Úvod, cíle: Poruchy ejakulace jsou po míšním poranění velmi časté. U nemocných s přáním vlastního potomka je většinou nutný asistovaný odběr spermií. Penilní vibrostimulace (PVS) je neinvazivní metoda k získání ejakulátu u pacientů s lézí nad úrovní T10. Cílem práce je zhodnocení efektivity a bezpečnosti PVS, vyhodnocení kvality a dalšího využití ejakulátu. Metodika: Od 2010 do 2014 jsme provedli PVS u 20 pacientů. Poranění krční (C) míchy bylo u 13 pacientů, hrudní (T) míchy u sedmi pacientů. Průměrný věk byl 30,8 let. Průměrná doba od úrazu do PVS byla 64 měsíců. První PVS mělo sedm nemocných déle než 3,5 roku a 13 do 3,5 roku od úrazu. K PVS jsme užili přístroj FERTI CARE Multicept. Výsledky: Ejakulace jsme dosáhli u 11 pacientů (55 %), z toho devět mělo poranění C míchy a dva T míchy. U nemocných se začátkem PVS do 3,5 roku od poranění byla úspěšnost 77 % a nad 3,5 roku 14 %, p = 0,027. Při srovnání ejakulátu z druhé PVS došlo ke zvýšení celkového počtu spermií (p = 0,03), koncentrace spermií (p = 0,025) a počtu progresivně pohyblivých spermií (p = 0,027) oproti první PVS. Získaný ejakulát je v 72 % pouze kryoprezervován k pozdějšímu využití. U dvou pacientů byl ejakulát použit k fertilizaci. Jeden nemocný měl azoospermii. Autonomní dysreflexie se vyskytla v průběhu PVS u sedmi pacientů, příznaky odezněly do 3 min. Závěr: PVS je bezpečná metoda asistovaného odběru ejakulátu. Při opakování PVS jsme zjistili zlepšení kvality ejakulátu. Obtained ejaculate is mostly (72%) cryopreserved only for further use. The ejaculate was used for fertilisation in two patients. One patient had azoospermia. Autonomic dysreflexia during PVS occurred in seven patients, symptoms resolved within 3 mins. Conclusion: PVS is a safe method for sperm retrieval. Semen quality improved with repeated PVS. The ejaculate was successfully used for fertilisation in two patients. Příspěvek je věnován problematice bezdomovců a žebráků. Podává základní informace o této sociální kategorii lidí zjevně chudých, žijících na okraji majoritní společnosti a vyznačujících se svým specifickým způsobem života. Jen dobře informovaný člověk je schopný pochopit složitou problematiku vzájemné tolerance a rovnosti všech lidí bez biologických a kulturních rozdílů (projevy rasismu, náboženské nesnášenlivosti, xenofobie a intolerance), včetně nezbytné péče o zdravotně postižené a sociálně znevýhodněné skupiny obyvatelstva. Povinností každého jedince, který chce žít v humánní společnosti, je snažit se svým způsobem a možností pomáhat znevýhodněným spoluobčanům v jejich životě, k možnosti navrácení do stylu života majoritní společnosti. Nutností je i šíření empatie k těmto skupinám lidí, kteří se ne vlastní vinou často dostanou do těžké životní situace. Cíl práce: Upozornit na problematiku apendicitidy v graviditě. Typ práce: kazuistika. Vlastní pozorování: Formou kazuistiky prezentovaný výskyt perforované apendicitidy s difuzní peritonitidou ve 27. týdnu gravidity. Následný porod císařským řezem ve 36. týdnu a komplikované pooperační období a infekční komplikace v šestinedělí. Závěr: Apendicitida v těhotenství je spojována se zvýšenou mateřskou a fetální morbiditou. Perforace apendixu přispívá ke zvýšenému riziku dalších komplikací, které zahrnují např. předčasný porod, abort a mateřské komplikace, zejména infekční. Včasná diagnóza s následnou operací může snížit mateřskou i fetální morbiditu., Objective: To draw attention to the issue of appendicitis in pregnancy. Design: Case report. Case report: We present a case study in the form of a case of occurrence of appendicitis with peritonitis in 27 weeks pregnancy. And describes the birth and childbed, which is complicated by infectious complications. Conslusion: Appendicitis in pregnancy is associated with increased maternal and fetal morbidity and even more in case of peritonitis. Delay surgical intervention correlates with increased risk of perforation, which contributes