Přehled problematiky: Magnézium je čtvrtým nejčastějším kationtem v lidském organizmu a po draslíku druhým kationtem v intracelulárním prostoru. Účastní se jako kofaktor enzymů metabolických reakcí, tvorby a utilizace energie a je nezbytný k udržení elektrolytové rovnováhy. Tělo dospělého člověka obsahuje asi 0,043 % (535-730 mmol) magnézia, což je v přepočtu asi 22-30 g (60 % v kostěném skeletu, 40 % intracelulárně ve svalovině a měkkých tkáních, 1 % (7,5-10 mmol) je obsaženo v extracelulární tekutině). Denní potřeba magnézia se odhaduje na 10-20 mmol, tj. 200-400 mg. Ve vztahu k podané dávce pak platí, že čím vyšší podaná dávka, tím nižší procento rezorbce ze střeva. Z doporučené denní dávky 15 mmol (365 mg) se rezorbuje pouze okolo 1/3. Význam magnézia v těhotenství: Důsledkem manifestního nedostatku magnézia může být: 1. zvýšené riziko potratu - předčasný porod (inkompetence hrdla děložního, předčasný odtok vody plodové), 2. placentární insuficience a hypotrofizace plodu, 3. rozvoj gestózy u matky, 4. zvýšené riziko astmatu a ekzému u dětí. Magnézium sulfát, podávaný těhotným ohroženým předčasným porodem (23+0 -30+0), snižuje riziko dětské mozkové obrny u přežívajících novorozenců. Souhrn: Nedoporučuje se používat intravenózní magnézium sulfát v indikaci léčby hrozícího předčasného porodu déle jak 5-7 dní. Tato omezení se nijak nevztahují na perorální užívání magnézia., Objectives: Magnesium is the fourth most common cation in the human body, and the second cation to potassium in the intracellular space. It participates as a cofactor of enzymes in the metabolic reactions, creation and utilization of energy and is necessary for the maintenance of electrolyte balance. Úvod: Monoklonální gamapatie nejistého významu (MGUS) je bezpříznakový a potenciálně maligní stav charakterizovaný nezhoubnou klonální proliferací plazmatických buněk secernujících monoklonální imunoglobulin (MIG, M-protein) detekovatelný v séru a/nebo v moči při absenci zhoubného lymfoproliferativního stavu. Je známo, že část jedinců s MGUS transformuje v některou z maligních forem monoklonálních gamapatií. Mezi známé faktory určující míru rizika progrese patří zejména kvantita, typ M-proteinu a poměr volných lehkých řetězců imunoglobulinu (VLŘ) umožňující účelnou stratifikaci jedinců s MGUS. Někteří autoři pokládají i za možný faktor progrese přítomnou supresi hladin polyklonálních imunoglobulinů. Cíl: Cílem studie bylo provedení pilotní analýzy hladin párů těžkých/lehkých řetězců imunoglobulinu (HLC) v souboru jedinců s MGUS a porovnání výsledků s hladinami M-proteinu vyšetřených gelovou elektroforézou. Dále pak provedení porovnání hladin polyklonálních imunoglobulinů a hladin alternativního páru HLC s cílem ověření stupně imunitní parézy v závislosti na rizikovosti MGUS. Metody: Analyzovaná sestava zahrnovala 148 vzorků sér od jedinců s MGUS (102 IgG, 28 IgA, 18 IgM typu), kteří byli v závislosti na hladinách, typu M-proteinu a hodnotě indexu VLŘ stratifikováni do 4 rizikových skupin (nízké, nízké-střední, vysoké-střední a vysoké riziko transformace). Pro stanovení hladin VLŘ, hladin polyklonálních imunoglobulinů a hladin HLC bylo využito platformy turbidimetru SPA Plus. Statistická analýza byla provedena pomocí Spearmanovy korelační analýzy a Mann-Whitneyeho U testu s Bonferroniho korekcí. Výsledky: Při porovnání hladin HLC byla v souboru IgG MGUS zjištěna silná korelace s hladinami M-proteinu jak v případě IgGκ (r = 0,698, p = < 0,0001), tak v případě IgGλ (r = 0,847, p = < 0,0001), součet obou párů izotypu IgG velmi silně koreloval s celkovými hladinami IgG v séru (r = 0,893, p = < 0,0001). Obdobné výsledky byly získány u MGUS s IgA izotypem MIG. V případě izotypového páru IgAκ byla zjištěna středně silná korelace s hladinami M-proteinu (r = 0,601, p = 0,018), v případě IgAλ pak silná korelace (r = 0,733, p = 0,004), přičemž součet obou HLC párů vykazoval velmi silnou korelaci s hladinami celkového IgA ( r = 0,941, p = < 0,0001). Při