- The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. During colonoscopical inspection, in 1218 weeks after the first infusion at 80% of the patients had observed disappearance of ulcers and erosion. In terms of 6 months of supervision of relapse of disease was not noted. All new directions in treatment of the IDI, apparently, are perspective, but while are proved only theoretically and had no practical development. - The authors demonstrate the possibility of improving surgical results by the reduction of perioperative bleeding in thoracic surgery associated with extended resection procedures. We focused on patients in whom the expected perioperative blood loss was greater than 500 ml. The first group consisted of patients with lung cancer stage III A after neoadjuvant chemotherapy had been indicated to extend the resection procedure. The second group consisted of patients with chest wall and mediastinum tumors of various etiologies. The third group consisted of patients with post-inflammatory thoracic complications in whom combined decortication and pleurectomy was necessary. - Cíl: Cílem studie bylo vyhodnotit vztah mezi hodnotami indexu IgG a počtem oligoklonálních IgG pásů (OCB) v mozkomíšním moku (CSF) pacientů s roztroušenou sklerózou (SM). Materiál a metodika: Soubor 150 pacientů sestával ze 41 mužů a 109 žen (ve věku od 18 do 68 let, průměrný věk byl 36,6 ±10,1 let). U mozkomíšního moku odebraněho lumbální punkcí byla posouzena intratekální syntéza s využitím indexu IgG a stanovením OCB. Počet alkalických OCB v CSF byl stanoven metodou izoelektrické fokusace. K vyhodnocení statistické významnosti výsledků byla rovněž použita Pearsonova korelační analýza, x2 test homogenity, Mannův-Whitneyův test, párový t-test (parametrický) a jednovýběrový Wilcoxonův test (neparametrický). Výsledky: Pozitivní korelace mezi IgG indexem a počtem OCB nebyla nalezena. Ani Mannův-Whitneyův test neprokázal podstatný rozdíl v hodnotách indexu IgG u pacientů s OCB > 2 a < 2. Závěr Studie nepotvrdila korelaci mezi hodnotami IgG indexu a počtem OCB v CSF u pacientů s roztroušenou sklerózou., Objective: The aim of this study was to assess the correlation between IgG index values and the number of the oligoclonal IgG bands (OCB) in the cerebrospinal fluid (CSF) of patients with multiple sclerosis (MS). Material and Methods: The set of 150 patients consisted of 41 males and 109 females (aged 18–68, mean 36.6 ± 10.1 years). The CSF collected by a lumbar puncture was examined evaluating intrathecal synthesis using the IgG index and determining OCB. The number of alkaline OCB in the CSF was assessed using the method of isoelectric focusation. Pearson's correlation analysis, and homogeneity χ2 test , Mann-Whitney test, paired-sample t-test (parametric) and Wilcoxon signed-ranks test (nonparametric) were used to evaluate the statistical significance of the results. Results: No positive correlation between the IgG index and the number of OCB was found. Mann-Whitney test also did not demonstrate any significant difference of the IgG index values in patients both with the OCB number ≥ 2 and < 2. - Tuberkulózní postižení střev je v rozvinutých zemích vzácné onemocnění, které postihuje zejména imigranty a imunokompromitované pacienty. Klinický obraz může imitovat nitrobřišní infekce, nádorová onemocnění či Crohnovu chorobu. Diferenciální diagnostika Crohnovy choroby a břišní tuberkulózy může být pro klinika a patologa svízelná, v obou případech se jedná o chronický granulomatózní zánět s podobnou symptomatologií. Prezentujeme případ 53letého pacienta s perforační peritonitidou při tuberkulóze střeva, u kterého byla původně mylně diagnostikována Crohnova choroba. Průběh choroby, rozsah a povaha nálezu byly v každé fázi léčby příčinou kritických komplikací. S touto zkušeností předáváme i doporučení: břišní tuberkulózu zvažovat v diferenciálně diagnostické rozvaze každého chronického střevního onemocnění. Klíčová slova: střevní tuberkulóza − Crohnova choroba − kritické komplikace, Intestinal tuberculosis is a rare disease in developed countries, affecting mainly immigrants and immunocompromised patients. Clinical presentation may mimic many other abdominal diseases such as intra-abdominal infections, tumors, and Crohn´s disease. Differential diagnosis of Crohn´s disease and intestinal tuberculosis poses a dilemma to clinicians and pathologists as both