- Introduction: Obstruction of the appendiceal lumen is the primary cause of appendicitis. The most common causes of luminal obstruction are fecaliths and lymphoid follicle hyperplasia. Additionally, bacterial infections or enteric and systemic viral diseases can cause a reaction of the lymphoid follicle. Case presentation: An 11-year-old boy with active phase of chickenpox presented on our Pediatric surgery emergency department under the impression of acute appendicitis. An appendectomy was performed on the same day. An inflamed and edematous retrocecal appendix was removed during surgery. Histological investigation of the appendix revealed transmural acute inflammation, with diffuse proliferation of inflammatory cells, with characteristic intranuclear inclusion surrounded by a clear halo. The PCR analysis of peripheral blood and appendix tissue specimen revealed positive VZV DNA. - Huntingtonova choroba (HD) je autozomálně dominantní neurodegenerativní onemocnění způsobené zvýšením počtu polyglutaminových repetic (> 35 repetic) v genu pro protein huntingtin. HD je charakteristická pomalými progresivními změnami pohybového aparátu a osobnosti, kdy tyto změny jsou často doprovázeny ztrátou tělesné hmotnosti. Do dnešního dne není znám přesný mechanizmus patofyziologie choroby. Poruchy pohybových funkcí reflektují masivní poškození specifických částí mozku (striatum), které bylo popsáno u pacientů s HD. V roce 2013 Sbodio et al [1] popsali zvýšené množství proteinu Acyl‑CoA binding domain containing 3 (ACBD3) ve striatu HD pacientů. Protein ACBD3 hraje nezastupitelnou roli v mnoha buněčných procesech, a to především díky interakci s různými vazebnými partnery. ACBD3 je esenciální při neuronálním dělení, neurodegeneraci, udržení lipidové homeostáze, stresové odpovědi, virové replikaci, apoptóze, udržení struktury golgiho komplexu. V této práci jsme prokázali nepřítomnost proteinu ACBD3 v mitochondriích v lidských kožních fibroblastech a navíc jsme potvrdili, že změny celkové hladiny proteinu ACBD3 ve fibroblastech HD pacientů nejsou konzistentní., Huntington’s disease (HD) is an autosomal‑dominant neurodegenerative disease caused by the expansion of polyglutamine repeats (> 35 repeats) in the nuclear gene for the huntingtin protein. HD is characterized by slow progressive changes in motor behaviour and personality that are sometimes accompanied by weight loss. To date, the exact mechanisms of HD pathophysiology have not been defined. Impaired motor behaviour reflecting massive and selective destruction of the striatum has been observed in patients with HD. Sbodio et al. [1] reported in 2013 that Acyl‑CoA binding domain containing 3 (ACBD3) protein levels were elevated in the striatum of HD patients and connected with higher neurotoxicity in HD. The ACBD3 protein plays essential roles in many different cellular functions via interactions with a multitude of partners. ACBD3 is involved in neuronal stem cell self‑renewal, neurodegeneration, lipid homeostasis, stress resistance, intracellular vesicle trafficking, organelle maintenance, viral replication and the apoptotic response. Herein, we found that ACBD3 in not present in the mitochondria in skin fibroblasts. - In the last decade the health of young people in Ukraine as well as in many other countries of the world significantly deteriorated according to the results of screening tests. Because of this the main task of prophylactic medicine is development and introduction of new methods of early diagnostics of diseases. The aim of the work to assess the peculiarities of adaptation of students with different types of vegetative regulation. Material and methods: 415 second year students of Lugansk State Medical University were tested. The mechanisms of physiological functions of the body regulation were assessed by registration of cardiac rhythm variability (CRV). Analysis of CRV was performed by three methods: statistical, geometrical, and spectral analysis. The express method proposed by Shlyk, N.I. (2009) was later used to determine the prevailing type of vegetative regulation. To assess adaptability the method of R.M. Baevskij (1979) was used. Results: In the result of investigation 4 types of functional states of regulatory systems were determined: I type 295 (71 %) of tested, II in 14 (3 %), III in 96 (23 %) and IV only in 10 (2%). I type of functional state of regulatory systems in the wakeful state at rest was characterized by moderate prevalence of central regulation of heart rhythm (MPCR), decreased activity of autonomic regulation. It was noticed that some of the parameters of CRV (like RR, SDNN, RMSSD and pNN50) which characterize autonomic regulation, were statistically lowere (р≤0.01) compared to type III, and some (like AMo and SI) higher, in both female and male students. Summarized spectrum capacity and components of its wave structure (HF, LF, VLF) which characterize central regulation were statistically significantly lower (р≤0.01) for the I type of regulation compared to the III one which is characterized by moderate prevalence of autonomic regulation of the heart rhythm. In female students with type I autonomic regulation compared to male students such CRV parameters as RR and AМo were statistically significantly lower (р≤0.05), while the parameters of total potency (TP) and high frequency waves (HF) were higher (р≤0.05), which could point to more centralized heart rhythm regulation in male compared to female. The evaluation of regulatory systems overstress allowed to determine the state of adaptation and risk of overstress or breakdown in students with different types of autonomic regulation. - Cíl: Cílem této studie je poskytnout orientační normy k Addenbrookskému kognitivnímu testu (ACE‑<span class=""></span>R) a jeho jednotlivým kognitivním doménám pro českou populaci. Soubor a metodika: Do studie bylo zařazeno 143 osob (89 žen a 54 mužů) ve věku 55–<span class=""></span>89 let. Subjekty byly zástupci zdravé populace, bez onemocnění mozku, smyslové poruchy a závažného psychiatrického onemocnění, s MMSE nad 27 bodů. Osoby byly podrobeny detailnímu anamnestickému rozhovoru pro zjištění případných onemocnění mozku v minulosti a pro zjištění míry soběstačnosti v běžných denních činnostech. Osoby s poškozením mozku v anamnéze či porušenou soběstačností v běžných denních činnostech nebyly do studie zařazeny, stejně jako osoby s MMSE nižším než 27 bodů. Srovnání hodnot ACE‑<span class=""></span>R a jeho složek u mužů a žen bylo provedeno pomocí Mannova‑<span class=""></span>Whitneyova testu a srovnání hodnot u čtyř skupin daných věkem a vzděláním pomocí Kruskalova‑<span class=""></span>Wallisova testu. Výsledné p‑<span class=""></span>hodnoty byly korigovány na mnohonásobné testování užitím Bonferroniho korekce. Hraniční skóry byly vytvořeny na úrovni 2. a 7. percentilu. Výsledky: Byla prokázána negativní korelace s věkem (p < 0,001; r = –<span class=""></span>0,43) a pozitivní korelace s proměnnou vzdělání na celkový výsledek v testu (p < 0,001; r = 0,41) i v dosažených výsledcích některých jednotlivých kognitivních domén. Vliv pohlaví nebyl statistický prokázán. Hraniční skór pro všechny subjekty ve věku 55–<span class=""></span>89 na úrovni 2. percentilu je 74 bodů pro celkový skór v Addenbrookském kognitivním testu a 79 bodů na úrovni 7. percentilu. Závěr: Studie poskytuje orientační normy pro českou populaci pro Addenbrookský kognitivní test a nabízí bázi pro vytvoření kvalitních norem za předpokladu navýšení počtu subjektů., Aim: The aim of this study was to provide benchmark normal values for Addenbrooke’s cognitive examination (ACE-R) and its domains for the Czech population. Methods: The study included 143 healthy subjects (89 women and 54 men) aged 55–89 years, without brain injury, neurodegenerative disease, severe hearing or visual impairment and without a psychiatric disease, with MMSE above 27 points. Participants were interviewed in detail to ascertain any previous brain injury history and to determine the level of self-sufficiency in daily living activities. Individuals with a history of brain injury or impaired self-sufficiency were excluded from the study. ACE-R values and values for its domains in men and women were compared with Mann-Whitney test and values for the four age and education groups were compared using the Kruskal-Wallis test. P-values were corrected for multiple testing using Bonferroni correction. Results: Cut-off scores were set at 2nd and 7th percentile. Negative correlation with age (p < 0.001, r = –0.43) and positive correlation with education (p < 0.001, r = 0.41) were statistically significant for the overall test performance and also for the performance in individual cognitive domains except for the Attention and orientation domain. The effect of sex was not statistically significant. The cut-off score for the total score in Addenbrooke’s cognitive examination for all subjects aged 55–89 years is 74 points at the 2nd percentile and 79 points at the 7th percentile. Conclusion: The study suggests cut-off scores for the Czech population of ACE-R and provides the basis for the development of Czech norms for this test. - V souboru 39 případů adenokarcinomu in situ děložního hrdla byly sledovány: věk pacientek, cytologické, kolposkopické a histologické nálezy, operační výkony a definitivní řešení. Medián věku byl 38 let (24–75). Prebioptická suspekce žlázové léze byla vyslovena v 7 cytologických (18 %) a v 7 kolposkopických (18 %) nálezech. U 28 