Empagliflozin je nový lék používaný ke snížení hyperglykemie u pacientů s diabetem 2. typu. Patří do nejnovější třídy antidiabetik, tzv. gliflozinů, které zabraňují zpětnému vstřebávání glukózy prostřednictvím inhibice sodíko-glukózového transportéru SGLT2. Způsobují tak terapeutickou glykosurii, díky níž dojde ke ztrátě přibližně 70 g glukózy denně. Tím dochází nejen k poklesu glykemie, ale i k poklesu tělesné hmotnosti, protože tato vyloučená glukóza nemůže být použita jako energetický substrát. Ve studiích fáze 3 byla prokázána terapeutická účinnost empagliflozinu v monoterapii, kombinaci s ostatními perorálními antidiabetiky i inzulinem. Dalším příznivým účinkem empagliflozinu je mírný pokles krevního tlaku. Empagliflozin byl v Evropské unii schválen k léčbě pacientů s diabetem 2. typu v květnu 2014, v ČR je dostupný od října 2014 a od 1. února 2015 je empagliflozin (pod obchodním názvem Jardiance?) částečně hrazen ze zdravotního pojištění pro léčbu pacientů s diabetem 2. typu buď v kombinaci s metforminem, nebo v kombinaci s inzulinem (s nebo bez metforminu)., Empagliflozin is a new medicine used to reduce hyperglycemia in patients with type 2 diabetes. It belongs to the most advanced class of antidiabetic drugs, known as gliflozins, which prevent reabsorption of glucose through inhibiting SGLT2 sodium-glucose cotransporter. Thereby they cause therapeutic glycosuria, thanks to which a loss of approximately 70 g of glucose per day occurs. This not only effects the decrease in glycemia, but also the loss of body mass, since this excreted glucose cannot be used as an energetic substrate. The studies within phase 3 have proven the therapeutic efficacy of empagliflozin in monotherapy, in combination with the other oral antidiabec drugs and insulin. Another favourable effect of empagliflozin is a slight decrease in blood pressure. Diabetická nefropatie postihuje 20–40 % pacientů s diabetem a je hlavní příčinou selhání ledvin. Diabetici s alterovanou funkcí ledvin mají omezené terapeutické možnosti vzhledem k riziku akumulace perorálních antidiabetik a jejich metabolitů při redukované glomerulární filtraci. Dobrá metabolická kontrola je důležitá v časných fázích nefropatie ke snížení rizika progrese a ve stadiu selhání ledvin redukuje riziko progrese aterosklerózy a zlepšuje životní prognózu. Metabolizmus inzulinu je při renálním selhání změněn, clearance inzulinu je prodloužena a zvyšuje se riziko hypoglykemií. Inzulinová analoga krátkodobě působící mají rychlejší absorpci a dlouhodobě působící nižší riziko hypoglykemií. Mohou tak pozitivně ovlivnit kompenzaci diabetu u pacientů s diabetem a renální dysfunkcí., The main cause of renal failure is diabetic nephropathy which affects 20–40 % of diabetic patients. Diabetics with altered renal function have restricted therapeutic options due to the risk of accumulation of oral antidiabetic drugs and of their metabolites at a reduced glomerular filtration rate. Good metabolic control is very important during the early phases of nephropathy for reducing the risk of progression and in the stage of renal failure reduces the risk of progression of atherosclerosis and improves life prognosis. Snížení kardiovaskulárního rizika je hlavním cílem léčby diabetes mellitus 2. typu. V současné době je intenzivně studován vliv konkrétních typů antidiabetické léčby na kardiovaskulární komplikace. Inkretinová léčba diabetu založená na účincích glukagon-like peptidu 1 (GLP-1) respektive stimulace jeho receptoru GLP-1 agonisty představuje z pohledu možného snížení rizika kardiovaskulárních komplikací jeden z nejnadějnějších přístupů. Experimentálními studiemi bylo prokázáno, že podávání GLP-1 či jeho agonistů vede ke zlepšení endoteliální dysfunkce, snížení krevního tlaku a k ochraně myokardu při experimentálně vyvolané ischemii. Klinické studie s GLP-1 agonisty konzistentně prokazují při dlouhodobém podávání kromě zlepšení kompenzace diabetu také pokles krevního tlaku, snížení hmotnosti a zlepšení lipidogramu spolu s mírným zvýšením tepové frekvence. V tomto článku se zaměřujeme na kardiovaskulární účinky GLP-1 agonisty liraglutidu. Předběžná analýza výskytu kardiovaskulárních komplikací ve studiích III. fáze s liraglutidem potvrzuje kardiovaskulární bezpečnost této léčby. The effects of various antidiabetics on the cardiovascular complications are currently under careful scrutiny. Incretin-based therapy that utilizes the effects of glucagon-like peptide 1 (GLP-1) or stimulation of its receptor by GLP-1 receptor agonists represents one of the most promising approaches from the potential cardiovascular risk reduction point of view. Experimental studies have shown that the GLP-1 and GLP-1 agonists treatment improves endothelial function, decrease blood pressure and protects myocardium during experimentally-induced ischemia. Clinical studies with GLP-1 receptor agonists consistently show that, in addition to good