analýze jedinců s IgM typem byly taktéž zjištěny velmi silné korelace hladin IgMκ (r = 0,904, p = 0,001) resp. IgMλ (r = 0,783, p = 0,013) a hladin M-proteinu. Nejsilnější korelace byly opět zjištěny mezi součtem obou izotypových párů a celkových hladin IgM imunoglobulinu v séru (r = 0,967, p = < 0,0001). Při porovnání míry suprese hladin polyklonálních imunoglobulinů u jednotlivých rizikových tříd v souboru IgG MGUS byly zjištěny signifikantně vyšší hladiny IgA imunoglobulinu u jedinců s nízkým resp. nízkým středním rizikem ve srovnání se skupinou s vysokým středním rizikem (p = 0,0001 resp. p = 0,047). Při analýze skupiny MGUS s IgA izotypem byly zjištěny významně vyšší hladiny IgM imunoglobulinu u skupiny s nízkým středním rizikem nežli u jedinců s vysokým středním resp. vysokým rizikem (p = 0,035 resp. p = 0,017). V souboru IgG MGUS bylo rovněž provedeno porovnání hladin alternativních izotypových párů mezi jednotlivými rizikovými skupinami. U jedinců s dominantní sekrecí IgGκ byly zjištěny signifikantně vyšší hladiny alternativního páru IgGλ ve skupině nízkého a nízkého středního rizika nežli ve skupině vysokého středního rizika (p = 0,003 resp. p = 0,006). V případě dominantní sekrece IgGλ byly zjištěny významně vyšší hladiny IgGκ u jedinců s nízkým resp. nízkým středním rizikem nežli ve skupině s vysokým středním rizikem (p = 0,0006 resp. p = 0,009). Závěr: Analýza potvrdila velmi těsnou korelaci mezi hladinami M-proteinu a stanovením HLC, která se jeví jako pomocná ev. alternativní metoda stanovení M-proteinu s výhodou u atypických výsledků elektroforézy (zejména u IgA izotypu) či v případě velmi nízkých koncentrací M-proteinu s výhodou výpočtu indexu Igκ/Igλ. Odhalená souvislost míry imunosuprese v závislosti na rizikovosti MGUS a zejména jasně patrný přínos stanovení alternativního HLC páru přináší další aspekt k pochopení souvislosti biologie, chování a případné maligní evoluce MGUS s výhodou získání dobře měřitelného parametru. Využití platformy SPA Plus se jeví jako rychlé, efektivní, s vysokou výpovědní hodnotu a v neposlední řadě i s příznivými ekonomickými aspekty, Introduction: Monoclonal gammopathy of undetermined significance (MGUS) indicates an asymptomatic and potentially malignant state characterised by benign clonal proliferation of plasma cells secerning (discharging) monoclonal immunoglobulin (MIG, M-protein) detectable in serum and/or urine in the absence of malignant lymphocyte proliferation. It has been observed that in some patients, MGUS transforms into one of the malignant forms of monoclonal gammopathy. The wellknown factors determining the risk degree of progression include in particular the quantity, M-protein type and the ratio of immunoglobulin free light chains (FLC) allowing the efficient stratification of MGUS patients. In addition, several authors consider the present suppression of polyclonal immunoglobulin levels to be a potential factor of progression. Aim: The study aimed at conducting a pilot analysis of levels of immunoglobulin heavy/light chains pairs (HLC) in a group of MGUS patients and at comparing the results with the M-protein levels detected by gel electrophoresis. Furthermore, the polyclonal immunoglobulin levels and the levels of HLC alternative pairs were to be compared with a view to verify the degree of immune paresis depending on the MGUS risk degree. Methods: The analysed set comprised 148 serum samples of MGUS patients (102 IgG, 28 IgA, 18 IgM type) who were stratified into 4 risk groups (low, low-intermediate, high-intermediate, and high risk of transformation) according to the levels, M-protein type and FLC index values. FLC levels, polyclonal immunoglobulin levels, and HLC levels were determined by means of the SPA Plus turbidimeter platform. In the statistical analysis, Spearmans rank correlation and Mann-Whitney U Test with Bonferroni correction were applied. Results: When comparing the HLC levels, the IgG MGUS group revealed strong correlation