nemocných (72 %) byla současně přítomna koexistující skvamózní léze. V 16 případech (46 %) z 35 konizací byly popsány histologicky pozitivní okraje léze a u 15 z nich byl proveden opakovaný nebo definitivní operační výkon. V něm byl ve 2 případech již invazivní adenokarcinom. Klíčová slova: adenokarcinom in situ – cytologie – děložní hrdlo – kolposkopie – pozitivní okraje, In a series of 39 cases of adenocarcinoma in situ of the uterine cervix, age of patients, cytological, colposcopical and histological findings, operations and definitive solutions have been analyzed. Median age was 38 (24 – 75). Praebioptic suspicion of a glandular laesion was expressed in 7 cytological (18 %) and 7 colposcopical (18 %) findings. In 28 patients (72 %) the coexistent squamous laesion was concurrently present. In 16 cases (46 %) from 35 conisations histologically positive margins were described and in 15 of them repeated or definitive operation was performed. - Hyperparatyreóza (HPT) patří k onemocněním, jejichž incidence stoupá. Převážnou většinu primárních hyperparatyreóz (asi 80 %) tvoří adenomy. Karcinom příštítných tělísek je velmi vzácný. K lokalizaci adenomů příštítných tělísek se standardně používá ultrasonografie, scintigrafie, nejčastěji 99mTc-MIBI a CT. Při stanovení diagnózy primární hyperparatyreózy je operační výkon standardním léčebným postupem. Nezastupitelné postavení má peroperační ultrasonografie. V průběhu tří let bylo na našem pracovišti operováno celkem 21 pacientů s diagnózou adenom příštítného tělíska, který byl histologicky potvrzen u 19 pacientů. Náš malý soubor operovaných pacientů vykazuje úspěšnost 90,5 %., Hyperparathyreosis is one of the diseases whose incidence has been increasing. Vast majority of hyperparathyreosis (about 80 %) is made up by adenomas. Carcinoma of parathyroid glands is very rare. Ultrasonography, scintigraphy, most frequently 99mTc-MIBI and CT are standardly being used for localization of the adenomas of parathyorid glands. Operation is a standard medical method in the process of determinating the diagnosis of primary hyperparathyreosis. Peroperative ultrasonography has irreplaceable position. 21 patients with diagnosis of adenoma of parathyroid glands which was histologically confirmed in 19 patiens have been operated in our workplace during three years. - Cíl: Průzkumné šetření zjišťovalo schopnosti spolupráce nemocných při léčbě Warfarinem a dodrţování léčebného reţimu v průběhu dlouhodobé antikoagulační terapie Warfarinem. Konkrétně byla mapována a vyhodnocena úroveň edukace sestrou, informovanost a adherence k léčbě u pacientů léčených Warfarinem. Metodika: Průzkumný soubor tvořilo 108 pacientů evidovaných v ambulantním léčení interní a kardiologické ambulance při interním oddělení Nemocnice ve Frýdku-Místku. Průzkumné šetření probíhalo v prosinci roku 2010 a lednu roku 2011. Pro sběr dat byla vyuţita metoda dotazníkového šetření. Výsledky: Výsledky průzkumného šetření ukazují, ţe naprostá většina pacientů léčených Warfarinem se aktivně podílí na léčebném procesu. Nemocným se od zdravotníků dostává poučení o léčbě, sestry se na edukaci podílejí v 75 %. Většina dotázaných dochází na pravidelné kontroly a zná zpaměti uţívanou dávku léku. Čtvrtina nemocných však není schopna uvést krvácení jako hlavní riziko léčby. Převáţná většina respondentů si je vědoma skutečnosti, ţe účinek léčby můţe být ovlivněn sloţením přijímané potravy, méně neţ polovina respondentů zná interakci Warfarinu s jinými léky či doplňky stravy. Nemocní mají na zřeteli, ţe je nutno informovat kaţdého lékaře o této léčbě, jen polovina respondentů však informuje i lékárníka. Závěr: Adherence k léčbě ze strany pacienta je základem úspěšné a bezpečné terapie Warfarinem. Cílem všech zdravotníků by měla být snaha o kvalitní a efektivní komunikaci s nemocnými, snaha o dosaţení co nejvyšší úrovně spolupráce, ochoty a schopnosti pacientů řídit se pokyny zdravotníků. Tím se výrazně zvýší úspěšnost léčby a eliminuje se riziko moţných komplikací léčby., Aim: Exploratory investigation ascertained capabilities of cooperation of patients when treated with warfarin and adherence to treatment regimen during long term anticoagulant therapy by warfarin. Specifically was mapped and assessed the level of education by nurse, foreknowledge and adherence to treatment in patients treated with warfarin. Methods: The exploratory set consisted of 108 patients, who were registered in ambulant treatment of internal and cardiac ambulance at internal department of Hospital in Frydek-Mistek. Exploratory investigation took place in December 2010 and January 2011. For data collection was used method of questionnaire survey. Results: The results of questionnaire survey show, that absolute majority of patients