to the increase of other complications, including premature birth, abortion and maternal complications. Alzheimerova choroba (ACH) se řadí mezi závažná neurodegenerativní onemocnění provázená oxidačním stresem. Produkty radikálových reakcí mohou difundovat z primárních míst a být detekovány v cerebrospinální tekutině (CSF) či krvi. Tyto produkty představují potenciální biochemické markery pro diagnostiku ACH. Nejvíce pozornosti je zaměřeno na analýzu CSF, protože odráží patologické změny v mozkové tkáni u ACH. V CSF byly potvrzeny zvýšené hladiny oxidačních produktů lipidů i proteinů. Hladiny vitaminů jsou snížené, nicméně existují i práce, které nenašly rozdíly oproti kontrolám. Některé studie se zaměřily na detekci produktů oxidačního stresu v krvi u ACH, ale výsledky nejsou konzistentní. Část prací ukazuje na nárůst oxidačních produktů a snížené hladiny antioxidantů v plazmě. Nicméně jiné práce tyto výsledky nepotvrdily. Z hlediska hledání diagnostického biomarkeru pro ACH má význam se zaměřit na specifické produkty peroxidace lipidů, tzv. lipofuscinoidní pigmenty (LFP), v erytrocytech. Na základě fluorescenčních analýz LFP je možné najít specifický produkt v krvi u ACH., Alzheimer´s disease (AD) is a serious neurodegenerative disorder accompanied by oxidative stress. Products of free radical reactions diffuse from primary sites and can be detected in cerebrospinal fluid (CSF) and blood. Such products represent potential biochemical markers for diagnosis of AD. Most studies are focused on CSF since its composition reflects pathological changes in the brain in AD. Increased levels of lipid and protein oxidative products have been found in CSF. Levels of vitamins were reduced in CSF. However, there are studies which show no difference between AD and controls. There is research on free radical products in blood in AD but results are not consistent. Several studies show increased oxidative products and reduced antioxidant in plasma. Nevertheless, others did not confirm it. Considering the investigation of a diagnostic biomarker for AD, specific end-products of lipid peroxidation, so called lipofuscin-like pigments (LFP), in erythrocytes represent an important possibility. Perigraft serom je poměrně vzácná komplikace, která může vzniknout po implantaci cévní protézy z dakronu nebo expandovaného polytetrafluoretylenu (ePTFE). Předkládáme kazuistiku 54letého pacienta s perigraft seromem okolo axillofemorálního bypassu (ePTFE protézou). Konečným řešením byla explantace této extraanatomické rekonstrukce a implantace aortobiilického bypassu s použitím cévní protézy z jiného materiálu. Článek diskutuje terapeutické možnosti této komplikace na základě publikovaných prací. Žádná guidelines ani jiná doporučení neexistují. Závěrem, přístup k léčbě perigraft seromu zůstává striktně individuální. Náhrada cévní protézy s použitím implantátu z jiného materiálu se zdá být nejlepší možností řešení pro recidivující perigraft seromy tam, kde jiné méně invazivní postupy selhaly., Perigraft seroma is quite a rare complication that may occur after implantation of Dacron or expanded polytetrafluoroethylene (ePTFE) vascular grafts. We report a case of a 54-year-old patient with perigraft seroma around an axillofemoral bypass (ePTFE graft). Definitive treatment involved the explantation of this extraanatomic bypass with perigraft seroma and the implantation of an aortobiiliac bypass using vascular prosthesis made of a different material. Based on published studies, therapeutic options for this complication are discussed. No guidelines or recommendations are available. In conclusion, the approach to perigraft seroma treatment remains strictly individual. Vascular graft replacement using grafts made of different material seems to be the best option in the case of recurring perigraft seroma, where less invasive procedures were not successful., and A. Perineální hernie je kýla prostupující defektem v