antidiabetic efficacy, its long-term administration decreases blood pressure, body weight and improves circulating lipid levels while slightly increasing heart rate. In this paper, we focus on the cardiovascular effects of GLP-1 receptor agonist liraglutide. Preliminary analyses of cardiovascular complications in phase III trials with liraglutide indicate its good cardiovascular safety. Diabetes mellitus 2. typu je heterogenní onemocnění, které vyžaduje personalizovaný přístup k léčbě s cíli přizpůsobenými možnostem a schopnostem pacienta a jeho dalším onemocněním tak, aby se zabezpečila dobrá kompenzace diabetu bez rizika hypoglykemických příhod a vývoje nebo progrese pozdních diabetických komplikací. Doporučení pro léčbu diabetu zařazují do 2. linie terapii bazálními inzuliny jako jednu z možností, a to ihned při selhání terapie metforminem a dietou. Diabetes 2. typu se stal v posledních 50 letech pandemickým onemocněním. Jeho výskyt roste nejrychleji v populaci seniorů, tj. osob starších 65 let. V řadě zemí žije v současnosti 1/4 osob trpících diabetem starších 65 let. Obor geriatrie se zabývá řadou odlišností u seniorů, které často vedou k poněkud rozdílným terapeutickým postupům ve srovnání s léčbou ostatních dospělých pacientů. Cílem této práce je poukázat na odlišnosti léčby seniora trpícího diabetem. Intenzita léčby diabetu u seniorů je definovaná především výskytem příznaků způsobených dekompenzací diabetu, které zhoršují kvalitu života a pravděpodobně zvyšují mortalitu. Léčebné cíle vyjádřené hodnotami HbA1c, glykemií na lačno a postprandiální, by měly být stanoveny individuálně dle věku, vstupní hodnoty HbA1c, přítomných komorbidit a míry křehkosti seniora. Snaha o redukci tělesné hmotnosti u osob vyššího věku je pravděpodobně nevhodná a možná dokonce škodlivá, fyzický pohyb naopak v každém věku snižuje mortalitu a zpomaluje svalový katabolizmus. Ideální je prostá chůze 20–30 min/den. Léčba sulfonylureou je s výjimkou „zdatných seniorů“ bez renální insuficience nevhodná, a možná dokonce škodlivá. Významně zvyšuje výskyt hypoglykemií a velmi pravděpodobně i celkovou mortalitu. Základem léčby diabetu u seniora je snaha o jakoukoliv pravidelnou tělesnou aktivitu. Z medikamentózní léčby je vhodné volit metformin nebo gliptin s pravidlem „start low, go slow“, tzn. začínat léčit nízkými dávkami léků a pozvolna je zvyšovat. Hlavním cílem léčby je udržet co nejdéle dobrou kvalitu života bez symptomů spojených s hyperglykemií s minimalizováním rizika vzniku hypoglykemie., Type 2 diabetes has become a pandemic disease over the past 50 years. Its incidence increases the most rapidly in the senior population, i.e. among people older than 65. In a number of countries 1/4 of the people with diabetes are now older than 65 years. Geriatrics now examines numerous differences regarding the senior patients, which often lead to somewhat different therapeutic procedures as compared to the treatment of other adult patients. This paper aims to show some different aspects of the treatment of an elderly patient with diabetes. The intensity of diabetes treatment in the elderly is mainly defined by the incidence of symptoms caused by diabetic decompensation which negatively affect quality of life and are likely to increase mortality. The treatment goals expressed by HbA1c, fasting and post-prandial glycemia, should be set individually based on age, initial HbA1c, present comorbidities and the level of frailty of an elderly patient. An effort to reduce weight regarding people at an older age is probably inappropriate and maybe even harmful, while physical activity reduces mortality and slows muscle catabolism at every age. Ideal is normal walking for 20–30 minutes a day. Except for “very fit elders” without renal insufficiency, the sulfonylurea treatment is unsuitable and perhaps even harmful. It significantly increases the incidence of different types of hypoglycemia and very likely overall mortality as well. The basis of diabetes treatment for the elderly is the effort to perform any regular exercise. In regard to medication treatment it is recommended to choose metformin or gliptin following the rule “start low, go slow“, i.e. start with low medication doses and increase them at a slow pace. Inkretinový hormon GLP1 (glucagon-like peptid 1) má kromě vlivu na pankreas také efekty systémové. Renální exprese receptorů pro GLP1 a DPP4 je popisována v řadě experimentálních studií. Aktivace receptorů pro GLP1 v ledvinách má diuretické a natriuretické efekty zřejmě přes renální tubulární buňky a sodíkové transportéry. Preklinicky inkretinová terapie snižovala albuminurii, ovlivňovala glomerulosklerózu, oxidativní stres a fibrózu v ledvinách. Diabetická nefropatie je hlavní příčinou selhání ledvin. Při renální insuficienci jsou možnosti funkční a zároveň bezpečné kompenzace diabetu omezené. SGLT2 a DPP4 inhibitory jsou nová účinná perorální antidiabetika s nízkým