstoupit intenzivní léčbu a sledování a také určité riziko chirurgicky nestážovaného onemocnění. Před léčbou je potřeba co nejpřesněji zhodnotit biologický charakter a rozsah onemocnění. Hormonální terapie je aplikována obvykle 6 měsíců. Response rate se pohybuje přibližně v hodnotách 60–80 %, recurrence rate 25–40 %. Live birth rate je uváděn okolo 30 %. Je potřeba informovat pacientky o reálné možnosti recidivy, obvykle dobře léčitelné, i o výhodě použít metody asistované reprodukce, o potřebě intenzivního follow-up a doporučení definitivní léčby po dokončení reprodukčních plánů., The diagnosis of endometrial carcinoma in young women of childbearing age is rare, but the incidence in growing. Fertility-preserving therapy of endometrial carcinoma could be considered in patients with a histological diagnosis of grade 1 stage IA endometrial carcinoma without myometrial invasion who wish to preserve their fertility, are willing to accept intensive treatment, close follow-up and risk of unstaged disease. Purpose: The aim of this phase III trial was to compare the efficacy and safety of bevacizumab alone with those of bevacizumab and capecitabine plus oxaliplatin (XELOX) as maintenance treatment following induction chemotherapy with XELOX plus bevacizumab in the first-line treatment of patients with metastatic colorectal cancer (mCRC). Patients and Methods: Patients were randomly assigned to receive six cycles of bevacizumab, capecitabine, and oxaliplatin every 3 weeks followed by XELOX plus bevacizumab or bevacizumab alone until progression. The primary endpoint was the progression-free survival (PFS) interval; secondary endpoints were the overall survival (OS) time, objective response rate (RR), time to response, duration of response, and safety. Results: The intent-to-treat population comprised 480 patients (XELOX plus bevacizumab, n = 239; bevacizumab, n = 241); there were no significant differences in baseline characteristics. The median follow-up was 29.0 months (range, 0–53.2 months). There were no statistically significant differences in the median PFS or OS times or in the RR between the two arms. The most common grade 3 or 4 toxicities in the XELOX plus bevacizumab versus bevacizumab arms were diarrhea, hand–foot syndrome, and neuropathy. Conclusion: Although the noninferiority of bevacizumab versus XELOX plus bevacizumab cannot be confirmed, we can reliably exclude a median PFS detriment >3 weeks. V uplynulých dvou desetiletích byl zaznamenán trend provádět u pacientek podstupujících radikální cystektomii kontinentní derivaci moči. Povzbudivé výsledky derivace moči s užitím ortotopické neoveziky (ON) u mužů a výsledky prvních studií prokazující onkologickou a funkční bezpečnost ON u žen byly impulzem pro zavedení této techniky i u žen. V uplynulých 20 letech byla uveřejněna celá řada článků popisujících výsledky derivace moči prostřednictvím ON u žen. Autoři tohoto minipřehledového článku hodnotí výsledky dostupných publikací, které se zabývají problematikou a dopadem této techniky na kvalitu života pacientů. Výsledky studií ukazují nižší incidenci lokální recidivy v močové trubici u žen než u mužů podstupujících stejný výkon. Plně kontinentních bylo 57–100 % pacientek během dne a 27–92 % pacientek i během noci. Výskyt chronické močové retence a potřeba provádět čistou intermitentní katetrizaci byly mnohem častější u žen. Porucha sexuální funkce byla méně častá u pacientek s ON než u pacientek podstupujících jiné formy derivace moči. Chronic urinary retention and the use of clean intermittent catheterization were much higher in female patients. Sexual function was less affected in women after ONB than those with other forms of urinary diversion. Funkční hypotalamická amenorea (FHA) je vedle gravidity a syndromu polycystických ovarií nejčastější formou sekundární amenorey. FHA je důsledkem poruch v pulzní sekreci gonadoliberinu (GnRH), které pak vedou k poruše sekrece gonadotropinů FSH a LH. FHA je obranou organizmu v situacích ohrožujících životní funkce, které jsou chráněny pak přednostně před funkcí rozmnožovací. FHA je reverzibilní, po ukončení extrémní zátěže se zvolna sama může upravit. Uvádějí se 3 hlavní typy funkční amenorey: FHA se vztahem k úbytku hmotnosti, FHA v důsledku stresu a se vztahem k nadměrnému fyzickému výkonu. Konečným výsledkem jsou komplexní hormonální změny charakterizované hlubokou hypoestrogenemií. Vedle nízkých estrogenů tyto pacientky mají mírnou hyperkortizolemii, nízké hladiny inzulinu v séru, nízký IGF1 a nízký celkový trijodtyronin. Zdraví těchto žen je ohroženo ve více směrech, včetně stavu kostí, kardiovaskulárního systému a stavu mentálních funkcí. Pacientky mají sníženou kostní denzitu a jsou pak ohroženy zvýšeným rizikem fraktur. Osteopenie a osteoporóza jsou hlavní dlouhodobou komplikací FHA. Kardiovaskulární komplikace spočívají v dysfunkci endotelu a změnách v lipidogramu. FHA results from the aberrations in pulsatile gonadotropin-releasing hormone (GnRH) secretion, which in turn causes impairment of the gonadotropins (follicle-stimulating hormone and luteinizing hormone). FHA is a form of the defence of organism in situations where life functions are more important than reproductive function. FHA is reversible; it can be normalized after ceasing the stress situation. There are three types of FHA: weight loss related, stress-related, and exercise-related amenorrhea. The final consequences are complex hormonal changes manifested by profound hypoestrogenism. Additionally, these patients present mild hypercortisolemia, low serum insulin levels, low insulin-like growth factor 1 (IGF-1) and low total triiodothyronine. Women’s health in this disorder is disturbed in several aspects including the