Zlomeniny lopatky představují otevřenou kapitolu traumatologie pohybového aparátu. Jejich pochopení není možné bez základních anatomických a klinických znalostí. Značná část zlomenin lopatky je sdružena s dalšími závažnými poraněními, především hrudníku. Zásadní pro diagnostiku a léčebný postup je rtg vyšetření, především obě Neerovy projekce a 3D CT rekonstrukce. Dosud používané klasifikace je třeba revidovat, neboť nereflektují reálné typy zlomenin. U dislokovaných zlomenin je nutno zvážit operační léčbu. Ta není urgentní a lze ji provést až do tří týdnů po primárním úrazu. Vzhledem k tomu, že jde o závažný, ale řídký typ poranění, je lépe tyto zlomeniny koncentrovat na specializovaná pracoviště., Scapular fractures are still a challenge in traumatology of the musculoskeletal apparatus. Their proper understanding is impossible without fundamental anatomical and clinical knowledge. A considerable part of scapular fractures is associated with other severe injuries, particularly to the chest. Essential for diagnosis and treatment of these fractures is radiographic examination, primarily both Neer projections and 3D CT reconstructions. The classifications used so far should be revised as they do not reflect real types of these fractures. Operative treatment should be considered in displaced scapular fractures. Such treatment is not urgent as these fractures may be operated on within up to three weeks of the primary injury. Evropa je regionem s nejvyšším výskytem neinfekčních onemocnění hromadného výskytu. Kardiovaskulární onemocnění patří mezi hlavní příčiny nemocnosti a úmrtnosti. Interakce mezi skladbou stravy, životním stylem a metabolizmem lipidů významně ovlivňují rozvoj aterosklerózy a jejích komplikací. Mortalita na ischemickou chorobu srdeční (ICHS) se v poslední dekádě 20. století výrazně snížila. Významné a pozitivní změny ve stravovacích návycích, zejména snížení příjmu nasycených mastných kyselin a jejich záměna za polynenasycené, přispěly ke snížení sérové hladiny cholesterolu v populaci. Pokles mortality na ICHS významně souvisel s vývojem tohoto rizikového faktoru, který nelze vysvětlit rozvojem farmakoterapie. Česká republika není jedinou zemí, v níž byly podobné změny stravovacích návyků ve vztahu k úmrtnosti na ICHS zaznamenány. Zlepšení stravovacích návyků se zdá být jednou z nejdůležitějších strategií v prevenci kardiovaskulárních onemocnění., Europe is a region, which is most severely affected by non-communicable diseases. Cardiovascular disease is the leading cause of disability and death. Complex interactions between diet, lifestyle, and lipoprotein metabolism significantly contribute to the development of atherosclerosis and its complications. Role of diet in prevention of coronary heart disease becomes sometime underestimated in comparison with pharmacological treatment. Coronary heart disease (CHD) mortality has declined substantially in the Czech Republic in the last decade of the 20th century. Significant and positive dietary changes, mainly reduction of saturated fatty acids intake and its replacement by polyunsaturated fatty acids, contributed to decline of the average serum cholesterol level in population. Fall in CHD mortality was attributable to reduction in this major cardiovascular risk factor and not driven by pharmacological intervention. The Czech Republic is not the only country where similar trends have been recorded. Jednou ze závažných oblastí projevů diabetes mellitus (DM) je muskuloskeletální systém. Tyto projevy významně zhoršují kvalitu života pacientů, proto jsou předmětem intenzivní diagnostiky a léčby. Významné místo v diagnostickém algoritmu zaujímají zobrazovací metody, jak RTG snímek, tak výpočetní tomografie (CT) a magnetická rezonance (MRI), v malé míře i ultrasonografie (USG). Článek podává přehled muskuloskeletálních (MSK) projevů a komplikací DM rozčleněných na přímé projevy DM, důsledky metabolických změn DM, přidružené syndromy se zvýšenou incidencí u DM a zánětlivé komplikace. Článek předkládá zobrazení časných změn, pokročilých změn, na sérii RTG snímků zachycuje vývoj patologických změn. Patofyziologie vzniku daných změn je zmíněna spíše okrajově, protože není náplní radiodiagnostiky. Závěrem je konstatováno, že na RTG pracovištích sloužících interním klinikám se jedná o častou indikaci zobrazení, že každou ze zobrazovacích metod je třeba využívat cíleně a že v mnoha ohledech nález ovlivňuje další léčbu pacienta, avšak že v některých diagnostických otázkách, zejména zánětlivých komplikací diabetické nohy, je stále ještě jednoznačná a spolehlivá interpretace zobrazeného nálezu problematická., The musculoskeletal system is one of the major regions, where changes caused by diabetes mellitus (DM) are often encountered causing severe impairment of the quality of patients´ lives. These changes have therefore become an important focus of diagnostic and therapeutic procedures, with imaging methods – both plain radiography, computed tomography (CT), magnetic resonance imaging (MRI) and to a lesser extent ultrasound (US), being their cornerstones. In the article the images of musculoskeletal (MSK) manifestations of diabetes mellitus are presented, structured into changes directly caused by DM, metabolic consequences of DM, syndromes with increased coincidence with DM and septic complications. The CT, MRI and radiographic images of both initial and extensive changes are being displayed as well as evolution of the changes in a radiographic series. The concerning pathophysiologic remarks are only basic, as they are not the primary focus of radiodiagnostics. In conclusion it is stated, that imaging of the MSK system in diabetic patients is a large issue in radiologic units serving internal medicine departments, that the imaging methods should be applied specifically and that the radiologic findings influence further management of the patients in many respects, but also, that in some diagnostic questions, namely concerning septic complications of the diabetic foot, a fully reliable and unequivocal interpretation is not always possible., and Jindra Brtková