are chronic granulomatous disorders with similar clinical features. We report the case of a 53 year-old man presenting with perforated intestinal tuberculosis in which the initial diagnostic work-up suggested Crohn´s disease. The severity of the findings resulted in critical complications during the course of treatment. This case points out the need for awareness of intestinal tuberculosis in the differential diagnosis of chronic intestinal disease. - Seminal vesicle secretion is important for increasing the stability of sperm chromatin, inhibition of the immune activity in the female reproductive tract and so on. Metronidazole (MTZ), a drug used for treatment of infections caused by anaerobic bacteria and protozoa, may have negative effects on the genital gland including the seminal vesicles. Curcumin exhibits antioxidant as well as anti-inflammatory properties. The present study aims to evaluate the negative effects of MTZ on the seminal vesicle structure and ameliorative effects of curcumin using stereological methods. Thirty balb/c mice were divided into six groups. The control group was received distilled water. The second and the third received higher doses of MTZ (500 mg/kg body weight/day) and MTZ (500 mg/kg/day) + 100 mg/kg/day curcumin, respectively. The fourth and the fifth were treated with lower doses of MTZ (165 mg/kg body weight/day) and MTZ (165 mg/kg body weight/day) + curcumin (100 mg/kg body weight/day), respectively. The sixth group received 100 mg/kg body weight/day curcumin. All the administrations were done by oral gavages for 14 days. After 30 days, seminal vesicles were removed. Stereological study of the seminal vesicle structure revealed a significant reduction in gland and vesicular fluid volume in MTZ-treated (higher or lower doses) animals. Curcumin protected the reduction of both parameters in therapeutic-dose treated animals. Metronidazole treatment does not induce structural changes in the seminal gland; however, it can have a significant impact on its secretion ability. - Úvod: Česká republika zaujímá v incidenci kolorektálního karcinomu (KRK) čelní místo ve světě. Radikální odstranění nádoru představuje nejúčinnější část v rámci současné multimodální terapie. Autoři prezentují průběžné výsledky roboticky asistované léčby u nemocných s karcinomem rekta (KR). Metoda: Vlastní soubor obsahuje 61 roboticky operovaných pacientů pro KR. U posledních 31 pacientů byla data získávána prospektivně. Analyzovány byly epidemiologické údaje, perioperační ukazatele, komplikace a onkologické parametry. Výsledky: Roboticky asistovanou operační léčbu pro karcinom rekta na našem pracovišti podstoupilo 61 nemocných: 34 mužů a 27 žen průměrného věku 62 let (33–80 let). Neoadjuvantní onkologická léčba byla indikována u 46 % pacientů. Průměrná krevní ztráta činila 187 ml, u tří nemocných byly podány transfuzní přípravky. Šest operací jsme byli nuceni konvertovat, u 16 pacientů jsme zaznamenali komplikace. Insuficience anastomózy se vyskytla u 10 % pacientů, u 4 nemocných si stav vyžádal operační léčbu. Žádný nemocný nezemřel. Rekurenci maligního onemocnění jsme diagnostikovali u 3 (5 %) pacientů. U 27 nemocných byla (od roku 2013) stanovena kvalita mezorektální excize (ME) a metrické stanovení cirkumferentního resekčního okraje – (y)pCRO. Ve dvou případech byla potvrzena pozitivita (y)pCRO a přibližně u čtvrtiny operantů nebyla ME kompletní. Závěr: Chirurgická léčba je v interdisciplinárním terapeutickém procesu u KR klíčová. Naše průběžné výsledky jsou srovnatelné se závěry některých literárně uveřejněných sdělení. Da Vinci systém je bezpečným manipulátorem v léčbě KR a operatérovi přináší nesporné výhody při preparaci ve stísněném pánevním prostoru. V současnosti chybí zhodnocení přínosu robotické operativy pro pacienta v oblasti břišní chirurgie z dlouhodobého hlediska a s dostatečným počtem operantů, resp. vysokým stupněm evidence based medicine (EBM). Vysoká pořizovací cena přístroje, jednotlivých instrumentů s vybavením a nesystémová úhrada představují významnou překážku v širším využití robotického systému v léčbě KR i jiných abdominálních malignit v ČR., Introduction: The incidence of colorectal cancer (CRC) in the Czech Republic is reported to be one of the highest on the global scale. Radical tumor removal has been observed to be the most effective part in the context of current multimodal therapy. The