rizikem hypoglykemie a příznivým nebo neutrálním vlivem na tělesnou hmotnost. Těmito vlastnostmi se zásadně odlišují od častěji používaných derivátů sulfonylurey. Současná léčba diabetu 2. typu je poměrně komplikovaná a pozice těchto nových léků v léčebném schématu zatím není jednoznačně definována. Personalizovaná léčba diabetu na základě molekulárních markerů zatím není v praxi možná. Často až klinické zkušenosti určují postavení nových antidiabetik v léčbě pacientů s diabetem 2. typu. Tyto zkušenosti jsou pak postupně modifikovány novými klinickými studiemi. V současné době stojí lékař před otázkou, který z moderních léků volit. Léčba diabetes mellitus 2. typu je v současnosti účinná, ale bohužel ještě není ideální. Novou skupinu perorálních antidiabetik představují DPP4 inhibitory, tzv. gliptiny. V roce 2015 byla ukončena již 3. velká mortalitní klinická studie s gliptiny u pacientů s diabetes mellitus 2. typu a kardiovaskulárním onemocněním. Jedná se o studie TECOS, SAVOR a EXAMINE s léky sitagliptin, saxagliptin a alogliptin. Studie zahrnuly téměř 37 000 nemocných. Tyto studie prokázaly kardiovaskulární bezpečnost DPP4 inhibitorů, přesto ale zůstává otevřená otázka výskytu srdečního selhání. Prokázaná je i účinnost na snižování glykemie, resp. glykovaného hemoglobinu a bezpečnost z pohledu onemocnění pankreatu. DPP4 inhibitors (gliptins) are a new group of peroral antidiabetic drugs. The third clinical mortality study with gliptins in patients with diabetes mellitus type 2 was finished in 2015. The studies are known under acronym TECOS, SAVOR and EXAMINE and the tested drugs are sitagliptin, saxagliptin and alogliptin. The studies included about 37 000 patients. The studies confirmed the cardiovascular safety of the DPP4 inhibitors, but the question about increased heart failure remains open. The effectiveness of lowering glycaemia (glycated haemoglobin) was confirmed and also the pancreatic safety is confirmed. Diabetes mellitus a zejména diabetes mellitus 2. typu představuje jeden z nejvýznamnějších rizikových faktorů kardiovaskulárních onemocnění. Z hlediska ovlivnění KV rizika má enormní význam nejen pozitivní ovlivnění glykemie, ale také léčba diabetické dyslipidemie a arteriální hypertenze. Předložená práce poskytuje analýzu terapie antihypertenzivy a hypolipidemiky u všech diabetiků registrovaných u VZP v období let 2010–2013. V uvedeném období bylo s diagnózou diabetes mellitus léčeno 866 570 pojištěnců VZP, přičemž většinu tvořili diabetici nezávislí na inzulinu. Přibližně polovina pacientů byla dispenzarizována diabetology a polovina lékaři jiných specializací. Z antihypertenziv byli pacienti nejčastěji léčeni léky ovlivňujícími systém renin-angiotenzin, dále pak betablokátory a diuretiky. Preskripce hypolipidemické léčby, zejména preskripce statinů, v souladu s platnými doporučeními, pozvolna narůstá, nicméně i nadále zůstává nedostatečná (v roce 2013 zůstalo bez hypolipidemické léčby 43,6 % diabetiků z ambulancí diabetologů a 51,3 % diabetiků z ambulancí praktických lékařů!). V doplňující analýze dat za rok 2014 jsme zjistili, že v tomto roce bylo léčeno z celkového počtu osob s farmakologickou léčbou diabetu nějakým hypolipidemikem 57,3 % osob. Nedostatečné využití zaznamenává rovněž kombinační hypolipidemická léčba, dosud zastoupená zejména kombinací statinu s fibrátem. Nicméně v následujících letech očekáváme na základě pozitivních výsledků subanalýzy studie IMPROVE-IT nárůst kombinační hypolipidemické terapie, resp. kombinační léčby statiny v kombinaci s ezetimibem. To influence cardiovascular risk there is enormous important not only positive influence of glycemia, but also the treatment of diabetic dyslipidemia and hypertension. The present work provides an analysis of lipid-lowering and antihypertensive therapy for all diabetics registered with General Health Insurance Company in the period 2010–2013. In this time 866 570 patients with diabetes mellitus registered with General Health Insurance Company were treated, the majority of them were diabetics independent on insulin. Approximately half of the patients were observed by dialectologists and half of them by the doctors of other specialization. Out of antihypertensive medication, patients were most often treated by drugs that affect the renin-angiotensin system, as well as beta-blockers and diuretics. Prescription of the lipid-lowering therapy, especially prescription of statins, in accordance with the guidelines, is increasing, but remains insufficient (at 2013 43.6% diabetics treated by dialectologists and 51.3% diabetics treated by GP´s didn´t have lipid lowering therapy). Inadequate use of combination lipid-lowering therapy was recorded too, still represented mainly by combination of statin and fibrate, but in coming years we expect (based on the positive results of the subanalysis IMPROVE-IT study), an increase of combination therapy statin and ezetimibe.