with M-protein levels both in the case of IgGκ (r = 0.698, p = < 0.0001), and in the case of IgGλ (r = 0.847, p = < 0.0001), the summation of both pairs of the IgG isotype correlated considerably with the total levels of IgG in serum (r = 0.893, p = < 0.0001). Similar results were acquired in MGUS with the IgA isotype of MIG. In case of the IgAκ isotype pair, medium-strong correlation with M-protein levels was detected (r = 0.601, p = 0.018), and strong correlation in case of the IgAλ (r = 0.733, p = 0.004), whereas the summation of both HLC pairs showed very strong correlation with levels of the total IgA (r = 0.941, p = < 0.0001). Moreover, analyses of IgM type patients revealed very strong correlation of IgMκ levels (r = 0.904, p = 0.001) or IgMλ (r = 0.783, p = 0.013) and M-protein levels. Again, the strongest correlation was found between the summation of both isotype pairs and the total levels of IgM immunoglobulin in serum (r = 0.967, p = < 0.0001). Comparison of the suppression degree of polyclonal immunoglobulin levels in individual risk classes in the IgG MGUS group brought significantly higher levels of IgA immunoglobulin in patients with low or low-intermediate risk as opposed to the group with high-intermediate risk (p = 0.0001 and p = 0.047 respectively). When analysing the MGUS group with IgA isotype, notably higher levels of IgM immunoglobulin were evident in the group with low-intermediate risk than in patients with high-intermediate or high risk (p = 0.035 and p = 0.017 respectively). In addition, the levels of alternative isotype pairs were compared among the individual risk groups of the IgG MGUS group. Patients with dominant IgGκ secretion showed significantly higher levels of the IgGλ alternative pair in the group of low and low-intermediate risk than in the group of high-intermediate risk (p = 0.003 and p = 0.006 respectively). In case of dominant IgGλ secretion, considerably higher IgGκ levels were detected in patients with low or low-intermediate risk than in the group of high-intermediate risk (p = 0.0006 and p = 0.009 respectively). Conclusion: The analysis confirmed a very close correlation between M-protein levels and HLC determination which appears to be an auxiliary or alternative method of M-protein determination with an advantage in case of atypical electrophoresis results (in particular in IgA isotypes) or by very low M-protein concentrations with the benefit of Igκ/Igλ index calculation. The discovered connection between the degree of immune suppression depending on the MGUS risk level and especially the clearly visible benefit of determining the alternative HLC pairs contributes with another aspect to understanding the links between the biology, behaviour, and the potential malignant evolution of MGUS with the advantage of obtaining a well-measurable parameter. Interventions of paediatric obstructive sleep apnea syndrome are complex, varied and multidisciplinary. The goal of the treatment is to restore optimal breathing during the night and to relieve associated symptoms. Evidence suggests that the surgical intervention with removal of the tonsils and adenoids will lead to significant improvements in the most incomplicated cases, as recently reported from a meta-analysis. However, post-operative persistence of this syndrome in paediatric population is more frequent than expected, which supports the idea of the complexity of this syndrome. Adenotomy alone may not be sufficient in children with OSAS, because it does not address oropharyngeal obstruction secondary to tonsillar hyperplasia. Continuous positive airway pressure can effectively treat this syndrome in selected groups of children, improving both nocturnal and daytime symptoms, but poor adherence is a limiting factor. For this reason, CPAP is not recommended as first-line therapy for OSAS when adenotonsillectomy is an option. It is now being investigated the incorporation of nonsurgical approaches for milder