authors present their preliminary results of robotic assisted treatment of rectal carcinoma (RC). Method: The observed group includes 61 patients who underwent robotic assisted treatment for rectal cancer. The data were collected prospectively in the last 31 patients. Analyses were conducted on epidemiological data, perioperative outcomes, complications and oncological results. Results: Robotic assisted treatment of RC was performed in 61 patients: 34 men and 27 women, mean age of 62 years (33–80). Neoadjuvant oncological treatment was indicated in 46% of the patients. Average blood loss was 187 ml, transfusions were administered in three cases. Conversion to open procedure was performed 6 times, and 16 patients had postoperative complications. Anastomotic leak was observed in 10% of the patients, and 4 patients undewent surgical treatment. No patient died. Local recurrence of the cancer was diagnosed in 3 (5%) patients. The quality of mesorectal excision (ME) and the circumferential resection margin [(y)pCRM] have been determined in 27 patients since 2013. Positive (y)pCRM was recorded in two cases and incomplete ME was observed in 25.8% of the patients. Conclusion: Surgical treatment for RC is pivotal in multimodal therapy. Our preliminary results are similar to the conclusions in other published studies. The da Vinci robotic system is a safe manipulator in the treatment of RC and provides indisputable benefits to the surgeon when operating in the narrow pelvic space. However, the benefits of robotic treatment in abdominal surgery are yet to be evaluated in patients (with respect to long-term results, sufficient number of patients or a high EBM level of evidence). The high purchase price of the robotic device, individual instruments with equipment and non-systemic compensation constitute a significant hindrance that prevents wider use of the robotic system in the treatment of RC and other abdominal malignancies in the Czech Republic., and D. - Purpose: This study aimed to evaluate prostate volume changes and prostate motions during radiotherapy.Methods: In 2010, twenty-five patients were treated for prostate cancer by external beam radiotherapy with implanted fiducial markers. Coordinates of three gold markers on kilovoltage images were calculated daily. Volume changes in target structure were observed through changes in intermarker distances. Differences in patient position between laser-tattoo alignment and gold marker localization were evaluated. Intrafraction motion was assessed by measuring marker displacement on kilovoltage images acquired before and after fraction delivery. Results: Prostate shrinkage was observed in 60% of patients. The average shrinkage was 7% of the prostate’s initial volume. Corrections after laser-tattoo alignment remained mostly below 1 cm. The difference between marker centroid position on the actual images and the planning images was 2 ± 1 mm on average. The extension of intrafraction movements was 7.6 ± 0.2 mm on average. Conclusions: In our retrospective study, the possibility for prostate volume changes during radiotherapy was revealed. - V letech 1959–2006 bylo u českých mužů evidováno 81 884 nových karcinomů prostaty (KP) s nárůstem z 12,3 na 96,1 na 100 tis. mužů. Úmrtnost se zvýšila z 21,3 na 27,2 na 100 tis. mužů. Podíl KP ze všech nádorů se zvýšil u incidence ze 7,1 % na 13,2 %, u mortality z 6,7 % na 8,9 %. V letech 1989–2005 vzrostla prevalence KP z 98 na 408,5 na 100 tisíc mužů a dosáhla téměř 20 500 žijících. Z 11 183 nemocných s vícečetnými KP bylo evidováno 4 608 primárních a 6 575 následných případů, představujících 17,4 % ze všech evidovaných KP v letech 1976–2005. Z primárních KP bylo léčeno 89,7 % duplicit a 10,3 % multiplicit. Počet primárních KP vzrostl na 282 v roce 1996 a pak klesal, následných KP trvale rostl na 705 v roce 2005. Téměř polovina KP se vyskytla ve věku 70–79 let a třetina ve věku 50–69 let. Poměr 1 115 (21,3 %) synchronních a 4 121 (78,7 %) metachronních následných novotvarů byl 1:3,7 s průměrnou dobou vzniku následného novotvaru 4,1 roku. Z 5 236 následných novotvarů po primárních KP byly nepočetnější nádory kůže 28,6 %, trávicí 26,7 %, močové 17,8 % a respirační 10,9 % soustavy. Z 5 613 novotvarů před KP jako druhou diagnózou, bylo 35,3 % nádorů kůže, 21 % trávicí, 19,6 % močové a 8,7 % respirační soustavy. Z nemocných s primárním KP k 17. 10. 