- V těhotenství je anafylaktická reakce naštěstí sice relativně vzácná, její důsledky ale mohou vést až k závažnému postižení matky i plodu. Ve většině publikovaných případů těžké anafylaxe v těhotenství bylo těžké postižení dokonce spíše na straně plodu než matky. Vznik anafylaxe se většinou nedá předvídat, první symptomy jsou někdy netypické, pouze lokální, ale rychle mohou přecházet v generalizované postižení celého organismu. Ať už jsou zprostředkovány protilátkami typu IgE (pravá anafylaxe), nebo vznikají na jiném podkladě (anafylaktoidní reakce), klinický obraz bývá obdobný. Je třeba myslet na to, že i klinicky nenápadné počáteční symptomy mohou mít průběh závažný a život ohrožující. Každé opomenutí a prodleva v diagnostice a léčbě anafylaxe může vést k fatálnímu konci. O úspěšném výsledku léčby rozhoduje velkou měrou medicínská zkušenost a včasnost zásahu. Protože je toto téma v současné literatuře zmiňováno spíše jen výjimečně, je cílem tohoto článku shrnout problematiku anafylaxe v těhotenství z hlediska patofyziologie, klinického průběhu, diagnostiky i léčby., Anaphylaxis in pregnancy is a relatively rare reaction but it can lead to serious maternal and fetal impairment. In published cases of severe anaphylaxis in pregnancy there were even more severe disabilities in the fetus than the mother. Development of anaphylaxis is usually unpredictable, the first symptoms are sometimes atypical and only local, but can move quickly in a generalized impairment of the whole organism. Whatever they are mediated by IgE antibodies (right anaphylaxis), or are caused on another basis (anaphylactoid reactions), the clinical picture is similar. It should be kept in mind that even clinically inconspicuous initial symptoms may progress into serious and life-threatening state. Any omission and delay in diagnosis and treatment of anaphylaxis can lead to fatal end. A successful outcome is largely dependent on experience and timeliness of medical intervention. - Východiska: Metody léčby klasické trigeminální neuralgie (cTN) isou rozličné stejně jako jejich patofyziologie. Pacienti s cTN, u kterých nebyla medikamentózni léčba účinná nebo vyvolala nežádoucí Účinky, jsou kandidáty chirurgické léčby. 07; Prezentace výsledků léčby 1775 pacientů s cTN léčených perkutánní radiofrekvenční rizotomií (pRFR). Je rovněž diskutován současný stav chirurgické léčby cTN. Metodika: V období od 1983 do 2001 bylo v Neurochirurgickém ústavu v Bělehradě metodou pRFR léčeno 1775 pacientů s onemocněním cTN. Autoři analyzovali inidální a dlouhodobě výsledky, tr jako např. vliv rozličných typů cTN, dosažení faciální hypestezie po pRFR a předchozích chirurgických procedur v trigeminové oblasti na výsledky léčení. Výsledky: Dobrých iniciálních výsledků bylo docíleno u 96 % pacientů; recidivita po 5 letech se objevila u 21 % pacientů. Výsledky byly úspěšnější U pacientů, kteří prodělali krátkodobou, epizodickou, ostrou bolest v porovnání s těmi, jejichž bolesti zahrnovaly i složku stálé, tupé bolesti. Úvodní stupeň a doba trvání faciální hypestezie vyvolané pRFR nejen zvýšila úspěšnost ve zmírnění bolesti, ale také tíži neurologických komplikací. Závěr: pRFR nebo operace gamma nožem jsou doporučovány jako metody první volby v rámci chirurgické léčby pacientů s cTN. Mikrovaskulámí dekomprese by měla být zvážena u mladších pacientů s jednoznačnou neuroradiologickou diagnózou neurovaskulámí komprese v oblasti trigeminal root entry zone, nebo po selhání peí perkutánních neurochirurgických procedur., Background: Treatment of classic trigeminal neuralgia (cTN) has been as diverse as explanations of its pathophysiology. The patients with cTN refractory to medical treatment are candidates for surgical procedures. Aims: We report our results on 1775 patients treated for cTN with percutaneous radiofrequency rhizotomy (pRFR). We also discuss the current state of surgical management of cTN. Methods: In the period from 1983 to 2001, 1775 patients underwent pRFR for relief of cTN at the Institute of Neurosurgery in Belgrade. The authors analyzed the initial results and long-term