Psoriasis patients are often susceptible to cardiovascular diseases (CVD), including atherosclerosis. Traditional markers (biochemical and inflammatory) and diagnostic tools could detect occlusive but not subclinical atherosclerosis. Carotid intima-media thickness (CIMT), has recently been recognised as a non invasive diagnostic tool for identification of premature atherosclerosis. Therefore we evaluated 80 psoriasis patients and 80 age sex matched healthy controls for serum leptin levels and apolipoprotein B/apolipoprotein A-I ratio (apoB/apoA-I ratio) in relation with CIMT of carotid artery. Carotid intima-media thickness and carotid plaques were simultaneously measured by carotid sonography. Serum concentration of leptin and apolipoprotein were measured using enzyme-linked immuno sorbent assay (ELISA) and nephelometry respectively. Raised CIMT correlated to age of onset of the disease, serum leptin and apoB/apoA-I ratio in psoriasis patients. Taking into account, values that were above the 75 percentile of the three markers (leptin, apoB/apoA-I ratio and CIMT) the odds ratio was 4.26 (2.06-8.80 CI). Leptin and apoB/apoA-I ratio showed significant cumulative association with CIMT. Results of predictive analysis supports measurement of CIMT along with estimation of serum leptin and apoB/apoA-I ratio for prediction of premature atherosclerosis in psoriasis patients. and K. Cíl: Naším záměrem bylo zjistit diagnostické charakteristiky Montrealského kognitivního testu (MoCA) v tréninkové podobě pro lepší administraci u pacientů s Alzheimerovou nemocí (AN) poprvé v českém písemnictví. <p align="">Soubor a metodika: Záznamový formulář jsme obohatili o instrukce pro vyšetřovaného, instrukce k administraci a návod ke skórování. Obsah a skórování zůstaly zachovány a odpovídají předchozí české verzi. Upravili jsme návod k administraci. Zpětný překlad záznamového archu a návodu do angličtiny byl zkontrolován, schválen a nazván kanadským tvůrcem jako tzv. tréninková verze MoCA‑CZ1. Tímto testem jsme vyšetřili 30 pacientů s již diagnostikovanou mírnou demencí způsobenou Alzheimerovou nemocí podle nových doporučení NIA‑AA (78 ? 6 let, 13 ? 3 let vzdělání, 56 % žen) a 30 kognitivně normálních seniorů (NOS) podobného věku, vzdělání a pohlaví. <p align="">Výsledky: Oproti standardní verzi obsahují záznamové archy tréninkové české podoby MoCA‑CZ1 navíc další informace, s nimiž předpokládáme snadnější a spolehlivější provádění a vyhodnocování testu. Celkové skóry MoCA‑CZ1 byly významně nižší u pacientů s mírnou demencí způsobenou AN (18 ? 5 bodů) než u NOS (26 ? 3 body) (p < 0,00001). Celkové skóry NOS nezávisely na věku, pohlaví a vzdělání. Při porovnávání obou skupin podle analýzy receiver operating curve byl zjištěn optimální hraniční skór ≤ 23 bodů se senzitivitou 87 % a specificitou 84 %. Plocha pod křivkou byla 0,92. <p align="">Závěry: Tréninková MoCA‑CZ1 je volně dostupná na http://www.pcp.lf3.cuni.cz/adcentrum/testy.html. Výsledek testu může přispět k dřívějšímu záchytu mírné demence způsobené AN, a to časnou detekcí kognitivního deficitu. Při používání tréninkové verze MoCA‑CZ1 doporučujeme používat hraniční skór ≤ 23 bodů., Aim: Our aim was to determine diagnostic characteristics of the Montreal cognitive assessment (MoCA) using its training version for better administration in patients with Alzheimer’s disease (AD) for the first time in Czech literature. <p align="">Patients and methods: We complemented the test form with instructions for the patient, and with administration and scoring instructions. The content and the scoring were preserved from the previous Czech version. A separate Czech administration instruction manual was refined. English back translation of the data collection form and the manual was checked and approved by the Canadian author who also named it the training version. We examined 30 patients previously diagnosed with mild dementia due to Alzheimer’s disease according to the new NIA‑AA recommendations (78 ? 6 years, 13 ? 3 years of education, 56% women) and 30 cognitively normal seniors (NOS) of similar age, education and gender by MoCA‑CZ1. <p align="">Results: The training version of MoCA‑CZ1 contains additional information. We assume that this ensures reliability, and facilitates administration and scoring of the test. Total MoCA‑CZ1 scores were significantly lower in patients with AD (18 ? 5 points) than in NOS (26 ? 