Retrospektivní studie u žen do 49 let analyzovala novotvary následující po primárních nádorech cervixu (CC), dělohy (CU), vaječníků (CO) a primární novotvary před těmito gynekologickými nádory (GC), hlášené v Národním onkologickém registru ČR v letech 1976–2010. Celkem 3 727 pacientek těchto tří diagnóz představovalo 12,1 % z 30 794 nově evidovaných GC, spojených s 4 160 dalšími novotvary. Jejich vývoj se týkal a) 1 339 žen s primárním CC, 715 s CU, 664 s CO (s následnými 1 509 a 831, respektive 758 dalšími novotvary), b) 263 žen s následným CC, 212 s CU, 534 s CO (s předcházejícími 274 a 223, respektive 565 primárními novotvary). Průměrný interval vzniku následných novotvarů byl 14,2 roku po CC, 13,6 roku po CU a 10,2 roku po CO. Nejvíce z 3 098 následných nádorů bylo 21 % trávicího traktu, 16 % prsů, 13 % kůže, 11,7 % rodidel, 9,7 % dýchacích a 7,8 % močových cest; z nich během prvního roku po primárním GC bylo nejvíce nádorů vaječníků, endometria, cervixu a kolorekta. Nejvíce z 1 063 primárních novotvarů před GC bylo 26,7 % rodidel, 24,3 % prsů a 14,8 % trávicího traktu. Během 35 let počet žen s primárními GC klesal, zejména v zastoupení jejich časných klinických stadií, žen s následnými GC rostl, zejména jejich pokročilých stadií. Časná stadia byla evidována u 70,4 % žen s primárními GC a 54 % s následnými GC, pokročilá stadia u 29,5 % žen s následnými GC a 13,3 % s primárními GC, neznámá stadia zahrnovala asi 16,5 %. Ze studie se odhaduje asi 1100 případů pokročilých stadií u žen do 49 let přežívajících s GC v letech 1976–2010. Většina z hodnocených kritérií byla nepříznivá u žen s karcinomy ovarií. Zejména těmto případům v pokročilých stadiích by měly zabránit dispenzární opatření a preventivní intervence. Omezené finanční zdroje a přetrvávající rizika životního stylu neumožňují zpomalit v blízké budoucnosti nárůst primárních a následných novotvarů u onkologicky přežívajících žen nejen v gynekologii., The neoplasms following primary cancers of cervix (CC), uterus (CU), ovary (CO), and primary neoplasms before these gynaecological cancers (GC), were analyzed in a retrospective study in survived females aged 0–49 years, notified in the National Czech Cancer Registry between 1976 and 2010. A total 3,727 patients of these three diagnoses, presented 12.1 % of 30,794 newly registered GC, were associated with 4,160 other neoplasms. Their development was concerned with a) 1,339 females with primary CC, 715 with CU and 664 with CO (followed by 1,509 and 831, respectively 758 other neoplasms) and b) subsequent 263 CC, 212 CU and 534 CO (preceded by 274 and 223, respectively 565 primary neoplasms). The average interval of occurrence of subsequent neoplasms was 14.2 years after CC, 13.6 years after CU and 10.2 years after CO. The most frequent of 3.098 subsequent neoplasms were 21 % of digestive tract, 16 % breast, 13 % skin, 11.7 % female genital, 9.7 % respiratory and 7.8 % urinary organs; the most frequent during the first year after primary GC were those of the ovarial, endometrial, cervical and colorectal cancers. The most frequent of 1,062 primary neoplasms before GC were 26.7 % cancers of female genital organs, 24.3 % breast and 14.8 % digestive tract. During 35 years the number of females with primary GC has decreased, especially of their representation at the early clinical stages, the females with subsequent GC has increased, especially at the advanced stages. There were recorded early stages in 70.4 % females with primary GC and 54 % with subsequent GC, advanced stages in 29.5 % females with subsequent GC and 13.3 % with primary GC, the unknown stages included about 16.5 %. In our study are estimated about 1100 cases of advanced stages in females aged 49 years survived with GC in 1976–2010. Most of the evaluated criteria were unfavorable among females with ovarian cancers. Mainly the cases at advanced stages must be prevented by the dispensary guidelines and preventive interventions. Úvod: Odběr ledvin z dárce s nezvratnou zástavou oběhu (DCD – donor