forms and for residual OSAS after surgical intervention. Althought adeno-tonsillar hypertrophy is the most common for OSAS in children; obesity is emerging as an equally important etiological factor. Therefore an intensive weight reduction program and adequate sleep hygiene are also important lifestyle changes that may be very effective in mitigating the symptoms of this syndrome. Pharmacological therapy (leukotriene antagonists, topical nasal steroids) is usually use for mild forms of OSAS and in children with associated allergic diseases. Acenocoumarol is a vitamin K antagonist that is used for the treatment of acquired and congenital, both arterial and venous, thrombotic diseases. Its use is complicated by the narrow therapeutic range. Bleeding following oral anticoagulation, despite rare, remains the major complication. Most cases of hemorrhagic episodes usually require short hospitalization and transfusion, while surgical drainage of the hematoma is not recommended. However, in cases that conservative treatment isn’t successful, surgical intervention remains an option. Pacienti s nezvladatelnou chronickou obstipací by měli být vyšetřeni fyziologickými testy poté, co se vyloučí strukturální a mimostřevní příčiny. Je nutné nejprve vyčerpat veškeré možnosti konzervativní léčby. Mělo by se zdůraznit, že zejména nové léky, jako jsou prucaloprid a linaclotid, se zdají být velkým krokem vpřed v léčbě pacientů s chronickou obstipací. Pokud je indikovaný chirurgický výkon, mnoho let byla operací volby subtotální kolektomie s IRA, i když segmentální resekce jsou také dobrou volbou u izolovaných megasigmoidů, sigmoidokél či rekurentních sigmoideálních volvulů. V dnešní době by se měly nejprve vyzkoušet méně invazivní postupy, jako například modulace sakrálního nervu (SNM). Pokud je neúspěšná, dá se pak uvažovat o kolektomii. Všeobecně by se pacientům s poruchou motility gastrointestinálního traktu (gastrointestinal dysmotility syndrome- GID) nemělo nabízet chirurgické řešení z důvodu očekávaných špatných výsledků. Navíc pacienti s psychiatrickou poruchou by měli být aktivně odrazováni od resekce, protože mívají horší prognózu. Pacientům musí být vysvětleno, že bolesti anebo nadýmání budou pravděpodobně přetrvávat, i když chirurgický výkon znormalizuje počet stolic. Pacienti se souvisejícími potížemi mohou profitovat ze stomie bez resekčního výkonu jakožto léčebného výkonu i diagnostického pokusu. Kolektomie není volbou léčby pro bolest anebo nadýmání. Ve většině případů se dají poruchy vyprazdňování úspěšně léčit konzervativně. Nicméně v dnešní době existuje také široký výběr chirurgických řešení. Každá metoda má své místo v armamentáriu kolorektálního chirurga, ale jejich přesná role ještě není definována. Cílem této práce je nabídnout stručný přehled, jak diagnostikovat a léčit chronickou obstipaci z pohledu kolorektálního chirurga. Chirurgická léčba chronické obstipace není rutinní a provádí se jen ve výjimečných případech. Ale nejprve je nutno si říci, že tzv. „příliš dlouhé střevo“ (dolichocolon) není nikdy indikované k chirurgickému výkonu. Cílem této práce je prezentovat stručný přehled možných mechanismů obstipace, diagnostických metod a nástrojů a různé možnosti konzervativní a chirurgické léčby. Dále je nutné si vždy pamatovat, že obstipace nemusí být jen příznakem, ale může být i jiným onemocněním!, Patients with intractable chronic constipation should be evaluated with physiological tests after structural disorders and extracolonic causes have been excluded. Conservative treatment options should be tried unstintingly. It should be pointed out that especially new drugs such as prucalopride and linaclotide seem to be a big step forward in treating patients with chronic constipation. If surgery is indicated, for many years subtotal colectomy with IRA was the treatment of choice, although segmental resections were also a good option for isolated megasigmoid, sigmoidocele or recurrent sigmoid