2007 žilo 22,4 % a zemřelo 77,6 %, s následným KP žilo 31,6 % a zemřelo 68,4 % mužů. Vzhledem k výskytu KP ve spojení s dalšími novotvary není u české populace zatím dostatečná intervence ke zdravému životnímu stylu také vzhledem k očekávanému počtu 42 tisíc mužů, žijících s KP v roce 2015., In Czech men there were registered 81,884 new prostate cancers (PC) during 1959–2006, increased from 12.3 to 96.1 per 100,000 men. The average time between the primary and subsequent neoplasms was 4.1 years. The most frequent from 5,236 subsequent neoplasms after primary PC there were 28,6 % cancers of skin, 26,7 % gastrointestinal cancers, 17,8 % of urinary and 10,9 % in respiratory tract. The most frequent from 5,613 neoplasms before PC as the second diagnosis there were 35,3 % cancers of skin, 21 % gastrointestinal, 19,6 % urinary and 8,7 % respiratory tract. From patients with primary PC survived 22,4 % and died 77,6 %, from subsequent PC survived 31,6 % and died 68,4 % men up to 17. 10. 2007. - Cíl: Cílem práce je charakterizovat klinické a elektrofyziologické nálezy dvou typů ulceromutilující dědičné neuropatie CMT2B a HSN1 u tří českých rodin s molekulárně geneticky objasněnou příčinou. Soubor: Popisujeme tři rodiny s dědičnou senzitivní neuropatií s celkem 16 postiženými. Metodika: Na základě neurologického a elektromyografického vyšetření, výskytu podobných obtíží u příbuzných byla stanovena diagnóza senzitivní, převážně axonální neuropatie. Následně bylo provedeno sekvenování genů SPTLC1 a RAB7 v rodinách A, B, C a u dalších 24 nepříbuzných pacientů s klinickým podezřením na dědičnou senzitivní neuropatii. Výsledky: V rodině B byla nálezem mutace p.C133Y v SPTLC1 genu prokázána hereditární senzitivní neuropatie typ 1 (HSN1). V rodinách A a C byla nálezem mutací p.L129F a p.V162M v genu RAB7 prokázána choroba Charcot‑Marie‑Tooth typ 2B (CMT2B). Všechny tři mutace již byly dříve popsány a fenotyp odpovídá popisu pacientů z jiných zemí. DNA vyšetření dalších 24 nepříbuzných pacientů však příčinu onemocnění neobjasnilo. Závěr: Ulceromutilující dědičné neuropatie CMT2B a HSN1 jsou v české populaci, podobně jako v jiných zemích vzácné, ale klinické projevy jsou jasně poznatelné, pokud jsou zohledněny a správně získány genealogické údaje – rodinná anamnéza. Tyto tři popsané rodiny jsou dosud jediné známé v ČR s objasněnou příčinou dědičné senzitivní neuropatie v důsledku mutací v RAB7 a SPTLC1. Objasnění příčiny ulceromutilující senzitivní neuropatie má význam nejen pro upřesnění genetické a klinické prognózy a pro cílenou genetickou prevenci, ale u pacientů s mutacemi v SPTLC1 genu možná i pro cílenou terapii se substitucí L‑serinem, která bude testována v klinické studii (Boston, USA), Aim: The goal was to clinically and electrophysiologically characterize two types of ulceromutilating hereditary neuropathy CMT2B and HSN1 in three Czech families where molecular genetic cause was confirmed. Patients: We describe three families, overall 16 affected patients, with hereditary sensory neuropathy. Methods: The diagnosis of sensory, predominantly axonal neuropathy was done on the basis of neurological and electrophysiological examination. Sequencing of the SPTLC1 and RAB7 genes was done in families A, B, C and 24 unrelated patients with clinical suspicion for HSN. Results: Hereditary sensory neuropathy type 1 (HSN1) caused by the p.C133Y mutation in the SPTLC1 gene was confirmed in family B and the Charcot-Marie-Tooth type 2B (CMT2B) caused by p.L129F and p.V162M mutations in the RAB7 gene was confirmed in families A and C. All three mutations have been previously described. DNA examination of 24 unrelated patients did not reveal the cause of their disease. Conclusion: As in other countries, ulceromutilating hereditary neuropathies CMT2B and HSN1 are rare in the Czech population. However, clinical manifestations are clearly recognizable if correctly obtained genealogical data – family history – is properly taken into account. The three families described here are the only known families with hereditary neuropathies caused by mutations in RAB7 and SPTLC1 in the Czech Republic. Clarification of the cause of ulceromutilating sensory neuropathy is crucial for genetic and clinical prognosis, including targeted genetic prevention, but possibly also for an L-serin therapy in SPTLC1 mutation patients to be tested in a clinical study (Boston, USA)., and D.