after circulatory death) je důležitou součástí transplantačních programů v celosvětovém měřítku. V České republice byla první transplantace ledvin z DCD úspěšně provedena v roce 2002. Autoři diskutují vlastní a literární zkušenosti s tímto programem. Snahou je zvýšit potenciál odběru ledvin z DCD v České republice. Metoda: Od roku 2002 do roku 2015 bylo v plzeňském Transplantačním centru odebráno celkem 44 ledvin z DCD. Technikou byl „in situ“ odběr pomocí „double baloon triple lumen“ katetru při zachování 5–10 min. „no-touch“ intervalu. Metoda hypotermické pulzatilní perfuze byla použita k testování viability odebraných ledvin. Transplantováno bylo celkem 28 ledvin příjemcům o průměrném věku 51,1 roku (26–73 let). Dle maastrichtských kritérií bylo 16 (57,1 %) ledvin ve skupině II, 8 (28,6 %) ve skupině III a 4 (14,3 %) ve skupině IV. Výsledky: 30denní pooperační mortalita byla 0 a morbidita 10,7 tj. 14,3 % (N=4). Primární afunkce ledviny byla přítomna u 2 (7,1 %), opožděný nástup funkce pak u 5 (17,9 %) nemocných. Jeden rok, 5 a 10 let po transplantaci žije 100 %; resp. 86,4 % a 76,7% nemocných a ve stejných intervalech je funkčních 92,9; 69,6 a 61,9 % transplantovaných ledvin. Dlouhodobé výsledky jsou plně srovnatelné s transplantacemi ledvin od dárců se smrtí mozku. Závěr: DCD jsou významným zdrojem ledvin pro transplantace. Transplantace ledvin z DCD je metodou logisticky, ekonomicky a personálně náročnou s velmi dobrými dlouhodobými výsledky., Introduction: Kidney procurement from donors after circulatory death (DCD) is an important part of worldwide transplantation programmes. The first kidney transplantation from DCD was successfully performed in the Czech Republic in 2002. Method: Forty four kidneys from DCD were procured in the Transplant Centre of Pilsen between 2002 and 2015. We used the technique of “in situ“ procurement with the double balloon triple lumen catheter and 5−10 minutes of the no-touch interval. The method of pulsatile hypothermic perfusion was used to test the viability of the kidneys. Twenty eight recipients with mean age 51.1 (26−73) years were transplanted. Sixteen (57.1%) kidneys were from the 2nd, 8 (28.6%) from the 3rd and 4 (14.3%) from the 4th category according to the Maastricht criteria. Results: 30-day mortality and morbidity rates were 0 and 10.7% i. e.14.3% respectively (N=4). Primary non-function was presented in 2 (7.1%), and delayed graft function in 5 (17.9%) cases. One, five and ten years of recipient and graft survival rates were 100%, 86.4% and 76.7%; and 92.9%, 69.6% and 61.9%, respectively. The long-term results are fully comparable with kidneys transplanted from donors after brain death. Conclusion: DCD are an important source for kidney transplantation. Práce prezentuje prenatální diagnostiku, možnosti terapie a následnou péči u plodů a novorozenců s kongenitálním chylothoraxem na podkladě zhodnocení 14 případů na naší klinice v letech 1993 až 2012. Povšechný hydrops plodu byl popsán ve 4 případech (29 %), v 10 případech (71 %) byla provedena odlehčovací amniodrenáž a ve 3 případech (21 %) prenatální odlehčovací punkce hrudníku. Spontánně bylo porozeno 11 plodů (78 %), hrudní drenáž byla nezbytná v průměru 9 dnů po porodu. Mortalita dosáhla v souboru 29 % (4 případy). V článku popisujeme vyšetřovací algoritmy, terapeutické prenatální i postnatální postupy a je zdůrazněna nutnost mezioborové spolupráce., The work presents prenatal diagnosis, treatment and care options in fetuses and neonates with congenital chylothorax based on a detailed evaluation of 14 cases recorded in our department between years 1993 and 2012. Generalized fetal hydrops was described in 29% of cases, therapeutic amnio drainage performed in 71% and prenatal thoracentesis in 21% of cases. 