treated with warfarin is actively participating in the treatment process. The patients receive from the health care providers education about the treatment, nurses are participating in education in 75%. Majority of respondents have regular check ups and knows by heart the dose of the drug. A quarter of patients is not able to state bleeding as the main risk of treatment. The vast majority of respondents are aware of the fact, that the treatment effect may be influenced by the composition of the diet, less than half of the respondents know the warfarin interactions with other medication or supplements. Patients have in mind that every doctor should be informed about this treatment, only half of the respondents, however, informs as well the pharmacist. Conclusion: Adherence to treatment by the patient is the foundation of a successful and safe warfarin therapy. The goal of all health professionals should be to aim for quality and effective communication with patients, effort to achieve the highest level of cooperation, willingness and ability of patients to follow medical instructions. - Cíl: Nepravidelné dávkování inzulinu je jedním z hlavních problémů spojených s inzulinovou terapií u pacientů s diabetem 2. typu; jeho skutečný rozsah však není přesně znám. Cílem průzkumu provedeného v ČR v rámci mezinárodního projektu GAPP2TM – Globální přehled přístupů pacientů a lékařů (Global Attitudes of Patients and Physicians) proto bylo ověřit, jaký je výskyt a dopad nepravidelného užívání bazálních inzulinových analog u pacientů s diabetem 2. typu, jakým způsobem a z jakých důvodů k těmto nepravidelnostem dochází, a jakým způsobem zdravotníci o nepravidelné aplikaci inzulinu s pacienty diskutují. Metodika: Projekt GAPP2TM je mezinárodní průřezovou studií provedenou on-line formou dotazníku prostřednictvím internetu u pacientů s diabetem 2. typu léčených inzulinovými analogy a u lékařů a zdravotníků, kteří tyto pacienty ošetřují. Průzkum proběhl ve dvou vlnách v celkem 17 zemích světa; do první vlny, která byla dokončena začátkem roku 2012, bylo zahrnuto 6 zemí, do druhé, dokončené v roce 2014, pak dalších 11 zemí včetně České republiky. Průzkum měl za cíl získat data o pohledech pacientů a lékařů na některé aspekty léčby diabetu 2. typu inzulinem a o přetrvávajících problémech v této oblasti v reálné každodenní praxi. Zabýval se zejména incidencí a zvládáním lehkých hypoglykemií a nepravidelnostmi v aplikaci inzulinu. V části zaměřené na adherenci k aplikaci bazálního inzulinu byly sledovány 3 typy nepravidelností v léčbě inzulinem: vynechání dávky, časový posun dávky (? 2 hodiny oproti předepsanému času) a snížení dávky, ve všech v případech během uplynulých posledních 30 dnů před vyplněním dotazníku. Dále bylo zkoumáno chování pacientů ve vztahu k této problematice a jejich vnímání těchto problémů. Výsledky: Výsledky průzkumu ukázaly, že nepravidelné dávkování inzulinu je v České republice méně časté než v ostatních zemích, v nichž probíhal výzkum GAPP2TM. I tak se ovšem týká cca pětiny všech nemocných s DM 2. typu léčených inzulinovými analogy v režimu bazál-bolus nebo pouze bazál. Při poslední nepravidelné aplikaci bazálního inzulínu se jednalo o záměr pacienta při úplném vynechání dávky ve 13 % případů, posun aplikace byl záměrný ve 23 % a snížení dávky inzulínu v 61 % případů. Nejčastěji hlášenými důvody bylo snížení rizika hypoglykemie a dodržování doporučení zdravotníků. Obavu z vynechání dávky bazálního inzulinu má 40 % českých pacientů a 35 % by mělo pocit viny, pokud by svou dávku bazálního inzulinu vynechali (u pacientů s intenzivním inzulinovým režimem je to 47 %). Pouze 60 % pacientů si uvědomuje, že vynechávání dávek bazálního inzulinu může mít negativní dopad na jejich dlouhodobý zdravotní stav. Dotazovaní lékaři měli podezření, že během pravidelných kontrol hlásí nižší počet vynechaných dávek bazálního inzulínu nebo vynechávání zcela zatajuje přibližně třetina pacientů. Tuto skutečnost však připustilo jen 11 % pacientů užívajících bazální inzulin a 15 % pacientů léčených intenzifikovaným režimem. Čtvrtina předepisujících lékařů navíc uvádí, že se svými pacienty nepravidelné užívání bazálního inzulinu během kontrol běžně neprobírá. Závěry: Ačkoli pacienti s diabetem 2. typu dodržují v České republice předepsané schéma léčby bazálním inzulinem častěji než v jiných zemích účastnících se průzkumu GAPP2TM, vynechání, časový posun a snížení dávky inzulinu je poměrně časté a ze strany zdravotníků si zasluhuje během pravidelných kontrol více pozornosti a cílenou edukaci., Objective: Irregular insulin dose is one of the main problems associated with insulin therapy in patients with type 2 diabetes; its extent is not known precisely. The aim of survey conducted in the Czech