Je rozdíl v cestě podání a mechanizmu syntézy substitučních látek?, Cardiovascular diseases count among common causes of death despite the improvement of their diagnostics, treatment and knowledge of etiopathogenesis. Ten percent of patients with acute myocardial infarction and unstable angina die to 90 days after the disease manifestation in Europe. During one year foUow-up the mortality represents 17 %. This values lead to scientific research development in the area of protective methods. One of them seems to be estrogen replacement, e.g. in menopausal women (HRT - hormone replacement therapy). The low estrogen level in men is investigated as a possible cause of higher incidence of cardiovascular diseases (estrogens are pro¬ duced from testosterone via aromatization) in comparison with women under 50 years of age. [1] Considering a hormone dehciency in elderly and old population (in men mainly partial androgen deficiency - PADAM, in women estrogen deficiency) an other question arises - is it effective to administer to men „women protective hormones", which decrease a risk of cardiovascular diseases? Have estrogens protective effects on cardiovascular system? Are there differences in the way of administration and synthesis mechanism of replaced substances?, Zbyněk Veselský, and Lit. 11
Podáváme přehled současných názorů na diagnostiku a léčbu chronického srdečního selhání u starší populace. Principy diagnostiky a léčby se v zásadě neliší. U starších nemocných nalézáme větší zastoupení žen a větší zastoupení diastolického srdečního selhání. Výběr lékových skupin i dávky jsou stejné u mladší i starší populace, redukce dávek některých lékových skupin je spíše závislá na stavu renálních funkcí., We give an overview of the contemporary views of the diagnostics and the treatment of chronic heart failure at the aged population. The diagnostical and therapeutical principles do not differ in principal. Elder patients are more represented by women and there is a bigger representation of the diastoUc heart failure. The choice of drug groups and of the doses is the same at younger and older population, the reduction of the doses of some drug groups depends more on the condition of renal functions., Jindřich Špinar, Jiří Vítovec, Lit: 20, and Souhrn: eng