3 points) (p < 0.00001). Total scores of NOS did not correlate with age, gender or education. Analysis of receiver operating curve comparing the two groups found optimal cut off score of ≤ 23 points with sensitivity of 87% and specificity of 84%. BACKGROUND: The goal of this prospective study was to determine the frequency of micrometastases in patients with squamous cell carcinoma (SCC) of the oral cavity, pharynx and larynx in whom elective neck dissection was indicated (cN0). PATIENTS AND METHODS: A total of 12 patients (10 males and 2 females) were enrolled in the study. The age ranged 42-73 years (median 62 years). Elective neck dissection was performed in all patients (8 ipsilateral, 4 bilateral) and a total of 256 lymph nodes were removed and sent for microscopic examination. RESULTS: The presence of tumor cells in cervical lymph nodes was found in 5/12 (42%) patients. Micrometastases of SCC were found in two patients and isolated tumor cells (ITC) in two other patients. In the remaining one patient with oropharyngeal SCC, a micrometastasis of papillary thyroid carcinoma (PTC) was detected. Positive lymph nodes were localized in level II in three patients with SCC of larynx, hypopharynx and tongue base, respectively, in level I in one patient with SCC of oral tongue and in level III in one patient with PTC. CONCLUSION: Our results indicate that SCC of head and neck has a high potential for creating micrometastases which frequency is higher compared to clinically detected macrometastases. Cíl: Cílem studie bylo zhodnotit přínos elektrofyziologického vyšetření v diferenciální diagnostice lumbální spinální stenózy (LS) a diabetické polyneuropatie (DPN). Soubor a metodika: Elektrofyziologické vyšetření bylo provedeno u 68 pacientů s klinicky symptomatickou LS, u 28 pacientů s DPN a 32 zdravých dobrovolníků. Výsledky: Vyšetřené elektrofyziologické parametry z horních končetin (latence F vlny n. ulnaris a amplituda senzitivního neurogramu n. radialis), všechny hodnocené latence z dolních končetin (F vlny n. tibialis, H reflexu m. soleus, spinální latence MEP) a amplituda senzitivního neurogramu n. suralis signifikantně odlišovaly pacienty s LS a DPN. ROC analýza nicméně prokázala pouze dva elektrofyziologické parametry jako efektivní nezávislé faktory pro odlišení pacientů s LS a DPN: latenci ulnarisové F vlny (optimální diskriminační hodnota je 24,2 ms, senzitivita 82,7 % a specificita 63,9 %) a amplitudu senzitivního neurogramu n. radialis (optimální diskriminační hodnota je 10,5 ?V, senzitivita 75,5 % a specificita 58,2 %). Vícerozměrná diskriminační analýza poskytla kanonické skóre s nejsilnější prediktivní hodnotou. Závěry: Elektrofyziologické vyšetření z horních končetin je užitečné v diferenciální diagnostice LS a DPN, nicméně nejlepší diskriminační efekt vykazuje kanonické skóre, které zahrnuje několik elektrofyziologických parametrů. Methods: Electrophysiological parameters were examined in 68 patients with clinically symptomatic LS, in 28 patients with DPN, and in a group of 32 healthy volunteers. Results: Electrophysiological parameters evaluated from the upper extremities (F-wave latency of the ulnar nerve and radial SNAP amplitude), all the latencies measured in the lower extremities (tibial F-wave, soleus H-reflex and spinal latency of MEP), and the sural SNAP amplitude contributed significantly to distinguishing the LS from DPN patients. ROC analysis, however, disclosed only two electrophysiological parameters as effective in independent discrimination between the LS and DPN patient groups: ulnar F-wave latency (cut-off point at 24.2 ms, sensitivity 82.7% and specificity 63.9%); and radial SNAP amplitude (cut-off point at 10.5 ?V, sensitivity 75.5%, specificity 58.2%). Multivariate discrimination provided a canonical score with the most powerful predictive value of all. Cíl: Posoudit možnosti diagnostiky cho-lesteatomu středouší pomocí magnetické rezonance (MR) za použití non-echo-planární difúzně-vážené (non-EPI DWI) sekvence. Metodika: Prospektivní studie zahrnovala všechny pacienty, kteří byli indikování k primární nebo revizní operaci cholesteatomu středouší v období mezi 10/2012 až 1/2015. Všichni tito pacienti před operací podstoupili vyšetření MR za použití non-EPI DWI techniky. Šlo o 25 pacientů, u dvou s oboustranným postižením. Deset bylo před primární operací, 17 před revizním výkonem. MR vyšetření bylo zhotoveno v rozmezí 0-106 dnů (medián 2 dny) před operací. Výsledky MR vyšetření byly srovnány s operačním a definitivním histologickým nálezem. Výsledky: MR vyšetření s non-EPI DWI sekvencí mělo pro diagnostiku cholesteatomu senzitivitu 93,75 %, specificitu 93,75 %, pozitivní prediktivní hodnotu 93,75 % a negativní prediktivní hodnotu 93,75 %. Nejmenší správně diagnostikovaný cholesteatom měl velikost 4 mm, největší 20 mm, průměr 11 mm. Závěr: Vyšetření MR s použitím non-EPI DWI sekvence je spolehlivou metodou k zobrazení cholesteatomu středouší. Lze ji využít jak u pacientů před primárním operačním výkonem, tak u pacientů před second-look či revizní operací., Aim: To evaluate the possibility of diagnosis of middle ear cholesteatoma on MRI using non-echo-planar diffusion-weighted imaging (non-EPI DWI). Method: The prospective study included all patients indicated to primary or second-look surgery of middle ear cholesteatoma between 10/2012 and 1/2015. All these patients underwent MRI using non-EPI DWI technique before the surgery. There were 25 patients, two patients had bilateral disease. 