skeletal system, cardiovascular system, and mental problems. Patients manifest a decrease in bone mass density, which is related to an increase in fracture risk. Therefore, osteopenia and osteoporosis are the main long-term complications of FHA. Hlavními sledovanými ukazateli kvality po resekci rekta pro karcinom jsou operační radikalita, pooperační komplikace, počet recidiv a přežití. Poruchy funkce rekta, které postihují 25–50 % nemocných, zůstávají v pozadí. Tyto poruchy zahrnují vysokou frekvenci stolice, urgenci, evakuační poruchy, inkontinenci a sexuální a urologické dysfunkce. Léčba nádorového onemocnění prsu patří nejen do rukou chirurgů a onkologů, ale také do rukou rehabilitačních lékařů a fyzioterapeutů, kteří mají nezastupitelné místo v následné léčbě. Komplexním cílem léčby je zajistit co nejrychlejší uzdravení, a to včasným předcházením všem možným komplikacím, tzn. i těm, které vznikají v pohybovém aparátu. Rehabilitace po operaci prsu má hospitalizační a posthospitalizační fázi. Fáze hospitalizační rehabilitace je zaměřena především na prevenci pooperačních komplikací – dechových, tromboembolických. Posthospitalizační fáze má několik cílů: předcházet vzniku sekundárních reflexních změn v pohybovém aparátu, edukovat pacientky, jaká režimová opatření jsou nutná k prevenci vzniku a rozvoje sekundárního lymfedému, snažit se o včasný záchyt prvních známek lymfedému a časné zahájení léčby, pokud vznikl. Nejúčinnější léčbou lymfedému je komplexní dekongestivní terapie doplněná dlouhodobým podáváním léků pro systémovou enzymoterapii. Intervence v oblasti rehabilitační a fyzikální medicíny může být u pacientek po operaci prsu velmi prospěšná, protože umožní předejít příznakům, které snižují kvalitu života (pohybová omezení, ztráta soběstačnosti v aktivitách denního života apod), a následně vedou u onkologických pacientek ke vzniku deprese a s ní souvisejícímu poklesu imunity., The treatment of breast cancer is not only in the hands of surgeons and oncologists, but it also involves the irreplaceable follow-up care of rehabilitation doctors and physiotherapists. The comprehensive goal of the treatment is to achieve recovery in the shortest possible time, specifically through early prevention of all kinds of complications, including those developing in the locomotive organs. The rehabilitation following breast surgery consists of hospitalization and post-hospitalization stages. The stage of hospitalization rehabilitation is primarily focused on the prevention of postoperative complications – breathing, thromboembolic. The post-hospitalization stage is mainly aimed at the prevention of development of secondary reflexive changes in locomotive organs and the prevention of secondary lymphedema. The most efficient prevention to prevent lymphedema from developing is the complete decongestive therapy complemented with systemic enzyme therapy. Inkontinence moči patří k nejčastějším zdravotním problémům zejména ženské populace, kterým trpí v celosvětovém měřítku stamilióny lidí. Zejména v posledních dvou desetiletích jsme svědky rostoucího zájmu o tuto problematiku jak ze strany odborné, tak i laické veřejnosti. Nejčastějšími formami inkontinence, se kterými se v praxi setkáváme, jsou stresová a urgentní inkontinence. Fyzioterapie hrála v léčbě inkontinence vždy spíše okrajovou úlohu a tradičně byla indikována pouze v léčbě stresové formy. Práce předkládá nový pohled na roli fyzioterapie v léčbě močové inkontinence. Díky pokrokům v posledních letech je dnes na správně indikovanou a kvalifikovaně prováděnou fyzioterapii pohlíženo jako na plnohodnotnou a široce použitelnou metodu léčby jak stresové, tak urgentní inkontinence a hyperaktivního měchýře (symptomový komplex zahrnující závažné urgence, zpravidla provázené frekvencemi a někdy též urgentní inkontinencí)., Urinary incontinence is one of the most common health problems especially in the female population. Endometrióza znamená přítomnost tkáně endometria mimo dutinu děložní. Hlubokou infiltrující endometriózu (deep infiltrating endometriosis – DIE) představují rektovaginální léze, infiltrující onemocnění střev, močového měchýře, močovodu a DIE se vzácně objevuje i v dalších lokalizacích. Negynekologické komplikace DIE rozdělujeme do dvou hlavních kategorií: samotné onemocnění zahrnuje pánevní a břišní bolest, střevní striktury, opakující se krvácení z konečníku, dyschezii, chronickou anémii, dysurii, hematurii, stenózu ureterů, ztrátu ledvinných funkcí a komplikace léčby, ty jsou charakterizovány vznikem fistul, dysfunkcí močového měchýře a opakujícími se záněty urinárního traktu. DIE přináší riziko pro vznik malignit zasažených orgánů. DIE může být léčena konzervativně, nebo chirurgicky. Předkládaná práce je přehledem gastrointestinálních a urologických komplikací DIE a předkládá také kazuistiku pacientky centra pro léčbu endometriózy v nemocnici ve Znojmě (ČR)., Endometriosis is the presence of endometrial tissue outside the endometrial cavity. Deep infiltrating endometriosis (DIE) includes rectovaginal lesions as well as infiltrative disease to the bowel, urinary bladder, ureter and other rare locations. Non gynecological complications of DIE can be divided into two major categories: complications of the disease itself which include pelvic and abdominal pain, bowel stricture, cyclic rectal bleeding, dyschezia, chronic anemia, dysuria, hematuria, ureteral stenosis as well as loss of kidney function and complications of the treatment which include fistula formation, bladder void dysfunction and recurrent urinary tract infections. DIE has potential for malignant transformation in the affected organ. DIE may be managed conservatively or surgically. However definitive treatment is surgical.