Republic in the international project GAPP2™ – Global Attitudes of Patients and Physicians was to determine the incidence and the impact of irregular use of basal insulin analogues in patients with type 2 diabetes, to point out the reasons for these irregularities and to focus on how physicians discuss irregular application of insulin with patients. Methods: The project GAPP2™ is an international cross-sectional study performed on-line via the Internet using a questionnaire filled by diabetic patients treated with insulin analogues and physicians who treat these patients. The survey was conducted in two steps in 17 countries; the first step included 6 countries and was completed in the beginning of 2012, the second step involved 11 other countries including the Czech Republic with termination in 2014. The survey was designed to obtain the views of patients and physicians on certain aspects of insulin treatment and persistent issues in this field in the real daily practice. Special focus was on the incidence and management of hypoglycaemia as well as on irregularities of insulin application. In the part dedicated to adherence to basal insulin application were observed three types of irregular insulin therapy: missed dose, time imprecision of dose (? 2 hours vs. the prescribed time) and dose reduction in all cases in the past 30 days before completing the questionnaire. In addition, it was investigated the attitude and relation of patients to these issues. Results: The results have shown that irregular insulin dose in the Czech Republic is less frequent than in other countries involved in the GAPP2™ research. Nevertheless, approximately one fifth of diabetic patients using insulin analogues in basal-bolus or only basal therapy regimen is related to this problem. The last irregular insulin application was due to missed dose in 13% of cases, time imprecision in 23% and reduction of dose in 61% of cases. The most commonly reported reason was risk reduction of hypoglycaemia and the recommendations of health professionals. Fear of missed dose is present in 40% Czech patients and 35% would feel guilty if their insulin dose is missed (up to 47% in patients with intensified insulin regimen). Only 60% patients are aware of negative impact on their long-term health after missed dose of basal insulin. Questioned doctors have suspected that the patients report lower number of missed doses during regular medical check because one third of patients doesn´t admit missed dose. However, this fact conceded only 11% of patients on basal insulin and 15% of patients on intensified insulin therapy. Quarter of prescribing doctors admit that they usually don´t discuss with patients irregularities in basal insulin treatment. Conclusion: Although, type 2 diabetes patients in the Czech Republic follow prescribed basal insulin therapy scheme more often than patients in other countries participating in the survey GAPP2 ™, missed dose, time imprecision and reduction of dose is quite common and it deserves more attention from medical side during regular medical check together with appropriate education of patients., and Martin Prázný

- Zlatým standardem léčby karcinomu endometria je chirurgická léčba. Adjuvantní radioterapie ve formě brachyterapie či zevní radioterapie je indikována u středně a vysoce rizikového karcinomu, kde snižuje zejména výskyt lokoregionální recidivy. U vysoce rizikového karcinomu jsou však pacientky ohroženy i generalizací onemocnění, což je důvodem pro zvažování zařazení adjuvantní chemoterapie do léčebného schématu. Chemoterapie může být použita současně s radioterapií jako konkomitantní chemoradioterapie nebo sekvenčně po skončení radioterapie. Článek popisuje výsledky prací, které použily adjuvantní chemoterapii. Výsledky nejsou zatím zcela jednoznačné, na závěry velkých studií stále čekáme. Přesto u vysoce rizikového karcinomu endometria již dnes v klinické praxi zvažujeme použití chemoterapie., Surgical treatment is the gold standard for treating endometrial carcinoma. Adjuvant radiotherapy in the form of brachytherapy or external radiotherapy is indicated in moderate and high-risk carcinoma where it particularly reduces the rate of locoregional recurrence. In high-risk carcinoma, however, patients are also at risk of disease generalization, which is the reason for considering the inclusion of adjuvant chemotherapy in the treatment regimen. Chemotherapy can be used concomitantly with radiotherapy as concomitant chemoradiotherapy or sequentially after radiotherapy termination. The article describes the results of papers that have utilized adjuvant chemotherapy. The results have not been entirely unequivocal so far, and the conclusions of large studies are yet to be published.
