The study aimed to investigate the role of aggression and impulsivity in smoking among male adult students. It was hypothesized that there would be significant difference between smokers and non- smokers on the traits of aggression and impulsivity. The sample of two hundred male adult students was selected from different universities of Islamabad. Two subscales of Impulsiveness and Aggression were administered to measure the personality traits of aggression and impulsivity. Descriptive statistics and t-test was calculated for analysis of data. Results showed that there is significant differences in aggression and impulsivity between smokers and non-smokers. Aldosterone blocker Spironolactone has antiinflammatory, antiproliferative and antioxidative effects, that is why pathogenetically it is expedient to use it in complex therapy of rheumatoid arthritis. Material and methods: 46 patients with RA took 2550 mg/day of Spironolactone during 12 months as an addition to standard therapy, the comparison group consisted of 47 patients that got only standard therapy, all the patients were fully examined prior and post the treatment. Results: complex RA therapy leads to improved VAS, HAQ, the antioxidative potential index F; decreased blood concentrations of TNFα, ICAM1, FGF and VEGF in contrast to standard therapy. Complex therapy made reduced the DAS 28 more > 0.6. Conclusions: applying of Spironolactone in complex therapy of rheumatoid arthritis contributes to more pronounced improvement in indices of articular syndrome and patients life quality, reduce of antiinflammatory, antiproliferative and angiogenic cytokines, and more effectively reduces the activity of the disease comparing to standard therapy., Elena Komarova, Borys Rebrov, and Literatura
Cíle: Pseudoaneurysma intrarenálních arterií (RAP) může představovat život ohrožující komplikaci, k níž může dojít po parciální resekci ledviny. Autoři hodnotí incidenci RAP, u něhož zatím nedošlo k ruptuře (detekováno pomocí trojrozměrné CT angiografie – CTA), časně po minimálně invazivní parciální resekci ledviny (včetně laparoskopické a robotické operace). Dále hodnotí možnost využití nefrometrického skóre při predikci vzniku RAP. Metody: V období od února 2012 do listopadu 2013 podstoupilo 101 pacientů s renálními tumory minimálně invazivní parciální resekci ledviny. Tři až čtyři dny po operaci byla provedena CTA a radiologové (zaslepeně) diagnostikovali RAP. Pomocí logistického regresního modelu analyzovali autoři faktory, které mohou potenciálně ovlivňovat vznik RAP. Výsledky: Míra incidence RAP časně po minimálně invazivní parciální resekci (MIPR) byla nečekaně vysoká – 21,7 % (detekováno pomocí CTA). U pacientů s RAP byl přítomen signifikantně větší tumor (p = 0,02) a významně vyšší skóre parametru N indikující blízkost tumoru k renálnímu sinu (p = 0,01) než u pacientů, u nichž nedošlo ke vzniku RAP. Multivariátní analýza však prokázala, že ani celkové nefrometrické skóre ani hodnota jednotlivých komponent neměly významný vliv na vznik RAP. Velikost tumoru představovala jediný významný nezávislý prediktor vzniku RAP po minimálně invazivní resekci ledviny. Závěr: Parametr N nefrometrického skóre představuje významný prediktor vzniku RAP podle univariátní analýzy, nikoli však podle multivariátní analýzy. Komponentu N nefrometrického skóre tedy lze považovat za jeden z faktorů užitečných pro predikci vzniku RAP., Objectives: Renal artery pseudoaneurysm (RAP) can be a life threatening complication after partial nephrectomy. We investigated the incidence of unruptured RAP detected by 3-dimensional computed tomography arteriography (CTA) in the early period after minimally invasive partial nephrectomy (MIPN), including laparoscopic and robotic partial nephrectomy. We also examined the usefulness of nephrometry scoring system to predict the occurrence of RAP. Methods: From February 2012 to November 2013, 101 patients underwent MIPN for renal masses. CTA was performed 3–4 days after surgery, and the radiologists made a diagnosis of RAP in the blinded manner. The factors influencing the occurrence of RAP were analyzed with the logistic regression model. Results: The incidence of RAP was unexpectedly high at 21.7% when detected by CTA in the early period after MIPN. The RAP group showed a significantly larger tumor size (p = 0.02) and a significantly higher N component score (p = 0.01) compared to the No-RAP group. However, Neither the sum of nephrometry score nor each component showed significant influence on the occurrence of RAP according to the multivariate analysis. Tumor size was the only significant independent factor to predict the occurrence of RAP after MIPN. Conclusions: The N component of the nephrometry scoring system is a significant predictor for RAP by the univariate analysis, but not by the multivariate analysis. The condition of the protective barrier of the stomach in patients with duodenal ulcer associated and non-associated with H. pylori infection has been studied. The state of gastric mucosal barrier was studied in 72 patients with peptic ulcer disease association with H. pylori and 26 patients without H. pylori by determining the content of insoluble glycoproteins and its fractions in the basal portion of the gastric juice. In a comparative perspective the state of these indicators were studied depending on the association with H. pylori and the severity of the disease. In patients with H. pylori, damages of protective mucus barrier are characterized by the significant shortage of fucose, which is considered to be the main component of the insoluble glycoproteins. In patients without H. pylori, status of mucosal barrier is characterized by uniform and less significant reduction in the content of fractions of insoluble glycoproteins. In this patients the reduction of insoluble glycoproteins is not dependent on the frequency of exacerbation. In this article, the results of the morphological study of the cerebral arteries in the patients with the hemorrhagic ischemic stroke on the background of metabolic syndrome were submitted. We established that under hemorrhagic stroke on the background of metabolic syndrome one could observe the atherosclerotic damages in the form of plaques, hyalinosis of vessels walls, destructive and necrotic changes of the middle coat of vessel wall, which are, in our opinion, the main reason in morphogenesis of hemorrhagic stroke. The atherosclerotic affection of the cerebral vessels with the lipidosis and fibrotic plaques in the vessel wall, which caused in disorganization of the vessel wall in the form intimae proliferation, structural reconstruction of the internal elastic membrane which resulted in narrowing of the vessel lumen and development of ischemia is the morphological basis for the ischemic stroke with the metabolic syndrome.