78% of fetuses were delivered spontaneously; thoracic drainage was necessary nine days after birth in average. Mortality reached 29%. V databázi Globocan 2008 (IARC) bylo za rok 2008 z celosvětového počtu 12,663 milionů nových nádorů odhadováno 2,445 milionů v EU, ze 7,565 milionů zemřelých téměř 1,235 milionů v EU a z pětileté prevalence 28,8 milionů asi 6,617 milionů v EU. Početnější prevalence se týkala ve světě 18 % nádorů prsů, 11 % kolorekta, 11 % prostaty, 6 % plic, 5,5 % žaludku, 5,5 % cervixu, 4 % měchýře a 4 % dělohy, v EU 20 % ZN prsů, 14 % kolorekta, 18 % prostaty, 5 % plic, 4 % melanomu, 3 % ledvin, 3 % dělohy a 3 % NH lymfomu. V roce 2030 mohou počty nových nádorů dosáhnout ve světě 21 milionů případů, v EU 3 miliony, v ČR 93 tisíc (dg. C00-D09). Z předběžných celkových výdajů 288 572 mil. Kč na zdravotnictví za rok 2011 uhradily zdravotní pojišťovny 217 653 mil., z toho 19 217 mil. na onkologii. V letech 1990–2011 vzrostly celkové náklady na zdravotnictví o 257 534 mil. Kč s podílem 7,58 % na hrubém domácím produktu. Průměrné výdaje VZP na léčbu jednoho nádoru v roce 2010 dosáhly 19 030 Kč s nejvyššími náklady u novotvarů mízní a krvetvorné tkáně, mozku a CNS, trávicího ústrojí, úst a hltanu, dýchacích cest. Výkonnost globální ekonomiky nedosahuje potřebných prostředků na zdravotnictví. Český veřejný dluh překračuje 1 680 miliard Kč v době dokončení tohoto článku. I když celkové výdaje obyvatel na zdravotní péči rostou, zdá se, že jeho pokračující zadlužování zvýší také dluhy nemocnic a zdravotních pojišťoven. Dostupné počty nádorů a výdajů na zdravotnictví a jejich zvyšování potvrzují nutnost důsledné prevence rakoviny, efektivní přidělování a využívání prostředků na zdravotní péči., In 2008 (Globocan 2008; IARC), 12.663 million new cancer cases occurred worldwide and, of them, 2.445 million in the EU. Here preliminary total health care expenditures in 2011 were 288,572 million CZK; of these, health insurance companies covered 217,653 million, with 19,217 million per one cancer treatment. Between 1990 and 2011, the total health expenditures increased by 257,534 million CZK, accounting for 7.58 % of the gross domestic product. In 2010 the General Health Insurance Company paid on average 19,030 CZK for the treatment of one cancer case, with the highest expenditures for neoplasms of the lymphoid and haemotopoietic tissues, brain and central nervous system, digestive organs, oral cavity and pharynx, and respiratory organs. The global economic performance cannot provide enough resources for public health care. The Czech public debt exceeded 1,680 billion CZK at the time this article was written. Although out-of-pocket money paid by cancer patients for treatment is growing, the continuing public indebtedness is likely to increase debts of hospitals and health insurance companies. Léčba diabetes mellitus 2. typu je v současnosti účinná, ale bohužel ještě není ideální. Novou skupinu perorálních antidiabetik představují DPP4 inhibitory, tzv. gliptiny. V roce 2015 byla ukončena již 3. velká mortalitní klinická studie s gliptiny u pacientů s diabetes mellitus 2. typu a kardiovaskulárním onemocněním. Jedná se o studie TECOS, SAVOR a EXAMINE s léky sitagliptin, saxagliptin a alogliptin. Studie zahrnuly téměř 37 000 nemocných. Tyto studie prokázaly kardiovaskulární bezpečnost DPP4 inhibitorů, přesto ale zůstává otevřená otázka výskytu srdečního selhání. Prokázaná je i účinnost na snižování glykemie, resp. glykovaného hemoglobinu a bezpečnost z pohledu onemocnění pankreatu. Klíčová slova: bezpečnost – gliptiny – účinnost, The treatment of diabetes mellitus type 2 is effective, but still is not optimal. DPP4 inhibitors (gliptins) are a new group of peroral antidiabetic drugs. The third clinical mortality study with gliptins in patients with diabetes mellitus type 2 was finished in 2015. The studies are known under acronym TECOS, SAVOR and EXAMINE and the tested drugs are sitagliptin, saxagliptin and alogliptin. The studies included about 37 000 patients. The studies confirmed the cardiovascular safety of the DPP4 inhibitors, but the question about increased heart failure remains open. The effectiveness of lowering glycaemia (glycated haemoglobin) was confirmed and also the pancreatic safety is confirmed.