volvulus. Nowadays, less invasive procedures like sacral nerve modulation (SNM) should be tried first. If unsuccessful, colectomy can still be considered. In general, patients with a gastrointestinal dysmotility syndrome (GID) should not be offered any surgical options because of their anticipated poor results. Moreover, patients with psychiatric disorders should be actively discouraged from resection, as they tend to have a poorer prognosis. Patients must be counseled that pain and/or bloating will likely persist even if surgery normalizes bowel frequency. Patients with associated problems may be better served by having a stoma without resection as both a therapeutic maneuver and a diagnostic trial. Colectomy is not an option for the treatment of pain and/or abdominal bloating. In most cases outlet obstruction can be treated successfully with a conservative approach. However, nowadays there are also a variety of surgical options on the market. Each technique has its special place in the armamentarium of a colorectal surgeon but its exact role is not defined yet. The aim of this article is to give a brief overview, how to diagnose and treat chronic constipation from the standpoint of a colorectal surgeon. Surgical treatment of chronic constipation is not routine and is performed only in exceptional cases. But one thing first: a “too long gut” (dolichocolon) per se is never an indication for surgery. Our case-based review focuses on limb salvage through operative management of Charcot neuroarthropathy of the diabetic foot. We describe a case, when a below-knee amputation was considered in a patient with chronic Charcot foot with a rocker-bottom deformity and chronic plantar ulceration. Conservative treatment failed. Targeted antibiotic therapy and operative management (Tendo-Achilles lengthening, resectional arthrodesis of Lisfranc and midtarsal joints, fixation with large-diameter axial screws, and plaster cast) were performed. On the basis of this case, we discuss options and drawbacks of surgical management. Our approach led to healing of the ulcer and correction of the deformity. Two years after surgery, we observed a significant improvement in patient's quality of life. Advanced diagnostic and imaging techniques, a better understanding of the biomechanics and biology of Charcot neuroarthropathy, and suitable osteosynthetic material enables diabetic limb salvage. and T. Many aspects of surgical treatment of intestinal malrotation in children remain to be debatable. In the opinion of the majority of the specialists, surgical treatment is required after the diagnosis taking into account serious complications of intestinal malrotation. Purpose. The purpose of this research was to conduct an analysis of surgical tactics and operative treatment method for isolated and associated intestinal malrotations in children. Material and methods. We observed 123 children at the age of one day to 15 years with malrotation during the period of 2002 to 2013. Results. We presented the data from observing 123 children at the age of one day to 15 years with various clinical-anatomic forms of intestinal malrotation over from 2002 to 2013. In 62 patients (50.4%), the evidences of the high intestinal obstruction were prevalent, while 61 (49.6%) showed signs of low intestinal obstruction. 116 patients (94,3%) were given operative intervention: radical – 95(81,9%) and palliative – 21 (18,1%). In 56 % of the cases, various simultaneous surgeries were required. There are proposed differential approaches in relation to anatomic form of malrotation and possibility of the fixation of large intestine in the physiological position. Conclusion. The results obtained from the operative treatment are presented. Background. Nonunion of the lateral humeral condyle are of the complex pathology of the elbow joint, occurring relatively often and resulting in disability of children. The treatment of nonunion of the lateral humeral condyle of humerus with