10 patients were before primary surgery, 15 patients before second-look surgery. The MRI was performed from 0 to 106 days (median 2 days) before surgery. The results of the MR investigation were compared with those obtained by surgery and confirmed by definitive histopatological evaluations. Results: The sensitivity of 93,75%, specificity of 93,75 %, positive predictive value 93,75 %, negative predictive value 93,75% was reached in detection of the middle ear cholesteatoma using MR with non-EPI DWI. The smallest correctly diagnosed cholesteatoma had the diameter of 4mm, the largest 20 mm, mean 11 mm. Conclusion: MR investigation using non-EPI DWI sequence is reliable method for the middle ear cholesteatoma diagnosis. Ultrazvukové měření děložního hrdla představuje dobře zavedenou metodu predikce předčasného porodu. Nicméně sensitivita a pozitivní predikční hodnota ultrazvukové cervikometrie zůstává stále relativně neuspokojivá. Proto je snaha upřesnit predikci předčasného porodu využitím dalších metod v kombinaci s ultrazvukovou cervikometrií. Jedna skupina výzkumných prací se zabývá hodnocením různých parametrů z cervikovaginálního sekretu – nejvýznamnější představuje fetální fibronektin (fFN). Pozitivní test na fFN ukazuje relativně nízkou pozitivní predikční hodnotu stran předčasného porodu, nicméně výhodou je vysoká negativní predikční hodnota – bezmála 95%. Data z posledních studií ukazují užitečnost hodnocení fFN spolu s cervikometrií. Nedávné objevy ukázaly, že T regulační lymfocyty (CD4+CD25+CD127 - dále zkráceně Treg lymfocyty) mohou hrát důležitou roli v patofyziologii předčasného porodu. Cílem výzkumu na našem pracovišti je vyhodnotit vztah mezi hladinou Treg lymfocytů a ultrazvukovou délkou děložního hrdla k riziku předčasného porodu. Pilotní data z naší práce ukazují, že kombinované hodnocení uvedených parametrů představuje mnohem lepší možnost predikce předčasného porodu než hodnocení obou parametrů samostatně. Je však potřeba dalšího výzkumu, který bude mít za cíl vyhodnotit korelaci mezi sníženými hladinami Treg lymfocytů a předčasným porodem., Ultrasound measurement of the uterine cervix represents a well-established method in the prediction of preterm labour. However, the sensitivity and positive predictive value of ultrasound cervicometry still remain rather unsatisfactory. Hence, the effort to provide more specific predictions of premature labour using other applicable methods in combination with ultrasound cervicometry. One group of studies have focused on determining various parameters from cervicovaginal secretion - the most important one being fetal fibronectin (fFN). A positive fFN test in the birth canal shows a relatively low positive predictive value in terms of premature birth; however, it does provide the advantage of a high negative predictive value, which is about 95%. Data from latest studies show the usefulness of fFN in combination with cervicometry. Recent discoveries suggest that T regulatory lymphocytes (CD4+CD25+CD127 - Tregs) may play an important role in the pathophysiology of preterm labour. The purpose of our study was to assess the relationship between maternal circulating Tregs and the sonographically measured uterine cervix and the risk of preterm labour. We can show for the first time that the combined assessment of Treg cell count and cervical length provides a much better prediction of preterm delivery that either parameter used on its own. Cíle: Pseudoaneurysma intrarenálních arterií (RAP) může představovat život ohrožující komplikaci, k níž může dojít po parciální resekci ledviny. Autoři hodnotí incidenci RAP, u něhož zatím nedošlo k ruptuře (detekováno pomocí trojrozměrné CT angiografie – CTA), časně po minimálně invazivní parciální resekci ledviny (včetně laparoskopické a robotické operace). Dále hodnotí možnost využití nefrometrického skóre při predikci vzniku RAP. Metody: V období od února 2012 do listopadu 2013 podstoupilo 101 pacientů s renálními tumory minimálně invazivní parciální resekci ledviny. Tři až čtyři dny po operaci byla provedena CTA a radiologové (zaslepeně) diagnostikovali RAP. Pomocí logistického regresního modelu analyzovali autoři faktory, které mohou potenciálně ovlivňovat vznik RAP. Výsledky: Míra incidence RAP časně po minimálně invazivní parciální resekci (MIPR) byla nečekaně vysoká – 21,7 % (detekováno pomocí CTA). U pacientů s RAP byl přítomen signifikantně větší tumor (p = 0,02) a významně vyšší skóre parametru N indikující blízkost tumoru k renálnímu sinu (p = 0,01) než u pacientů, u nichž nedošlo ke vzniku RAP. Multivariátní analýza však prokázala, že ani celkové nefrometrické skóre ani hodnota jednotlivých komponent neměly významný vliv na vznik RAP. Velikost tumoru představovala jediný významný nezávislý prediktor vzniku RAP po minimálně invazivní resekci ledviny. Závěr: Parametr N nefrometrického skóre představuje významný prediktor vzniku RAP podle univariátní analýzy, nikoli však podle multivariátní analýzy. Komponentu N nefrometrického skóre tedy lze považovat za jeden z faktorů užitečných pro predikci vzniku RAP., Objectives: Renal artery pseudoaneurysm (RAP) can be a life threatening complication after partial nephrectomy. We investigated the incidence of unruptured RAP detected by 3-dimensional computed tomography arteriography (CTA) in the early period after minimally invasive partial nephrectomy (MIPN), including laparoscopic and robotic partial nephrectomy. We also examined the usefulness of nephrometry scoring system to predict the occurrence of RAP. Methods: From February 2012 to November 2013, 101 patients underwent MIPN for renal masses. CTA was performed 3–4 days after surgery, and the radiologists made a diagnosis of RAP in the blinded manner. The factors influencing the occurrence of RAP were analyzed with the logistic regression model. Results: The incidence of RAP was unexpectedly high at 21.7% when detected by CTA in the early period after MIPN. The RAP group showed a significantly larger tumor size (p = 0.02) and a significantly higher N component score (p = 0.01) compared to the No-RAP group. However, Neither the sum of nephrometry score nor each component showed significant influence on the occurrence of RAP according to the multivariate analysis. Tumor size was the only significant independent factor to predict the occurrence of RAP after MIPN. Conclusions: The N component of the nephrometry scoring system is a significant predictor for RAP by the univariate analysis, but not by the multivariate analysis. Objective: The study aimed to evaluate plasma adiponectin (ADP), adipocyte fatty acid binding protein (A-FABP) and fibroblast growth factor 21 (FGF 21) levels as potential predictors of severity of acute pancreatitis (AP) in day 4 after admission. Simultaneously, the classical proinflammatory makers were analysed as well. Settings: Department of Clinical Biochemistry and 2nd Department of Internal Medicine, Faculty Hospital Olomouc, I. P. Pavlova 6, 775 20 Olomouc, Czech Republic; Hospital Písek, Karla Čapka 589, 397 23 Písek, Czech Republic. Study design: The study was conducted in subjects with acute pancreatitis (n = 84, 37 females, 47 males). The analyses were performed in the groups according to the mild/severe classification of AP, and partly in the computed-tomography severity index (CTSI) score subgroups. Results: From adipokines, only FGF 21 tended to be higher in the severe AP subgroup in day 4. The multiple regression analysis revealed a positive association of A-FABP with procalcitonin (PCT), whilst FGF 21 was associated positively with ADP and negatively with C-reactive protein (CRP) in patients with the severe AP. The receiver-operator characteristics (ROC) analysis confirmed that not adipokines, but only CRP and interleukin 6 (IL-6) were suitable as potential predictors of the disease severity. The cut-off values were established for both parameters: 100 mg/L for CRP and 37 ng/L for IL-6, with negative predictive values (NPV) 96 % and 92 %, and positive predictive values (PPV) 39 % and 29 %, respectively. Výsledky: Ze sledovaných adipokinů měl u pacientů s těžkou AP pouze FGF 21 tendenci k vyšším hladinám v den 4 po přijetí. Za pomoci mnohočetné regresní analýzy byla zjištěna pozitivní asociace A-FABP s prokalcitoninem (PCT), zatímco FGF 21 asocioval u těžké AP pozitivně s ADP a negativně s C-reaktivním proteinem (CRP). ROC (receiver-operator characteristics) analýza potvrdila, že pouze CRP a interleukin 6 (IL-6) jsou využitelné jako potenciální prediktory závažnosti onemocnění. Pro oba parametry byly stanoveny cut-off hodnoty: 100 mg/l pro CRP a 37 ng/l pro IL-6, s negativními prediktivními hodnotami (NPV) 96 % a 92 %, a s pozitivními prediktivními hodnotami (PPV) 39 % a 29 %.