follow- up data, such as the influence of different types of cTN, achieved facial hypesthesia after pRFR and prior trigeminal surgical procedures on the treatment outcome. Results: Initial positive results were obtained in 96% of the patients, while the recurrence rate after five years accounted for 21%, taking into consideration the whole series. Better results were achieved in patients with short episodic, sharp pain than in patients suffering from pain containing a more constant, throbbing component. Unfortunately, the initial degree and duration of facial hypesthesia provoked by pRFR, not only increased the success in pain relief, but also the degree of neurological complications. Conclusion: We recommend pRFR or gamma knife surgery (GK) as the first procedure for surgical treatment in majority of patients suffering from cTN. Microvascular decompression (MVD) should be considered for younger patients with clear radiological diagnosis of neurovascular compression at the trigeminal root entry zone or after failed percutaneous neurolytic procedures., Eugen Slavík, D. - Clinical oncologists have been focusing their efforts on attempting to define risk groups of patients with unusual biological reactions to the recommended therapy regimens using molecular biology techniques. The aims of our study were: (i) to find a design and validate a method for fast and reliable analysis of the D1853N (5557G>A) genetic polymorphism in the ATM (ataxia-telangiectasia mutated) gene; (ii) to use side-directed mutagenesis to generate ATM 5557A-positive DNA (reference ATM5557A DNA); and (iii) to analyze a group of patients suffering from cervical carcinoma with adverse responses to radiotherapy. The 5557A variant was found in three of twenty women (15%). Our data show that the prevalence of the 5557A allelic variant in cervical cancer subjects with adverse responses after irradiation probably does not differ from the prevalence common in Caucasians. - This article aims to share the results of research conducted in the Fergana chemical plant of furan compounds (FCPFC) in Uzbekistan.19 workers of the Furan compounds plant, in Fergana, Uzbekistan, were tested. By neutron activation analysis method, we have studied microelement composition of saliva, blood, dental hard tissue, and the level of Ca, Zn, Fe, and Ag in these subjects. We have detected that the level of chemical elements in dental hard tissue, blood, and saliva of these workers was subject to negative changes as compared to the analysis results from those in the control group. The research results have practical value for the prophylaxis, treatment, and health resumption of the people living in rugged ecological environment and workers who are engaged with harmful substances in chemical industry. The articles touch, what we think, is important about analytical quality in the medical laboratory The articles will address topics such as metrology (philosophy of the measurand), statistics, internal quality control (IQC), external quality assessment (EQA), critiques to the discipline. All contributions follow the ASAP-concept, meaning As Simple As Possible. Here, we explore the „ASAP“ concept for analytical quality specifications derived from biological variation. The application of the concept results for the great majority of the mainstream serum-, plasma-, and blood-analytes in CVa values within boundaries of 0.4 – 15 % and for Delta-SE within boundaries of 0.2 – 10 %. The take-home message is: analytical quality in the medical laboratory must NOT be viewed in absolute terms („a CVa of 2 % is good“), BUT in relative terms. - Tato práce vysvětluje základy vnitřní kontroly kvality v kostce. V prvé řadě je IQC nezbytnou součástí získání spolehlivého výsledku měření pacientských vzorků. Vytvoření účinné IQC strategie vyžaduje základní statistické znalosti (průměr, směrodatná odchylka a jejich intervaly spolehlivosti). K tomu jsou také potřebné základní znalosti o silových funkcích. Výběr IQCpravidel je vždy kompromisem mezi danou schopností systému detekovat chyby a pravděpodobností falešného