cubitus valgus remains controversial. Purpose of this report was improvement of the results of surgical treatment of the nonunion of the lateral humeral condyle of humerus with cubitus valgus of the lateral humeral condyle of humerus with cubitus valgus with use of differential approach to the surgical strategy. Material and methods. We were observing 28 children (17 boys and 11 girls) with nonunion and longstanding nonunion of lateral condyle of humerus, with various degrees of severity. There were used MRI and Xray investigations for differential study of the patients divided into 3 groups in relation to stability and nonstability of the nonunion of the lateral humeral condyle of humerus with cubitus valgus of the lateral humeral condyle of humerus with cubitus valgus. Results. All 28 patients lateral humeral condyle nonunions with cubitus valgus achieved union within sixty five days after operative procedure using Ilisarovs technique. The mean postoperative humerusulna angle was 6,0 degrees of cubitus valgus. All of reverse Tosteothomies healed uneventfully, and there was no loss of correction postoperatively. The mean duration of followup was 7 years. The overall results were excellent in 15(53,5%) patients, good in 11(39,3%) patients, and fair in 2(7,2%) patients. All 28 patients lateral humeral condyle nonunions with cubitus valgus achieved union within sixty five days after operative procedure using Ilisarovs technique. The mean postoperative humerusulna angle was 6,0 degrees of cubitus valgus. All of reverse Tosteothomies healed uneventfully, and there was no loss of correction postoperatively. Suspenze přední poševní stěny spočívá v rekonstrukci nativní tkáně vaginální cestou. Cílem operace je korekce stresové močové inkontinence, k níž došlo v důsledku hypermobility močové trubice, a případně souvisejícího prolapsu přední poševní stěny jakéhokoli stupně. Ideálním kandidátem pro tento výkon je pacientka s laterální cystokélou (1. a 2. stadium). Operace může být indikována rovněž u pacientek s cystokélou závažnějšího stupně, v tomto případě je však nezbytná další rekonstrukce v podobě suspenze klenby poševní a korekce posteriorního kompartmentu (enterokéla, rektokéla). V našem článku popisujeme techniku rekonstrukce a uvádíme naše počáteční (dosud příznivé) zkušenosti. Operace je krátká (trvá přibližně jednu hodinu), je spojena s minimální morbiditou a nemá žádný vliv na mikční funkci. Operace neovlivňuje hloubku ani délku pochvy – po zhojení sutury se tedy pacientka může vrátit k sexuální aktivitě. Prof. Zimmern (UT Southwestern Medical Center, Dallas, USA) prezentoval na poslední konferenci SUFU a AUA dlouhodobé výsledky po 15 letech., The anterior vaginal wall suspension is a native tissue repair procedure done vaginally to correct stress urinary incontinence secondary to urethral hypermobility and any associated degree of anterior vaginal wall compartment prolapse. The procedure is ideally suited for women with lateral defect cystocele (stages I and II). The procedure can be applied for more advanced degrees of cystocele, but additional repairs will be necessary including vault suspension and posterior compartment repairs (enterocele, rectocele). The technique of the procedure is reported herein, along with our early experience which has been favorable so far. The procedure is short (around one hour), with limited morbidity and no impact on voiding function. Vaginal depth and size are not compromised; so sexual activity can be resumed once the vaginal suture lines have healed. Histopatologické rysy obou v současnosti definovaných forem myotonické dystrofie (MD1 i MD2) jsou velmi podobné, vykazují charakteristický komplex změn. Vyšetření svalových biopsií in situ může kromě toho přinést i zjištění přítomnosti expanzí repetic CUG resp. CCUG v transkriptu mutované DNA, která tvoří genetický základ onemocnění. Expandované úseky lze znázornit ve tkáňových řezech metodou fluorescenční in situ hybridizace (FISH) jako fokální intranukleární inkluze. Pro posouzení významu svalové biopsie v době, kdy je diagnostika MD1 a MD2 založena na DNA analýze lymfocytů z periferní krve, jsme analyzovali výsledky histopatologického vyšetření souboru 34 nemocných, u kterých byla biopsie provedena na základě klinického podezření na myotonickou dystrofii. Diagnóza MD byla potvrzena analýzou DNA pro oba typy onemocnění. FISH byla provedena u 27 z nich. Mutační analýzou DNA byla potvrzena MD1 v 13 případech, MD2 v 10 případech, u 11 pacientů nebyla diagnóza MD1 či MD2 potvrzena. Ve většině případů byl bioptický histopatologický nález u prokázané MD hodnocen buď jako typický pro MD1 či MD2, anebo nález kompatibilní s touto diagnózou. Bioptické nálezy u některých pacientů s MD byly atypické: ve 2 případech byla přítomna zánětlivá infiltrace, v jednom případě obraz připomínající kongenitální myopatii. Naproti tomu histopatologické rysy MD byly zjištěny v některých biopsiích, u nichž diagnóza MD1 ani MD2 nebyla potvrzena. Fokální intranukleární inkluze prokazované FISH byly prokázány u 23 pacientů – ve shodě s výsledky analýzy DNA – u 13 s MD1 a u 10 s MD2. Metodou volby při podezření na myotonickou dystrofii je mutační analýza DNA z periferní krve. V případě provedené svalové biopsie z důvodu atypického klinického obrazu, který nevedl k podezření na MD, však může zhodnocení biopsie vést k dodatečnému molekulárně-genetickému vyšetření pacienta. Metoda FISH v biopsii je zajímavá z teoretického pohledu, neboť umožňuje zkoumat retinovanou RNA v jádrech jednotlivých buněk a tkání postižených pacientů, a tak může napovědět o jejich možné účasti v patogenezi onemocnění., The histopatological features of both so far defined types of myotonic dystrophy (DM1 and DM2) are very similar, the affected muscles show a typical pattern of changes. The examination of muscle biopsies in situ may also bring about revealing of expansions of CUG or CCUG repetitons in transcripts of the mutated DNA, the genetic basis of the diseases, which may be demonstreated by FISH (fluorescence in situ hybridization) as intranuclear focal accumulation of the transcript. In order to evaluate the importance of muscle biopsy including FISH at the time, when the diagnosis of DM is based on DNA analysis from blood lymphocytes, we analysed the results of histopathological examination of 34 patients, where the biopsy was indicated by clinical suspicion of myotonic dystrophy. The diagnosis was verified by mutation analysis of DNA for both DM1 and DM2. The examination was supplemented by FISH in 27 patients. The diagnosis of DM1 was confirmed in 13 patients, DM2 in 10 patients, the diagnosis of DM1 or DM2 was not confirmed in 11 patients. The biopsy brought about confirmation or precision-specification of the clinical diagnosis or suggested the diagnosis of DM in the majority of the patients. The bioptic findings in some patients with DM were atypical: in two cases there was inflammatory infiltration, in one case a picture resembling congenital myopathy. On the other hand, histopathological features of myotonic dystrophy were present in several biopsies where the diagnosis of DM was not confirmed. Focal accumulation of CUG/CCUG repeats in myonuclei was demonstrated by fluorescence in situ hybridization – in accordance with the DNA examination – in 23 patients, 13 with DM1 and 10 with DM2. Mutation analysis of the DNA isolated from blood leukocytes represent the method of choice if the diagnosis of DM is suspected. On the other hand, muscle biopsy may reveal histopathological features that may indicate subsequent molecular genetic examination of the patient if the results of the clinical examination are equivocal or atypical. FISH in the biopsy may reveal the intranuclear focal accumulation of the RNA transcripts in cells and tissues of the affected patiens and, in this way, to suggest a possible participation of them in the pathogenesis of the disease., Zdeněk Lukáš, I.