pánevním dnu. Primární hernie jsou extrémně vzácné, častěji se jedná o onemocnění sekundární po operacích v pánvi, zejména po abdominoperineálních resekcích či exenteracích pánve. Jako nejvýznamnější rizikové faktory pro vznik onemocnění jsou uváděny poruchy hojení perineální rány a neoadjuvantní chemoradioterapie u nemocných s maligním onemocněním. Lze očekávat nárůst výskytu tohoto onemocnění v souvislosti s využíváním laparoskopických přístupů a ELAPE. I přes značný odstup od první popsané operace chybějí soubory pacientů, studie i doporučené postupy řešení. Autoři prezentují kazuistiku 66letého nemocného se sekundární perineální hernií po předchozí laparoskopické abdominoperineální resekci rekta pro adenokarcinom, u něhož byla provedena plastika kýly z abdominálního přístupu a k uzávěru defektu bylo využito stopkovaného laloku m. rectus abdominis., Perineal hernia is defined as a protrusion of intra-abdominal viscera through a defect in the pelvic floor. Primary hernias are extremely rare; secondary (postoperative) hernias following pelvic surgery, especially abdominoperineal resection or pelvic exenteration, are more common. Impaired perineal wound healing and neoadjuvant chemoradiotherapy in cancer patients are defined as main risk factors. A growing incidence of this complication is expected in connection with an increasing use of laparoscopic approaches and ELAPE. Despite the considerable time since the first described secondary perineal hernia in literature, patient series, prospective studies and treatment guidelines are still missing. First stadium juveniles of P. angustus were reared under controlled seasonal conditions to maturity, reproduction and death. Individuals born in any one breeding season either had a 1-year or a 2-year life cycle (cohort-splitting). The life cycle was annual for individuals born in the first part of the breeding season (May-August), but became biennial for those born later (August-October). Two phenomena were involved: (1) Only individuals reaching the penultimate stadium (stadium VII) before a critical period at the end of spring could become adult in the breeding season following that of their birth. After this time, stadium VII individuals entered into aestivation and only became adult in the second autumn after their birth. (2) Females becoming adult in autumn entered reproductive dormancy and only laid eggs in the following spring. Overall, individuals born at the start of the breeding season easily reached stadium VII before the critical period and were able to breed at I year, whereas individuals born at the end of the breeding season reached stadium VII after the critical period, then had two consecutive periods of dormancy and only bred at 2 years age. Individuals from the same nest born in the middle of the breeding season (August) could have either annual or biennial life cycles, depending on whether they reached stadium VII before or during aestivation. Perioperační péče o diabetika je specifická nejen nezbytností adekvátní korekce glykemie, ale také výskytem komplikací, které se u nediabetiků nevyskytují. Dosavadní antidiabetickou léčbu je proto většinou nutno přizpůsobit perioperačnímu období, u malých a krátkých chirurgických výkonů to však není většinou nutné. Perioperační péči je nezbytné u diabetiků pečlivě zorganizovat a naplánovat. K hlavním zásadám patří adekvátní monitoring glykemií a inzulinové terapie, zajištění nutrice a provedení výkonu ráno., The perioperative care of diabetic patients is specific due to the necessity of adequate glycaemic correction, as well as owing to the occurrence of complications not taking place in nondiabetic patients. The present antidiabetic treatment therefore mostly needs to be adjusted to the perioperative period, but this is largely not necessary in the case of small and short surgeries. The perioperative care of diabetic patients must be carefully organized and scheduled.