zamítnutí. Použití konceptu přípustné celkové chyby může být užitečné při rozhodování o nejúčinnějších kontrolních pravidlech pro daný konkrétní analyt. To však vyžaduje též základní znalosti o analytických požadavcích na kvalitu. Praktické faktory však mohou někdy vést laboratoř k odchylkám od ideálního IQC konceptu: i) použití a práce s „nevhodným“ průměrem, protože je souběžně kontrolováno několik systémů / modulů pro stanovení téhož analytu, ii) použití a práce s „nevhodnou“ směrodatnou odchylkou, protože se musí zohlednit a respektovat variabilita mezi jednotlivými šaržemi. V práci jsou prezentovány běžné problémy spojené s těmito postupy. Tyto postupy mohou být i užitečné pro aplikování pacientských percentilů jako nástroje provádění IQC., This essay tries to put the very basics of internal quality control (IQC) into a nutshell. First, IQC is a necessary part for obtaining a valid result for a patients’ specimen. Establishing an effective IQC strategy requires basic statistical knowledge (mean, standard deviation and their confidence intervals). It also requires basic knowledge of power functions. The selection of an IQC-rule is always a compromise between power for error detection and probability of false reactions. Application of the allowable total error concept may be helpful in deciding about the most effective control rule for a specific analyte. This, however, requires decent knowledge about establishing analytical quality specifications. Practical considerations may lead the laboratory to deviate from the ideal IQC concept: i) working with the „wrong“ mean because several systems/modules have to be controlled for an analyte; ii) working with the „wrong“ standard deviation because lot-to-lot variation needs to be accommodated. - Úvod: Výsledky poskytované chirurgické péče v širším přesahu jsou obtížně dosažitelné. Práce testovala systém účtování zdravotní pojišťovně z hlediska využitelnosti dat. Metoda: Analyzováno bylo 6879 operací tříselné kýly v období 2008–2015. Využit byl systém účtování v DRG Vojenské zdravotní pojišťovny České republiky. Výsledky: Operace provedlo 156 pracovišť. V režimu jednodenní chirurgie bylo provedeno 3,8 % (248) výkonů, z toho 77 % (193) bylo operováno laparoskopicky. Za hospitalizace bylo provedeno 96,2 % (6631) výkonů, medián délky hospitalizace byl 4 dny. U hospitalizovaných pacientů byla otevřená plastika bez síťky provedena v 36,54 % (2423), otevřená plastika se síťkou v 19,84 % (1316), laparoskopická plastika v 43,61 % (2892). Reoperace pro recidivu v souboru byla v 1,85 % (127), po otevřené plastice bez síťky v 2,76 % (66), po otevřené plastice se síťkou v 1,66 % (22), po laparoskopické plastice v 1,19 % (37). Predikce recidiv do 5 let byla 2,42 % celkem. Hodnoceny byly i další parametry. Závěr: Studie potvrdila využitelnost systému pojištění k získání relevantních dat o léčbě vybrané diagnózy., Introduction: Information about actual results of provided surgical care is difficult to obtain. We tested the insurance company billing system in terms of data usability. Methods: 6,879 groin hernia surgeries carried out from 2008 to 2015 were analysed using registry data of one health insurance company. Results: Groin hernia surgeries were performed by 156 providers in the Czech Republic. Day surgeries were used in 3.8% cases (248), of which laparoscopic surgeries were used in 77% (193) of patients. The other 96.2% (6,631) surgeries were carried out on inpatients with the median hospital stay of 4 days. Of this number: open procedures without the use of a mesh were done in 36.54% (2,423), open procedures using a mesh in 19.84% (1,316), and laparoscopic procedures in 43.61% (2,892) cases. Reoperations for recurrence after hernia repair were done in 1.85% (127) of all cases, while 2.76% (66) reoperations were done after open procedures without the use of a mesh, 1.66% (22) after open procedures using a mesh, and 1.19% (37) after laparoscopic surgeries. The prediction rate for recurrent hernia incidence within 5 years was 2.42% in all cases. Others parameters were also evaluated.
